Botulinum Toxin Type A Satisfaction in Different Neurological Disorders

Background: The botulinum toxin type A (BoNT-A) is used in a wide range of neurological diseases. We aimed to study the overall patients/caregivers’ satisfaction with BoNT-A treatment in different neurological conditions. Methods: Prospective study included patients who had received at least two BoNT-A treatment sessions. They were asked to rate overall treatment satisfaction at the peak of treatment effect on a 1 to 10 scale (1 = not at all satisfied;10 = fully satisfied). Subjects with a rating of 1to3 were classified as not at all satisfied, those with a rating of 4 to7 as somewhat satisfied, and those with a rating of 8 to10 as very satisfied. Treatment satisfaction questionnaire for medicine (TSQM) was assessed at the end of observational period. Quality of life QOL was assessed before BoNT-A treatment and at the last visit. Results: The study was conducted from first April 2014 to August 2021. 548 patients were identified with mean age 43.66 ± 14.50. Most of participants 389 (71%) were female. At the end of observational period, the mean satisfaction was 7.28 ± 1.78. There was a highly significant difference (P P P = 0.001). Conclusion: Satisfaction with BoNT-A therapy for different neurological disorders is overall good. The highest patient satisfaction was observed with primary focal HH, and the least satisfaction was observed in writer’s cramp. BoNT-A therapy improved QOL.

Assessment of the Diagnostic Value of Plasma Level of Von Willebrand Factor and ADAMTS13 in Patients with Cerebral Infarction

Background: Von Willebrand Factor (VWF) is a large multimeric plasma gly-coprotein that plays an important role in primary haemostasis. VWF together with ADAMTS13 plays a key role in arterial thrombosis. VWF is integral for platelet adhesion to collagen fibres and atherosclerotic plaues and platelets aggregation under high shear conditions. Upon vascular wall damage, plasma VWF binds to collagen in the exposed endothelial matrix and platelet glycoprotein triggers platelet aggregation and thrombous formation. In the early stages of acute ischaemic stroke, VWF increased significantly and the ADAMTS13 level decreased. The increase of plasma VWF and FVIII in a patient with ischaemic stroke indicated an increased risk of complications and severe neurological deficit. Objective: to evaluate the validity of VWF and ADFAMTS13 as diagnostic markers of cerebral infarction by comparing the two markers’ validity in diagnosis. Methods: this case control study enrolled 38 cerebral infarction patients 20 acute infarction and 18 recurrent infarction. Patients were compared with 65 age and sex with control patients. We performed the VWF assay using the minividas immunofluorescence technique and the ADAMTS13 assay using the ELISA technique. Results: VWF level of acute infarction patients (280.97 ± 109.83 IU/dl) while its level in recurrent infarction (264.93 ± 172.8 IU/dl) were highly significantly increased compared to control (75.8 ± 16.24 IU/dl). ADAMTS13 level of acute infarction patients was (506.11 ± 225.24 ng/dl) and (1386.03 ± 667.64 ng/dl) in recurrent infarction patients with highly significant differences between the acute and control group and non-significant different between recurrent and control. ADAMTS13 is an excellent diagnostic test in acute cerebral infarction but a weak diagnostic test in recurrent cerebral infarction but it is a good diagnostic test in differentiation between acute and recurrent cerebral infarction patients. The Salvadora persica(***)*** stick,usually known as miswak,is still being em-ployed as an oral hygiene agent for plaque and gingivitis *** study aims to assess the antibacterial,antibiofilm,antioxidant,and phytochemical profile of *** *** ***-sica was purchased from a local market,grinded and extracted with petroleum *** disk dif-fusion,microdilution,and micro-plate assays were performed to evaluate the antibacterial and an-tibiofilm activities of the prepared extract at different concentrations against𝛽-lactam resistance Streptococcus *** radical scavenging 2,2-diphenyl-1-picrylhydrazyl(DPPH)and stable radical cationic chromophore,2,2-azinobis-(3-ethylbenzothiazoline-6-sulfonate)(ABTS)methods were used to determine their antioxidant *** and spectrometric analyses were performed using gas chromatography-mass(GC-MS)*** minimum inhibitory concentration(MIC)of *** extract against𝛽-lactam resistance Streptococcus species ranged from 6.25 to 12.5 mg/*** maximum suppression of biofilm formation by *** extract was observed at MIC with a percentage of 68.66%,against Streptococcus *** *** ex-tract exhibited antioxidant activity with IC 50 of 20μg/mL and 35μg/mL from DPPH and ABTS,*** phytochemical characterization showed the presence of 22 compounds with ma-jor compounds;benzyl isothiocyanate(36.21%)and n-hexadecanoic acid(27.62%).The *** extract exhibited antibacterial activity against𝛽-lactam resistant Streptococcus species,showing a promising natural alternative that could be a treatment option.

The present study addressed the properties affecting the activity of a new heterogeneous catalyst Fe/Ti-NaY in the ultrasonic irradiated for decolorization of amaranth dye in aqueous solution. The catalyst was prepared by the ion exchange and impregnation method. Different characterization techniques, i.e. XRD, AFM, TEM and SEM/EDAX were done to characterize the properties of the fresh and used catalyst. This catalyst was able to keep its stability and high activity without any noticeable reduction in the decolorization efficiency of amaranth after three cycles of reaction. The X-ray diffraction proved the high stability and the crystallinity for the reuse catalyst. The COD removal for the fresh and used catalyst with and without calcination was 57%, 36% and 20% respectively. The hydrophilic properties of the heterogeneous catalyst increased slightly due to the loading of Ti and Fe ions.

Postoperative Analgesia Following Caesarean Section: Intravenous Patient Controlled Analgesia Versus Conventional Continuous Infusion

Background: Management of postoperative pain after caesarean section (C/S) requires a balance between pain relief and undesirable side effects of drugs and technique. In order to improve postoperative pain management after caesarean section, we compared intravenous patient controlled analgesia (IV-PCA) with our current hospital practice, which is continuous opioid infusion. Method: We enrolled one hundred and twenty patients in our prospective randomized trial after an uneventful elective caesarean section under spinal anaesthesia. All patients received 0.5 mg/kg bolus of pethidine on first complaint of pain or at 120 minutes after institution of spinal anaesthesia. Depending upon the randomization, Group P received IV-PCA with 0.15 mg/kg bolus pethidine with 10-minute lockout and Group C received continuous pethidine infusion at a rate of 0.15 mg/kg/hr. Statistical analysis: For qualitative variables means and standard deviations were computed and analyzed by T-test, Mann Whitney U test and repeated measures ANOVA. Frequency and percentages were computed for qualitative data and analyzed by Chi-Square and Fischer exact test. A p-value of less than 0.05 was treated as significant. Results: The numeric rating score for pain, need for rescue analgesia and incidence of nausea and vomiting was significantly lower (p-value < 0.001) in IV-PCA group as compared to continuous infusion group at 6, 12 and 24 hours postoperatively, 98% of the patients were satisfied with pain management in Group P as compared to 70% (p < 0.001) in Group C. Conclusion: Our results showed improved pain control, less need for rescue analgesia for breakthrough pain, lower incidence of nausea and vomiting and greater patient satisfaction with IV-PCA. In the absence of preservative free narcotics for intrathecal use, postoperative pain management can be significantly improved by using IV-PCA instead of continuous opioid infusion in patients undergoing caesarean section.

In the last few years, interest in burnup calculations using Monte Carlo methods has increased. Previous burnup codes have used diffusion theory for the neutronic portion of the codes. Diffusion theory works well for most reactors. However, diffusion theory does not produce accurate results in burnup problems that include strong absorbers or large voids. MCNPX code based on Mont Carlo Method, is used to design a three dimensional model for a BWR fuel assembly in a typical operating temperature and pressure conditions. A test case was compared with a benchmark problem and good agreement was found. The model is used to calculate the distribution of pin by pin power and flux inside the assembly. The effect of axial variation of water （coolant） density, and of control rods motion on the neutron flux and power distribution is analyzed. The effect of addition of Gd2O3 to natural uranium （0.711%） on both the thermal neutron flux and normalized power are analyzed. The concentration of U^235, U^238, Pu^239, and its isotopes is also calculated at burn-up 50 GWD/T.

AIM： To investigate whether the degree of rectal distension could define the rectum functions as a conduit METHODS： Response of the rectal and anal pressure to 2 types of rectal balloon distension, rapid voluminous and slow gradual distention, was recorded in 21 healthy volunteers （12 men, 9 women, age 41.7 ± 10.6 years）. The test was repeated with sphincteric squeeze on urgent sensation. RESULTS： Rapid voluminous rectal distension resulted in a significant rectal pressure increase （P 〈 0.001）, an anal pressure decline （P 〈 0.05） and balloon expulsion. The subjects felt urgent sensation but did not feel the 1^st rectal sensation. On urgent sensation, anal squeeze caused a significant rectal pressure decrease （P 〈 0.001） and urgency disappearance. Slow incremental rectal filling drew a rectometrogram with a ＂tone＂ limb representing a gradual rectal pressure increase during rectal filling, and an ＂evacuation limb＂ representing a sharp pressure increase during balloon expulsion. The curve recorded both the ist rectal sensation and the urgent sensation. CONCLUSION： The rectum has apparently two functions： transportation （conduit） and storage, both depending on the degree of rectal filling. If the fecal material received by the rectum is small, it is stored in the rectum until a big volume is reached that can affect a degree of rectal distension sufficient to initiate the defecation reflex. Large volume rectal distension evokes directly the rectoanal inhibitory reflex with a resulting defecation.
