Clinical features, treatment strategies and outcomes of craniocervical junction arteriovenous fistulas: a cohort study of 193 patients

Background Craniocervical junction(CCJ)arteriovenous fistulas(AVFs)are rare.The current treatment strategies for AVFs with different angioarchitecture need to be clarified.The present study aimed to analyse the correlation between angioarchitecture and clinical characteristics,share our experience in treating this disease and identify risk factors associated with subarachnoid haemorrhage(SAH)and poor outcomes.Methods A total of 198 consecutive patients with CCJ AVFs from our neurosurgical centre were retrospectively reviewed.The patients were grouped according to their clinical manifestations,and their baseline clinical characteristics,angioarchitecture,treatment strategies and outcomes were summarised.Results The patients’median age was 56 years(IQR 47–62 years).The majority of patients were men with 166(83.8%)patients.The most common clinical manifestation was SAH(52.0%),followed by venous hypertensive myelopathy(VHM)(45.5%).The most common CCJ AVFs type was dural AVF,with 132(63.5%)fistulas.The most frequent fistula location was C-1(68.7%)and dural branch of vertebral artery(70.2%)was the most involved arterial feeders for fistulas.The most common direction of venous drainage was descending intradural drainage(40.9%),followed by ascending intradural drainage(36.5%).Microsurgery was the most common treatment strategy applied for 151(76.3%)patients,15(7.6%)patients were treated with interventional embolisation only,and 27(13.6%)received both interventional embolisation and microsurgical treatment.The learning curve for microsurgery only was analysed by cumulative summation method,and the turning point was the 70th case,and blood loss in post-group was lower than that in pre-group(p=0.034).At the last follow-up,there were 155(78.3%)patients with favourable outcomes(modified Rankin Scale(mRS)<3).Age≥56(OR 2.038,95%CI 1.039 to 3.998,p=0.038),VHM as the clinical manifestation(OR 4.102,95%CI 2.108 to 7.982,p<0.001)and pretreatment mRS≥3(OR 3.127,95%CI 1.617 to 6.047,p<0.001)were significantly associated with poor out

Tourmaline is a common borosilicate mineral in granite and pegmatite， and it is also an important carrier to record the physicochemical evolution information of magmas. However， the genetic relationship between pegmatite-type rare metal mineralization potential and tourmaline-bearing pegmatites is not clear， and the tourmaline geochemical indication of the mineralization type of rare metals needs to be supplemented. In the middle Altun region， extensive tourmaline-bearing pegmatite dykes are prevalent within and surrounding the Tugeman complex. In this paper， the petrology and geochemical characteristics of tourmaline within the pegmatite of the Tugeman Be-Li deposit have been comprehensively analyzed. The research delves into the genetic evolution and metallogenic potential of the pegmatite， emphasizing the indicative role of tourmaline geochemistry in discerning mineralization types in the Tugeman Be-Li deposit. Major and trace element analysis of tourmaline reveals varying contents of FeO （9.37 wt.%–17.10 wt.%）， Al2O3（28.64 wt.%–35.22% wt.%）， and MgO （0.13 wt.%–2.47 wt.%） in the Tugeman Be–Li pegmatite. Li， Zn， and Sn contents range from 204 ppm to 1 590 ppm， 1 143 ppm to 4 089 ppm， and 74 ppm to 396 ppm， respectively. The tourmaline analyzed in this study is magmatic origin and belongs to schorl series， exhibits substitution relationships， including (Na， Fe2+)(X□， Al)-1， (Fe2+， Fe3+)（Mg， Al）-1， (Al，X□)(R2+， Na)-1，（Na， Mg）(X□， Al)-1andX□NaR2+-1. The trace element data of tourmaline show that lithium has four enrichment stages in different mineral crystallization phases. Notably， the tendency of tourmaline in the Tugeman Be-Li pegmatite to evolve towards elbaite implies a magmatic evolution sequence from low to high， transitioning through microcline-albite pegmatite， muscovite-microcline-albite pegmatite， albite pegmatite， and microcline pegmatite. The positive definiteness of real quadratic forms with convolution structures plays an important rolein stability analysis for time-stepping schemes for nonlocal operators. In this work, we present a novel analysistool to handle discrete convolution kernels resulting from variable-step approximations for convolution operators.More precisely, for a class of discrete convolution kernels relevant to variable-step L1-type time discretizations, weshow that the associated quadratic form is positive definite under some easy-to-check algebraic conditions. Ourproof is based on an elementary constructing strategy by using the properties of discrete orthogonal convolutionkernels and discrete complementary convolution kernels. To our knowledge, this is the first general result onsimple algebraic conditions for the positive definiteness of variable-step discrete convolution kernels. Using theunified theory, we obtain the stability for some simple nonuniform time-stepping schemes straightforwardly.

Herein, the influence of the concentration design and comprehensive performance of the sulfate-phosphoric mixed acid system electrolyte is investigated to realize an electrolyte that maintains high energy density and stable operation at high temperatures. Static stability tests have shown that VOPO4 precipitation occurs only with vanadium(V) electrolyte. The concentration of vanadium ion of 2.0–2.2 mol·L^(–1), phosphoric acid of 0.10–0.15 mol·L^(–1), and sulfuric acid of 2.5–3.0 mol·L^(–1) are suitable for a vanadium redox flow battery in the temperature range from –20 to 50 ℃. The equations for predicting the viscosity and conductivity of electrolytes are obtained by the response surface method. The optimized electrolyte overcomes precipitation generation. It has 2.8 times higher energy density than the non-phosphate electrolyte, and a coulomb efficiency of 94.0% at 50 ℃. The sulfate-phosphoric mixed acid system electrolyte promotes the electrode reaction process, increases the current density, and reduces the resistance. This work systematically optimizes the concentrations of composition of positive and negative vanadium electrolytes with mixed sulfate-phosphoric acid. It provides a basis for the different valence states and comprehensive properties of sulfate-phosphoric mixed acid system vanadium electrolytes under extreme environments, guiding engineering applications.

In recent years,endoscopic resection,particularly endoscopic submucosal dis-section,has become increasingly popular in treating non-metastatic superficial esophageal squamous cell carcinoma(ESCC).In this evolving paradigm,it is crucial to identify factors that predict higher rates of lymphatic invasion and poorer outcomes.Larger tumor size,deeper invasion,poorer differentiation,more infiltrative growth patterns(INF-c),higher-grade tumor budding,positive lymphovascular invasion,and certain biomarkers have been associated with lymph node metastasis and increased morbidity through retrospective reviews,leading to the construction of comprehensive nomograms for outcome prediction.If validated by future prospective studies,these nomograms would prove highly applicable in guiding the selection of treatment for superficial ESCC.

In this paper we prove that isometries with respect to the Kobayashi metric between certain domains having boundary points at which the boundary is infinitely flat extend continuously to the boundary.The strategy is to reestablish the Gehring-Hayman-type Theorem for these complex domains.Furthermore,the regularity of boundary extension map is given.

Chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS),also known as National Institutes of Health(NIH)type Ⅲ prostatitis,is a common disorder with an unclear etiology and no known curative treatments.Based on the presence or absence of leukocytes in expressed prostatic secretion(EPS),CP/CPPS is classifiedfurther into Illa(inflammatory)and Illb(noninflammatory)subtypes.However,the severity of symptoms is not entirely consistent with the white blood cell(WBC)count.Following the preliminary finding of a link between inflammatory cytokines and CP/CPPS,we performed this clinical study with the aim of identifying cytokines that are differentially expressed according to whether the prostatitis subtype is Ⅲa or Ⅲb.We found that granulocyte colony-stimulating factor(G-CSF),interleukin-18(IL-18),and monocyte chemoattractant protein-1(MCP-1)levels were significantly elevated and interferon-inducible protein-10(IP-10)and platelet-derived growth factor-BB(PDGF-BB)levels were downregulated in the EPS of patients with type Ⅲa prostatitis.In a word,it is a meaningful study in which we investigate the levels of various cytokines in EPS according to whether prostatitis is the Ⅲa or Ⅲb subtype.The combination of G-CSF,IL-18,MCP-1,IP-10,and PDGF-BB expression levels could form a basis for classification,diagnosis,and therapeutic targets in clinical P/CPPS.

Increasing operational stability of journal bearing in hydraulic suspension micro-pump by herringbone grooved structure

The operational stability of radial journal bearings is the bottleneck that limits the reliability of hydraulic suspension micropump. Due to self-excited vibrations, the whirl amplitude of the plain journal bearing(PJB) is large at high rotational speeds,which will accelerate wear failure. It has been proven that employing herringbone grooved journal bearing(HGJB) is an effective method to reduce the whirl amplitude and improve operational stability. However, enhancing the stability of journal bearings in micro-pumps by herringbone grooved structures has rarely been studied, and its effect needs to be verified. We validated the mechanism of the stability improvement with the CFD method and compared the dynamic characteristics of HGJB and PJB by rotor dynamics evaluation and experiment. The experimental results show that under the same conditions the whirl amplitude of the HGJB decreases by 29.61% in the x direction and by 24.09% in the y direction compared with that of the PJB. This study reveals the effect of the herringbone grooved structure on the operational stability of bearings and may provide guidance for the reliability improvement of hydraulic suspension micro-pump.
