Surgical management of primary hyperparathyroidism guided by double-phase Tc-99m-MIBI scintigraphy

Objective:The aim of this work was to study the clinicopathological features of cases with primary hyperparathyroidism (PHPT) referred to National Cancer Institute (NCI), Cairo University in the last six years and to study the role of nuclear medicine techniques in the initial diagnosis and preoperative localization directing surgical management of these cases. Methods: This study included 27 patients with PHPT properly diagnosed and treated in the NCI, Cairo University from January 2005 to December 2010. Preoperative neck U/S and Tc-99m-MIBI scintigraphy were done in all cases. If preoperative localization detected a single lesion, unilateral exploration was done. Bilateral exploration was done if multiple foci of active parathyroid glands or no lesions were detected. Results: This study included 27 patients (8 males and 19 females). The median age was 43 years (range from 19 to 68 years). All cases presented with bone disease in the form of bony pain in 23 patients, bony swellings in 15 patients and pathological fractures in 6 cases. Twenty one patients (77.8%) had single adenoma, 5 cases (18.5%) had parathyroid hyperplasia, and only one case had double adenomas. The sensitivity of neck U/S in detecting single adenoma was 61.9% (13/21), with 81% positive predictive value (PPV) while the sensitivity of Tc-99m-MIBI scintigraphy was 90.5% (19/21) with 100% PPV. Based on preoperative localization tests, unilateral exploration was done in 19 patients with solitary adenomas. All cases in this group were cured with no reported case of persistent or recurrent hypercalcemia. Conclusion: Presentation of PHPT may mimic malignant bone tumors but fortunately these patients were correctly diagnosed with bone scan which confirm the presence of metabolic bone disease rather than bone metastases. Smart prediction of liquefaction-induced lateral spreading

The prediction of liquefaction-induced lateral spreading/displacement(Dh)is a challenging task for civil/geotechnical *** this study,a new approach is proposed to predict Dh using gene expression programming(GEP).Based on statistical reasoning,individual models were developed for two topographies:free-face and gently sloping *** with a comparison with conventional approaches for predicting the Dh,four additional regression-based soft computing models,*** process regression(GPR),relevance vector machine(RVM),sequential minimal optimization regression(SMOR),and M5-tree,were developed and compared with the GEP *** results indicate that the GEP models predict Dh with less bias,as evidenced by the root mean square error(RMSE)and mean absolute error(MAE)for training(i.e.1.092 and 0.815;and 0.643 and 0.526)and for testing(i.e.0.89 and 0.705;and 0.773 and 0.573)in free-face and gently sloping ground topographies,*** overall performance for the free-face topology was ranked as follows:GEP>RVM>M5-tree>GPR>SMOR,with a total score of 40,32,24,15,and 10,*** the gently sloping condition,the performance was ranked as follows:GEP>RVM>GPR>M5-tree>SMOR with a total score of 40,32,21,19,and 8,***,the results of the sensitivity analysis showed that for both free-face and gently sloping ground,the liquefiable layer thickness(T_(15))was the major parameter with percentage deterioration(%D)value of 99.15 and 90.72,respectively.

Nowcasting and forecasting solar irradiance are vital for the optimal prediction of grid-connected solar photovoltaic (PV) power plants. These plants face operational challenges and scheduling dispatch difficulties due to the fluctuating nature of their power output. As the generation capacity within the electric grid increases, accurately predicting this output becomes increasingly essential, especially given the random and non-linear characteristics of solar irradiance under variable weather conditions. This study presents a novel prediction method for solar irradiance, which is directly in correlation with PV power output, targeting both short-term and medium-term forecast horizons. Our proposed hybrid framework employs a fast trainable statistical learning technique based on the truncated-regularized kernel ridge regression model. The proposed method excels in forecasting solar irradiance, especially during highly intermittent weather periods. A key strength of our model is the incorporation of multiple historical weather parameters as inputs to generate accurate predictions of future solar irradiance values in its scalable framework. We evaluated the performance of our model using data sets from both cloudy and sunny days in Seattle and Medford, USA and compared it against three forecasting models: persistence, modified 24-hour persistence and least squares. Based on three widely accepted statistical performance metrics (root mean squared error, mean absolute error and coefficient of determination), our hybrid model demonstrated superior predictive accuracy in varying weather conditions and forecast horizons. A novel prediction method for solar irradiance, targeting both short-term and medium-term forecast horizons, is proposed to improve PV power production. Tendoscopic Surgery for Peroneal Tendons Instability: A Literature Review

Introduction: There has been a surge in the use of tendoscopic surgery for treating peroneal tendons instability. The novelty of this approach demanded a literature review of its indications, limitations, and clinical outcomes. Aim: a literature review of the clinical studies reporting on tendoscopic peroneal tendon stabilisation surgery along with its outcomes and complications. Methods: We carried out a comprehensive review of the literature up until September 2022 with an extensive search of the MEDLINE, Embase and Cochrane library databases. Results: Initial search resulted in 66 articles. Four duplicate articles were removed. Further 30 articles were excluded after title and abstract screening. Eight studies satisfied the inclusion criteria and were included in this review. Articles were analysed for outcomes and complications. Conclusion: The tendoscopic technique for peroneal tendon instability is an effective and safe surgical technique with very low failure rate. Levels of Evidence: Level IV.

Comparison between seismic analysis of twisting and regular 52-story towers considering soil-structure interaction

A dynamic analysis of both twisting and regular towers is carried out to determine the results of considering soil-structure interaction(SSI)on high-rise *** addition,the difference between the seismic performance of using twisting towers over regular ones is *** twisting tower is a simulation of the Evolution Tower(Moscow).The towers’skeletons consist of RC elements and rest on a reinforced concrete piled-raft *** soil model is considered as multi-layered with the same soil properties as the zone chosen for the analysis(New Mansoura City,Egypt).The only difference between both towers is their shape in *** whole system is modelled and analyzed in a single step as one full 3D model,which is known as the direct approach in *** analyses are carried out using finite-element software(Midas GTS NX).Dynamic output responses due to three records of seismic loads are proposed and presented in some *** on the results,it is concluded that SSI has a considerable effect on the dynamic response of tall buildings mainly because of the foundation flexibility,as it leads to lengthening the vibration period,increasing the story drift and the base shear for both cases.

In this paper,we investigate the existence of solutions and analyze the large-time behavior for Gurtin-Maccamy population model involving conformable fractional *** a preliminary step,we construct a generic structure of the solution associated with our proposed model by utilizing some basic properties and tools of conformable fractional *** establish the existence of a unique solution of the given model with the given initial *** last,by using the upper and lower solutions for the characteristic equation,we define the upper and lower boundaries for the obtained solution and describe the large-time behavior of the total population.

Nonlinear wave dispersion in monoatomic chains with lumped and distributed masses:discrete and continuum models

The main objective of this paper is to investigate the influence of inertia of nonlinear springs on the dispersion behavior of discrete monoatomic chains with lumped and distributed *** developed model can represent the wave propagation problem in a non-homogeneous material consisting of heavy inclusions embedded in a *** inclusions are idealized by lumped masses,and the matrix between adjacent inclusions is modeled by a nonlinear spring with distributed ***,the model is capable of depicting the wave propagation in bi-material bars,wherein the first material is represented by a rigid particle and the second one is represented by a nonlinear spring with distributed *** discrete model of the nonlinear monoatomic chain with lumped and distributed masses is first considered,and a closed-form expression of the dispersion relation is obtained by the second-order Lindstedt-Poincare method(LPM).Next,a continuum model for the nonlinear monoatomic chain is derived directly from its discrete lattice model by a suitable continualization *** subsequent use of the second-order method of multiple scales(MMS)facilitates the derivation of the corresponding nonlinear dispersion relation in a closed *** novelties of the present study consist of(i)considering the inertia of nonlinear springs on the dispersion behavior of the discrete mass-spring chains;(ii)developing the second-order LPM for the wave propagation in the discrete chains;and(iii)deriving a continuum model for the nonlinear monoatomic chains with lumped and distributed ***,a parametric study is conducted to examine the effects of the design parameters and the distributed spring mass on the nonlinear dispersion relations and phase velocities obtained from both the discrete and continuum *** parameters include the ratio of the spring mass to the lumped mass,the nonlinear stiffness coefficient of the spring,and the wave amplitude.

Modeling environmental sensitivity and risk assessment of PAHs in sediments along two marine coastal areas in Egypt

Modeling the environmental sensitivity index(ESI)is vital in the planning and management processes,especially along the coastal *** main objectives of the study were to define the main sources of polycyclic aromatic hydrocarbons(PAHs),establish an ESI model,and measure the risk of PAHs on humans and the environment using various *** methodology of the research involved analyzing PAHs according to the EPA 550 methodology in sediments of the coastline in two important coastline areas in Egypt,namely the Gulf of Suez and the Mediterranean Sea,modeling the sensitivity of PAHs using ArcGIS 10.5 by integrating different physical,biological,and land use/cover factors,identifying the sources,and performing a risk *** sediment samples were collected from each *** produced sensitivity map could explain the highly sensitive areas along the Mediterranean Sea and Gulf of Suez coastlines(especially areas that were near drains'outlets,industrial sites,and sea ports).The highest mean PAH concentrations were Acenaphthylene and Fluorine in the Mediterranean Sea and Suez Gulf,***'s obvious that the individual PAH concentrations were within the ERL,ERM,and Pel threshold limits,indicating no adverse biological *** ratios of BaA/BaA+Chy,Flu/Flu+Py,and Ant/Ant+Phe gave an indication that most PAH sources were petrogenic(63.64%)and others(36.36%)were from petroleum *** principle component analysis(PCA)indicated a diverse correlation among the isomer ratios and LU/LC *** carcinogenic risk values were in the very low category(elines and seawater free of oil spillage act
