Does Positive Airway Pressure Therapy Result in Improved Sleep Quality?

Introduction: Positive airway pressure (PAP) therapy is the gold-standard for obstructive sleep apnea (OSA) management. While it is known that PAP is efficacious for controlling breathing events during sleep when it is worn at the right pressure for the amount of time prescribed, there is less clear data on how well it improves sleep quality. There are few studies that have examined the effectiveness of PAP therapy on sleep quality. Methods: OSA participants (n = 241) from a larger trial examining a PAP adherence were included. Participants were provided with PAP instruction and followed at 2 months and 4 months. PAP adherence was measured as the number of hours per night at prescribed pressure, an objective measure of treatment adherence. The Pittsburgh Sleep Quality Index (PSQI) was used as the primary measure of sleep quality. Results: The PSQI was significantly correlated with PAP adherence at both the 2-month and 4-month time points, such that lower sleep quality was associated with lower PAP use. This finding held for the sleep disturbance subscale of the PSQI. Over 55% of those using PAP therapy at the 4-month time point continued to report significantly disturbed sleep. Discussion: This study shows that PAP therapy does not appear to improve sleep quality to a degree that would be expected. Over half of those patients using PAP therapy still experienced disturbed sleep. Whether the disturbed sleep is directly attributable to the PAP device itself or to disturbed sleep secondary to uncontrolled OSA when PAP is not worn is worthy of further investigation.

低调陈述是一种为了满足交际需要而故意用弱化的或有节制的词语代替较直接的、唐突的言辞的修辞手段，同时它也是委婉语的一种实现手段。随着语用学的发展，低调陈述被赋予了新的活力，低调陈述成为一种被广泛使用的语用策略。它可以通过多种词汇、语法和语篇手段获得意想不到的礼貌、幽默、讽刺效果。C-R-A（合作-关联-顺应）模型是廖巧云在2005年提出的，这是一个言语交际的三维模型，它把语用学三大理论合作原则、关联理论和顺应论结合起来，旨在取长补短，优势互补，建立一个可以从哲学、认知和社会三个维度解释交际过程的模型。在这个模型中，合作是成功交际的先决条件，关联是必要条件，顺应是调节机制。通过对交际言语的阐释初步证明了它的解释力，但是仍需检验。本文采用定性分析方法在C-R-A模型下解读低调陈述言语行为，旨在通过研究初步解决四个问题：（1）基于以往研究对低调陈述进行重新定义和分类;（2）说话者和听话者在C-R-A模式简单模型下的认知过程;（3）低调陈述在C-R-A模式扩展模型下的普遍路径;（4）通过研究验证对C-R-A模式作了一定程度地改进。笔者认为，（1）低调陈述可以根据实现手段分为词汇低调陈述、语法低调陈述和语境低调陈述;（2）C-R-A模型简单模式过于简化，因此在运用此模型对低调陈述进行解读时应该将合作、关联和顺应这三个关键词深化和丰富;（3）参见图形5.9，低调陈述在C-R-A模型扩展模式下是一个合作、最佳关联和隐性顺应的成功交际过程;（4）C-R-A模型扩展模式在对复杂低调陈述言语行为进行解读时，应根据话轮的数量及交际双方是否具有共同的交际目的而片开，不应将复杂的交际过程局限在一个演示模型下。

One of the most unique properties of two-dimensional carbides and nitrides of transition metals(MXenes)is their excellent water dispersibility and yet possessing superior electrical conductivity but their industrial-scale application is limited by their costly chemical synthesis *** this work,the niche feature of MXenes was capitalized in the packed-bed electrochemical reactor to produce MXenes at an unprecedented reaction rate and yield with minimal chemical waste.A simple NH4F solution was employed as the green electrolyte,which could be used repeatedly without any loss in its ***,both fluoride and ammonium were found to play critical roles in the electrochemical etching,functionalization,and expansion of the layered parent materials(MAXs)through which the liberation of ammonia gas was *** electrochemically produced MXenes with excellent conductivity,applied as supercapacitor electrodes,could deliver an ultrahigh volumetric capacity(1408 F cm^(−3))and a volumetric energy density(75.8 Wh L^(−1)).This revolutionary green,energy-efficient,and scalable electrochemical route will not only pave the way for industrial-scale production of MXenes but also open up a myriad of versatile electrochemical modifications for improved functional MXenes.

Available Approaches of Remediation and Stabilisation of Metal Contamination in Soil: A Review

Anthropogenic activities, such as mining of natural resources, manufac-turing industries, modern agricultural practices and energy production have resulted in the release of heavy metals with resultant harmful im-pacts in some natural environments. Toxic heavy metals are harmful to living organisms even at low concentrations. Therefore, heavy metal contaminated sites should be remediated as heavy metals do not decompose into less harmful substances and are retained in the soil. Conventional methods are used for remediation of heavy metal contaminated soils such as heavy metal extraction, immobilization and removal of soils to landfill produce large quantities of toxic products including insoluble hydroxides and are rarely cost effective. The advent of bioremediation technologies like biosparging, bioventing and bioaugmentation has provided an alternative to conventional methods for remediating heavy metal contaminated soils. A subset of bacteria found in the rhizosphere has been found to increase the tolerance of plants to heavy metals in soil. These bacteria commonly known as plant growth promoting rhizobacteria or Plant Growth Promoting Rhizobacteria (PGPR) are showing promise as a bioremediation technique for the stabilisation and remediation of heavy metal contami-nated sites. PGPR can improve plant growth via a variety of mechanism including fixing atmospheric N to improve N status and making plants more tolerant of heavy metals. Scattered literature is harnessed to review the principles, advantages and disadvantages of the available technologies for remediating heavy metal contaminated soils and is presented.

Purpose:To examine the effects of a school-based karate intervention on academic achievement,psychosocial functioning,and physical fitness in children aged 7-8 ***:Twenty schools in 5 different European countries(2 second-grade classrooms per school)participated in a cluster randomized controlled trial(Sport at School trial).Participants were assigned to either a control group,which continued with their habitual physical education lessons,or to an intervention group,which replaced these lessons with a 1-year karate intervention(Karate Mind and Movement program).A total of 721 children(344 girls and 377 boys,7.4±0.5 years old,mean±SD)completed the study,of which 333 and 388 were assigned to the control group and intervention group,*** included academic performance(average grade),psychosocial functioning(Strengths and Difficulties Questionnaire for parents),and different markers of physical fitness(cardiorespiratory fitness,balance,and flexibility).Results:The intervention provided small but significant benefits compared to the control group for academic achievement(d=0.16;p=0.003),conduct problems(d=-0.28;p=0.003),cardiorespiratory fitness(d=0.36;p0.05).Conclusion:A 1-year school-based karate intervention was effective in improving academic achievement,conduct problems,and physical fitness in primary school *** results support the inclusion of karate during physical education lessons.

Coverage and Information Treatment of Zika in Ecuador:Analysis of News From National Newspapers El Universo and El Telégrafo

Zika is an infectious disease that emerged as an epidemic in Latin America in 2015 and arrived in Ecuador at the start of *** research analyzes the coverage that two national Ecuadorian newspapers gave to this sanitary *** El Telégrafo and El Universo,the first one of government domain and the other one of a private company,were chosen to be studied,and interviews with journalists who reported before,during and after the epidemic,were *** results showed that:In most news articles,a single source is handled,preferably the official or institutional one;that the majority of articles were about the international situation rather than local or national information;that alarm content is privileged over the preventive;and finally that the earthquake that hit the Ecuadorian coast in April 2016 weakened the coverage of Zika,even though the epidemic was at its outbreak at the time in the *** is a need for journalists to be trained to cover these types of emergencies in order to better inform the people and follow up diseases cycles in order to arrange their own news agenda and work more on solution approaches.

Time-to-event has become one of the primary endpoints of many clinical trials. Comparing treatments and therapies using time-to-event (or “survival”) data requires some care, since survival differences may occur either early or late in the follow-up period, depending on various factors such as the initial potency or the duration of efficacy of the drugs. In this work, we investigate the effect of the CIMAvax?EGF vaccine therapy on the survival of patients with non-small cell lung cancer, using stratified and unstratified weighted log-rank tests. Weighted log-rank tests are designed to identify early and late survival differences between treatments. Using these tests, we conclude that the vaccine is more efficient than the standard therapy among patients less than 60 years of age.

Effects of Velocity and Thermal Slip Conditions with Radiation on Heat Transfer Flow of Ferrofluids

To analyze the thermal convection of ferrofluid along a flat plate is the persistence of this study. The two-dimensional laminar, steady, incompressible flow past a flat plate subject to convective surface boundary condition, slip velocity in the presence of radiation has been studied where the magnetic field is applied in the transverse direction to the plate. Two different kinds of magnetic nanoparticles, magnetite Fe3O4 and cobalt ferrite CoFe2O4 are amalgamated within the base fluids water and kerosene. The effects of various physical aspects such as magnetic field, volume fraction, radiation and slip conditions on the flow and heat transfer characteristics are presented graphically and discussed. The effect of various dimensionless parameters on the skin friction coefficient and heat transfer rate are also tabulated. To investigate this particular problem, numerical computations are done using the implicit finite difference method based Keller-Box Method.
