Feasibility of Upfront Debulking Surgery versus Neoadjuvant Chemotherapy Followed by Interval Debulking Surgery for Advanced Ovarian Cancer

Background: Inappropriately ovarian cancer cannot be detected until an advanced stage. Radical debulking surgery is considered the cornerstone in the management of advanced ovarian cancer pointing to complete tumor resolution. Unless optimal debulking cannot be achieved, these patients gain little benefit from surgery. Neoadjuvant chemotherapy (NACT) has been recommended as a novel therapeutic modality to a diversity of malignant tumors when the disease is not willing to optimal surgical resection at the time of diagnosis or the patient who unfit for aggressive debulking surgery. The purpose of this study is to compare survival in the patient with advanced ovarian cancer (stage III/IV) underwent primary debulking surgery followed by adjuvant chemotherapy (PDS-ACTR) to those who received neoadjuvant chemotherapy followed by interval debulking surgery (NACT-IDS). Results: Neoadjuvant chemotherapy (NACT-IDS) showed significant complete cytoreduction and decreased in surgical morbidity in comparison to primary debulking surgery (PDS-ACTR). NACT-IDS showed significant improvement in progression-free survival (P-value 0.002) and overall survival (P-value 0.03) in comparison to PDS-ACTR. Response to NACT and residual volume were the two independent prognostic factors for overall survival. Conclusion: NACT-IDS for advanced ovarian cancer (III/IV) resulted in higher frequency of complete resection with no residual tumor, less post-operative surgical morbidity and significant increase progression-free survival and overall survival. Both responses to NACT and residual tumor volume were the two independent prognostic factors for survival in ovarian cancer.

A Single Institutional Phase II Randomized Trial of Whole Brain Radiation Therapy with or without Irinotecan for the Treatment of Brain Metastases from Solid Tumours

Background: The relatively suboptimal results of whole brain radiation therapy (WBRT) alone in eradication of brain metastases and an attempt to improve outcomes with WBRT have led to studies combining radiotherapy with chemotherapy drugs that could act as radiosensitizers with a rationale of improving local tumor control. Materials and Methods: This randomized phase II study evaluated the use of Irinotecan concomitant with 37.5 Gray (Gy) of WBRT in 2.5 Gy daily fractions × 5 days each week for 3 weeks versus Whole WBRT alone in patients with brain metastasis (BM) from solid tumors. Fifty patients were randomized to receive either WBRT alone or concomitant with three irinotecan IV infusions 80 mg/m2, 2 hrs before RT on days 1, 8, and 15. Results: The objective response rate (ORR) was significantly improved in patients receiving Irinotecan with radiotherapy versus radiotherapy alone (48% vs. 28%;p = 0.048). The median time to progression of brain metastasis was significantly longer in the irinotecan and WBRT arm as compared to the WBRT arm (8 vs. 5 months;p < 0.001). There was no significant difference in survival between treatment arms (p = 0.361). Irinotecan with radiotherapy was generally well tolerated and did not interfere with the delivery of WBRT. Conclusions: Irinotecan concomitant with WBRT was well tolerated and significantly improved local control of BM compared with WBRT alone. These findings require confirmation in a phase III trial with addition of quality of life assessment.

Lissajous Orbits at the Collinear Libration Points in the Restricted Three-Body Problem with Oblateness

In the present work, the collinear equilibrium points of the restricted three-body problem are studied under the effect of oblateness of the bigger primary using an analytical and numerical approach. The periodic orbits around these points are investigated for the Earth-Moon system. The Lissajous orbits and the phase spaces are obtained under the effect of oblateness.

Predictors of Complete Early Virological Response to Pegylated Interferon and Ribavirin in Egyptian Patients with Chronic Hepatitis C Genotype 4

We aim to determine the baseline factors associated with partial and cEVR by analyzing the data of 1861 Egyptian patients treated for 12 weeks with a course of Peg-IFN plus RBV. Base line data of 1861 Egyptian patients with chronic hepatitis C coming at Cairo-Fatemic Hospital for HCV treatment were studied including full clinical, Ultrasonographic examination, laboratory evaluation and liver biopsy. The most significant variables in relation to complete early virological response were low Hb level (13 gm/dl) with p 0.01, the stage of fibrosis p value < 0.05 and the grades of inflammation p value were associated with less achievement of c EVR. We conclude that identifying the most significant predictors of response such as Hb, stage of fibrosis F, at baseline before initiating treatment is mandatory to predict which patient will be more expected to achieve a cEVR and thus reducing the side-effects and healthcare costs associated with interferon therapies.

Coronary heart disease is among the most prevalent and costly of all global health problems. Coronary artery bypass grafting (CABG) has been increasingly used since the 1960s. Coronary heart disease is a leading cause of death worldwide, including Sudan. Self-efficacy and functional capacity is a critical factor for quality of life in patients who has undergone CABG as well as for their caregivers from their families. Aim of the Study: To explore the effects of an educational program on self-efficacy and functional capacity of patients after CABG. 1) There will be no significant difference between the intervention group and control group concerning knowledge regarding Self-efficacy and functional capacity post CABG surgery. 2) The application of our educational program for the patients will enhance the patients with CABG surgery and improve knowledge about Self-efficacy and functional capacity post CABG surgery. 3) There are some socio demographic factors that affect the response to the educational program. Methods: This was a quasi-experimental, interventional and hospital-based study, including pre-test, a post-test and follow-up test, with a control group, conducted during the period from September 2014 to June 2019. Patients were recruited consecutively to the cases and control group using convenience sampling. The sample consisted of 76 patients;41 as cases and 35 as control group. The sample included those who had undergone CABG in Khartoum State. Relevant data were collected using a checklist to monitor the effect of a health education program on self-efficacy and functional capacity of the patients. Result: The results showed marked improvement in patients’ self-efficacy and functional capacity between pretest and posttest for the study group;p-value = (0.001) in the health status of cases in comparison to the control group. Project portfolio management is a major challenge for some *** most organizations,there are a large number of projects active at the same time,some not necessarily delivering value or not aligned with their strategic *** universities face a lot of uncertainties when selecting and prioritizing the projects that make up their *** addition,the achievement of those who are aligned with the strategy of the university becomes a great *** to ensure good project portfolio management,the implementation of selection and prioritization methods and processes becomes *** the project portfolio management to be effective,it is necessary to establish a structured method adapted to the needs and strategy of the *** this context,this paper proposes a method for selecting and prioritizing projects within the framework of the portfolio management dedicated to universities,which can promote harmony between the university’s strategy,the needs and the priority objectives for enable better *** method is based on the processes of the COBIT 5 good practice framework,and on the multi-criteria decision-making methods AHP,TOPSIS and the WSM technique,thus,it proposes seven project selection criteria based on the five axes IT governance strategies and two catalysts derived from COBIT 5 *** evaluation and validation of this method was applied in the portfolio management of the Abdelmalek Essaadi Moroccan University(AUE).The result shows that this proposed method has made it possible to make a better selection and prioritization of the portfolio of projects of Abdelmleek Essaadi University having the most value.

Immunotherapies boosting the immune system's ability to target cancer cells are promising for the treatment of various tumor types,yet clinical responses differ among patients and ***,there has been increasing interest in novel cancer immunotherapy practices aimed at triggering T cell-mediated anti-tumor ***-directed cytotoxicity mediated by T lymphocytes has become a central focal point in the battle against cancer utilizing the immune *** molecular and cellular mechanisms involved in the actions of T lymphocytes have directed new therapeutic approaches in cancer immunotherapy,including checkpoint blockade,adoptive and chimeric antigen receptor(CAR)T cell therapy,and cancer *** review addresses all the strategies targeting tumor pathogenesis,including metabolic pathways,to evaluate the clinical significance of current and future immunotherapies for patients with cancer,which are further engaged in T cell activation,differentiation,and response against tumors.

Clinical Significance of Serum Galectin-1 and Its Tissue Immunohistochemical Expression in Serous Ovarian Carcinoma Patients

Objectives: Serous ovarian carcinoma (SOC) is the commonest ovarian carcinoma type with poor prognosis due to early metastasis and first presentation with advanced stage. In this work, we investigated serum level of Galactin-1 (Gal-1) and its tissue immunohistochemical expression in SOC patients at different stages trying to find out its significance as a diagnostic and prognostic marker. Patients and methods: The study included 95 females I-Control group: Twenty five healthy females;II-Patients group: Seventy females diagnosed as SOC at different stages;Stage I: 8 cases, Stage II: 12 cases, Stage III: 32 cases and Stage VI:18 cases. Serum Galectin-1 and CA-125 were measured by ELIZA and tissue Galectin-1 was assessed by immunohistochemistry. All patients were followed for up to 3 years after surgery. Results: Serum Gal-1 and CA-125 levels were significantly higehr in SOC patients compared to controls (p 0.001). We found a direct positive statistically significant correlation between serum Gal-1 and CA125 levels (p 0.001). Serum Gal-1 at cut off value > 135 ng/ml was superior to CA-125 a cut off value > 49 u/ml with sensitivity, specificity of 100%, vs 88.57, 96% for CA-125. Serum Gal-1 was significantly associated with tumor stage (p 0.001). Immunohistochemistry showed that patients with strong Gal-1 expression had higher serum level (p = 0.002). Stromal and tumor Gal-1 expression were significantly correlated with tumor grade (p 0.001) and stage (p = 0.001). Serum Gal-1, CA-125 and IHC Gal-1 expression were associated with poor survival (p 0.001, p = 0.009 and p = 0.002) respectively. Conclusion: Serum Gal-1 and its tissue IHC expression are useful diagnostic and prognostic markers for SOC patients.

Antihyperglycemic effect of Juniperus phoenicea *** alloxan-induced diabetic rats and diterpenoids isolated from the fruits

Objective:To explore the traditional use of Juniperus phoenicea L.(***)growing in Egypt as antidiabetic ***:The antihyperglycemic activities of the crude 80%ethanol and successive extracts of leaves and fruits of the plant were investigated in alloxan-induced diabetic rats after collecting blood samples through retro-orbital puncture *** a consequence of the biological results,phytochemical investigation of the chloroform fraction of fruits was carried out by column chromatography and thin layer ***:Results revealed the reduction in blood glucose levels in rats,which were significantly different from control at 4 and 8 weeks(P<0.01).The highest antihyperglycemic activity was exhibited by the crude extracts of fruits and leaves of which the potency was 83.6%and 81.9%,respectively,after 8 weeks,comparing to metformin drug(100%potency).Chloroform fractions of leaves and fruits were the most potent fractions(potencies were 70.3%,71.4%),respectively,along with ethyl acetate fraction of fruits(71.4%).Phytochemical investigating of the chloroform fraction of fruits resulted in the isolation and identification of 5 abietane ***,7-dehydroabietanone,sugiol,6-α-hydroxy-7-oxoferruginol,totarolone and a labdane diterpenoid,varodiol were isolated for the first time from the fruits of *** growing in *** identification of these compounds was based on spectroscopic analysis:1hydrogen-nuclear magnetic resonance and electron impact mass spectrometry,comparing the results with the ***:It has become clear that leaves and fruits of the Egyptian *** provide effective antihyperglycemic action in diabetic rats as was reported in folk *** high contents of terpenoids in the non-polar fractions may attribute to the antidiabetic effect of the plant.

Treatment Outcome of Pharmacokinetics-Based Dosing of Docetaxel and Fluorouracil in Advanced Head and Neck Cancer Patients

Introduction: Docetaxel, Cisplatin and 5-Fluorouracil (DPF) became the standard induction chemotherapy in advanced Head and Neck Cancer (HNC) but associated with high toxicity rate. Several studies reported higher response rates with better tolerability when chemotherapy dose is calculated based on Pharmacokinetics (PK) versus conventional Body Surface Area (BSA). Patients and Methods: Thirty nine patients with stage III and IV HNC who received induction DPF were included in the study. Dose of cycle 1 was BSA-based then Docetaxel and 5-FU doses were PK-adjusted starting from cycle 2 whereas Cisplatin dose was BSA-based throughout the study. Results: After median follow up period of 14 months the median overall survival (OS) and progression free survival (PFS) were 15.1 and 10.6 months respectively. Twenty nine patients were available for response assessment. Seven patients (24.1%) achieved complete response while partial response encountered in 19 patients (65.5%) with and Overall response rate of 89.6%. Both treatment related side effects and mortality significantly decreased after the application of PK dose adjustments (p-value 0.007 and 0.01 respectively). Conclusion: PK-guided dose adjustments of 5-FU and Docetaxel in DPF regimen can significantly decrease the treatment related side effects and mortality without compromising the tumor response rate. A randomized clinical trial is needed to compare the PK-guided dose adjustment with the standard BSA based protocol.
