Laryngopharengeal Reflux in Gastroesophageal Reflux Disease: Does “Silent Laryngopharengeal Reflux” Really Exist?

Background: Gastroesophageal reflux disease (GERD) is a disorder resulting from the reversed flow of gastroduodenal contents into the esophagus, and producing different symptoms, while laryngopharyngeal reflux (LPR) is a disorder resulting from the reversed flow of gastric contents into the hypopharynx. The aim of this work is to evaluate LPR in cases of GERD. Methods: The present study was performed on fifty GERD patients diagnosed by gastroscope. LPR was assessed by reflux symptoms score (RSI) and reflux finding score (RFS). Accordingly, patients are classified into: Group I = 25 patients with manifest LPR, and Group II = 25 control patients without LPR symptoms. Results: GERD accounts for 17.4% of attendants of gastroscope unit, where manifest LPR accounts for 29.1% of GERD cases recording mean RSI and RFS 16.48 and 8.44 respectively. Silent LPR accounts for 8% recording mean RFS 7. Conclusion: There is a significant direct proportional relationship between severity of GERD and the RSI and RFS (p = 0.015 and 0.005 respectively).

BACKGROUND Hepatocellular carcinoma(HCC)is the fifth most common *** expression of microRNAs(miRNAs)-326,miRNA-424,and miRNA-511 has been associated with the diagnosis and prognosis of HCC in different ***,limited information is available regarding their expression in Egyptian HCC *** To assess the role of circulating miRNAs-326,miRNA-424,and miRNA-511 in Egyptian HCC *** This prospective observational study included 70 HCC patients and 25 healthy *** circulating levels of these three miRNAs were evaluated by real-time *** operating characteristic curve analysis was used to test the diagnostic accuracy of micro RNA expression *** All miRNAs were differentially expressed in HCC patients;miRNAs326 and miRNA-424 were upregulated,while miRNA-511 was *** miRNA-326 and miRNA-424 showed sensitivity and specificity of 97%,71.4%,and 52%,60%,respectively,to differentiate HCC from ***,miRNA-326 was associated with survival and could differentiate between Child grades(A vs B);miRNA-424 significantly differentiated early vs intermediate stages of HCC;while miRNA-511 was significantly correlated with response to modified Response Evaluation Criteria in Solid Tumors(m RECIST).CONCLUSION We conclude that miRNA-326,miRNA-424,and miRNA-511 have diagnostic and prognostic roles in Egyptian patients with hepatitis C virus-related HCC and should be considered for better disease management.

BACKGROUND Pancreatic cystic lesions(PCLs) are common in clinical practice. The accurate classification and diagnosis of these lesions are crucial to avoid unnecessary treatment of benign lesions and missed opportunities for early treatment of potentially malignant *** To evaluate the role of cyst fluid analysis of different tumor markers such as cancer antigens [e.g., cancer antigen(CA)19-9, CA72-4], carcinoembryonic antigen(CEA), serine protease inhibitor Kazal-type 1(SPINK1), interleukin 1 beta(IL1-β), vascular endothelial growth factor A(VEGF-A), and prostaglandin E2(PGE2)], amylase, and mucin stain in diagnosing pancreatic cysts and differentiating malignant from benign *** This study included 76 patients diagnosed with PCLs using different imaging modalities. All patients underwent endoscopic ultrasound(EUS) and EUS-fine needle aspiration(EUS-FNA) for characterization and sampling of different *** The mean age of studied patients was 47.4 ± 11.4 years, with a slight female predominance(59.2%). Mucin stain showed high statistical significance in predicting malignancy with a sensitivity of 87.1% and specificity of 95.56%. It also showed a positive predictive value and negative predictive value of 93.1% and 91.49%, respectively(P < 0.001). We found that positive mucin stain, cyst fluid glucose, SPINK1, amylase, and CEA levels had high statistical significance(P < 0.0001). In contrast, IL-1β, CA 72-4, VEGF-A, VEGFR2, and PGE2 did not show any statistical significance. Univariate regression analysis for prediction of malignancy in PCLs showed a statistically significant positive correlation with mural nodules, lymph nodes, cyst diameter, mucin stain, and cyst fluid CEA. Urban transportation,especially bus transportation,is an important sign of development in every city in the *** average waiting time for passengers at correspondence stations of buses is one of the most important measures of effectiveness of bus *** the best of our knowledge,the studies in the literature are about maximizing the number of synchronizations in those correspondence stations whose objective is to minimize the waiting time in the *** classical definition of synchronization used in the literature related to a time *** this work,we introduce a new definition of synchronization of two buses in network *** this context,we present a mathematical formulation of the synchronization bus timetabling problem as a multi-objective program,where we use the new meaning for synchronization of two buses in the network *** the problem is NP-hard,we adapt a multi-agent approach to solve *** experiments show that after adapting the multi-agent approach using our proposed definition,we obtain high-quality solutions compared to the classical definition.

In this paper, we apply the method of directly defining the inverse mapping introduced by Liao and Zhao [On the method of directly defining inverse mapping for nonlinear differential equations, Numer. Algorithms 72（4）（2016） 989-1020] to the problem of prostate cancer immunotherapy. We extend this method in two directions： first, we apply the method to a system of nonlinear ordinary differential equation, and second, we propose a new technique for finding the base functions in the considered algorithm.

This study presents a new systematic algorithm to optimize the durability of reinforced recycled aggregate *** proposed algorithm integrates machine learning with a new version of the firefly algorithm called chaotic based firefly algorithm(CFA)to evolve a rational and efficient predictive *** CFA optimizer is augmented with chaotic maps and Levy flight to improve the firefly performance in forecasting the chloride penetrability of strengthened recycled aggregate concrete(RAC).A comprehensive and credible database of distinctive chloride migration coefficient results is used to establish the developed algorithm.A dataset composite of nine effective parameters,including concrete components and fundamental characteristics of recycled aggregate(RA),is used as input to predict the migration coefficient of strengthened RAC as output,k-fold cross validation algorithm is utilized to validate the hybrid *** numerical benchmark analyses are applied to prove the superiority and applicability of the CFA algorithm in predicting chloride *** show that the developed CFA approach significantly outperforms the firefly algorithm on almost tested functions and demonstrates powerful *** addition,the proposed strategy can be an active tool to recognize the contradictions in the experimental results and can be especially beneficial for assessing the chloride resistance of RAC.
