Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

Background: Preterm prelabor rupture of membranes (PPROM) is a major cause of Pretem Birth (PTB), Pretem Birth (PTB) is the most significant cause of perinatal morbidity and mortality worldwide. Cervical length (CL), posterior uterocervical angle (PUCA) and anterior uterocervical angle (AUCA) have been postulated in several studies to have an important role in prediction of PTB. Up to our knowledge, this is the first study that combines the three cervical parameters in prediction of latency period in women with PPROM. Distribution of Different Organotin and Organolead Compounds in Sediment of Suez Gulf

Organotin and organolead compounds were determined in sediments of the Suez Gulf The concentrations of Tributyltin (TBT) ranged from 0.27 to 2.77 with an average value of 1.37 μgg-1;dry wt. However, the concentrations of dibutyltin (DBT) ranged from 0.07 to 2.27 with an average value of 0.58 μgg-1;dry wt. A significant correlation was found between TBT and DBT with r = 0.82, (p = 0.05) indicating that the occurrence of DBT is mainly related to the degradation of TBT. Generally, the high concentration of TBT was attributed to shipping activity in harbours. In addition, Diphenyltin (DPhT) concentrations ranged from not detected to 2.09 with an average of 1.10 μgg-1 dry wt. Antifouling agents, industrial discharge and the influence of sewage discharge are the main sources of pollution by DPhT compounds in Suez Gulf. On the other side, organolead (OLC) concentrations ranged from 10.88 - 440.2 with an average of 168.7 ngg-1;dry wt. A significant setting of OLC recorded in sediments of Suez Gulf was mainly attributed to cars exhaust and/or spelling and direct evaporation of fuels.

Performance Assessment of Some Flax (<i>Linum usitatissimum</i>L.) Varieties Using Cluster Analysis under Sandy Soil Conditions

To assess the performance and the variability of 49 flax varieties based on agronomic parameters using cluster analysis, a field experiment was conducted in 2011/2012 and 2012/2013 winter seasons at the farm of the National Research Center at Nubria district, Albehira Governorate, Egypt. The results revealed high significant difference among all flax varieties in all the studied characters. Letwania-9 and Evelen cultivars surpassed all other varieties in seed and oil yields/fed. On the other hand, D variety gave the lowest value of seed yield/fed and F variety gave the lowest values of oil% and oil yield/fed. While, G Variety surpassed all other varieties in straw yield/fed. Blanka variety recorded the lowest values of straw yield/fed and biological yield/fed, while, Posna variety gave the lowest values of technical stem length (cm). In cluster analysis, all the studied characters were used to construct a distance matrix using the Euclidian coefficient, and generate dendrogram showing dissimilarity among all the varieties. Distance coefficient was ranged from 1.2 between Piltstar and Litwania-1 varieties and 10.8 between Posna and G varieties, which reveal the genetic diversity among varieties. The varieties can be divided into 4 groups based on cluster analysis.

Removing Iron Ions Contaminants from Groundwater Using Modified Nano-Hydroxyapatite by Nano Manganese Oxide

In this article, we study modified nano-hydroxyapatite (HAp) by nano manganese oxide (Mn3O4) as adsorbent material to remove iron ions from groundwater. Different parameters were studied to option optimum conditions of removing such as contact time, pH, initial concentration, a dosage of adsorbent, agitation speed and temperature. Kinetics studies included first order (R2 = 0.915), pseudo-first order (R2 = 0.936), second order (R2 = 0.948), pseudo-second order (R2 = 0.995), elovich equation model (R2 = 0.977), intraparticle diffusion (R2 = 0.946), Natarajan and Khalaf (R2 = 0.915) were carried out, the obtained results revealed that the pseudo-second order is the best to describe the adsorption process because the correlation coefficient is approaching one (R2 = 0.995). Adsorption isotherm was calculated by using Freundlich, Langmuir and Temkin constants, adsorption capacity from Langmuir model was 0.606 mg/g. Thermodynamic parameters (ΔG, ΔH = ?51 KJ/mol, and ΔS = ?142 (KJ/mol)) for the adsorption process were also calculated and discussed.

The structure of 2-carboxamidomethylene-4-thiazolidinone 1 has two active methylene groups: one is endocyclic and close the carbonyl group, where the other is branched at position 2 and close to endocyclic amino group. Thus, the reactivity of compound 1 towards arylidenemalononitrile and ethyl α-cyanocinnamates as electrophiles was studied. Consequently, a new route to the synthesis of pyrano[2,3-d]thiazoles 4a-d and 7a-d has been developed through heterocyclization of carboxamidomethylene -4-thiazolidinone 1 with some α-cyanocinnmonitriles derivatives.

Effect of Oxygen Therapy by Venturi Mask versus Non Invasive Ventillation on the Outcome of Patients Who Devolope Hypoxia after Open Heart Surgery

Background: Cardiothoracic surgery is a common cause of acute respiratory failure and is associated with increased morbidity and mortality. We aimed to compare the outcomes of open heart surgery patients treated using oxygen delivery devices with patients who receive NIV as a first-line therapy for hypoxemic respiratory failure. Materials and Methods: 40 patients who developed acute hypoxemic respiratory failure after open heart surgery and admitted to cardiothoracic ICU 20 patients received NIV and 20 patient received oxygen by venture mask. For all patients the following measurements were performed before and after CPAP AND Venture use: CBC, blood urea, serum creatinine body temperature, chest X-ray, Arterial blood gases (arterial pH, sodium bicarbonate, pcO2, SpO2 and PaO2-to-FiO2 ratio). Results: Mean PO2 and SO2 have increased after using of both venture and Cpap, increase in both PCO2 and HCO3 levels after using Venturi mask, CPAP mask was superior to venturi mask in avoiding the need of intubation, decreasing The ICU stay median length and also median length of hospitalization, all were lower in CPAP group than venture group. Also the mortality rate was lower in CPAP group than the venturi group. Conclusion: Using CPAP mask in severe AHRF following open heart surgery can avoid intubation, decreases the levels of tachypnea and arterial hypoxemia, decreases ICU stay, the length of hospitalization and also decreases the mortality rate compared with patients receiving high-concentration oxygen therapy with venture mask.

Prevalence of Cerebral Atherosclerosis among Patients with Metabolic Syndrome: A Case Control Study on Egyptian Subjects

Background: The metabolic syndrome (MetS) is a clustering of vascular risk factors that tend to increase the risk of occurrence of diabetes mellitus type 2 (DMT2), cardiovascular and cerebro-vascular ischemic events. The aim of the present study is to investigate the association of extra-cranial and intracranial arterial atherosclerosis as well as silent brain infarction (SBI) with MetS, thereby determining the potential cerebrovascular atherosclerotic risk of MetS in the Egyptian population. Methods: A case control study was conducted on 50 Egyptian subjects with MetS and 30 without with age range from 40 - 60 years old. All participants were free from cerebrovascular ischemic events [stroke or transient ischemic attack (TIA)]. All participants underwent complete neurological examination, assessment of the diagnostic criteria for MetS, carotid and transcranial duplex ultrasonography (U/S) and brain MRI. Results: Preclinical carotid atherosclerosis (athero-sclerotic plaques with ely, there was a highly significant association between MetS and SBI even after adjustment for age and other confounders (P value = 0.001). Conclusion: Metabolic syndrome is an important factor associated with mild to moderate atherosclerosis (<50% stenosis) and silent brain infarcts among asymptomatic individuals. Interventions to reduce MetS are important for prevention of subclinical and clinical cerebral atherosclerotic disease.

PAHs in Sediments along the Semi-Closed Areas of Alexandria, Egypt

Sediment samples were collected from 49 sampling stations along the semi-closed areas of Alexandria coasts, Egypt. Total concentrations of 15 out of 16 EPA-PAHs in sediments were varied from 4.2 to 886 ng.g–1 with an average value of 176 ng.g–1 (dry wt). The average total organic carbon (TOC) percent was varied from 0.04 to 7.65%. Higher concentration of total pyrolytic hydrocarbons (∑COMB) than total fossil hydrocarbons (∑COMB), declared that atmospheric fall-out is the significant source of PAHs to marine sediments of the semi-closed area of Alexandria. The selected marked compounds and special PAHs compound ratios (phenanthrene/anthracene;fluoranthene/pyrene;∑COMB/ ∑EPA-PAHs) suggest the pyrogenic origins, especially traffic exhausts, are the dominant sources of PAHs in most locations. Interferences of rather petrogenic and pyrolytic PAH contaminations were noticed in the harbours, especially marine area due to petroleum products deliveries and fuel combustion emissions from the ships staying alongside the quays.

Predictors of long term survival after hepatic resection for hilar cholangiocarcinoma:A retrospective study of 5-year survivors

AIM:To determine predictors of long term survival after resection of hilar cholangiocarcinoma(HC) by comparing patients surviving > 5 years with those who survived 5 ***:There were 34(14%) long term survivors(5 year survivors) among the 243 *** survivors were younger at diagnosis than those surviving less than 5 years(mean age,50.47 ± 4.45 vs 54.59 ± 4.98,p = 0.001).Gender,clinical presentation,preoperative drainage,preoperative serum bilirubin,albumin and serum glutamic-pyruvic transaminase were similar between the two *** level of CA 19-9 was significantly higher in patients surviving 5 year,includingyoung age(p = 0.001),serum CA19-9(p = 0.0001),non-cirrhotic liver(p = 0.02),major hepatic resection(p = 0.001),caudate lobe resection(p = 0.006),well differentiated tumour(p = 0.03),lymph node status(0.008),R0 resection margin(p = 0.0001) and early postoperative liver cell failure(p = 0.02).CONCLUSION:Liver status,resection of caudate lobe,lymph node status,R0 resection and CA19-9 were demonstrated to be independent risk factors for long term survival.
