Triple Negative Breast Cancer(TNBC)immunotherapy has recently shown promising ***,some TNBC patients presented with *** of the reasons was attributed to the excessive release of cytokines at the tumor microenvironment(TME)such as Tumor necrosis factor alpha(TNF-α)and Interleukin-10(IL-10).Fine regulation of these cytokines’levels via non-coding RNAs(ncRNAs)might alleviate the immune quiescent nature of TME at TNBC ***,the extrapolation of ncRNAs as therapeutic tools is highly ***,disentanglement the nature for the isolation of natural compounds that could modulate the ncRNAs and their respective targets is an applicable translational therapeutic ***,this study aimed to targeting the chief immune suppressive cytokines at the TME(TNF-αand IL-10)via ncRNAs and to examine the effects of Rosemary aerial parts extract on the expression levels of these ncRNAs in *** revealed miR-17-5p as a dual regulator of TNF-αand ***,an intricate interaction has been shown between miR-17-5p and the oncogenic lncRNAs:MALAT1 and *** down of MALAT1 and/or H19 caused an induction in miR-17-5p and reduction in TNF-αand IL-10 expression ***-17-5p was found to be down-regulated,while TNF-α,IL-10,MALAT1 and H19 were up-regulated in BC *** expression of miR-17-5p in MDA-MB-231 cells reduced TNF-α,IL-10,MALAT1 and H19 expression levels,as well as several BC *** a translational approach,ursolic acid(UA)isolated from rosemary induced the expression of miR-17-5p,MALAT1 and decreased H19 expression *** conclusion,this study suggests miR-17-5p as a tumor suppressor and an immune-activator miRNA in BC through tuning up the immunological targets TNF-α,IL-10 at the TME and the oncological mediators MALAT1 and H19 lncRNAs.

Cupping （Hijama in Arabic） is an ancient, holistic method for the treatment of a variety of diseases. Though the exact origin of cupping therapy is a matter of controversy, its use has been documented in early Egyptian and Chinese medical practices. Diverse human civilizations have contributed to the historical development and continuation of cupping therapy. This narrative review describes the history of cupping, historical definitions, cupping instruments and uses of cupping therapy. electronic searches of relevant databases （PubMed, Google Scholar and OvidSP） were conducted using keywords and Boolean operators. Manual searches and references of published articles and books were also conducted. A number of articles （N = 625） were retained for extensive review, and finally 83 articles were included in this paper. The historical descriptions of cupping therapy were found in ancient human civilizations of the Eastern and Western world. There were inconsistent data concerning the origin of cupping, definitions, instruments, procedures, definite advancements and research in Hijama over centuries. Cupping therapy fell out of favor in 17th and mid-18th centuries but recovered popularity in modem medicine. Currently, cupping therapy is used for health promotion, prophylaxis and treatment of a variety of diseases around the world. Cupping therapy with a good safety profile has a checkered history and is a well-recognized traditional method for managing medical conditions. Currently, the scope of cupping therapy is expanding, and a growing body of research is providing additional evidence-based data for the further advancement of cupping therapy in the treatment of a variety of diseases.

In the present investigation, the tensile properties of A356/Al2O3 nanocomposites at both ambient and elevated temperatures were studied. The A356/Al2O3 nanocomposites were fabricated using a combination between the rheocasting and squeeze casting techniques. The A356 matrix alloy was reinforced with Al2O3 nanoparticulates having average sizes of 60 nm and 200 nm with different volume fractions up to 5 vol%. The results revealed that the A356/Al2O3 nanocomposites exhibited better mechanical properties than the A356 monolithic alloy. Such improvement in the mechanical properties was observed at both room and elevated temperatures up to 300°C. Increasing the volume fraction and/or reducing the size of Al2O3 nanoparticulates increase both the tensile and yield strengths of the nanocomposites.

To assess the effect of the dispersion of Al2O3 nanoparticles into A356 Al alloy on both the electrical and thermal conductivities, A356/Al2O3 metal matrix nanocomposites (MMNCs) were fabricated using a combination of rheocasting and squeeze casting techniques. Two different sizes of Al2O3 nanoparticles were dispersed into the A356 Al alloy, typically, 60 and 200 nm with volume fractions up to 5 vol%. The effect of the nanoparticles size and volume fraction on the electrical and thermal conductivities was evaluated. The results revealed that the A356 monolithic alloy exhibited better electrical and thermal conductivities than the MMNCs. Increasing the nanoparticles size and/or the volume fraction reduces both the thermal and electrical conductivities of the MMNCs. The maximum reduction percent in the thermal and electrical conductivities, according to the A356 monolithic alloy, were about 47% and 38%, respectively. Such percentages were exhibited by A356/Al2O3MMNCs containing 5 vol% of nanoparticles having 60 and 200 nm, respectively.

Hypertensive diseases of pregnancy, mainly PET, consist of several diseases which are associated with increased morbidity and mortality rates both maternal and fetal wise. Its incidence among pregnant females is about 3% - 10%. This study included 500 pregnant PET women, and they categorized to mild and severe forms, after exclusion of cases with essential hypertension, renal disease and SLE. All pregnant ladies underwent history taking, abdominal examination (general and local), obstetric ultrasound, Doppler ultrasound, laboratory investigations;treatment received (antihypertensive medications, magnesium sulphate (MgSO4) and steroids) and finally follow up till delivery. Regarding complications, PET can result in serious fetal/neonatal or maternal complications. Background: Peutz-Jeghers syndrome (PJS) is a rare autosomal dominant disorder characterized by gastrointestinal hamartomatous polyps and mucocutaneous pigmentations in the mouth, facial skin, hands & feet. Small bowel obstruction, intussusception, bleeding, intestinal and extra-intestinal malignancies are the major complications of PJS. The aim of this study is to analyze the clinical characteristics, preoperative diagnosis, and surgical management of PJS associated-intussusception in adults. Patients and Methods: This study included 5 cases with intussusception in PJS patients presented to Surgical Oncology Unit, General Surgery Department, Tanta University Hospital, Egypt and Hamad General Hospital, Hamad Medical Corporation, Qatar, between October 2011 and March 2016. Patients’ demographics were collected. After thorough clinical examination, abdominal X-ray, US, & CT scan were done. All the patients were submitted to midline laparotomy with resection anastomosis of the affected bowel segment. Results: The mean age was 28.4 years. Female: male ratio was 3:2. Abdominal pain was the most common presenting complaint with or without intestinal obstruction manifestations. Palpable abdominal mass was found in 3 patients (60%). Intussusception was proved pre-operatively in all the cases by abdominal ultrasound and CT scan. The intussusception was found in the jejunum in 3 patients, ileum in 1 patient, & in 1 patient, there was double intussusception (one jejunal & one ileo-cecal). Histopathological examination revealed the presence of typical Peutz-Jeghers hamartomatous polyp. No morbidity or mortality was reported at a mean follow-up period of 32 months. Conclusion: Family history, physical examination, abdominal ultrasound and CT scan were important in the diagnosis of acute intussusception caused by PJS. Surgical management of PJS associated intussusception is the recommended treatment to relieve patient’s symptoms and to avoid missing underlying malignancy. Patient

Background: We have developed a Hip Fracture Classification which stratifies patients into 4 groups upon admission according to fracture complexity. This classification considers patho-anatomic and physiologic parameters and was developed to help identify high-risk patients, with more surgical demands and consequently higher incidence of morbidity. Materials and Methods: Data was gathered prospectively for a cohort of 273 consecutive patients admitted over a 12- month period between 2008 and 2009 and classified according to the Hip Fracture Complexity Classification (C0 - C3). The following outcome measures (mortality and length of hospital stay) were reviewed at thirty days and one year. Result: The overall mortality was 4.4% at 30 days and 20% at 1 year. Purpose: The introduction of light weight three-dimension meshes into the field of inguinal hernia repair showed excellent outcomes. Ultrapro Hernia System (UHS) is one of these three-dimension meshes that allow reinforcing the pre-peritoneal space with minimal fixation. It improves the patient quality of life with rapid resumption of the daily activities. The aim of this study is to evaluate the short-term outcomes of open mesh repair of primary complex inguinal hernia using UHS mesh. Methods: Between November 2013 and November 2015, seventy five male patients with complex primary inguinal hernia were submitted to open inguinal hernia mesh repair using the UHS. Results: The mean age was 46.1 years. Thirty three patients had inguinoscrotal hernias, 14 with Nyhus type IIIA, 22 with Nyhus type IIIB, & 6 patients had strangulated hernias. The mean operative time was 48 minutes. No operative complications were recorded. Post-operatively, the mean VAS scale on 1 day, 1 week, & 1 month was 3.22, 1.2, & 0.3 respectively. The mean duration of oral analgesics use was 2.1 days. None of the patients reported any chronic pain at 12 months post-operatively. Two cases of superficial wound infection were recorded that were treated by oral antibiotics. No recurrence was reported during a mean follow-up period of 26 months. Conclusion: Inguinal hernia repair using UHS is an effective technique that combines the advantages of the anterior and the pre-peritoneal approaches. It improves the patient’s quality of life with minimal recurrence rates. A larger number of patients with longer follow up periods are needed to increase the validity of our results.

目的:评估羊膜移植(AMT)在治疗睑球黏连中的应用效果。方法:这项无对照干预性病例研究于2013/01~2015/12,包括连续就诊的12名患者共14眼。患者均接受永久性羊膜移植,以治疗睑球黏连,使用冷冻保存或冷冻干燥的羊膜(AM)。这12例(14眼)患者,年龄在26~62岁,平均43.38±11.25岁,其中8例(4名男性,4名女性)10眼为翼状胬肉切除术后继发,4例患者(1名男性,3名女性)斜视手术后继发。所有患者距上次手术至少6mo。术后以恢复深度稳定的穹窿为手术成功,无瘢痕或炎症发生,6mo的随访期间无眼球运动的限制。结果:随访期平均为7±4.2mo(6-9mo)。在所有14只眼中,手术后3wk均观察到AM完全上皮化,手术部位无炎症表现。总共14眼中有8眼(成功率57%)显示穹窿重建成功,穹窿深,无复发。有4眼(29%)表现出部分成功,中度深度的穹窿和中度瘢痕。有2眼(14%)显示重建失败,穹窿完全闭合。有7眼手术后视力改善,7眼视力保持稳定。AMT术后并发症少,14眼中1眼(7%)发生严重的结膜反应和眼球运动受限。而在术后前3mo中,14眼中有2眼发生化脓性肉芽肿(14%),予以手术切除,局部糖皮质激素注射。结论:单用AMT是一种治疗睑球黏连安全有效的方法。考虑到与角膜缘切除有关的潜在不良反应,AMT也是一种有效的重建穹窿以修复各种眼表疾病所致睑球黏连的方法。
