Granulosa Cell Tumors of the Ovary: Retrospective Analysis of 17 Cases

Background: Granulosa cell tumors (GCTs) are rare neoplasms with a relatively favorable prognosis. They are characterized by a prolonged history and a tendency to late recurrences. It is the most common type of sex cord-stromal tumors. Aims: To analyze, to report and to better understand the clinico-pathologic features and results of treatment, and prognostic factors of these tumors. Materials and Methods: A retrospective single-institutional review 17 cases of GCTs were treated in National Cancer Institute—Cairo University from January 2010 till December 2014. The clinical and pathological characteristics, treatment, and outcomes of patients with ovarian GCTs were analyzed. Results: Data from 17 patients were obtained. The median age was 54 years (range;14 - 72). Abdominal pain was the most common presentation (64.7%). The mean tumor size was 14 cm (range;7 - 23 cm). The majority of our patients were stage I (n = 11;64.7%), while (n = 3;17.6%) had stage III and (n = 2, 11.8%) were stage IV. Only one case (5.9%) had an unknown stage (explored outside NCI). The majority of cases were of adult type disease (n = 14) and low grade pathology (n = 10). In follow-up period (median = 42 months;ranging 9 - 60) three patients relapsed;the median overall survival time was not reached yet, however, the estimated 3-year survival was 72.5%. Conclusion: Granulosa cell tumors are rare neoplasms of the ovaries. They progress slowly and often are diagnosed in an early stage. Surgery is the main line of treatment. Prolonged post-therapeutic follow-up is necessary. Definition of proper prognostic factors is mandatory.

Continuous Inferior Alveolar Nerve Block Using an Indwelling Catheter and Inferior Maxillary Artery Embolization for the Management of Atypical Trigeminal Neuralgia

A 40-years-old female patient with severe right facial pain with a throbbing component along the mandibular division of the trigeminal nerve resistant to medical management. Continuous inferior alveolar nerve block with local anesthetics using an indwelling catheter provided a complete pain resolution for the patient for 2 weeks, after which the catheter got infected and was removed. A trial of balloon occlusion of the right internal maxillary artery provided complete resolution of the throbbing component of the patient’s pain. This was followed by permanent embolization with multiple coils.

Extracranial Head and Neck, Peripheral Nerve Tumors: A 10-Year Experience at NCI, Egypt

Background: Peripheral nerve tumors (PNT) originate from the neural crest and typically formed from Schwann cell sheath. Considering the number of nerves in this area, they form a very small percentage of neoplastic lesions of the head & neck. A solitary mass in the lateral neck frequently confronts the surgeon. An uncommon cause for such a nodule is a neoplasm arising from nerve tissue with PNTs being the commonest of them to face. Aims: To study the clinico-pathological characteristics, presentations, surgical approaches, postoperative complications & outcome of management. Materials & Methods: a single institution combined retrospective & prospective study. A retrospective analysis of all cases presented NCI-Cairo University with H&N extracranial PNST candidate for surgery from Jan 2009 until the end of Dec 2015 & prospective analysis of all cases with the same presentation presented to the NCI from Jan 2016 until the end of June 2019. 23 cases were included. Data were collected then analyzed. Results: Mean age was 34.7 years with female to male ratio 1.9:1. Mean size was 5.4 cm. Commonest presentation was asymptomatic neck lump located laterally in the neck (9 cases, 39%). CT scanning was done in 17 cases (74%). As regard pathology, we have only 4 malignant cases (17%). Most tumors (9 cases, 39%) originated from cranial nerves. 18 cases (78%) were resected through the neck. Complication occurred in 13 cases (56%), most of them during tumor dissection from its nerve origin or root. Vascular complications occurred in 6 cases (26%) due to bleeding controlled with ECA ligation (facial ischemia, hemiparesis & paresthesia). Cranial nerve injury (XI, XII) occurred in two cases (8.7%) & Speech defects in other two 2 cases (8.7%) due to maxillary resection. Postoperative?radiation was given to all 4 malignant cases (17%) of MPNST with Postoperative Chemotherapy to all cases experienced relapse after adjuvant RT. Management of Thymic Neoplasms: Egyptian NCI Experience

Background: Thymic tumors are rare entity with little information regarding outcomes after therapy with curative intent. They are heterogeneous group of neoplastic lesions with wide spectrum of morphologic appearances. They show different presentations with a variable & unpredictable evolution ranging from indolent attitude to highly infiltrative and metastasizing one. The optimal treatment includes surgical resection, chemotherapy, and radiotherapy. Objectives: To analyze, to report & to better understand the clinico-pathologic features, results of treatment, and prognostic factors of these tumors. Materials and Methods: From 2008 to 2014, 13 patients (8 men, 5 women) underwent surgical resection of thymic tumor at a mean age of 47 years. Patient demographics, extent of surgical resection, and outcomes were compiled. Demographic variables, use of chemotherapy or radiotherapy, peri-operative variables, recurrence rates, and long term survival were analyzed retrospectively. The Masaoka stage and tumor diameter were recorded along with other variables that potentially influenced survival such tumor grade, site & number of metastatic disease. Results: The mean age of our patients was 47 years (range: 21 to 58 years) & 8 cases (61.5%) were men. The distribution of patients according to the Masaoka staging at presentation was, stage I (6 cases, 47%), stage II (3 cases, 23%), stage III in (1 patient, 7%), and stage IV (3 cases, 23%). Neoadjuvant chemotherapy was administered to 3 patients (23%) whose tumors were deemed to be more locally advanced precluding R0 resection from the start & 2 patients (15%) received neoadjuvant concomitant chemo-radiotherapy. No patient demonstrated an associated immunologic disorder such as myasthenia gravis. In all patients (13 cases, 100%) pathologic confirmation of thymic tumor was by CT guided fine needle aspiration/biopsy. Combined Sacral Nerve Roots Stimulation and Low Thoracic Spinal Cord Stimulation for the Treatment of Chronic Pelvic Pain

Some pelvic pain syndromes are very resistant to medical treatment. Several studies have demonstrated that sacral neuromodulation, which has been successfully used for the treatment of bladder dysfunction, incontinence, urinary retention and urinary frequency [1]-[3], can be successfully used for the treatment of chronic pelvic pain [4]-[7]. Several studies have also demonstrated significant involvement of dorsal column pathways in the transmission of visceral pelvic pain [8] and the successful use of spinal cord stimulation for the treatment of chronic pelvic pain [9]. We report three cases of severe chronic pelvic pain that failed conservative treatment modalities. Placement of a combined sacral nerve roots stimulator and a low thoracic spinal cord stimulator resulted in a significant pain relief and improvement in daily life activities. We believe that this combination may help patients suffering from chronic pelvic pain resistant to medical management.

Change in Impedance with Change in Posture during Spinal Cord Stimulator Placement

In this article we discuss the effect of posture on impedance during the placement of a spinal cord stimulator. Although, according to several studies, there appears not to be a specific and consistent change in impedance with change in posture, we noticed on multiple occasions that if the impedance remains very high during spinal cord stimulator (SCS) lead placement, asking the patient to sit up from the prone position may drop the impedance down. This could be explained by having air around the lead, from using loss of resistance to air technique in finding the epidural space, which moves up when sitting the patient up, or having the lead immersed in epidural fat. Sitting the patient up from the prone position should be tried if the impedance remains very high before aborting the procedure and taking the lead out.

Biological treatments as a mean to improve feed utilization in agriculture animals——An overview

As a result of agriculture practices, million tons of agriculture are produced as a secondary or by-products; however, with low nutritive values. Many methods are applied to improve the nutritive value and increase its utilization in ruminant＇s nutrition. The biological treatments are the most common with more safe-treated products. In most cases, the biological treatments are paralleled with decreased crude fiber and fiber fractions content with increased crude protein content. Direct-fed microbial and exogenous enzymes to animal are other ways of biological methods for improving nutritive value of feeds. Here in this review, we will try to cover the biological treatments of by-products from different sides view with different types of animals and different animal end-products.

Variability in the Content of 25α- and 25β-Steroidal Sapogenins among Thirty Morphologically Different Sudanese Seed Accessions of Fenugreek

Seeds of Fenugreek （Trigonellafoenum-graecum L.）, an international spice commodity, are much consumed in Sudan as a food, for flavoring and as a folk-remedy for several ailments, together with local beliefs in nutraceutical benefits. The authors have noted an immense variability in colour and other morphological characters of Fenugreek seeds offered in local Sudanese markets and wondered whether this variability is extended to their chemical composition. Steroidal sapogenins are important chemical constituents of Fenugreek seeds in view of their potential as precursors for the commercial synthesis of steroid drugs and their continually revealed beneficial biological activities. In this study, thirty Sudanese Fenugreek seed accessions collected from different geographical regions in Sudan were analyzed for their 25α-, 25β- and total sapogenin content using a simple and specific infra-red spectroscopic method. The seed accessions exhibited much morphological variability particularly in outer seed coat color and size. Preparative TLC followed by gravimetric analysis showed that steroidal sapogenins, mostly diosgenin and yamogenin, represented more than 70% of the steroids of Fenugreek seeds. Infra-red spectroscopic analysis showed that total 25α-sapogenins （calculated as diosgenin） varied from 0.65% to 1.68%. Total 25β-sapogenin （calculated as yamogenin） varied from 0.38% to 2.03%. The content of total sapogenins （α ＋ β epimeric forms） varied between 1.24% and 3.0% of the oven-dry weight of seeds. β-sapogenins （mostly yamogenin） were dominant over α-sapogenins （diosgenin） in most Sudanese Fenugreek seed accessions.

Cardiac and pericardial tumors:A potential application of positron emission tomography-magnetic resonance imaging

Cardiac and pericardial masses may be neoplastic, benign and malignant, non-neoplastic such as thrombus or simple pericardial cysts, or normal variants cardiac structure can also be a diagnostic challenge. Currently, there are several imaging modalities for diagnosis of cardiac masses; each technique has its inherent advantages and disadvantages. Echocardiography, is typically the initial test utilizes in such cases, Echocardiography is considered the test of choice for evaluation and detection of cardiac mass, it is widely available, portable, with no ionizing radiation and provides comprehensive evaluation of cardiac function and valves, however, echocardiography is not very helpful in many cases such as evaluation of extracardiac extension of mass, poor tissue characterization, and it is non diagnostic in some cases. Cross sectional imaging with cardiac computed tomography provides a three dimensional data set with excellent spatial resolution but utilizes ionizing radiation, intravenous iodinated contrast and relatively limited functional evaluation of the heart. Cardiac magnetic resonance imaging(CMR) has excellent contrast resolution that allows superior soft tissue characterization. CMR offers comprehensive evaluation of morphology, function, tissue characterization. The great benefits of CMR make CMR a highly useful tool in the assessment of cardiac masses.(Fluorine 18) fluorodeoxygluocse(FDG) positron emission tomography(PET) has become a corner stone in several oncological application such as tumor staging, restaging, treatment efficiency, FDG is a very useful imaging modality in evaluation of cardiac masses. A recent advance in the imaging technology has been the development of integrated PET-MRI system that utilizes the advantages of PET and MRI in a single examination.
