Nano albumin bound-paclitaxel in pancreatic cancer: Current evidences and future directions

Pancreatic cancer(PDAC) is an aggressive and chemoresistant disease, representing the fourth cause of cancer related deaths in western countries. Majority of patients have unresectable, locally advanced or metastatic disease at time of diagnosis and the 5-year survival rate in these conditions is extremely low. For more than a decade gemcitabine has been the cornerstone of metastatic PDAC treatment, although survival benefit was very poor. PDAC cells are surrounded by an intense desmoplastic reaction that may create a barrier to the drugs penetration within the tumor. Recently PDAC stroma has been addressed as a potential therapeutic target. Nano albumin bound(Nab)-paclitaxel is an innovative molecule depleting tumor stroma, through interaction between albumin and secreted protein acidic and rich in cysteine. Addition of nab-paclitaxel to gemcitabine has showed activity and efficacy in metastatic PDAC first-line treatment improving survival and overall response rate vs gemcitabine alone in the MPACT phase Ⅲ study. This combination represents one of the standards of care in advanced PDAC therapy and is suitable to a broader spectrum of patients compared to other schedules. Nab-paclitaxel is under investigation as a backbone of chemotherapy in novel combinations with target agents or immunotherapy in locally advanced or metastatic PDAC. In this article, we provide an updated and critical overview about the role of nab-paclitaxel in PDAC treatment based on the latest advances in preclinical and clinical research. Furthermore, we focus on the use of nab-paclitaxel within the context of metastatic PDAC treatment landscape and we discuss about future implications in the light of current clinical ongoing trials.

Towards curative therapy in gastric cancer:Faraway, so close!

Although recent diagnostic and therapeutic advances have substantially improved the survival of patients with gastric cancer(GC), the overall prognosis is still poor. Surgery is the only curative treatment and should be performed in experienced centers. Due to high relapse following surgery, complementary and systemic treatment aimed at eradicating micrometastasis should be performed in most cases. Cytotoxic treatments are effective in downstaging locally advanced cancer, but different sensitivities and toxicities probably exist in different GC subtypes. Current treatment protocols are based primarily on clinical data and histological features, but molecular biomarkers that would allow for the prediction of treatment responses are urgently needed. Understanding how host factors are responsible for inter-individual variability of drug response or toxicity will also contribute to the development of more effective and less toxic treatments.

Barriers and facilitators of implementing child injury surveillance system

Purpose: Child injuries are a global public health problem and injury surveillance systems (ISS) can be beneficial by providing timely data. However, ISS implementation has challenges. Opinions of stakeholders of ISS implementation barriers and facilitators are a good source to understand this phenomenon. The aim of this study is to investigate barriers and facilitators of implementing ISS in Iran. Methods: This is a qualitative study. Data were gathered through interviews with 14 experts in the field of child injury and prevention from Iranian Ministry of Health and Medical Education (MOHME), medical univers让ies, pediatrics hospitals, general hospitals and health houses during January 2017 to September 2017. Data collection and analysis con tinued until data saturation. Data were analyzed using con tent analysis through identifying meaning units. Results: Barriers were classified in three main categories and nine subcategories including management barriers (including performance, coordination and cooperation, supervision and attitude), weakness in data capture and usage (including data collection, data recording and data dissemination) and resource limitation (including human and financial resources). Facilitators identified in three areas of policy making (including empowerment and attitude), management (including organization, function and cooperation and coordination) and data recording and usage (including data collection/distTibution and data recording). Conclusion: The most important barrier is lack of national policy in child injury prevention. The most important facil让ator is improving MOHME function through passing supportive regulations. Effective data usage and dissemination of information to those requiring data for policy making can help reduce child injuries. Coalition of stakeholders helps overcome existing barriers.

Our aim is to evaluate the association between body mass index(BMI)and preoperative total testosterone(TT)levels with the risk of single and multiple metastatic lymph node invasion(LNI)in prostate cancer patients undergoing radical prostatectomy and extended pelvic lymph node *** BMI,basal levels of TT,and prostate-specific antigen(PSA)were evaluated in 361 consecutive patients undergoing radical prostatectomy with extended pelvic lymph node dissection between 2014 and 2017・Patients were grouped into either nonmetastatic,one,or more than one metastatic lymph node invasion *** association among clinical factors and LNI was *** was detected in 52(14.4%)patients:28(7.8%)cases had one metastatic node and 24(6.6%)had more than one metastatic *** the overall study population,BMI correlated inversely with TT(r=-0.256;P28 kg m^2(P28 kg m^2(P-0.048).In our study,overweight and obese patients had a higher risk of harboring multiple prostate cancer lymph node metastases and lower TT levels when compared to patients with normal BMI.

Macroplastics on Soil-Plant System: Inhibiting Effects of Macroplastics on the Growth of Green Amaranth (<i>Amaranthus viridis</i>)

In recent time Bangladesh faces a serious problem of soil pollution due to plastic contamination. However, the degree of the extent to which the effects of plastics on plant growth occur is not properly identified. An experiment was conducted to measure the effects of mixed plastic (polyethylene and disposable plastic glass) on the growth of Amaranthus viridis. Different doses of mixed plastics (T0, T1, T2, and T3) were applied with a fixed amount of soil for each of the treatments e.g., T0 (control), T1 (10 gm mixed plastics/3kg soil), T2 (15 gm mixed plastics/3kg soil) and T3 (20 gm mixed plastic/3kg soil), and the growth response of Amaranthus viridis against plastic was observed for six consecutive weeks. The growth was measured in terms of plant height and girth diameter. The results showed that the presence of mixed plastic had a significant effect on the growth of Amaranthus viridis and particularly in treatment T3 (3 kg soil/20gm mixed plastic), the plants showed a slower growth response compared to control and the rest of the treatments applied in case of both plant height as well as girth diameter. The statistical analysis (one-way Analysis of Variance) also proved the significance of the treatments (p-values < 0.05) for six consecutive weeks. The experiment was successfully able to set an index on which plastics had their effects on the growth of green amaranth. In addition, the obtained data will be helpful in future research of the study in determining the possible effects of plastic on plant growth viz. green amaranth.

Unravelling the Functions of Regulatory T Cells during Infection

Accumulating evidences have suggested that Treg have an active role in the regulation of immunity to infection. Treg suppress not only autoimmune responses but also other immune responses for instance, during acute infections, against commensal microbes in inflammatory diseases or during chronic illness. Treg have been shown to limit exacerbated inflammation to avoid collateral tissue damage. Treg are also suggested to provide early protective responses in some viral infections as the permitting timely entry of effector cells in infected tissue. Furthermore, Treg have been shown to contribute to form memory pool after resolution of infection. In this review, we survey and analysis our current knowledge and relative dynamics of Treg in a wide range of infection settings and elaborate the examples in which these cells are of critical importance in conferring tolerance, suppressing pathogenesis, inducing protection and optimizing immunity to eliminate infection.

Cholera continues to be one of the most common causes of morbidity and mortality among children and adults in developing countries. Vaccine against cholera is an approach in the control of this epidemic and pandemic disease. From the development of very early oral cholera vaccine, advances in vaccine development documented due to a good illustration of the epidemiology, outbreak strategy, and pathophysiology of the disease causing pathogen. The newer-generation oral cholera vaccines are safe and guarantee a high level of protection during outbreak settings for several years. Yet infants and young children in developing countries are hyporesponsive to vaccines and show poor protection against cholera. In this review, we survey and analyse our current knowledge on the etiology of cholera, its clinical manifestation, global epidemiology and elaborate the vaccine candidates, which are effective against the pathogen and the corresponding immune responses to the available vaccines. These reviews comprehensively cover the salient features of recent discoveries related to Vibrio cholerae virulence, past and present vaccine candidates and their advantages and disadvantages with their development strategies. We believe that the advances that have been included in this review will give a comprehensive insight to the prevention and control of cholera outbreaks and development of effective cholera vaccines.

太极拳不仅是一种体育运动项目,又是中华传统文化的一个重要组成部分,它在全世界非常受欢迎。现在太极拳的科学规范的传播方式与标准问题,已经影响到太极拳在国际上的普及与推广。在太极拳界,技术训练历来被重视,而理论方面经常被练习者所忽略。这样就会对拳术的发展有所限制,其原因是由于文化的差异性。通过几百年的训练经验后创造的,太极拳理论是一种使用中华传统文化语言来解释太极拳动作的理论。一些拳理的基本要领,如“掤”、“虚实分明”、“缠丝劲”等训练要求,对大部分的练习者来说,理解这些难度很大。原因在于这些概念只有在掌握中华传统文化基础上才能理解。为了有助于练习者能理解训练理论的内涵,我们使用科学方法来分析太极拳动作技术,使用一种国际通用的生物力学方法来说明太极拳运动的文化本质。太极拳的拳理概念很多,本文要研究的是关于太极拳理论中的“旋腰转脊、旋腰转胯”,用生物理学方法来解释太极拳搂膝拗步动作中,脊、腰、髋部的运动变化。而这三个身体部位的连接部位就是骨盆,由于运动中骨盆沿规定轴线在旋移,所以脊、腰、髋部,亦随其协调同步运动。本文采用运动生物力学中运动学和动力学的测量方法,对太极拳练习者骨盆运动的本质进行了研究,得出不同技术水平练习者完成杨式太极拳搂膝拗步过程中,骨盆运动的一些技术特征指导。太极拳动作中,骨盆运动是以“旋移”为主。骨盆绕Z-垂直轴和Y-冠状轴转动幅度变化要大,绕X-矢状轴转动幅度变化要小。前移的时候身体不是直进直退,而是旋移地往前移。如果骨盆的某一个角度方向变化小,其他角度方向变化就大了。这样动作会顺利地行进,而且能够保持动作的旋移性。以保持身体“中正”,骨盆绕X-矢状轴转动幅度变化要小。在步法过程中,重心向回旋移并左右脚跟着地的时候,因为绕Z-垂直轴的幅度要大,所以骨盆绕Y-冠状轴后仰。骨盆后仰的时候,躯干从头到尾闾变成一条直线,可以更好地当作身体的转轴,以便更好地转动在Z-垂直轴。在双支撑阶段,脚跟着地至弓步,骨盆绕Z-垂直、X-矢状以及Y-冠状等三轴转动角度达到曲线的最大变化。这个现象是“掤”的反应,是骨盆转动并准备蓄劲的意思。

Factors Associated with Physical-Activity Performance by Older Individuals in a Medium-Sized City in S^o Paulo State, Brazil

Physical activity has been scientifically discussed as fundamental in the process of healthy ageing. Hence, this study aimed at determining the factors that influence older people to perform physical activities. The complete IPAQ （International Physical Activity Questionnaire） was applied to a population-based sample consisting of 364 elderly persons in the city of Botucatu, SAo Paulo, Brazil. Days of physical activity performed by the older people were considered by taking into account household and leisure activities. Models for count data were fitted by including socio-demographic variables as well as those related to life satisfaction. It was shown that housework physical-activity performance is associated with female, who predominantly showed to be more active in all levels. Male seemed to be more predisposed to perform lighter recreation, sports and leisure-time physical activities, such as walking. Additionally, poor schooling showed to he decisive for not performing physical activities both at home and during leisure.
