A recent study by Zoufaly et *** in The Lancet Respiratory Medicine describes encouraging data from the first severe COVID-19 patient successfully treated with human recombinant soluble angiotensin-converting enzyme-2(hrsACE2).1 The published data document upon treatment of an adaptive immune response,the disappearance of the virus swiftly from the serum,the nasal cavity and lungs,and a reduction of inflammatory cytokine levels that are critical for COVID-19 ***,the use of hrsACE2 did not impede the generation of neutralizing antibodies,leading to a significant clinical improvement of the treated patient.

Histopathological changes in the oesophageal mucosa in Egyptian children with corrosive strictures: A single-centre vast experience

BACKGROUND The caustic ingestion continues to be a major problem worldwide especially in developing countries. The long-term complications include stricture and increased life time risk of oesophageal carcinoma. Patients suffered from corrosive induced oesophageal strictures have more than a 1000-fold risk of developing carcinoma of the *** To determine the possibility of oesophageal mucosal dysplasia after prolonged dilatation in post corrosive *** This observational study was conducted at the Paediatric Endoscopy Unit in Cairo University Children's Hospital. It included children of both sexes older than 2 years of age who had an established diagnosis of post-corrosive oesophageal stricture and repeated endoscopic dilatation sessions for more than 6 mo. All patients were biopsied at the stricture site after 6 mo of endoscopic dilatation. A histopathological examination of an oesophageal mucosal biopsy was performed for the detection of chronic oesophagitis, inflammatory cellular infiltration and *** The mean age of the enrolled children was 5.9 ± 2.6 years; 90% of the patients had ingested an alkaline corrosive substance(potash). The total number of endoscopic dilatation sessions were ranging from 16 to 100 with mean number of sessionswas 37.2 ± 14.9. Histopathological examination of the specimens showed that 85%of patients had evidence of chronic oesophagitis(group A) in the form of basal cell hyperplasia, hyperkeratosis and subepithelial fibrosis. Functional evaluation of the filtering bleb by ultrasound biomicroscopy after trabeculectomy with mitomycin C

AIM： To study the role of ultrasound biomicroscopy（UBM） in the assessment of the bleb function after trabeculectomy with mitomycin C（MMC）. METHODS： This was a cross-sectional study including all cases had undergone trabeculectomy with MMC 0.2 mg/mL for 3 min. Participants were recruited from the follow-up cases at Department of Ophthalmology, Tanta University in the period from August 2015 to August 2016. Full history taking and ophthalmological examination were performed. Intraocular pressure（IOP） was measured using Goldmann applanation tonometry and the history of prescribed postoperative antiglaucoma medications was recorded. Accordingly, the trabeculectomy blebs were divided clinically into successful blebs when the IOP post-operative was ≤18 mmHg without medications. Scanning examination of the filtering bleb using UBM examination was performed at the period from 2 to 36 mo after trabeculectomy（13.6±9.7 m o）. RESULTS： This study included 33 trabeculectomy filtering blebs of 25 patients（9 males and 16 females）. The mean age of patients was 53.2±14.2 y（range 25-71y）. We had 20 eyes with complete success, 9 eyes with qualified success, and 4 eyes with failure after trabeculectomy with MMC. The blebs were classified into successful blebs（IOP ≤18 mmHg without antiglaucoma medications）, others were classified into qualified successful and failed blebs. There was a significant correlation between certain UBM findings（intra-bleb reflectivity, draining sub-scleral tract, bleb height, and intrableb fluid cysts） and cases with different grades of clinical functional success（P〈0.01）.CONCLUSION： UBM is an objective tool in the functional assessment of the post-trabeculectomy filtering bleb through a significant correlation between certain UBM parameters and the different grades of clinical functional success.

BACKGROUND Diffusion-weighted magnetic resonance imaging has shown promise in the detection and quantification of hepatic fibrosis. In addition, the liver has numerous endogenous micro-RNAs(miRs) that play important roles in the regulation of biological processes such as cell proliferation and hepatic *** To assess diffusion-weighted magnetic resonance imaging and miRs in diagnosing and staging hepatic fibrosis in patients with chronic hepatitis *** This prospective study included 208 patients and 82 age-and sex-matched controls who underwent diffusion-weighted magnetic resonance imaging of the abdomen, miR profiling, and liver biopsy. Pathological scoring was classified according to the METAVIR scoring system. The apparent diffusion coefficient (ADC) and miR were calculated and correlated with pathological *** The ADC value decreased significantly with the progression of fibrosis, from controls(F0) to patients with early fibrosis(F1 and F2) to those with late fibrosis(F3 and F4)(median 1.92, 1.53, and 1.25 × 10^(-3) mm^2/s, respectively)(P = 0.001).The cut-off ADC value used to differentiate patients from controls was 1.83 × 10^(-3) mm^2/s with an area under the curve(AUC) of 0.992. Combining ADC and miR-200 b revealed the highest AUC(0.995) for differentiating patients from controls with an accuracy of 96.9%. The cut-off ADC used to differentiate early fibrosis from late fibrosis was 1.54 × 10^(-3) mm^2/s with an AUC of 0.866. The combination of ADC and miR-200 b revealed the best AUC(0.925) for differentiating early fibrosis from late fibrosis with an accuracy of 80.2%. The ADC correlated with miR-200 b(r =-0.61, P = 0.001), miR-21(r =-0.62, P = 0.001), and miR-29(r = 0.52,P = 0.001).CONCLUSION Combining ADC and miRs offers an alternative surrogate non-invasive diagnostic tool for diagnosing and staging hepatic fibrosis in patients with chronic hepatitis C.

Influence OF anticancer drugs on DNA methylation in liver of female mice

Epigenetic changes such as DNA methylation regulate gene expression in normal development. Methotrexate and Adriamycine are antineoplastic drugs that target DNA and enzymes acting on DNA. We aimed to evaluate their cytotoxic effect on cell lines and on female mice to investigate the in vivo influence of both drugs on the DNA methylation and subsequently the protein expression. The total level of DNA methylation showed a significant reduction from 62.2% to 36.7%, 36.6% as compared to control group, when using different doses of MTX and ADR. Hepatic protein pattern revealed five bands with low MW (16 - 6.1 KDa) in acute and LD50 doses. In conclusion DNA methylation is influenced by anticancer drugs in a dose - dependent manner. Some specific protein fragments may be considered as a potential markers associated with high dose of anticancer drugs.

Efficacy of Some Antibiotics in Curing Resistant <i>Escherichia coli</i>Infection

There is growing interest in re-evaluation of older antibiotics with the wide spread of pathogen resistance, especially gram negative bacteria, which impair treatment of some infections. In contrast various studies have reported that some antibiotics have efficacy in clearing resistant bacterial infections. On account of that it was interesting to evaluate the efficacy of erythromycin, chloramphenicol and/or tenoxicam in curing and/or relieving wound infection of highly resistant Escherichia coli and investigate the possible mechanisms beyond their antibacterial activity. This was achieved through evaluating highly resistant E. coli strains in vitro using agar dilution and in vivo rat models of E. coli infected wound and acute inflammation by carrageenin, where possible mechanisms were evaluated through measuring immunological mediators and histopathological examination. This study revealed that in vivo, erythromycin alone or in combination with tenoxicam significantly improved the healing of infected skin wounds with E. coli irresspective of resistancy in vitro. In addition to the improvement of immunological mediators involved in inflammatory reaction, oxidative stress and in cytokines expression as response to the bacterial infection in vivo. On the other hand chloramphenicol neither alone nor in combination with tenoxicam, achieved any significant effect. Tenoxicam didn’t show antimicrobial activity alone nor in combination with tested antibiotics in vitro, but it has shown synergestic activity in combination with tested antibiotics in vivo. Thus we concluded that immunomodulatory activity of erythromycin through anti-inflammatory and antioxidant effects was the possible mechanisms by which this antibiotic had healed infection with resistant E. coli in vivo, despite its resistancy to this antibiotic in vitro.

Assessment of EEG Changes in Neonatal Sepsis at Al-Zahraa University Hospital’s NIC Unit

Background: EEG is one of the most sensitive instrumental techniques for identifying sepsis-related neurological complications and a valuable tool in the ICU when clinical assessment is difficult. Aim of the Study: To detect EEG changes in neonates with evidence of sepsis. Patients and Methods: This was a prospective observational study for detecting EEG changes in cases of proven neonatal sepsis. This study was conducted at the NICU of Al Zahraa University Hospital for a period from October 2019 to March 2020. Our studied neonates were 50. This study included full-term newborns (37 to 42 weeks of gestational age) with clinical and laboratory evidence of sepsis (early and late onset). Neonatal birth asphyxia, jaundice, congenital malformations, suspected inborn error of metabolism and neonates electrolytes disturbances were excluded from the study. We record EEG changes during 20 min By an electroencephalogram (EEG) with abioelectric amplifier (Deltamed, Bourgogne), (Philips) Screen. Results: Among our studied neonates (neonates with evidence sepsis), 44.0% developed clinical seizures, 81% of the seizures group had abnormal EEG, 35.7% of No seizures group had abnormal EEG. Integrated Action of Mixture Rates and Nitrogen Levels on Quantity and Quality of Forage Mixture from Egyptian Clover and Barley in Sandy Soil

Effect of seeding rate mixture from Egyptian clover (Trifolium alexandrinum L.) E.C. and barley (Hordum vulgare L.) B, N-fertilization levels and their interaction on quantity and quality of forage yield. Two field experiments were conducted during winter season of 2012/2013 and 2013/2014 in sandy soil at research and production station of National Research Centre, Al Nubaria district, Al Behaira Governorate, Egypt. The experiment included two factors first was five mixtures from seeding rates (24 kg EC/fed. – 50 kg B/fed. – 18 kg EC + 12.5 kg B/fed. – 12kg EC + 25 kg B/fed. – 6 kg EC + 37.5 kg B/fed.) and second factor was three N levels (30-45-60 kg N/fed.). Two cuts were obtained at 60 and 100 DAS. Results indicated significant differences between mixture rates for all studied characters in both cuts 60 and 100 DAS. Significant differences between N levels for all studied characters at 60 DAS, also, forage yield /fed. and DM% at 100 DAS. It can be introducing forage mixture of 18 kg EC + 12.5 B had balanced character in forage yield/fed., DM%, carbohydrate % and protein % at both cuts 60 and 100 DAS.

Objective:Preservation of the pelvic autonomic nerves in order to lower bladder and sexual dysfunction after radical rectal cancer surgery & to evaluate functional outcome,local ***:A prospective study was undertaken on Egyptian *** one patients participated in the study in the period from December 2002 till June 2004 where they underwent radical surgery but with preservation of the pelvic autonomic nerves this was followed by adjuvant pelvic ***:Six months,1-year and 2-year follow-up of urinary function was complete in 32 out of 41(78%),30 out of 41(73%) and 27 out of 41 patients(65%) respectively There was no statistically significant correlation between the extent of nerve preservation and the reported minor voiding *** of the patients reported major *** months,1-year and 2-year follow-up of sexual function revealed that 22 out of 41 patients(53%) were sexually *** out of 4l patients(7.3%) developed local recurrence.38(92.7%) patients were free of local recurrence,regarding patients who received adjuvant radiotherapy 3 out of the 34(8.8%) patients developed local pelvic recurrence while 9 patients(26.5%) developed distant metastases(3 of them did not receive adjuvant chemotherapy),while patients who received adjuvant chemotherapy,2 out of 20 patients(10%) developed local recurrence while distant metastases developed in 6 patients(30%).Conclusion:Preservation of the pelvic autonomic nerves minimizes bladder and sexual dysfunction especially in male patients after rectal cancer surgery.
