Long-term results of Ahmed glaucoma valve implantation in Egyptian population

AIM： To evaluate the long-term results and complications of Ahmed glaucoma valve （AGV） implantation in a cohort of Egyptian patients. METHODS： A retrospective study of 124 eyes of 99 patients with refractory glaucoma who underwent AGV implantation and had a minimum follow-up of 5y was performed. All patients underwent complete ophthalmic examination and intraocular pressure （lOP） measurement before surgery and at ld, weekly for the 1st month, 3, 6mo, and ly after surgery and yearly afterward for 5y. lOP was measured by Goldmann applanation tonometry and/or Tono-Pen. Complications and the number of anti-glaucoma medications needed were recorded. Success was defined as lOP less than 21 mm Hg with or without anti-glaucoma medication and without additional glaucoma surgery. RESULTS： Mean age was 23.1±19.9y. All eyes had at least one prior glaucoma surgery, lOP was reduced from a mean of 37.2±6.8 to 19.2±5.2 mm Hg after 5y follow-up with a reduced number of medications from 2.64±0.59 to 1.81±0.4. Complete and qualified success rates were 31.5% and 46.0% respectively at the end of follow-up. The most common complications were encapsulated cyst formation in 51 eyes （41.1%）, complicated cataract in 9 eyes （7.25%）, recessed tube in 8 eyes （6.45%）, tube exposure in 6 eyes （4.8%） and corneal touch in 6 eyes （4.8%）. Other complications included extruded AGV, endophthalmitis and persistent hypotony. Each of them was recorded in only 2 eyes （1.6%）. ~ CONCLUSION： Although refractory glaucoma is a difficult problem to manage, AGV is effective and relatively safe procedure in treating refractory glaucoma in Egyptian patients with long-term follow-up. Encapsulated cyst formation was the most common complication, which limits successful lOP control after AGV implantation. However, effective complications management can improve the rate of success.

Objective:To study the occurrence and prevalence of dermatomycosis in special patient populations suffering from diabetes,cancer and heart ***:A total of 640 patients suffering from diabetes,280 patients suffering from cancer and 210 patients suffering from heart diseases from Cairo City were evaluated for dermatophytosis at the dermatology clinics of four different hospitals from January 2005 to December ***:The presence of diabetes looks a risk factor for tinea pedis and tinea *** cruris and tinea unguium were not common among diabetics,while tinea capitis and tinea versicolor were completely *** capitis followed by tinea pedis are the most common among cancer *** cases were recorded for tinea among 210 patients with heart diseases of which tinea capitis and tinea versicolor were recorded once while the other clinical types of tinea were estimated in 6-8 patients for each ***:The present investigation shows that diabetes remains to be a risk factor for dermatophytosis and cancer comes next due to the use of radioactive irradiation.

BACKGROUNDThere has been a worldwide increase in the reported incidence of inflammatorybowel disease (IBD) in children over the past 2-3 decades. The hepatobiliary (HB)manifestations of IBD have been well-studied in children in industrialized anddeveloped countries but are infrequently reported in low- and middle-incomecountries (LMIC) such as *** determine the prevalence of the HB manifestations in a cohort of Egyptianchildren with *** cross-sectional observational study was carried out over a period of 6 mo(between June 2013 to December 2013, at the Paediatric Hepatology andGastroenterology Units of Cairo University Children's Hospital, which is thelargest paediatric tertiary care centre in the *** study included 48 patients with confirmed IBD based upon clinical,laboratory, endoscopic and histopathological features, 29 (60.4%) were ***-four patients (50%) had ulcerative colitis (UC), 11 (22.9%) had Crohn'sdisease (CD) and 13 (27.1%) had unclassified-IBD (IBD-U), which was formerlyknown as indeterminate colitis. The mean age of the patients at the time ofpresentation was 8.14 (± SD 4.02) years and the mean age at the time of studyenrolment was 10.16 (± SD 4.19) years. All patients were screened for HBmanifestations by physical examination, liver function tests, imaging and liver biopsy when indicated. HB disorders were confirmed in 13 patients (27.1%).Transaminases were elevated in 3 patients (6.3%). Two patients (4.2%) hadelevated biliary enzymes (one was diagnosed as primary sclerosing cholangitis(PSC) and the other was diagnosed with PSC/autoimmune hepatitis overlapsyndrome and the third patient had hepatitis C virus infection. Biomedical applications of body area networks (BANs) are evolving, where taking periodic medical readings of patients via means wireless technologies at home or in the office will aid physicians to periodically supervise the patient’s medical status without having to see the patient. Thus, one important objective of BANs is to provide the doctor with the medical readings that can be collected electronically without being in close proximity to the patient. This is done through the measurement of the patient’s physiological signals via means of wearable sensors. This paper investigates wireless BAN cooperation via actual measurements of human movement kinematics and electrocardiogram (ECG), which are believed to provide patients with easy healthcare for continuous health-monitoring. The collected information will be processed using specially designed software, which in turn will enable the patient to send a full medical chart to the physician’s electronic device. In this way, physicians will have the ability to monitor their patients more efficiently.

Hypoxia, due to impaired cerebral blood flow, has hazardous effects on brain structure and function. To minimize as much as possible the neurological consequences from hypoxic-ischemic (HI) brain injury, neuro-protective strategies are urgently required. Vitamin E has been shown to have protective effects against cerebral ischemia, possibly due to its anti-oxidant effects. Thirty albino rats, of both sexes, were obtained from the animal house at King Khalid University Hospital, King Saud University. They were divided into three groups;each included 10 animals: Group A was considered as a control one, animals of Group B were subjected to a permanent link to the carotid arteries on both sides and animals of Group C underwent permanent link to carotid arteries on both sides and concomitantly were given Vitamin E as an anti-oxidant. Animals of Group C were injected by Vitamin E (equivalent to 15 mg/day), into the peritoneal cavity as a single dose for a week and after the prescribed period the mice were sacrificed under deep anesthesia and their brains were extracted and prepared for an electron microscopic study of brain tissue. Specimens from animals of Group B showed a large number of neurons that had been deteriorated. Mitochondria were the most affected organelles. There were a large number of dark cells which probably resulted from shrunken nerve cells and exhibited opaque nuclei. The number of affected nerve cells was much lower in brain tissues from animals of the Group C which revealed absence of dark cells. The study did not disclose any similar changes in brain tissues of the control group animals. Our results suggested that treatment with Vitamin E after hypoxia-ischemia led to a neuro-protective effect that appeared in reduction of cell death of neurons. Thus, the present study provides an evidence that Vitamin E protects the brain tissue of the consequences of hypoxia caused by ischemia in the tested experimental animals. Obesity and overweight are prevalent in Saudi Arabia (24.95% & 31.80%). Also, childhood obesity in the country is a challenge, with 6% - 10% of preschool and school-age children. The burden of being overweight and obese is disastrous. Therefore, the Saudi Diabetes Scientific Society constituted a team to develop a guideline. The team reviewed the local Clinical Practice Guidelines for the Prevention and Management of Obesity in Saudi Arabia;and conducted a rigorous review of relevant evidence-based scientific literature. After a thorough assessment, a consensus was reached to use the Australian guideline as the main guideline to be adapted and localized to be suitable for the Saudi people. To avoid duplication of efforts, the team adopted the grading of evidence used by the Australian guideline. The updated version was presented in a workshop, and the recommendation of the participants was included. The final draft was distributed for review, and comments were included. This document presents the result of such an effort as a local guideline.

BACKGROUND Pancreatic cystic lesions(PCLs) are common in clinical practice. The accurate classification and diagnosis of these lesions are crucial to avoid unnecessary treatment of benign lesions and missed opportunities for early treatment of potentially malignant *** To evaluate the role of cyst fluid analysis of different tumor markers such as cancer antigens [e.g., cancer antigen(CA)19-9, CA72-4], carcinoembryonic antigen(CEA), serine protease inhibitor Kazal-type 1(SPINK1), interleukin 1 beta(IL1-β), vascular endothelial growth factor A(VEGF-A), and prostaglandin E2(PGE2)], amylase, and mucin stain in diagnosing pancreatic cysts and differentiating malignant from benign *** This study included 76 patients diagnosed with PCLs using different imaging modalities. All patients underwent endoscopic ultrasound(EUS) and EUS-fine needle aspiration(EUS-FNA) for characterization and sampling of different *** The mean age of studied patients was 47.4 ± 11.4 years, with a slight female predominance(59.2%). Mucin stain showed high statistical significance in predicting malignancy with a sensitivity of 87.1% and specificity of 95.56%. It also showed a positive predictive value and negative predictive value of 93.1% and 91.49%, respectively(P < 0.001). We found that positive mucin stain, cyst fluid glucose, SPINK1, amylase, and CEA levels had high statistical significance(P < 0.0001). In contrast, IL-1β, CA 72-4, VEGF-A, VEGFR2, and PGE2 did not show any statistical significance. Univariate regression analysis for prediction of malignancy in PCLs showed a statistically significant positive correlation with mural nodules, lymph nodes, cyst diameter, mucin stain, and cyst fluid CEA.
