Anesthetic Consideration on Neonatal Patient with Esophageal Atresia

Esophageal atresia is a disorder of the esophageal continuity with or without a connection with the trachea or the under-development esophagus. Most babies who suffer from esophageal atresia also have tracheoesophageal fistula (an abnormal connection between the esophagus and the trachea/windpipe). The possibility of these two conditions is accompanied by other anomalies such as congenital heart disease and anorectal malformations. Esophageal atresia can also be interpreted as malformations caused by the failure of the esophagus to develop a continual passage that may or may not establish a connection to the trachea (tracheoesophageal fistula) or it can be said that the esophageal atresia is the failure of the esophagus to form a channel from the pharynx to the stomach during embryonic development. Another understanding of esophageal atresia is an interruption in the growth of a segment of the esophagus and remains as thin sections without continual channels. Tracheal esophageal fistula is an abnormal connection between the trachea and esophagus. Miscellaneous types of esophageal atresia include: Type A. The incidence of Type A esophageal atresia was about 5% - 8%. Type A esophageal atresia occurs at each end of the esophageal sac, and is apart without fistula to the trachea. Type B incident is rare. Type B esophageal atresia occurs at each end of the esophagus, and there is fistula from the trachea to the upper segment of esophagus. Incidence of Type C esophageal atresia occurs about 80% - 95%. Type C esophageal atresia occurs at the proximal esophageal segment and ends at a dead-end sac, and distal segments are connected to the trachea or primary bronchus. Type D esophageal atresia is rare. In this type, upper and lower segment of the esophagus is connected to the trachea. Type E esophageal atresia is rare when compared with Types A and C esophageal atresia. In this type the esophagus and trachea are normal but they are connected with fistula.

Monitoring Frequency of Haemoglobin, Calcium and Phosphorus and the Impact on Correction in Patients on Maintenance Haemodialysis in Douala-Cameroon

Background: There is scarcity of data on the actual frequency of routine blood work-up for patients on maintenance haemodialysis and how this affects the achievement of therapeutic goals in sub-Saharan Africa. This study aimed to review these in a referral hospital in Cameroon. Methods: A prospective hospital-based cohort study carried out from November 2019 to April 2020 including patients on maintenance haemodialysis in Douala general hospital. Patients were followed during 6 months to determine the frequency of monitoring of haemoglobin, serum calcium and phosphorus. Targeted values for haemoglobin were ≥10 g/dL, 80 - 100 mg/L for serum calcium and 25 - 45 mg/L for serum phosphorus. Pearson correlation test was used to define the correlation between monitoring frequencies and blood values at the end of the study. Results: For all 154 patients included in the study, the median frequency of monitoring for haemoglobin was once every 8 week (IQR: 6 - 12) and once every 12 weeks (IQR: 8 - 24) for serum calcium and phosphorus. The proportion of patients who achieved haemoglobin, serum calcium and phosphorus targets at the end of the study were 27.4%, 63% and 74% respectively. No correlation was found between the frequency of monitoring and the haemoglobin, serum phosphorus and calcium levels at the end of the study. Conclusion: The frequency of monitoring of serum haemoglobin, calcium and phosphorus by patients in this study was low with a high prevalence of patients not achieving target value.

Effects of Different Levels of Garlic Powder Incorporated in the Feed Enriched with Stylosanthes guianensis on Reproduction and Pre-Weaning Growth Performances of Cavies (Cavia porcellus)

In order to increase the productivity of the local cavies as a meat animal, a study was carried out between November 2021 and March 2022 at the Research and Experimental Farm, Animal Nutrition and Production Research Unit of the Faculty of Agronomy and Agricultural Sciences (FAAS) and the Microbiology and Anti-Microbial Research Unite of the University of Dschang on 72 cavies aged between 4 and 5 months and weighing an average of 450 ± 50 g produced on the farm, including 60 females and 12 males. The females were distributed according to a completely randomized factorial design in twelve (12) rearing boxes numbered G1 to G12, with 15 females per group distributed in 03 boxes and receiving iso-nitrogenated experimental diet (18% protein) R0 enriched with 20% of Stylosanthes guianensis (0% garlic powder), R0.25 (0.25% garlic powder), R0.50 (0.50% garlic powder) and R0.75 (0.75% garlic powder). Females were identified by numbered ear tags and mated with a sex ratio of 5 females for 1 male. Drinking water enriched with vitamin C (01 tablet of 240 mg in 1.5 liters of water) was served ad libitum and renewed daily. The males were removed after 31 days. A form was established to record the weight of each pregnant female, any abortions during gestation, the date of parturition and the date of birth of each newborn, as well as the number of live-born or stillborn pups. After parturition, the weights of female and newborn cavies were recorded weekly until the 3rd week. The number of cavies that died before weaning and the number of live weaned cavies were also recorded. Reproductive performance significantly increased (p 0.05) fertility rate and net fertility rate, litter size, birth viability rate, and pre-weaning viability rate with rations containing garlic powder, compared to the control ration, especially with the R0.50 ration. Digital Control Design for the Boiler Drum BDT921

A boiler drum BDT921 that is installed in the Control Laboratory, Department of Mechatronics and Robotics Engineering, Faculty of Electric and Electronic Engineering, Universiti Tun Hussein Onn Malaysia (UTHM) is being used as a model plant to achieve the digital control system since its analog. Implementing a digital system to boiler quite a though work. This paper covers analysis from the experiment done to match with digital design that will be implemented to the real system. The digital control design will come up with the mathematical model and will be analyzed with MATLAB and SIMULINK software named as "Discrete Analysis of BDT921 Simulation". A proportional integral and derivative (PID) controller is being chosen as the control element in discrete form as the real system is using the same control element. The output responses behave as the second order system with a bit difference in rise times and peak times compared with data obtained from experiment. With regarding to the analysis done, the digital control can be implemented and for further viewing, to be controlled digitally with computer in the control room.
