Comparison of inebilizumab or rituximab in addition to glucocorticoid therapy for neuromyelitis optica spectrum disorders

AIM:To investigate the short-term efficacy and safety of inebilizumab for neuromyelitis optica spectrum disorders(NMOSD).METHODS:A total of 33 patients with NMOSD treated with inebilizumab(Group INB,n=15)or rituximab(Group RTX,n=18)in addition to high-dose glucocorticoids were included.Both groups underwent hormone shock therapy during the acute phase.Subsequently,Group INB received inebilizumab injections during the remission phase,while Group RTX received rituximab injections.A comparison of aquaporins 4(AQP4)titer values,peripheral blood B lymphocyte counts,and visual function recovery was conducted before and 8wk after treatment.Additionally,adverse reactions and patient tolerability were analyzed after using inebilizumab treatment regimes.RESULTS:Following inebilizumab treatment,there was a significantly improvement in the visual acuity of NMOSD patients(Pmab treatment showed a partial effect in preventing optic nerve atrophy(Pmab,which has previously demonstrated substantial therapeutic efficacy(P>0.05).Furthermore,inebilizumab exhibited higher safety levels than that of rituximab injections.CONCLUSION:The combination of inebilizumab and high-dose glucocorticoids proves to be effective.In comparison to rituximab injections,inebilizumab displays better tolerance and safety.Moreover,it demonstrates a partial effect in preventing optic nerve atrophy.Thus,it stands as an effective method to reduce the disability rates and improve the daily living ability of patients with NMOSD.

0 INTRODUCTION To relieve the short supply of suitable land for residing,cultivating,and commerce,it is accepted to reclaim land from lakes,river estuaries,and coastal regions in many countries(mancini et al.,2020).The large-scale reclamation activities have an irreversible effect on the mico-geomorphology and hydrodynamic system of seasonally flooded areas(Zhang et al.,2019;Yu et al.,2018).For example,coastal regions reclaimed extended the groundwater flow paths,as well as created groundwater level rise,resulting in water-logging in the reclaimed area(Hu et al.,2008).Similar changes in groundwater flow systems may alter groundwater-lake water interactions,and it can ultimately affect the water cycle in the lake areas.However,such detailed studies are presently lacking.In addition,natural exchanges between the lake and groundwater systems can be strongly affected by physical hydrological processes(Javadzadeh et al.,2020),chemical properties(Kamtchueng et al.,2015),and bioto behavior(Hancock et al.,2005).

Hydralazine represses Fpn ubiquitination to rescue injured neurons via competitive binding to UBA52

A major impedance to neuronal regeneration after peripheral nerve injury (PNI) is the activation of various programmed cell death mechanisms in the dorsal root ganglion. Ferroptosis is a form of programmed cell death distinguished by imbalance in iron and thiol metabolism, leading to lethal lipid peroxidation. However, the molecular mechanisms of ferroptosis in the context of PNI and nerve regeneration remain unclear. Ferroportin (Fpn), the only known mammalian nonheme iron export protein, plays a pivotal part in inhibiting ferroptosis by maintaining intracellular iron homeostasis. Here, we explored in vitro and in vivo the involvement of Fpn in neuronal ferroptosis. We first delineated that reactive oxygen species at the injury site induces neuronal ferroptosis by increasing intracellular iron via accelerated UBA52-driven ubiquitination and degradation of Fpn, and stimulation of lipid peroxidation. Early administration of the potent arterial vasodilator, hydralazine (HYD), decreases the ubiquitination of Fpn after PNI by binding to UBA52, leading to suppression of neuronal cell death and significant acceleration of axon regeneration and motor function recovery. HYD targeting of ferroptosis is a promising strategy for clinical management of PNI.

A Novel Defender-Attacker-Defender Model for Resilient Distributed Generator Planning with Network Reconfiguration and Demand Response

To improve the resilience of a distribution system against extreme weather,a fuel-based distributed generator(DG)allocation model is proposed in this study.In this model,the DGs are placed at the planning stage.When an extreme event occurs,the controllable generators form temporary microgrids(MGs)to restore the load maximally.Simultaneously,a demand response program(DRP)mitigates the imbalance between the power supply and demand during extreme events.To cope with the fault uncertainty,a robust optimization(RO)method is applied to reduce the long-term investment and short-term operation costs.The optimization is formulated as a tri-level defenderattacker-defender(DAD)framework.At the first level,decision-makers work out the DG allocation scheme;at the second level,the attacker finds the optimal attack strategy with maximum damage;and at the third level,restoration measures,namely distribution network reconfiguration(DNR)and demand response are performed.The problem is solved by the nested column and constraint generation(NC&CG)method and the model is validated using an IEEE 33-node system.Case studies validate the effectiveness and superiority of the proposed model according to the enhanced resilience and reduced cost.

Synthesis of gem-Difluoroalkenes via Ni-Catalyzed Three-Component Defluorinative Reductive Cross-Coupling of Organohalides,Alkenes and Trifluoromethyl Alkenes

gem-Difluoroalkenes are considered ideal isosteres for metabolically susceptible carbonyl groups in modern drug discovery and medicinal chemistry.In addition,gem-difluoroalkenes are used as versatile precursors for the synthesis of difluoroalkylated compounds and monofluoroalkenes.Therefore,a great deal of effort has been devoted to developing efficient methods for their preparation.The catalytic defluorinative functionalization of trifluoromethyl alkenes represents a useful strategy for the preparation of chiral gem-difluoroalkenes.However,most of these catalytic processes are still essentially limited to two-component defluorinative cross-couplings to form single C—C bonds.Due to the challenge of controlling chemoselectivity in the carbon-carbon bond forming events,three-component defluorinative cross-coupling involving multiple C—C bond formations has rarely been studied.We report a nickel-catalyzed three-component defluorinative reductive cross-coupling of organohalides,alkenes and trifluoromethyl alkenes.A variety of electron-rich and electron-deficient alkenes,as well as aryl and alkyl halides can efficiently participate in the formation of three-component cross-coupling products.This reaction proceeds under mild conditions and exhibits excellent functional group compatibility without requiring a pendant chelating group,providing a variety of functionalized gem-difluoroalkenes in good yields with excellent chemoselectivity.

The coexistence of venous thromboembolism(VTE)within patients with cancer,known as cancer-associated thrombosis(CAT),stands as a prominent cause of mortality in this population.Over recent years,the incidence of VTE has demonstrated a steady increase across diverse tumor types,influenced by several factors such as patient management,tumor-specific risks,and treatment-related aspects.Furthermore,mutations in specific genes have been identified as potential contributors to increased CAT occurrence in particular cancer subtypes.We conducted an extensive review encompassing pivotal historical and ongoing studies on CAT.This review elucidates the risks,mechanisms,reliable markers,and risk assessment methodologies that can significantly guide effective interventions in clinical practice.

Due to the complex pathophysiological mechanism, spinal cord injury (SCI) has become one of the most intractable central nervous system (CNS) diseases to therapy. Stem cell transplantation, mesenchymal stem cells (MSCs) particularly, appeals to more and more attention along with the encouraging therapeutic results for the functional regeneration of SCI. However, traditional cell transplantation strategies have some limitations, including the unsatisfying survival rate of MSCs and their random diffusion from the injection site to ambient tissues. The application of biomaterials in tissue engineering provides a new horizon. Biomaterials can not only confine MSCs in the injured lesions with higher cell viability, but also promote their therapeutic efficacy. This review summarizes the strategies and advantages of biomaterials reinforced MSCs transplantation to treat SCI in recent years,which are clarified in the light of various therapeutic effects in pathophysiological aspects of SCI.

Dear Editor,Cardiovascular disease is the leading cause of deaths worldwide,with coronary artery disease(CAD)accounting for approximately 50%of its mortality.Dual antiplatelet therapy,including aspirin and a P2Y12 inhibitor,is the most important treatment for CAD patients undergoing percutaneous coronary intervention(PCI)to prevent recurrent ischemic events and cardiac death.Clopidogrel is one of the commonly used P2Y12 inhibitors.However,up to 30%of patients treated with a standard dose of clopidogrel present with high on-treatment platelet reactivity(HOPR),which is associated with the increased ischemic risks[1].The causes of HOPR are multifactorial and complex.
