Dear Editor,Lake Baikal and its neighboring territories are an intermediate zone for the'steppe'and'arctic-like'rabies virus lineages in Russia.After the elimination of dog-mediated rabies during the early 1980s,this area remained rabies-free for over 25–30 years.A sudden reappearance of rabies occurred in this zone in the Republic of Buryatia in 2011–2012.A marginal part of the Mongolian steppe penetrates the Siberian taiga forests in this area,and human and animal rabies have been repeatedly recorded in the Republic of Buryatia from the end of

The Usefulness of Zhukovsky Double Balloon in Obstetric Hemorrhage

Objective:To evaluate the effectiveness of the use of a modified Zhukovsky double(vaginal and uterine)balloon to improve the results of treatment in women with obstetric hemorrhage.Methods:We conducted an observational controlled study including 701 puerperas,which were divided into two groups:The main group consisted of 508 women,who underwent a combined management,that is,traditional(transfusion of fresh frozen plasma,erythrocyte mass,uterotonics,hemostatic agents),surgical hemostasis in cases of caesarean section,and insertion of a double Zhukovsky balloon;while the comparison group included 193 patients,who were managed traditionally.The main group and the comparison group were divided into subgroups according to the main etiology of obstetric hemorrhage:ⅠA andⅠB-postpartum;ⅡA andⅡB-placenta accreta;ⅢA andⅢB-postpartum hysterectomy.Results:The most frequent causes of massive obstetric hemorrhage were atony of the uterus(39.2%),placenta previa(29.1%),and placental abruption(11.5%).Of the 508 balloon insertions,345(70.7%)were inserted for hemorrhage during cesarean section and 148(29.3%)for obstetric hemorrhage after spontaneous delivery.Among the obstetric hemorrhage at caesarean section,lower segment bleeding prevailed(78.1%),caused mainly by placenta previa,placenta accreta,placental abruption,polyhydramnios,and uterine scar defects.Combined management,including surgical hemostasis and insertion of vaginal and uterine balloon of Zhukovsky,reduced blood loss by 1.5 times and the number of hysterectomies by 6.72 times compared to controls.Conclusion:The use of a modified Zhukovsky double balloon in the management of obstetric hemorrhage may reduce the number of hysterectomies and the amount of blood loss.

Within this survey we describe the conceptual architecture of the infrastructure to measure PM2.5/PM10 concentration in the atmosphere over the Kyiv city using modern monitoring instruments.We define the requirements for information tools and network for informing Kyiv city community on the state of PM pollutions that will be created.This infrastructure will provide long-term PM2.5/PM10 observations that could be included in the AirBase network.The comprehensive review of in-situ and satellite measurements of PM2.5/PM10 is provided as well as the description current state-of-the-art for Air Quality monitoring with intelligent sensors and systems in Ukraine as-awhole and in Kyiv in particular.it is proposed to apply the concept of essential variables(EVs)used in Earth Observation to identify the variables that should be measured in priority when designing,deploying and maintaining observation systems.in this study we use and validate the global air quality products from Copernicus Atmosphere Monitoring Service obtained from modeling by GEOS-Chem model and other sources.The influence of PM and aerosols on a human health is estimated in terms of possible diseases and dangerous concentrations.

Novel MLH1 frameshift mutation in an extended hereditary nonpolyposis colorectal cancer family

AiM: To present novel frameshift mutation c.31delC [p.L11X] in the MLH1 gene identified in an extended Bulgarian hereditary non-polyposis colorectal cancer (HNPCC) family and to analyze the molecular and clinical findings within the pedigree concerning the proposal of adequate individual prophylactic strategy for all mutation carriers. METHODS: The pedigree of the family consists of 42 members in four generations. Search for mutations in the MLH1 and hMSH2 genes was performed in the pro-band. After PCR amplification of all exons including flanking intronic regions, amplicons were directly sequenced. RESULTS: The mutation was found in nine from the thirteen pedigree members who signed informed consent to participate in the study. in three adenocarcinomas, microsatellite instability and lack of the MLH1 protein expression were detected. The only one tubulovillous adenoma analyzed was microsatellite stable and the MLH1 protein showed an intact staining. CONCLUSiON: The newly described mutation c.31delC is HNPCC causative. Besides the typical clinical features of the syndrome, we found a specific pathologic manifestation such as moderate to high differentiated adenocarcinomas of the colon. One of the mutation carriers developed a benign giant cell soft tissue tumor. The primary tumor localizations were frequently extracolonic and detailed yearly gastrointestinal and gynecological examinations have been proposed to the mutation carriers. We emphasize the importance of including the HNPCC genetic counseling and testing as well in the following surveillance of all patients at risk in the services covered by the health insurance in Bulgaria.
