Utility of short-term telemetry heart rhythm monitoring and CHA2DS2-VASc stratification in patients presenting with suspected cerebrovascular accident

BACKGROUND Inpatient telemetry heart rhythm monitoring overuse has been linked to higher healthcare *** To evaluate if CHA2DS2-VASc score could be used to indicate if a patient admitted with possible cerebrovascular accident(CVA)or transient ischemic attack(TIA)requires inpatient telemetry *** A total of 257 patients presenting with CVA or TIA and placed on telemetry monitoring were analyzed *** investigated the utility of telemetry monitoring to diagnose atrial fibrillation/flutter and the CHA2DS2-VASc scoring tool to stratify the risk of having CVA/TIA in these *** In our study population,63(24.5%)of the patients with CVA/TIA and telemetry monitoring were determined to have no ischemic neurologic *** the 194(75.5)patients that had a confirmed CVA/TIA,only 6(2.3%)had an arrhythmia detected during their inpatient telemetry monitoring *** with a confirmed CVA/TIA had a statistically significant higher CHA2DS2-VASc score compared to individuals without an ischemic event(3.59 vs 2.61,P<0.001).CONCLUSION Given the low percentage of inpatient arrhythmias identified,further research should focus on discretionary use of inpatient telemetry on higher risk patients to diagnose the arrhythmias commonly leading to CVA/TIA.A prospective study assessing event rate of CVA/TIA in patients with higher CHA2DS2-VASc score should be performed to validate the CHA2DS2-VASc score as a possible risk stratifying tool for patients at risk for CVA/TIA.

Derivation of energy-based base shear force coefficient considering hysteretic behavior and P-delta effects

A modified energy-balance equation accounting for P-delta effects and hysteretic behavior of reinforced concrete members is derived. Reduced hysteretic properties of structural components due to combined stiffness and strength degradation and pinching effects, and hysteretic damping are taken into account in a simple manner by utilizing plastic energy and seismic input energy modification factors. Having a pre-selected yield mechanism, energy balance of structure in inelastic range is considered. P-delta effects are included in derived equation by adding the external work of gravity loads to the work of equivalent inertia forces and equating the total external work to the modified plastic energy. Earthquake energy input to multi degree of freedom（MDOF） system is approximated by using the modal energy-decomposition. Energybased base shear coefficients are verified by means of both pushover analysis and nonlinear time history（NLTH） analysis of several RC frames having different number of stories. NLTH analyses of frames are performed by using the time histories of ten scaled ground motions compatible with elastic design acceleration spectrum and fulfilling duration/amplitude related requirements of Turkish Seismic Design Code. The observed correlation between energy-based base shear force coefficients and the average base shear force coefficients of NLTH analyses provides a reasonable confidence in estimation of nonlinear base shear force capacity of frames by using the derived equation.

Research on dynamics and stability of machin-ing operations has attracted considerable attention. Cur-rently, most studies focus on the forward solution ofdynamics and stability in which material properties and thefrequency response function at the tool tip are known topredict stable cutting conditions. However, the forwardsolution may fail to perform accurately in cases whereinthe aforementioned information is partially known or var-ies based on the process conditions, or could involve sev-eral uncertainties in the dynamics. Under thesecircumstances, inverse stability solutions are immenselyuseful to identify the amount of variation in the effectivedamping or stiffness acting on the machining system. Inthis paper, the inverse stability solutions and their use forsuch purposes are discussed through relevant examples andcase studies. Specific areas include identification of processdamping at low cutting speeds and variations in spindledynamics at high rotational speeds.

Liver transplantation: Current status and challenges

Great progress has been made in the field of liver transplantation over the past two decades. This progress, however, also brings up the next set of challenges: First, organ shortage remains a major limitation, and accounts for a large proportion of wait list mortality. While living donation has successfully increased the total number of liver transplants done in Asian countries, the total number of such transplants has been stagnant in the western hemisphere. As such, there has been a significant effort over the past decade to increase the existing deceased donor pool. This effort has resulted in a greater use of liver allografts following donation after cardiac death(DCD) along with marginal and extended criteria donors. Improved understanding of the pathophysiology of liver allografts procured after circulatory arrest has not only resulted in better selection and management of DCD donors, but has also helped in the development of mechanical perfusion strategies. Early outcomes demonstrating the clinical applicability of both hypothermic and normothermic perfusion and its potential to impact patient survival and allograft function have generated much interest. Second, long-term outcomes of liver transplant recipients have not improved significantly, as recipients continue to succumb to complications of long-term immunosuppression, such as infection, malignancy and renal failure. Furthermore, recent evidence suggests that chronic immune-mediated injury to the liver may also impact graft function.

Aetiology of idiopathic granulomatous mastitis

Idiopathic granulomatous mastitis is a rare chronic inflammatory lesion of the breast that can clinically and radiographically mimic breast carcinoma. The most common clinical presentation is an unilateral, discrete breast mass, nipple retraction and even a sinus formation often associated with an inflammation of the overlying skin. The etiology of idiopathic granulomatous mastitis is still obscure. Its treatment remains controversial. The cause may be the autoimmune process, infection, a chemical reaction associated with oral contraceptive pills, or even lactation. Various factors, including hormonal imbalance, autoimmunity, unknown microbiological agents, smoking and α 1-antitrypsin deficiency have been suggested to play a role in disease aetiology. In this review, causing factors in the aetiology of idiopathic granulomatous mastitis are reviewed in detail.

黏土矿物对硫浮选有负面影响。在本研究中,研究了黏土矿物(高岭石-蒙脱石)对黄铜矿浮选的影响,旨在提出能够消除负面影响的解决方案。进行ζ-电位测量和浮选实验,采用中心复合设计方法,选取起泡剂浓度、分散剂浓度、泡沫高度、空气流速和黏土量为变量。在对测试结果的评估中,将这5个重要变量定为因变量。随着黏土量的增加,黄铜矿品位降低,而黄铜矿回收率、黄铁矿回收率和动态泡沫稳定性增加。黄铜矿浮选的负相互作用顺序为蒙脱石>高岭石,说明使用蒙脱石可获得较大的气泡直径。对矿石中的黏土类型(高岭石和蒙脱石)适用的浮选条件进行数学建模,该模型预测了与矿石的黏土比的相关条件随时间的变化,以及浮选获得的品位回收值。

Evaluation of in vitro digestibility of Aspergillus oryzae fungal biomass grown on organic residue derived‑VFAs as a promising ruminant feed supplement

Background As demand for high quality animal feed continues to raise,it becomes increasingly important to mini-mize the environmental impact of feed *** appealing sustainable approach to provide feed fractions is to use organic residues from agro-food *** this regard,volatile fatty acids(VFAs)such as acetic,propionic and butyric acids,derived from bioconversion of organic residues can be used as precursors for production of micro-bial protein with ruminant feed inclusion *** study aims to investigate the in vitro digestibility of the Asper-gillus oryzae edible fungal biomass cultivated on VFAs-derived from anaerobic digestion of *** produced fungal protein biomass,along with hay clover silage and rapeseed meal were subjected to various in vitro assays using two-stage Tilley and Terry(TT),gas,and bag methods to evaluate and compare its digestibility for application in ruminant *** The produced fungal biomass contained a higher crude protein(CP)(41%–49%)and rather similar neutral detergent fiber(NDF)(41%–56%)compared to rapeseed *** rumen in vitro dry matter digestibility(IVDMD)of the fungal biomass in the TT method ranged from 82%to 88%(statistically similar to that of the gas method(72%to 85%)).The IVDMD of fungal biomass were up to 26%and 40%greater than that of hay clover silage and rapeseed meal,*** type of substrate and bag method had pronounced effect on the fermentation products(ammonium-N(NH4+-N),total gas and VFAs).Fungal biomass digestion resulted in the highest release of NH4+-N(340–540 mg/L)and the ratio of acetate to propionate ratio(3.5)among subjected *** The results indicate that gas method can be used as a reliable predictor for IVDMD as well as fermenta-tion ***,the high IVDMD and fermentation product observed for Aspergillus oryzae fungal biomass digestion,suggest that the supplementation of fungal biomass will contribute to improving the rumen diges
