Survival predictors in patients treated with a molecular adsorbent recirculating system

AIM:To identify prognostic factors for survival in patients with liver failure treated with a molecular adsorbent recirculating system(MARS). METHODS:MARS is a liver-assisting device that has been used in the treatment of liver failure to enable native liver recovery,and as a bridge to liver transplantation(LTX).We analyzed the 1-year outcomes of 188 patients treated with MARS,from 2001 to 2007, in an intensive care unit specializing in liver disease. Demographic,clinical and laboratory parameters were recorded before and after each treatment.One-year survival and the number of LTXs were recorded.Logistic regression analysis was performed to determine factors predicting survival. RESULTS:The study included 113 patients with acute liver failure(ALF),62 with acute-on-chronic liver failure(AOCLF),11 with graft failure(GF),and six with miscellaneous liver failure.LTX was performed for 29% of patients with ALF,18% with AOCLF and 55%with GF.The overall 1-year survival rate was 74% for ALF,27% for AOCLF,and 73% for GF.The poorest survival rate,6%,was noted in non-transplanted patients with alcohol-related AOCLF and cirrhosis,whereas,patients with enlarged and steatotic liver had 55% survival.The etiology of liver failure was the most important predictor of survival(P<0.0001).Other prognostic factors were encephalopathy(P=0.001)in paracetamol-related ALF, coagulation factors(P=0.049)and encephalopathy(P=0.064)in non-paracetamol-related toxic ALF,and alanine aminotransferase(P=0.013)and factor V levels(P =0.022)in ALF of unknown etiology. CONCLUSION:The etiology of liver disease was the most important prognostic factor.MARS treatment appears to be ineffective in AOCLF with end-stage cirrhosis without an LTX option.

AIM:To determine the short-term cost-utility of mo-lecular adsorbent recirculating system(MARS) treatment in acute liver failure(ALF).METHODS:A controlled retrospective study was conducted with 90 ALF patients treated with MARS from 2001 to 2005.Comparisons were made with a historical control group of 17 ALF patients treated from 2000 to 2001 in the same intensive care unit(ICU) specializing in liver diseases.The 3-year outcomes and number of liver transplantations were recorded.All direct liver disease-related medical expenses from 6 mo before to 3 years after ICU treatment were determined for 31 MARS patients and 16 control patients.The health-related quality of life(HRQoL) before MARS treatment was estimated by a panel of ICU doctors and after MARS using a mailed 15D(15-dimensional generic healthrelated quality of life instrument) questionnaire.The HRQoL,cost,and survival data were combined and the incremental cost/quality-adjusted life years(QALYs) was calculated.RESULTS:In surviving ALF patients,the health-related quality of life after treatmeant was generally high and comparable to the age-and gender-matched general Finnish population.Compared to the controls,the average cost per QALY was considerably lower in the MARS group(64 732€ vs 133 858€) within a timeframe of 3.5 years.The incremental cost of standard medical treatment alone compared to MARS was 10 928€,and the incremental number of QALYs gained by MARS was 0.66.CONCLUSION:MARS treatment combined with standard medical treatment for ALF in an ICU setting is more cost-effective than standard medical treatment alone.

Do we need to screen every patient in intensive care unit for diabetes in community with high prevalence of diabetes?

Diabetes mellitus (DM) is marked as global health care challenge with almost 10% of the United States population being diagnosed with DM. A sizeable percentage of patients are oblivious of their disease, in spite of easily accessibility knowledge about its early signs and symptoms and rapid diagnostic modalities. Critically ill patients with undiagnosed DM are likely to have an increased mortality as compared to intensive care unit (ICU) patients with diagnosed DM. DM may have adverse effect on ICU patients causing organ failure and complications. Early Screening of patients at the risk of developing disease may prevent long term complications. Early screening and management may be beneficial as controlled DM patients have similar morbidity as non DM patients in ICU. An intense glycaemic and blood pressure control improves retinopathy and albuminuria, but may not affect the macrovascular outcomes.

Rural households' livelihoods diversification through termite utilization in depressed region of Zimbabwe

This study sought to examine the utility of termites to rural households in depressed regions of Bikita,Zimbabwe.Colonialism and its spread of European culture had viewed entomophagy with contempt resulting in reduced utilization and consumption of termites in most colonies.In our quest to understand how people in depressed regions utilized termites,both quantitative and qualitative methodologies were employed.Questionnaires and in-depth interviews were used to gather data during field work.Purposive and snowball sampling techniques were used in recruiting respondents.Excel and content analysis were used in data presentation and analysis.Results revealed that dampwood and drywood termites are climatic indicators for the rural communal farmers,a low-cost technology in weather forecasting.Subterrain and mold builders provide manure,relish,and are medicinal in nature.They are traded for income generation,and as for the poor,harvesting,processing,storage,and marketing is cost effective.Though considered nostalgic foods,most respondents highlighted that selling termites complimented other off-farm livelihood activities.Termite consumption and utilization provide a sustainable way for livelihood diversification in depressed regions and has partly addressed problems of food insecurity.

Effect of IOP based infusion system with and without balanced phaco tip on cumulative dissipated energy and estimated fluid usage in comparison to gravity fed infusion in torsional phacoemulsification

Background:To evaluate the effect of three different combinations of tip designs and infusion systems in torsional phacoemulsification(INFINITI and CENTURION)in patients with cataract.According to the manufacturer,two unique improvements in the Centurion are:active fluid dynamic management system and use of an intrepid balanced tip.The study specifically aimed to evaluate the beneficial effects,if any,of change in tip design and infusion system individually and in combination on both per-operative parameters as well as endothelial health over 6 months.Methods:One hundred and twenty six consenting patients of grade 4.0-6.9 senile cataract were randomized into three groups for phacoemulsification:Group A(n=42):Gravity fed infusion system and 45° Kelman miniflared ABS phaco tip;Group B(n=42):intraocular pressure(IOP)based infusion system and 45° Kelman miniflared ABS phaco tip;Group C(n=42):IOP based infusion system and 45° Intrepid balanced phaco tip.The cumulative dissipated energy(CDE),estimated fluid usage(EFU)and total aspiration time(TAT)were compared peroperatively.The endothelial parameters were followed up postoperatively for six months.Results:The three arms were matched for age(p=0.525),gender(p=0.96)and grade of cataract(p=0.177).Group C was associated with significant reductions in CDE(p=0.001),EFU(p<0.0005)as well as TAT(p=0.001)in comparison to the other groups.All three groups had comparable baseline endothelial cell density(p=0.876)and central corneal thickness(p=0.561).On post-operative evaluation,although all groups were comparable till 3 months,by 6 months,the percentage losses in endothelial cell density were significantly lower in group C as compared to the other groups.Conclusions:Use of an IOP based phacoemulsification system in association with use of the Intrepid balanced tip reduces the CDE,EFU and TAT in comparison to a gravity fed system with a mini flared tip or IOP based system with a mini flared tip while also providing better endothelial preservation thus favouring the use of an IOP

A Comparative Assessment of Disability Levels among Nigerian Outpatients with Schizophrenia and Type 2 Diabetes Mellitus

Background: Chronic illnesses are often associated with functional disability, thus compromising the ability to carry out everyday activities of daily living. The degree of disability depends on the severity and the type of illness experienced. Studies that compare the level of disability between people with chronic medical conditions and mental illnesses in North-Central Nigeria are scarce. This study aims to compare the disability levels between people with schizophrenia and Type 2 Diabetes Mellitus (T2DM) attending outpatient clinics at the Jos University Teaching Hospital, north-central Nigeria, and evaluate the factors associated with these conditions. Methods: It was a cross-sectional study with a total of 600 patients who were diagnosed with schizophrenia and T2DM, attending the Psychiatric and medical outpatient clinics of the Jos University Teaching Hospital, north-central Nigeria, between June 2017 and November 2017. The study assessed Psychotic and non-psychotic symptoms by applying the Brief Psychiatric Rating Scale (BPRS) among those with schizophrenia. We evaluated the level of disability by using World Health Organization Disability Assessment Schedule version 2.0 (WHODAS 2.00). Results: Disability was significantly higher among respondents with schizophrenia than those with T2DM. This difference occurred across all the domains except domain 2 (moving around). Marital status, living situation, occupation, and treatment adherence were significant common factors associated with disability in these conditions. In contrast, age, educational status, income level, and duration of illness were significantly associated with disability among respondents with T2DM only. Conclusion: Disability and its associated factors among people with chronic diseases, if identified early and proper interventions instituted, disability can be avoided or minimized among people with chronic illnesses.

Medication Non-Adherence and Disability among Outpatients with Schizophrenia in North-Central Nigeria

Background: About fifty percent of patients with Schizophrenia do not take their medication as prescribed. This scenario often results in disease progression and increased relapse rates, resulting in poor outcomes, including functional disability. Aim: This study was design to evaluate the relationship between medication non-adherence and disability in patients with Schizophrenia attending the outpatient clinic at the Jos University Teaching Hospital, north-central Nigeria, from June 2017 to November 2017. Methods: We carried out a cross-sectional study that employed a systematic sampling method to select 301 eligible subjects. We used the Medication Adherence Rating Scale (MARS) 10 and World Health Organization Disability Assessment Schedule version 2.0 (WHODAS 2.00) to assess medication adherence and levels of disability. Results: The results show that 39.9% of the respondents were medication nonadherent. Medication adherence correlated negatively with the total disability score and all the domains of disability. A low level of education and poor social support were significant predictors of non-adherence. Conclusion: Medication non-adherence and its associated factors, if identified early and proper interventions instituted, a disability could be avoided or minimized among people with Schizophrenia. Therefore, we recommend regular screening of patients with mental disorders for early identification and intervention for medication adherence.

Apple pomace is a rich source of dietary fiber that can be easily incorporated in bakery products to enhance the nutritional quality of the product.In the present study,apple pomace powder was utilized for formulating fiber-enriched muffin pre-mix.Apple pomace powder(APP)was supplemented in refined wheat flour at different concentration and 33%was found highly acceptable by the sensory panelists on a 9 point hedonic scale.Samples were analyzed for proximate composition,functional properties(water holding,swelling,foaming and emulsion),phenolic content and calorific value.Storage study of polyethylene and aluminium laminate pouch(ALP)packed fiber-enriched muffin pre-mix was also conducted for six months at ambient conditions.Cost evaluation showed that the cost of APP-based pre-mix was comparable to conventional wheat-based pre-mix.Results of this study support the possibility of producing high-fiber muffin pre-mix with desirable quality and quantity in a sustainable manner.
