Partially covered vs uncovered sphincterotome and postendoscopic sphincterotomy bleeding

AIM: To prospectively compare partially covered vs uncovered sphincterotome use on post-endoscopic biliary sphincterotomy (ES) hemorrhage and other complications. METHODS: All patients referred for therapeutic endoscopic retrograde cholangiopancreatography (ERCP) were randomly assigned to undergo ES either with a partially covered or an uncovered sphincterotome. Both patient and technical risk factors contributing to the development of post-ES bleeding were recorded and analyzed. The characteristics of bleeding was recorded during and after ES. Other complications were also compared. RESULTS: Three-hundred and eighty-seven patients were recruited in this study; 194 patients underwent ES with a partially covered sphincterotome and 193 with conventional uncovered sphincterotome. No statistical difference was noted in the baseline characteristics and risk factors for post-ES induced hemorrhage between the 2 groups. No significant difference in the incidence and pattern of visible bleeding rates was found between the 2 groups (immediate bleeding in 24 patients with the partially covered sphincterotome vs 19 patients with the uncovered sphincterotome, P = 0.418). Delayed bleeding was observed in 2 patients with a partially covered sphincterotome and in 1 patient with an uncovered sphincterotome (P = 0.62). No statistical difference was noted in the rate of other complications. CONCLUSION: The partially covered sphincterotome was not associated with a lower frequency of bleeding. Also, there was no difference in the incidence of other significant complications between the 2 types of sphincterotome.

Aggressive treatment of acute anal fissure with 0.5% nifedipine ointment prevents its evolution to chronicity

AIM: To investigate the efficacy of topical application of 0.5% nifedipine ointment in healing acute anal fissue and preventing its progress to chronicity. METHODS: Thirty-one patients (10 males, 21 females) with acute anal fissure from September 1999 to January 2005 were treated topically with 0.5% nifedipine ointment (t.i.d.) for 8 wk. The patients were encouraged to follow a high-fiber diet and assessed at 2, 4 and 8 wk post-treatment. The healing of fissure and any side effects were recorded. The patients were subsequently followed up in the outpatient clinic for one year and contacted by phone every three months thereafter, while they were encouraged to come back if symptoms recurred. RESULTS: Twenty-seven of the 31 patients completed the 8-wk treatment course, of them 23 (85.2%) achieved a complete remission indicated by resolution of symptoms and healing of fissure. Of the remaining four unhealed patients (14.8%), 2 opted to undergo lateral sphincterotomy and the other 2 to continue therapy for four additional weeks, resulting in healing of fissure. All the 25 patients with complete remission had a mean follow-up of 22.9 ± 14 (range 6-52) mo. Recurrence of symptoms occurred in four of these 25 patients (16%) who were successfully treated with an additional 4-wk course of 0.5% nifedipine ointment. Two of the 27 (7.4%) patients who completed the 8-wk treatment presented with moderate headache as a side effect of nifedipine. CONCLUSION: Topical 0.5% nifedipine ointment, used as an agent in chemical sphincterotomy, appears to offera significant healing rate for acute anal fissure and might prevent its evolution to chronicity.

A novel computational methodology for large-scale screening of MOFs is applied to gas storage with the use of machine learning *** approach is a promising trade-off between the accuracy of ab initio methods and the speed of classical approaches,strategically combined with chemical *** results demonstrate that the chemical properties of MOFs are indeed predictable(stochastically,not deterministically)using machine learning methods and automated analysis protocols,with the accuracy of predictions increasing with sample *** initial results indicate that this methodology is promising to apply not only to gas storage in MOFs but in many other material science projects.

Status of Traditional Chinese Medicine in Greece and its Approach on COVID-19 Pandemic

Although acupuncture was first introduced to Greece in about 1938,it was not until 1973 that an institution was created to teach it,where its study was limited to medical doctors(MDs).Traditional Chinese medicine(TCM)was not taught to non-MDs until *** today,TCM has not been integrated into the Greek health system,and in a few exceptions,TCM was mainly associated with pain management.A few private schools teach TCM theory and practice in the fields of acupuncture,moxibustion,herbology,gua sha,and tui na(Chinese massage).This study examines the use of Chinese single herbs and formulas for prevention or treatment during the COVID-19 pandemic in Greece,as well as issues with the availability of Chinese herbs and proposals for substitutes from the traditional Greek medicine point of view.

Surfaces of Revolution in the General Theory of Relativity

We present a class of axially symmetric and stationary spaces foliated by a congruence of surfaces of revolution. The class of solutions considered is that of Carter’s family [A] of spaces and we try to find a solution to Einstein’s equations in the presence of a perfect fluid with heat flux. This approach is an attempt to find an interior solution that could be matched to a corresponding exterior solution across a surface of zero hydrostatic pressure. The presence of a congruence of surfaces of revolution, described as the quotient space of the commoving observers, can be useful to the determination of the surface of zero pressure. Finally we present two formal solutions representing ellipsoids of revolutions.

<i>In Vitro</i>Evaluation of the Staining Effects of Two Intracanal Medicaments over a 3-Month Period

Aim: The aim of this study was to evaluate in vitro the tooth color alterations associated with two intracanal medicaments, calcium hydroxide and calcium hydroxide combined with aquatic solution of chlorexidine, for up to three months post-treatment. Materials & Methods: Thirty-one intact human anterior mature teeth were used. Black adhesive tape with a 4-mm diameter window was used to standardize the enamel surface intended for color analysis. After access cavity preparation, cleaning and shaping were completed with rotary nickel-titanium files. The teeth were randomly divided into two groups (n = 15), each according to the intracanal medicament used: (A) calcium hydroxide paste (UltraCalTM XSTM Ultradent Products, Inc., USA);(B) 2% chlorhexidine aquatic solution combined with pure calcium hydroxide powder. In one tooth, no medicament was placed (pilot). The enamel surfaces were colormetrically evaluated at the following time intervals: before placing the medicaments, immediately after placement, after 1 week as well as after 1, 2 and 3 months post-treatment. The CIE color parameters (L*, a*, b*) were recorded for each material and the corresponding color differences (ΔΕ) were calculated and statistically analyzed. Results: The most significant factor in tooth discoloration was the time intervals. The most important changes of the ΔΕ values were recorded after the 1st week and after the 3rd month. L* presented an overall increase in both groups, resulting in a higher lightness of the crown color. Only L* did show significant alterations during specific time intervals. Conclusions: There is no statistically significant relationship between the type of intracanal medicament and tooth color alteration. Time is statistically the most important factor affecting the discoloration ability of Ca(OH)2, either combined with chlorexidine or not. Consequently, the clinician should always take this into account before using such medicaments for a long-term treatment.

A Comparative <i>In-Vitro</i>Study of Sealing Ability of Four Different Materials Used in Furcation Perforation

The purpose of this study was to compare the sealing ability and marginal adaptation of four re-storative materials (MTA, Biodentine, portlant cement, and resin modified glass ionomer cement) used to repair iatrogenic furcation perforations. Eighty-four molars were treated endodontically, perforated in the middle of the pulp chamber floor with a round bur and separated randomly into 4 groups of 20 teeth each, while 4 teeth were used as positive and negative controls. The teeth were embedded in a moistened flower sponge and the perforations were filled with the appropriate restorative materials: Group 1: Biodentine;Group 2: MTA Angelus;Group 3: GC Fuji lining LC Paste Pak;Group 4: Aquafix Portland cement. The teeth remained in the soaked sponge for 28 days and then were submerged in basic fuchsine solution 1% for 48 hours. Dye penetration was evaluated after longitudinal sectioning of the teeth. Statistical analysis revealed that perforations restored with MTA exhibited the least microleakage with statistically significant difference among the other three groups (p < 0.05). The worst sealing ability was observed in the teeth restored with Aquafix Portland cement. No statistically significant difference was found between the groups of Biodentine and FC Fuji Lining Paste (p = 0.066).

AIM: To investigate whether smoking is associated with human papilloma virus(HPV) infection. METHODS: HPV infection is considered to be a neces-sary condition for cervical cancer development. The study population included 1291 women, aged 25-55 years, attending cervical cancer screening. All women had a Papanicolaou(Pap) test, with liquid-based cytology(Thinprep), an HPV-DNA test and an evaluation of smoking habits. The COBAS 4800 system was used for HPV-DNA testing, enabling identifi cation of the following high-risk HPV(hr HPV)-types: each of HPVs 16 and 18 separately, and HPVs 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, 66 and 68 as a cocktail. The evaluation of smoking habits was assessed using the smoking intensity index(SII), a variable formed as the product of cigarettes consumed per day by the days(years × 365) that a woman was a smoker, divided by 1000. RESULTS: There were 136 smokers among 238 women tested positive for hr HPV-types(HPVs 16, 18, 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, 66 and/or 68), and 463 smokers among 1053 hr HPV-negative women(OR = 1.7, P < 0.001). This association was attributed to the youngest age group of women, aged 25-34 years(OR = 2.3, P < 0.001), while there was no association in other age groups. The intensity of smoking(increasing SII) showed no statistically signifi cant association with hr HPV infection. Cervical infection with HPV 16 and/or HPV 18 was also not associated with age or smoking habits. Finally, no association was found between Pap test status and smoking habits or smoking intensity. CONCLUSION: Smoking appears to be associated with hr HPV infection of the uterine cervix, particularly in younger women. Further studies should investigate whether this association is based on causality and evaluate the role of other possible co-factors.
