Evaluation of mitral chordae tendineae length using four-dimensional computed

BACKGROUND Mitral valvuloplasty using artificial chordae tendineae represents an effective surgical approach for treating mitral regurgitation.Achieving precise measurements of artificial chordae tendineae length(CL)is an important factor in the procedure;however,no objective index currently exists to facilitate this measurement.Therefore,preoperative assessment of CL is critical for surgical planning and support.Four-dimensional x-ray micro-computed tomography(4D-CT)may be useful for accurate CL measurement considering that it allows for dynamic three-dimensional(3D)evaluation compared to that with transthoracic echocardiography,a conventional inspection method.AIM To investigate the behavior and length of mitral chordae tendineae during systole using 4D-CT.METHODS Eleven adults aged>70 years without mitral valve disease were evaluated.A 64-slice CT scanner was used to capture 20 phases in the cardiac cycle in electrocardiographic synchronization.The length of the primary chordae tendineae was measured from early systole to early diastole using the 3D image.The primary chordae tendineae originating from the anterior papillary muscle and attached to the A1-2 region and those from the posterior papillary muscle and attached to the A2-3 region were designated as cA and cP,respectively.The behavior and maximum lengths[cA(ma),cP(max)]were compared,and the correlation with body surface area(BSA)was evaluated.RESULTS In all cases,the mitral anterior leaflet chordae tendineae could be measured.In most cases,the cA and cP chordae tendineae could be measured visually.The mean cA(max)and cP(max)were 20.2 mm±1.95 mm and 23.5 mm±4.06 mm,respectively.cP(max)was significantly longer.The correlation coefficients(r)with BSA were 0.60 and 0.78 for cA(max)and cP(max),respectively.Both cA and cP exhibited constant variation in CL during systole,with a maximum 1.16-fold increase in cA and a 1.23-fold increase in cP from early to mid-systole.For cP,CL reached a plateau at 15%and remained elongated until end-systole,whereas for cA,aft

Background During the COVID-19 pandemic,decreased volumes of stroke admissions and mechanical thrombectomy were reported.The study’s objective was to examine whether subarachnoid haemorrhage(SAH)hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines.Methods We conducted a cross-sectional,retrospective,observational study across 6 continents,37 countries and 140 comprehensive stroke centres.Patients with the diagnosis of SAH,aneurysmal SAH,ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases,10th Revision,codes.The 3-month cumulative volume,monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before(1 year and immediately before)and during the pandemic,defined as 1 March-31 May 2020.The prior 1-year control period(1 March-31 May 2019)was obtained to account for seasonal variation.Findings There was a significant decline in SAH hospitalisations,with 2044 admissions in the 3 months immediately before and 1585 admissions during the pandemic,representing a relative decline of 22.5%(95%CI−24.3%to−20.7%,p<0.0001).Embolisation of ruptured aneurysms declined with 1170-1035 procedures,respectively,representing an 11.5%(95%CI−13.5%to−9.8%,p=0.002)relative drop.Subgroup analysis was noted for aneurysmal SAH hospitalisation decline from 834 to 626 hospitalisations,a 24.9%relative decline(95%CI−28.0%to−22.1%,p<0.0001).A relative increase in ruptured aneurysm coiling was noted in low coiling volume hospitals of 41.1%(95%CI 32.3%to 50.6%,p=0.008)despite a decrease in SAH admissions in this tertile.Interpretation There was a relative decrease in the volume of SAH hospitalisations,aneurysmal SAH hospitalisations and ruptured aneurysm embolisations during the COVID-19 pandemic.These findings in SAH are consistent with a decrease in other emergencies,such as stroke and myocardial infarction.

The main cause of late-onset hypogonadism(LOH)symptoms is decline of testosterone according to aging.Patients with LOH suffer from various signs including physical-,psychological-,and sexualrelated symptoms.It is obvious that the basic treatment for LOH is testosterone replacement therapy(TRT).However,TRT is not effective for a few patients in clinical practice,and it is so important to find out the fair candidates for TRT in patients with LOH symptoms.

Rare earth application for heat-resisting alloys

High-temperature oxidation resistance of Al2O3-and Cr2O3-forming heat-resisting alloys with rare earths(yttrium-implanted FeCrAl,-added FeCrAl,-added FeCrAlPt alloys,Y2O3-or CeO2-coated NiCrSi,yttrium-or lutetium-added NiCr and NiCrSi) was studied in oxygen at high temperatures,by mass gain measurements,mass change measurements,amount of spalled oxide,observation of surface appearance,X-ray diffraction(XRD),scanning electron microscopy(SEM),electron probe X-ray microanalysis(EPMA) and transmission electron microscopy(TEM).After oxidation at 1573 K for 18 ks in oxygen,oxide scale on FeCrAl alloy spalled from the entire surface,however,yttrium-implanted FeCrAl alloys showed good oxide adherence.After oxidation at 1473 K for 18 ks in oxygen,mass gain of FeCrAlY alloys decreased with increasing yttrium of up to 0.1 wt.% follwed by an increase with the yttrium content,and the mass gain of FeCrAl0.005Pt0.05Y alloy with appropriate additions of platinum and yttrium was lower than that of FeCrAl0.1Y alloy.Yttrium-added FeCrAl alloys showed good oxide adherence.TEM analysis revealed that the alumina/alloy interface of FeCrAl0.005Pt0.05Y alloy showed good coherency.The scale surface of FeCrAl alloy was rough,however,those of FeCrAlY and FeCrAlPtY alloys were smooth.Cyclic oxidation of NiCrSi,Y2O3-or CeO2-coated NiCrSi alloys was studied up to 10 cycles(1 cycle:300 s) at 1523 K in oxygen.Mass change of NiCrSi alloy increased up to 3 cycles and then decreased up to 10 cycles because of oxide spallation during cooling.On the other hand,mass change of Y2O3-or CeO2-coated NiCrSi alloy increased up to 10 cycles,and these alloys showed good oxide adherence.Granular Cr2O3 particles on Y2O3-coated NiCrSi alloy were in size smaller than these on CeO2-coated NiCrSi alloy.This result suggested that oxidation rate of Y2O3-coated NiCrSi alloy was lower than that of CeO2-coated NiCrSi alloy.After oxidation at 1473 and 1573 K for 18 ks in oxygen,mass gain of yttrium-or lutetium-added NiCr and NiCrSi alloys decreased.Oxide scales on NiCrSi

The process of life adjustment in patients at onset of glioma who are receiving continuous oral anticancer drug: A qualitative descriptive study

Objective: Patients with malignant gliomas have to adjust their daily lives because of the threat of impending death and declining abilities.Thus,clarifying the process of life adjustment in such patients would help them lead a normal life.To clarify the process of life adjustment in patients at the onset of glioma,who continuously receive oral anticancer drug.Methods: The study institution consisted of two designated cancer centers.Semi-structured interviews were conducted with 10 patients,and the data were analyzed using the Modified Grounded Theory Approach of Kinoshita (M-GTA).Results: As a core category representing the process of life adjustment in patients at the onset of glioma,"trials and errors for self-fulfillment even in a limited lifespan" was extracted.Patients began "seeking information about the unfamiliar life-threatening disease," and "imaging their uncertain lives after the disease and the resulting disability" while "conducting repeated trials and errors to establish coping methods according to their abilities." When facing difficulties in such trials and errors,they reported "losing self-confidence due to unexpected limitations." However,they regulated their feelings by "resigning themselves to their unchangeable reality." Contrarily,as "functional improvement enhanced their motivation to recover," they attempted to promote functional recovery and organized their daily lives in "fulfilling their desires in their limited lifespan." Conclusion: The process of life adjustment in patients with malignant gliomas involved identifying a way of living despite limited lifespan.To ensure appropriate nursing care for patients at the onset of glioma,it is important to help them establish coping methods in accordance with their abilities.

Software-defined networking(SDN)is a new paradigm that promises to change by breaking vertical integration,decoupling network control logic from the underlying routers and switches,promoting(logical)network control centralization,and introducing network programming.However,the controller is similarly vulnerable to a“single point of failure”,an attacker can execute a distributed denial of service(DDoS)attack that invalidates the controller and compromises the network security in SDN.To address the problem of DDoS traffic detection in SDN,a novel detection approach based on information entropy and deep neural network(DNN)is proposed.This approach contains a DNN-based DDoS traffic detection module and an information-based entropy initial inspection module.The initial inspection module detects the suspicious network traffic by computing the information entropy value of the data packet’s source and destination Internet Protocol(IP)addresses,and then identifies it using the DDoS detection module based on DNN.DDoS assaults were found when suspected irregular traffic was validated.Experiments reveal that the algorithm recognizes DDoS activity at a rate of more than 99%,with a much better accuracy rate.The false alarm rate(FAR)is much lower than that of the information entropy-based detection method.Simultaneously,the proposed framework can shorten the detection time and improve the resource utilization efficiency.

The matrix metalloproteinase stromelysin-3 cleaves laminin receptor at two distinct sites between the transmembrane domain and laminin binding sequence within the extracellular domain

The matrix metalloproteinase (MMP) stromelysin-3 (ST3) has long been implicated to play an important role in extracellular matrix (ECM) remodeling and cell fate determination during normal and pathological processes. However like other MMPs, the molecular basis of ST3 function in vivo remains unclear due to the lack of information on its physiological substrates. Furthermore, ST3 has only weak activities toward all tested ECM proteins. Using thyroid hormone-dependent Xenopus laevis metamorphosis as a model, we demonstrated previously that ST3 is important for apoptosis and tissue morphogenesis during intestinal remodeling. Here, we used yeast two-hybrid screen with mRNAs from metamorphosing tadpoles to identify potential substrate of ST3 during development. We thus isolated the 37 kd laminin receptor precursor (LR). We showed that LR binds to ST3 in vitro and can be cleaved by ST3 at two sites distinct from where other MMPs cleave. Through peptide sequencing, we determined that the two cleavage sites are in the extracellular domain between the transmembrane domain and laminin binding sequence. Furthermore, we demon strated that these cleavage sites are conserved in human LR. These results together with high levels of human LR and ST3 expression in carcinomas suggest that LR is a likely in vivo substrate of ST3 and that its cleavage by ST3 may alter cell-extracellular matrix interaction, thus, playing a role in mediating the effects of ST3 on cell fate and behavior ob- served during development and pathogenesis.

BACKGROUND Biliary atresia(BA)is a rare pediatric disease.AIM To compare the outcomes of laparoscopic portoenterostomy(Lap-PE)with those of laparotomy(Open-PE)at a single institution.METHODS The surgical outcomes of PE were retrospectively analyzed for patients with a non-correctable type of BA from 2003 to 2020.RESULTS Throughout the assessment period,119 patients received PE for BA treatment,including 66 Open-PE and 53 Lap-PE cases.Although the operation duration was longer(medians:for Open-PE,242 min;for Lap-PE,341 min;P80%for both groups for the first half year post surgery,followed by a gradual decrease with time;there were no statistically significant differences in the native liver survival rates for any durations assessed.CONCLUSION Lap-PE could be a standard therapy for BA.
