PNPLA3 and TM6SF2 polymorphisms in Brazilian patients with nonalcoholic fatty liver disease

BACKGROUND Nonalcoholic fatty liver disease(NAFLD)is becoming the most common chronic liver disease worldwide,with significant morbidity associated with nonalcoholic steatohepatitis(NASH).Genome-wide association studies demonstrated that the variants rs738409 C/G in the PNPLA3 and rs58542926 C/T in the TM6SF2 genes are determinants of inter-individual and ethnicity-related differences in hepatic fat content and NAFLD *** To investigate PNPLA3 and TM6SF2 genotype frequency and their association with NAFLD development and progression in Brazilian *** This cross-sectional case-control study enrolled 285 individuals from the Gastroenterology and Hepatology clinics at a university hospital in *** case patients(n=148)were confirmed to have NAFLD by the identification of hepatic steatosis on ultrasonography and exclusion of other causes of liver *** to the clinical protocol,patients underwent liver biopsy when at high risk for NASH and/or advanced fibrosis(n=65).Steatohepatitis was confirmed in 54 *** who did not have biopsy indication or NASH on histology were considered to have simple steatosis(n=94).The control group(n=137)was selected among patients that attended the Intestinal Disease clinic and was composed of subjects without abnormalities on abdominal ultrasonography and normal liver biochemical *** individuals underwent PNPLA3 and TM6SF2 genotype *** PNPLA3 CC,CG and GG genotype frequencies were 37%,44%and 19%,respectively,in NAFLD patients and were 58%,31%and 10%in controls(P<0.001).In a model adjusted for gender,age,body mass index and type 2 diabetes mellitus,the G allele increased the chance of NAFLD(OR=1.69,95%CI:1.21-2.36,P=0.002)and NASH(OR=3.50,95%CI:1.84-6.64,P<0.001).The chance of NASH was even higher with GG homozygosis(OR=5.53,95%CI:2.04-14.92,P=0.001).No association was found between G allele and the features of metabolic *** histological assessment,PNPLA3 genoty

Probiotics as a complementary therapeutic approach in nonalcoholic fatty liver disease

Nonalcoholic fatty liver disease(NAFLD) is currently recognized as one of the most common causes of chronic liver disease. It involves a spectrum of conditionsthat include pure steatosis without inflammation, steatohepatitis, fibrosis and cirrhosis. The key factor in the pathophysiology of NAFLD is insulin resistance that determines lipid accumulation in the hepatocytes and, thus, oxidative stress, which is followed by inflammatory response. However, NAFLD pathogenesis is still largely unknown and has been extensively investigated. Although life style modification with the aim of losing weight has been advocated to treat this disorder, its effectiveness is limited; additionally, there is no specific pharmacologic treatment until nowadays. Recent evidence suggests that the gut microbiota may play a role in the development of insulin resistance, hepatic steatosis, necroinflammation and fibrosis. Differences in gut microbiota between NAFLD patients and lean individuals as well as presence of small intestinal bacterial overgrowth in NAFLD subjects have been demonstrated. Furthermore, some data indicate that the immunoregulatory effects of probiotics may be beneficial in NAFLD treatment as they modulate the intestinal microbiota; improve epithelial barrier function and strengthen the intestinal wall decreasing its permeability; reduce bacterial translocation and endotoxemia; improve intestinal inflammation; and reduce oxidative and inflammatory liver damage. In this article, we review the clinical trials on the use of probiotics in the treatment of NAFLD and discuss the effects of these agents and their efficacy as an emerging therapeutic resource to treat NAFLD patients.

Nonalcoholic fatty liver disease(NAFLD) has been identified as one of the most prevalent chronic liver disease in adults and children populations. NAFLD is usually associated with the metabolic syndrome(MS), which is chiefly related to insulin resistance and its consequences. Insulin resistance has a crucial role in the pathogenesis of hepatic steatosis and potentially nonalcoholic steatohepatitis(NASH). Because of the contemporary epidemics of MS and obesity, the burden of NAFLD is also expected to rise. Unhealthy diets, such as the so-called western diet, are enriched in fructose, trans-fatty acids and saturated fat and seem to be associated with the development of NAFLD. In human studies, certain dietary sugars, particularly fructose, are used as a substrate for lipogenesis leading to hepatic fatty infiltration, inflammation, and possibly fibrosis. Other investigations have shown that fat consumption especially cholesterol and trans/saturated fatty acids are also steatogenic and seem to increase visceral adiposity. The identification of specific dietary components that favor the development of NASH could be important for the management of this disorder. This review focuses on the effects of different dietary approaches to prevent and treat NAFLD emphasizing the macronutrients and energy composition.

Do Cytokines Play a Role in Predicting Some Features and Outcome in Infective Endocarditis?

Objective: This study aimed to measure the serum concentrations of several cytokines in patients with infective endocarditis (IE), and correlate them with the C-reactive protein serum levels, echocardiographic findings and infecting microorganisms. Methods: Forty-five patients with definite IE according to the Duke’s criteria were included in the study. Ten healthy blood donors formed the control group. Serum levels of interleukin (IL) 1β, IL-6, IL-8, IL-10, IL-12 and tumor necrosis factor-α (TNF-α) were measured and compared between the groups, and also with some clinical and laboratory parameters of IE. Results: Patients with IE had significantly higher serum concentrations of the inflammatory mediators than the control patients. Median IL-12 and IL-1β levels were higher in staphylococcal than streptococcal IE. Except for TNF-α, the levels of all the other cytokines correlated with the C-reactive protein concentrations. In multivariate analysis, IL-10 and IL-12 serum levels remained as independent factors associated with the C-reactive protein concentrations. There was a significant correlation between IL-10 concentration and vegetation length. Conclusion: The inflammatory serum cytokines levels were elevated in IE in comparison to healthy controls. C-reactive protein concentrations correlated with cytokines levels. IL-1β and IL-12 serum concentrations were higher in staphylococcal when compared to streptococcal IE. IL-10 levels were associated with vegetation size.

The B1 dam of Co rrego do Feij～ao Mine, owned by Vale, S.A. mining company and located on the FerroCarv～ao stream, collapsed and injected 2.8 Mm3of clayey, silty, and sandy iron-and manganese-rich tailings into the Paraopeba River(Minas Gerais state, Brazil). The accident occurred on 25 January2019 and the tailings have been co-transported with coarser natural sediment since then, being partly trapped in the Igarape Weir reservoir located on the Paraopeba River nearly 45 km downstream the injection point. The general purpose of the current study was to model suspended sediment transport in the vicinity of the Igarape Weir aiming to assess the concomitant barrier effect imposed by this ***, the spatial distributions of suspended clay, silt, and very fine-grained sand fractions(CSS) of sediment were mapped around the Igarape Weir under low-flow(16 m3/s) and high-flow(5 to 10,000years return period stream discharge; 699—2,699 m3/s) regimes, using RiverFlow2D as the modelling tool. The concentrations of the various grain materials in the upstream and downstream sectors were quantified linking the barrier effect to concentration reductions in the direction of stream flow. It was also a study goal to calculate differences of iron and manganese concentrations in the sediment + tailings mixtures along the Paraopeba River. The study results showed reductions in the CSS between 6.6% and18%, from upstream to downstream of the Igarape Weir, related with backwater effects, free and submerged hydraulic jumps, bank sedimentation in periods of high flow, and streambed sedimentation controlled by channel sinuosity and tailings density. These reductions were accompanied by drops in the concentrations of iron and manganese present in the clay and silt fractions, which varied between 6% and42% under low flows and between 16% and 44% under high flows. Hydrocarbons in particulate samples from wildfire events in central Portugal in summer 2010

In summer 2010, twenty eight （14 PM=2.5 samples plus 14 samples PM2.5-10） smoke samples were collected during wildfires that occurred in central Portugal. A portable high-volume sampler was used to perform the sampling, on quartz fibre filters of coarse （PM=2.5-10） and fine （PM2.5） smoke samples. The carbonaceous content （elemental and organic carbon） of particulate matter was analysed by a thermal-optical technique. Subsequently, the particulate samples were solvent extracted and fractionated by vacuum flash chromatog- raphy into three different classes of organic compounds （aliphatics, polycyclic aromatic hydrocarbons （PAHs） and carbonyl compounds）. The organic speciation was performed by gas chromatography-mass spectrometry （GC-MS）. Emissions were dominated by the fine particles, which represented around 92% of the PM10. A clear predominance of carbonaceous constituents was observed, with organic to elemental carbon （OC/EC） ratios ranging between 1.69 and 245 in both size fractions. The isoprenoid ketone 6,10,14-trimethyl-2-pentadecanone, a tracer for secondary organic aerosol formation, was one of the dominant constituents in both fine and coarse particles. Retene was the most abundant compound in all samples. Good correlations were obtained between OC and both aliphatic and PAH compounds. Pyrogenic processes, thermal release of biogenic com- pounds and secondary processing accounted for 97% of the apportioned PM2.5 levels.

Modelling fuel loads of understorey vegetation and forest floor components in pine stands in NW Spain

In this study,310 destructively sampled plots were used to develop two equation systems for the three main pine species in NW Spain(***;*** and ***):one for estimating loads of understorey fuel components by size and condition(live and dead)and another one for forest floor *** systems of equations were simultaneously fitted for estimating fuel loads using overstorey,understorey and forest floor variables as *** systems of equations included both the effect of pine species and the effect of understorey compositions dominated by ferns-brambles or by woody species,due to their obvious structural and physiological *** general,the goodness-of-fit statistics indicated that the estimates were reasonably robust and accurate for all of the fuel *** best results were obtained for total understorey vegetation,total forest floor and raw humus fuel loads,with more than 76%of the observed variability explained,whereas the poorest results were obtained for coarse fuel loads of understory vegetation with a 53%of observed variability *** reduce the overall costs associated with the field inventories necessary for operational use of the models,the additive systems were fitted again using only overstorey variables as potential *** relationships for fine(<6 mm)and total understorey vegetation and total forest floor fuel loads were obtained,indicating the complexity of the forest overstorey-understorey and overstorey-forest floor ***,these models explained around 52%of the observed ***,equations estimating the total understorey vegetation and the total forest floor fuel loads based only on canopy cover were *** models explained only 26%-32%of the observed variability;however,their main advantage is that although understorey vegetation in forested landscapes is largely invisible to remote sensing,canopy cover can be estimated with moderate accuracy,allowin
