The etiology of upper gastrointestinal bleeding(UGIB)varies by age,from newborns to adolescents,with some of the causes overlapping between age *** particular causes such as vitamin K deficiency and cow’s milk protein allergy are limited to specific age groups,occurring only in neonates and infants,others such as erosive esophagitis and gastritis may be identified at all ***,the incidence of UGIB is variable throughout the world and in different hospital *** North America and Europe,most UGIBs are nonvariceal,associated with erosive esophagitis,gastritis,and gastric and duodenal *** recent years,the most common causes in some Middle Eastern and Far Eastern countries are becoming similar to those in Western ***,variceal bleeding still predominates in certain parts of the world,especially in South *** most severe hemorrhage arises from variceal bleeding,peptic ulceration,and disseminated intravascular *** is a credible indicator of a UGI source of bleeding in the majority of *** familiar with the most likely UGIB causes in specific ages and geographic areas is especially important for adequate orientation in clinical settings,the use of proper diagnostic tests, and rapid initiation of the therapy. The fundamental approach to the management of UGIBincludes an immediate assessment of severity, detecting possible causes, and providing hemodynamic stability,followed by early endoscopy. Unusual UGIB causes must always be considered when establishing a diagnosis inthe pediatric population because some of them are unique to children. Endoscopic techniques are of significantdiagnostic value, and combined with medicaments, may be used for the management of acute bleeding. Finally,surgical treatment is reserved for the most severe bleeding.

Splenic hamartomas(SHs)are uncommon,benign vascular lesions of unclear etiology and are mostly found incidentally on abdominal images,at surgery,or at *** the first case description,in 1861,less than 50 pediatric SH cases have been reported in the *** this article,we have performed an analysis of all SH cases in children published in the literature to date and presented our case of an 8-year-old male with *** lesions in children were shown to cause symptoms more often than in the adult *** observed SH sizes in children ranged from a few millimeters to 18 cm,and the symptomatic lesions were mostly larger or *** most common clinical finding was *** of hypersplenism were present in children with a single SH larger than 4.5 cm(diameter range:4.5-18.0 cm)and in those with multiple hamartomas,ranging from a few millimeters to 5 *** percent of patients with available laboratory findings had hematological abnormalities such as anemia,thrombocytopenia,or *** symptoms and signs included abdominal pain,recurrent infections,fever,night sweats,lethargy,growth retardation,and weight *** use of multiple imaging modalities may suggest the preoperative diagnosis of a splenic mass in children and determine the therapeutic ***,the final diagnosis of SH relies on histopathological ***,including total or partial splenectomy(PS),is the mainstay of SH *** M et *** in children WJCC https://*** 1910 April 16,2024 Volume 12 Issue 11 Although total splenectomy carries a greater risk of overwhelming post-splenectomy infection than PS it has remained the most performed surgical procedure in children with *** the majority of pediatric patients with symptomatic SH,resolution of symptoms and resolution or improvement of cytopenias occurred after surgical treatment.

Critical infrastructures (CI) are difficult to handle due to their complexity, size and the number of stakeholders involved. During emergency situations (e.g. fire or terrorist attack), a CI operator in the control room is faced with a flood of information coming from different sensors and legacy monitoring systems. Since in these situations, time is critical and the operators are under a great deal of pressure, a holistic management of all the technical systems and actors involved is needed, reinforced by the Recommendation and Decision Support System (RDSS) that helps emergency managers to take correct and timely decisions. One way to provide an adequate RDSS support to the operator is proposed in this paper which is based on an intelligent, event driven layer that sits on top of the legacy CI monitoring system. Powered by the complex event processing capabilities and facility data model implemented in the form of CI ontology, this layer processes events originating from different sources, conducts the situation and risk assessment, and reacts accordingly, either automatically or via recommendations proposed to emergency personnel. To validate the proposed approach, an event-driven RDSS was deployed on an airport use case (Nikola Tesla airport in Belgrade), as one of the most complex CIs.

Response of Growth Dynamics of Two Spring Geophytes to Light Regime in a Lime-Beech Forest

Blomaee accumulation, leaf longevity and growth rate of two spring forest geophytes, Scllla blfolla L. and Arum maculatum L. were estimated separately for three size groups within each population of these species. Despite the differences in leaf longevity, both species showed a similar pattern of blomass accumulation In relation to their phenologles and reproductive demands. Eco-physlological acclimation to changing light environment was assumed through photosynthetic parameters and dynamics of leaf area Index In the predominant size group of each species. A light response curve was measured under natural light for each species through the continuum of Its phenology to quantify the photosynthetic photon flux density at light saturation, light-saturated photosynthetic rate, light compensation point, and dark respiration. Light-saturated assimilation per leaf area basis, dark respiration rate and light compensation points were significantly higher in S. blfolla relative to A. maculatum. However, the acclimation of photosynthesis that would respond to light changes in environment was not found in S. bifolla. In contrast, In A. maculatum a marked shift In the light dependence of photosynthesis through the season was noticed, which resulted In a strong photosynthetic acclimation to the low-light conditions. Accompanied by significant leaf area Index, this efficient low-light photosynthesis enabled greater leaf longevity, and consequently longer accumulative period to A. maculatum. From the different parameters that we determined （both photosynthetic acclimation and growth strategy） it would appear that these species belong to two distinct subgroups： S. blfolla to the early and A. maculatum to the late vernals.

AIM To investigate changes in hepatic and splenic stiffness in patients without chronic liver disease during liver resection for hepatic *** Patients scheduled for liver resection for hepatic tumors were considered for enrollment. Tissue stiffness measurements on liver and spleen were conducted before and two days after liver resection using point shear-wave elastography. Histological analysis of the resected liver specimen was conducted in all patients and patients with marked liver fibrosis were excluded from further study analysis. Patients were divided into groups depending on size of resection and whether they had received preoperative chemotherapy or not. The relation between tissue stiffness and postoperative biochemistry was investigated. RESULTS Results are presented as median(interquartile range). 35 patients were included. The liver stiffness increased in patients undergoing a major resection from 1.41(1.24-1.63) m/s to 2.20(1.72-2.44) m/s(P = 0.001). No change in liver stiffness in patients undergoing a minor resection was found [1.31(1.15-1.52) m/s vs 1.37(1.12-1.77) m/s, P = 0.438]. A major resection resulted in a 16%(7%-33%) increase in spleen stiffness, more(P = 0.047) than after a minor resection [2(-1-13) %]. Patients who underwent preoperative chemotherapy(n = 20) did not differ from others in preoperative right liver lobe [1.31(1.16-1.50) vs 1.38(1.12-1.56) m/s, P = 0.569] or spleen [2.79(2.33-3.11) vs 2.71(2.37-2.86) m/s, P = 0.515] stiffness. Remnant liver stiffness on the second postoperative day did not show strong correlations with maximum postoperative increase in bilirubin(R^2 = 0.154, Pearson's r = 0.392, P = 0.032) and international normalized ratio(R^2 = 0.285, Pearson's r = 0.534, P = 0.003). CONCLUSION Liver and spleen stiffness increase after a major liver resection for hepatic tumors in patients without chronic liver disease.

A plasma spectroscopy experiment is described and its performances are given, ways oI excltauon u^ttc~,t tc~,,,,,~ ,,, free-burning plasma at atmospheric pressure is analyzed in detail. The fixed-grating spectrograph PGS-2 Carl Zeiss Jena is actually a variation on the very first spectrometer design, where photo-plate was replaced with a detector array of PIXIS-Roper Scientific camera. Abilities of the system are analyzed and its limitations are determined.

本文评述了聚合物复合材料固化现场监测传感器的应用现状与发展趋势.热电偶和压力传感器一直被使用,并且常常用其它传感器作为补偿,以增加固化过程中化学流变变化的实时信息.介电传感器是目前通用的,但它在传输分子固化过程的信息时,容量有其局限性. 一个值得注意的新动向是红外传输光学纤维传感器的使用,因为在红外光谱中含有极丰富的信息.远程红外光谱测定法是用于固化现场监测的最有希望的方法,它的应用关键在于中、远红外传输纤维的可用性.文章论述了硫族化物和卤化物型红外传输光纤技术的现状.
