Treatm.nt Outcom. of Pharm.cokinetics-Based Dosing of Docetaxel and Fluorouracil in Advanced Head and Neck Cancer Patients

Introduction: Docetaxel, Cisplatin and 5-Fluorouracil (DPF) becam. the standard induction chem.therapy in advanced Head and Neck Cancer (HNC) but associated with high toxicity rate. Several studies reported higher response rates with better tolerability when chem.therapy dose is calculated based on Pharm.cokinetics (PK) versus conventional Body Surface Area (BSA). Patients and m.thods: Thirty nine patients with stage III and IV HNC who received induction DPF were included in the study. Dose of cycle 1 was BSA-based then Docetaxel and 5-FU doses were PK-adjusted starting from.cycle 2 whereas Cisplatin dose was BSA-based throughout the study. Results: After m.dian follow up period of 14 m.nths the m.dian overall survival (OS) and progression free survival (PFS) were 15.1 and 10.6 m.nths respectively. Twenty nine patients were available for response assessm.nt. Seven patients (24.1%) achieved com.lete response while partial response encountered in 19 patients (65.5%) with and Overall response rate of 89.6%. Both treatm.nt related side effects and m.rtality significantly decreased after the application of PK dose adjustm.nts (p-value 0.007 and 0.01 respectively). Conclusion: PK-guided dose adjustm.nts of 5-FU and Docetaxel in DPF regim.n can significantly decrease the treatm.nt related side effects and m.rtality without com.rom.sing the tum.r response rate. A random.zed clinical trial is needed to com.are the PK-guided dose adjustm.nt with the standard BSA based protocol.

Experim.ntal investigation of viscous oil-water-sand flow in horizontal pipes:Flow patterns and pressure gradient

Fluid production from.unconsolidated reservoirs often leads in sand production,which poses a num.er of *** deposition in flowlines can result in significant pressure dips,pipe and facility dam.ge,and obstructions that decrease *** research is needed to understand the m.vem.nt and deposition of sand in oil-water-sand（O-W-S） *** article focuses on O-W-S flows in a 6-m.ter-long horizontal pipe with an inner diam.ter of 0.0381 *** study looks at the flow behavior of high viscosity oil-water（O-W）,water-sand（W-S）,and oil-water-sand（O-W-S） *** were carried out at 250 psig pressure in a laboratory flow test facility using various heavy synthetic oils（viscosities ranging from.3500 cP to 7500 cP at 25℃） and tap *** sand concentration varied from.1% to 10%,with an average sand particle diam.ter of 145 μm.and m.terial density of 2630 kg/*** cuts ranged from.0.0 to *** experim.ntal results revealed a m.nor change in pressure gradient between（O-W） and（O-W-S） ***,increasing the sand concentration in（O-W-S） flow resulted in higher pressure *** reduction factor of pressure gradient indicated that the highest decrease in pressure drop occurred at higher superficial oil ***,a direct relationship was observed between the reduction factor and the decrease in water *** results also showed that the m.nim.m.required transportation velocity for sand slurry decreased with increasing superficial oil velocity,while the m.nim.m.transportation condition increased with higher sand *** com.arison between the expected pressure gradient from.Bannwart and m.Kibben et *** the actual experim.ntal data dem.nstrated significant accuracy for the oil viscosities and superficial oil velocities used in the study.

Sm.rt prediction of liquefaction-induced lateral spreading

The prediction of liquefaction-induced lateral spreading/displacem.nt(Dh)is a challenging task for civil/geotechnical *** this study,a new approach is proposed to predict Dh using gene expression program.ing(GEP).Based on statistical reasoning,individual m.dels were developed for two topographies:free-face and gently sloping *** with a com.arison with conventional approaches for predicting the Dh,four additional regression-based soft com.uting m.dels,*** process regression(GPR),relevance vector m.chine(RVm.,sequential m.nim.l optim.zation regression(Sm.R),and m.-tree,were developed and com.ared with the GEP *** results indicate that the GEP m.dels predict Dh with less bias,as evidenced by the root m.an square error(Rm.E)and m.an absolute error(m.E)for training(i.e.1.092 and 0.815;and 0.643 and 0.526)and for testing(i.e.0.89 and 0.705;and 0.773 and 0.573)in free-face and gently sloping ground topographies,*** overall perform.nce for the free-face topology was ranked as follows:GEP>RVm.m.-tree>GPR>Sm.R,with a total score of 40,32,24,15,and 10,*** the gently sloping condition,the perform.nce was ranked as follows:GEP>RVm.GPR>m.-tree>Sm.R with a total score of 40,32,21,19,and 8,***,the results of the sensitivity analysis showed that for both free-face and gently sloping ground,the liquefiable layer thickness(T_(15))was the m.jor param.ter with percentage deterioration(%D)value of 99.15 and 90.72,respectively.

For the first tim., a highly crystalline porous shish-kebab structure with a high degree of crystallinity was obtained by using a com.ination of two m.thods for the form.tion of porous polym.ric m.terials. A treatm.nt procedure using supercritical carbon dioxide(sc CO_(2))was carried out for oriented ultrahigh m.lecular weight polyethylene(UHm.PE) film., which provided special conditions for the crystallization of dissolved UHm.PE m.crom.lecules on the surface of oriented UHm.PE crystals. The prepared porous m.terials were investigated by scanning electron m.croscopy(SEm. and differential scanning calorim.try(DSC). The particularity of the obtained porous shish-kebab is the absence of the am.rphous phase between lam.llar crystals(kebabs). The obtained pores had an oval shape, and they were oriented in the orientation direction of the UHm.PE m.crom.lecules. The pore size ranged from.0.05 μm.to 4 μm. Controlling the conditions for the crystallization of the UHm.PE m.crom.lecules using supercritical CO_(2) gives the possibility to control the size of both lam.llar disks and pores form.d.

Leptin influences estrogen m.tabolism.and increases DNA adduct form.tion in breast cancer cells

Objective: The elevated incidence of obesity has been paralleled with higher risks of breast cancer. High adiposity increases leptin secretion from.adipose tissue, which in turn increases cancer cell proliferation. The interplay between leptin and estrogen is one of the m.chanism. through which leptin influences breast carcinogenesis. An unbalanced estrogen m.tabolism.increases the form.tions of catechol estrogen quinones, DNA adducts, and cancer m.tations. This study aim. to investigate the effect of leptin on som. estrogen m.tabolic enzym.s and DNA adduction in breast cancer ***: High perform.nce liquid chrom.tography(HPLC) was perform.d to analyze the DNA adducts 4-OHE1[E2]-1-N3 adenine and 4-OHE1[E2]-1-N7 guanine. Reporter gene assay, real tim. reverse transcription polym.rase chain reaction(real tim. RT-PCR), and Western blot were used to assess the expression of estrogen m.tabolizing genes and enzym.s: Cytochrom. P-4501B1(CYP1B1), Nicotinam.de adenine dinucleotide phosphate-quinone oxidoreductase1(NQO1), and Catechol-O-m.thyl transferase(COm.).Results: Leptin significantly increased the DNA adducts 4-OHE1[E2]-1-N3 adenine and 4-OHE1[E2]-1-N7 ***, leptin significantly upregulated CYP1B1 prom.ter activity and protein expression. The luciferase prom.ter activities of NQO1 and m.RNA levels were significantly reduced. m.reover, leptin greatly reduced the reporter activities of the COm.-P1 and COm.-P2 prom.ters and dim.nished the protein expression of ***: Leptin increases DNA adduct levels in breast cancer cells partly by affecting key genes and enzym.s involved in estrogen m.tabolism. Thus, increased focus should be directed toward leptin and its effects on the estrogen m.tabolic pathway as an effective approach against breast cancer.

Long-term.changes in sea surface tem.erature(SST)within the southern Levantine Basin

Knowledge of sea surface tem.erature(SST)behaviour is vital for long-term.clim.te *** study highlights essential outcom.s about the distinguishable and unsurprising warm.ng of the SST along the southern border of the Levantine *** analysis is based on m.nthly SST data for the period *** southern Levantine Basin has undergone SST increase,during the last 71 *** this study,a consistent warm.ng trend has been found for the analysed SST data series,with a rate of 0.04℃/a,i.e.,0.4℃/(10 a).From.1975 to 1991 the m.an annual SST was 17.1℃,and this increased to be 19.2℃,over the period *** revealed two opposite trends of variability:a decreasing trend(−0.06℃/a)over the period 1975-1991,and an increasing trend(0.2℃/a)from.2002 to *** the period 1948-2018,positive m.an annual SST anom.lies had an average of 1.8℃,and negative anom.lies had an average of−1.1℃.The lowest SST total increase was found from.January to April,with values about 0.03℃,while the highest warm.ng appeared from.June to *** driving m.chanism. behind the SST changes need to be m.re investigated,to understand the future trends and im.acts of clim.te change in the Levantine Basin.

Depending on double com.ound synchronization and com.ound com.ination synchronization, a new kind of syn- chronization is introduced which is the double com.ound com.ination synchronization （DCCS） of eight n-dim.nsional chaotic system.. This kind m.y be considered as a generalization of m.ny types of synchronization. In the com.unica- tion, based on m.ny of drive and response system., the transm.tted and received signals will be m.re secure. Using the Lyapunov stability theory and nonlinear feedback control, analytical form.las of control functions are obtained to insure our results. The corresponding analytical expression and num.rical treatm.nt are used to show the validity and feasibility of our proposed synchronization schem.. The eight m.m.istor-based Chua oscillators are considered as an exam.le. Other exam.les can be sim.larly investigated. The proposed synchronization technique is supported using the m.TLAB sim.la- tion outcom.s. We obtain the sam. results of num.rical treatm.nt of our synchronization using sim.lation observations of our exam.le.

A Retrospective Study of Clinicoepidem.ological Aspects of Nasopharyngeal Cancer at NCI-Cairo (2000-2010)

Aim.of Work: The aim.of this work was to study the clinico-epidem.ological characteristics of the patients with nasopharyngeal carcinom. (NPC), analyse the prognostic factors of the disease and to analyse the results of different treatm.nt m.dalities and their effect on loco-regional, distal m.tastatic disease control and both overall survival (OS) and disease free survival (DFS) rates. Patients and m.thods: This is a retrospective study reviewing all adult nasopharyngeal carcinom. (NPC) patients who presented to the radiotherapy departm.nt—National Cancer Institute Cairo University in the period from.(2000-2010). Results: In this study, it was found that the m.an age was 45 years;m.st of the patients were of locally advanced stages. m.ltivariate cox proportional hazards regression identified T-stage, radiotherapy course com.letion & addition of chem.therapy as independent prognostic factors for local control (LC), DFS, & OS. The 5-year LC, DFS and OS rates for all studied patients were 38.2%, 33.5% & 37.2% respectively. The m.dian DFS was 26 m.nths and m.dian OS was 36.5 m.nths. Conclusion: This study m.tches the published data that support that radical concurrent chem.radiation is the m.instay of treatm.nt of locally advanced NPC, & that T-stage, m.stage, prescribed dose com.letion, response to initial treatm.nt are independent prognostic factors for survival. All m.asures should be taken to im.rove the local response during prim.ry treatm.nt as this will im.rove survival rates of patients with NPC.

Nanoscale electropolishing of high-purity nickel with an ionic liquid

High purity(>99.9% com.osition) nickel m.tal specim.ns were used in electropolishing treatm.nts with an acid-free ionic liquid electrolyte prepared from.quaternary am.onium.salts as a green polishing solution. Voltam.etry and chronoam.erom.try tests were conducted to determ.ne the optim.m.conditions for electrochem.cal polishing. Atom.c force m.croscopy(AFm. revealed nanoscale effectiveness of each polishing treatm.nt. Atom.c force m.croscopy provided an overall observation of the m.terial interface between the treated and unpolished regions. Surface m.rphology com.arisons sum.arized electrochem.cal polishing efficiency by providing root-m.an-square roughness averages before and after electrochem.cal polishing to reveal a m.rror finish six tim.s sm.other than the sam. nickel m.tal surface prior to electropolishing. This transition m.nifested in a m.rked change in root-m.an-squared roughness from.112.58 nm.to 18.64 nm.and producing a sm.oth m.rror finish. Finally, the m.chanism.of the ionic liquid during electropolishing revealed decom.osition of choline in the form.of a transient choline radical by acceptance of an electron from.the nickel-working electrode to decom.ose to trim.thylam.ne and ethanol.
