Serum Bilirubin as a Predictor of Coronary Artery Disease Severity in Patients Undergoing Primary Percutaneous Coronary Intervention

Introduction: Coronary artery disease (CAD) is one of the most common causes of death worldwide. In 2010, about 7 out of total 53 million deaths were due to ischemic heart disease. The aim of this study is to evaluate the relationship of serum bilirubin level with the severity and complexity of coronary artery disease (CAD) in the patients undergoing primary percutaneous coronary intervention (PCI). Materials and Methods: 70 patients with STEMI who were undergoing primary PCI were included in the study. All the patients included in the study were subjected to full routine investigations and standard coronary angiographic projections. Total bilirubin level was measured and the patients were divided into two groups. Group 1 was with serum TB (and Group 2 was with serum TB (>1 mg/dl). Severity and complexity of coronary artery lesions will be assessed using Gensini score. Results: After PCI, the two studied groups were compared regarding the number of vessels affected by one and more than one vessel disease. Single vessel disease was frequent in Group 2 (71%) with significant p value (0.003). Cardiac enzymes (troponin I) was more in Group 1 (S. Bil value (0.02). Also (ALT, AST) were more in Group 1 (p value = 0.01). By comparing the 2 groups, there was a significant difference regarding (EF) between both which was less in Group 1 (*** 1), p value significance (0, 0001). Also GENSENI was more in Group 1 (*** 1) with mean (80.35 vs 34.71) and significant pvalue (0.0001). There was a highly significant negative correlation between serum bilirubin & GENSENI score (r = -0.762, p value 0.0001). Regarding the incidence of complications, incidence was more in Group 1 (*** 1), which means a significant difference between both groups with significant p value (0.0001). There was a significant negative correlation between serum bilirubin & incidence of complications (R = -0.38, pvalue 0.001). Diagnostic value of dobutamine stress Doppler tissue imaging in diabetic patients with suspected coronary artery disease

Background: Coronary artery disease (CAD) is often silent in diabetic patients, and it is typically in ad-vanced stages of development by the time it manifests. Various forms of stress testing have been investigated to detect obstructive CAD in diabetes mellitus. Ob-jectives: To assess the diagnostic value of dobutamine stress pulsed-wave Doppler tissue imaging (DTI) compared with standard wall motion analysis in de-tection of myocardial ischemia in diabetic patients with suspected CAD. Methods: The study comprised 46 diabetic patients with suspected CAD underwent dobutamine stress echocardiography (DSE) with DTI within 4 weeks before coronary angiography(CA). Dobutamine infusion started at 5 μ/kg/min and in-creased up to 40 μ/kg/min with additional atropine during submaximal heart rate responses. In addition to wall-motion score index (WMSI) analysis, pulsed- wave DTI examination of basal and mid segments of posteroseptal, lateral, anterior, inferior and antero- septal walls was performed. Myocardial velocities were measured at rest in the apical 4, 3 and 2-cham- ber views. The measurements were repeated at low dose (10 - 15 μ/kg/min) and at peak stress (40 μ/kg/min). DTI measurements included peak systolic velocity (S), peak early diastolic velocity (E) and peak late diastolic velocity (A) and the results were com-pared to WMSI analysis. Patients were classified into two groups according to CA results;group (I) di-abetics with positive CA (n = 27) and group (II) di-abetics with negative CA (n = 19). Results: There was no significant difference between the two groups in duration of diabetes, global WMSI at rest or the ? changes (stress-rest/rest) of WMSI (P > 0.05). Global S and global E were significantly lower in group I compared to group II at peak stress (11.3 ± 3.7 cm/sec vs. 14.5 ± 2.2 cm/sec, p < 0.01) and (11.3 ± 1.6 cm/sec vs. 13.1 ± 2.1 cm/sec, p < 0.01) respectively. Cutaneous leishmaniasis in Al Hassa, Saudi Arabia: Epidemiological trends from 2000 to 2010

Objective: To study epidemiological trends related to cutaneous leishmaniasis (CL) in Al Hassa, an endemic area in Saudi Arabia. Methods: This retrospective study included the spatial/ temporal analysis of the reported cases of CL using the available surveillance database for the disease at the regional Vector Control Unit, from 2000 to 2010. Results: The incidence of CL was declining at a stable rate especially during the last 3 years of the study (2008-2010). An interesting finding was the percentage of expatriates affected was increasing over the last 10 years compared to that of the Saudis. Conclusions: A definite declining trend in the incidence of CL was observed in Al Hassa. Further studies are warranted to assess whether special public health measures are needed for better control of CL in expatriate populations in Saudi Arabia.

Chemical composition and antibacterial activity of essential oil of Launaea lanifera Pau grown in Algerian arid steppes

Objective: To evaluate the essential oil composition and the antibacterial activity of an Algerian endemic plant, Launaea lanifera Pau(L. lanifera), grown in arid steppe ***: L. lanifera essential oil was isolated from aerial parts by steam distillation and its chemical composition was evaluated by gas chromatographyflame ionization detector and gas chromatography with electron impact mass spectrometry. Furthermore, its in vitro antibacterial activity against four bacterial strains was tested following the agar disk diffusion ***: This species had a very low essential oil yield(0.005%). Twenty-four(92.6%)individual components were identified. The main constituents were hexahydrofarnesyl acetone(31.6%),(E)-b-ionone(8.5%),(E)-b-damascenone(7.0%), 2-methyltetradecane(3.8%), n-heptadecane(3.8%), limonene(2.8%) and b-caryophyllene(2.8%). No noteworthy antimicrobial activity was observed on the tested bacteria, neither Gram negative nor Gram ***: This is the first report on the volatile constituents and antibacterial activity of L. lanifera. The studied essential oil does not possess significant activity against the tested microorganisms.

Maize response to elevated plant density combined with lowered N-fertilizer rate is genotype-dependent

Increasing plant density and improving N fertilizer rate along with the use of high density-tolerant genotypes would lead to maximizing maize(Zea mays L.) grain productivity per unit land area. The objective of this investigation was to match the functions of optimum plant density and adequate nitrogen fertilizer application to produce the highest possible yields per unit area with the greatest maize genotype efficiency. Six maize inbred lines differing in tolerance to low N and high density(D) [three tolerant(T); L-17, L-18, L-53,and three sensitive(S); L-29, L-54, L-55] were chosen for diallel crosses. Parents and crosses were evaluated in the 2012 and 2013 seasons under three plant densities: low(47,600),medium(71,400), and high(95,200) plants ha-1and three N fertilization rates: low(no N addition), medium(285 kg N ha-1) and high(570 kg N ha-1). The T × T crosses were superior to the S × S and T × S crosses under the low N–high D environment in most studied traits across seasons. The relationships between the nine environments and grain yield per hectare(GYPH) showed near-linear regression functions for inbreds L54, L29, and L55 and hybrids L18 × L53 and L18 × L55 with the highest GYPH at a density of47,600 plants ha-1and N rate of 570 kg N ha-1and a curvilinear relationship for inbreds L17, L18, and L53 and the rest of the hybrids with the highest GYPH at a density of95,200 plants ha-1combined with an N rate of 570 kg N ha-1. Cross L17 × L54 gave the highest grain yield in this study under both high N–high-D(19.9 t ha-1) and medium N–high-D environments(17.6 t ha-1).

Sustainable Guideline for Developing the E-Waste Sector in Egypt

The amount of electronic waste generated globally is alarming especially that it is on the rise. The impact of the generated amounts is not only Environmental. The electronic waste sector is unregulated in most developing countries;this results in the informal sector being heavily involved in managing and handling it. Due to the lack of regulations, informal handlers are exposed to hazardous materials that affect both their health and health of the communities living in their proximity. Moreover, the informal sector involves illegal child and women labor under harsh conditions. In addition to the social impact, the lack of regulations leads to missing major economic opportunities associated with developing a recycling industry for the safe handling and material recovery from the generated e-waste volumes. Electronic waste may contain up to 60 different materials including precious and rare-earth metals with estimated equivalent economic value of 48 billion Euros. Egypt is one the countries which lack policies and regulations dedicated to e-waste, hence the Egyptian economy is missing the economic potential and job creation opportunities associated with developing the e-waste sector as well as facing major Environmental and health challenges due to the improper handling of the generated amounts. This paper briefly reviews the current situation of e-waste in Egypt. The paper then proposes an adapted guideline for the sustainable management of e-waste in Egypt. The guideline is based on the international regulations and guidelines especially those of countries similar to the Egyptian socioeconomic context. The produced guideline focuses on establishing a collection and recycling framework for e-waste in Egypt with EPR at its center. The guideline also establishes a financing system to help foster the recycling industry in Egypt while fulfilling the three main pillars of sustainability.

FPGA-Based Network Traffic Security: Design and Implementation Using C5.0 Decision Tree Classifier

In this work, a hardware intrusion detection system （IDS） model and its implementation are introduced to perform online real-time traffic monitoring and analysis. The introduced system gathers some advantages of many IDSs： hardware based from implementation point of view, network based from system type point of view, and anomaly detection from detection approach point of view. In addition, it can detect most of network attacks, such as denial of services （DOS）, leakage, etc. from detection behavior point of view and can detect both internal and external intruders from intruder type point of view. Gathering these features in one IDS system gives lots of strengths and advantages of the work. The system is implemented by using field programmable gate array （FPGA）, giving a more advantages to the system. A C5.0 decision tree classifier is used as inference engine to the system and gives a high detection ratio of 99.93%.

Optimal Solution of Multi-Choice Mathematical Programming Problem Using a New Technique

The study deals with the multi-choice mathematical programming problem, where the right hand side of the constraints is multi-choice in nature. However, the problem of multi-choice linear programming cannot be solved directly by standard linear or nonlinear programming techniques. The aim of this paper is to transform such problems to a standard mathematical linear programming problem. For each constraint, exactly one parameter value is selected out of a multiple number of parameter values. This process of selection can be established in different ways. In this paper, we present a new simple technique enabling us to handle such problem as a mixed integer linear programming problem and consequently solve them by using standard linear programming software. Our main aim depends on inserting a specific number of binary variables and using them to construct a linear combination which gives just one parameter among the multiple choice values for each choice of the values of the binary variables. A numerical example is presented to illustrate our analysis.

A Framework for Introducing Precision Agriculture Technologies in Egypt

Precision Agriculture(PA)has been used in many countries and serving the agricultural *** use of PA solutions intervened with many agricultural businesses and supported decision making using data *** Agriculture depends on weather,soil,plants,and water information that are essential for *** Agriculture depends on the use of several technologies such as image sensors,vision machines,drones,robots,machine learning,and artificial *** use of Precision Agriculture Technologies(PAT)depends on integration between devices,sensors,and systems to ensure the proper implementation of *** paper is generated from research on the applicability of PA in in Egypt that ended with a proposed framework for proper implementation of *** conducted research depended on a survey,focus group discussions,and an online questionnaire that reached 271 respondents from 19 Egyptian *** framework has been developed to enhance the role of an initiative leader to promote PAT through collaboration with other stakeholders in the agricultural *** proposed framework can be used by governmental,non-governmental entities,universities and private sector institutions and could be used at countries facing issues with land fragmentation,limited access to information,limited access to agricultural extension services,and increase in agricultural input’s prices.
