Serum ferritin in neonatal cholestasis: A specific and active molecule or a non-specific bystander marker?

Background: Serum ferritin(SF) and consequently hepatic iron have long been considered important in liver fibrosis progression. They have been studied in different liver diseases with no previous reports in neonatal cholestasis(NC). This study aimed to measure SF in different etiologies of NC and investigate its relation to hepatic iron and fibrosis. Methods: SF was measured in 75 infants, including 50 with NC and 25 with sepsis. SF was compared between these two groups. Biochemical parameters, hepatic iron grades, and liver fibrosis and other histopathological characteristics and correlated with SF were assessed in NC group. Finally, a comparison between intrahepatic cholestasis and obstructive etiology was performed. Results: SF was elevated in NC(1598 ± 2405 ng/mL) with no significant difference from those with sepsis( P = 0.445). NC and sepsis constituted augmenting factors leading to more elevation of SF(2589 ± 3511 ng/mL). SF was significantly correlated with hepatic iron grades( r = 0.536, P ell transformation( P = 0.009) and hepatocyte swelling( P = 0.023) than those with obstructive etiology(672 ± 566 ng/mL) and their prevalent pathological findings of ductular proliferation( P = 0.003) and bile plugs( P = 0.002). SF was unrelated to the grade of liver fibrosis( P = 0.058). Conclusions: SF is non-specifically elevated in NC, with positive correlation to hepatic iron grades. SF ≥ 803.5 ng/mL can predict higher grades( ≥ grade 3) of hepatic iron. However, an active role of increased SF and hepatic iron in disease progression remains questionable.

Facies Analysis and Biostratigraphy of the Miocene Sequence,Cairo-Suez District,Egypt

Miocene siliciclastic-carbonate deposits are widely exposed in Cairo-Suez District, Egypt. These deposits are underlain and overlain by continental sediments of Oligocene and post Miocene, respectively. Three stratigraphic sections were investigated at Gabals Geneife, Homeira and Gharra. Lithostrtigraphically, the Miocene sequence could be differentiated into two main rock units representing shallow deposits with relatively intermittent deep marine incursions. These are from base to top, Gharra Formation and Genefe Formation. Detailed macrofossils investigations led to the recognition of four macrofossil zones, namely Alectryonella plicatula-Crassostrea frondosa Range Zone, Echinolampas amplus-Scutella ammonis Range Zone, Chlamys（Macrochlamys） sardoa-Chlamys（Argopecten） submalvinae Range Zone, and Chlamys gentoni-Pecten（Oppenheimopecten） benedictus-Pecten（P.） ziziniae Assemblage Zone. Microfacies analysis and identified taxa indicated that the Miocene sequence was deposited in transgressive-regressive cycles ranged from near shore, warm shallow inner to middle shelf marine environments with storm influence during the accumulation of the oyster banks.

Effect of single subconjunctival injection of bevacizumab on primary pterygium:clinical,histopathological and immunohistochemical study

Transient elastography compared to liver biopsy and morphometry for predicting fibrosis in pediatric chronic liver disease: Does etiology matter?

AIM: To evaluate transient elastography (TE) as a noninvasive tool in staging liver fibrosis compared with liver biopsy and morphometry in children with different chronic liver ***: A total of 90 children [50 with chronic hepatitis C virus (HCV), 20 with autoimmune hepatitis (AIH) and 20 with Wilson disease] were included in the study and underwent liver stiffness measurement (LSM) using TE. Liver biopsies were evaluated for fibrosis, qualitatively, by Ishak score and quantitatively by fibrosis area fraction (FAF) using digital image analysis (morphometry). LSM was correlated with fibrosis and other studied variables using spearman correlation. A stepwise multiple regression analysis was also performed to examine independent factors associated with LSM. Different cut-off values of LSM were calculated for predicting individual fibrosis stages using receiver-operating characteristic curve. Cut-off values with optimal clinical performance (optimal sensitivity and specificity simultaneously) were ***: The majority of HCV group had minimal activity (80%) and no/mild fibrosis (72%). On the other hand, the majority of AIH group had mild to moderate activity (70%) and moderate to severe fibrosis (95%) and all Wilson disease group had mild to moderate activity (100%) and moderate to severe fibrosis (100%). LSM correlated significantly with both FAF and Ishak scores and the correlation appeared better with the latter (r = 0.839 vs 0.879, P < 0.0001 for both). LSM discriminated individual stages of fibrosis with high performance. Sensitivity ranged from 81.4% to 100% and specificity ranged from 75.0% to 97.2%. When we compared LSM values for the same stage of fibrosis, they varied according to the different etiologies. Higher values were in AIH (16.15 ± 7.23 kPa) compared to Wilson disease (8.30 ± 0.84 kPa) and HCV groups (7.43 ± 1.73 kPa). Histopathological changes in the oesophageal mucosa in Egyptian children with corrosive strictures: A single-centre vast experience

BACKGROUND The caustic ingestion continues to be a major problem worldwide especially in developing countries. The long-term complications include stricture and increased life time risk of oesophageal carcinoma. Patients suffered from corrosive induced oesophageal strictures have more than a 1000-fold risk of developing carcinoma of the *** To determine the possibility of oesophageal mucosal dysplasia after prolonged dilatation in post corrosive *** This observational study was conducted at the Paediatric Endoscopy Unit in Cairo University Children's Hospital. It included children of both sexes older than 2 years of age who had an established diagnosis of post-corrosive oesophageal stricture and repeated endoscopic dilatation sessions for more than 6 mo. All patients were biopsied at the stricture site after 6 mo of endoscopic dilatation. A histopathological examination of an oesophageal mucosal biopsy was performed for the detection of chronic oesophagitis, inflammatory cellular infiltration and *** The mean age of the enrolled children was 5.9 ± 2.6 years; 90% of the patients had ingested an alkaline corrosive substance(potash). The total number of endoscopic dilatation sessions were ranging from 16 to 100 with mean number of sessionswas 37.2 ± 14.9. Unified Fracture Design(UFD)bridges the gap between practices and theory in the hydraulic fracturing *** represents a technique to design hydraulic fracturing treatment with a particular amount of *** design could provide the maximum fold-of-increase(FOI)in productivity-index(PI)after hydraulic fracturing *** UFD optimization tool is very effective,but it has assumptions like any other *** assumption of UFD optimization technique;is a single-layer *** assumption does not align with the limited entry fracturing design *** limited entry fracturing,the frictional pressure is employed to offset the stress differences between multi-layers reservoirs to attain fluid injection through these layers,intended to deliver an optimal fracture conductivity in all *** drawback of this assumption is the underestimation of the actual value of FOI in *** paper aims to recast the original unified fracture design approach to extend the optimal UFD to a multilayer reservoir to predict the FOI in PI after limited entry fracturing *** recasting tool for this problem to find the optimum solution is Artificial Neural Networks(ANN).The architected ANN model is based on actual historical data of limited entry fracturing treatments.A statistical comparison between the proposed ANN model and classical UFD technique demonstrates that ANN model solution has a more reliable estimation of FOI in PI with the actual historical data.

Objectives:Pressure ulcer(PU)is one of the most common problem among the bedridden elderly and has significantly more burden on elderly and *** study is aimed to evaluate the effects of the training program for caregivers to prevent PUs among elderly residents at geriatric ***:A quasi-experimental design was used to carry out the current study.A purposive sample comprised of all formal(39 nurses)or informal caregivers(39)and all immobilized elderly residents(35)who are found in all geriatric centers in Helwan district.A structured questionnaire was used to assess caregivers’socio-demographic characteristics,knowledge,attitude,and observational checklists for their practice for prevention of PU at pre-and posttest and during ***:After the training program,there were improvements in the level of knowledge,practice,and positive attitude of caregivers about PU prevention with a statistically significant difference between pre-,post-,and follow-up training programs(Pelderly caregivers’attitudes as well as practice for prevention could be important in reducing the burden of PU among the ***,conducting a training program for caregivers at different geriatric homes about caring skills for elders can prevent PU,and using spontaneous reposition is very effective to prevent PUs.

Determination of a novel ACE inhibitor in the presence of alkaline and oxidative degradation products using smart spectrophotometric and chemometric methods

Simple, accurate, sensitive and validated UV spectrophotometric and chemometric methods were developed for the determination of imidapril hydrochloride （IMD） in the presence of both its alkaline （AKN） and oxidative （OXI） degradation products and in its pharmaceutical formulation. Method A is the fourth derivative spectra （D4） which allows the determination of IMD in the presence of both AKN and OXD, in pure form and in tablets by measuring the peak amplitude at 243.0 nm. Methods B, C and D, manipulating ratio spectra, were also developed. Method B is the double divisor-ratio difference spectrophotometric one （DD-RD） by computing the difference between the amplitudes of IMD ratio spectra at 232 and 256.3 nm. Method C is the double divisor-first derivative of ratio spectra method （DD-DR1） at 243.2 nm, while method D is the mean centering of ratio spectra （MCR） at 288.0 nm. Methods A, B, C and D could successfully determine IMD in a concentration range of 4.0-32.0 mg/mL. Methods E and F are principal component regression （PCR） and partial least-squares （PLS）, respectively, for the simultaneous determination of IMD in the presence of both AKN and OXI, in pure form and in its tablets. The developed methods have the advantage of simultaneous determination of the cited components without any pre-treatment. The accuracy, precision and linearity ranges of the developed methods were determined. The results obtained were statistically compared with those of a reported HPLC method, and there was no significant difference between the proposed methods and the reported method regarding both accuracy and precision.

This study examines the large amount of agricultural waste produced in Egypt between 2010 and 2019 by analysing data from various departments within the Agriculture *** also provides a comprehensive database on the biomass available from agricultural waste in Egypt and its potential applications for producing power,heat and chemical *** biomass-pyrolysis systems powered by solar energy are used,research demonstrates the potential to convert agricultural waste into a variety of chemical *** approach utilizes solar energy,a clean and renewable source,and has wide-ranging industrial and power generation *** Egypt’s reliance on agriculture,the country currently utilizes little biomass for energy production and has not previously used it as a source for creating chemical products,which could potentially save on the imported oil used in these *** findings of the study are graphically presented using histograms,pie charts,*** overall production of residues reached the 30-Mt level in *** it contains the most rice-cultivated land,Dakahlia(Lower Egypt region)produces the highest percentage of the total residue(37.17%).Qena governorate(Upper Egypt region)produces 1.14 metric tons of residues,with sugarcane accounting for 20.3%of the total.
