What Happens to Modifiable Cardiovascular Risk Factors over Time in Type 2 Diabetes? Experience from a Developing Country under Standard of Care Conditions

Introduction: The complications of diabetes mellitus are the main reason for the morbidity associated with this condition, its cost, and its effect on quality of life. We have previously reported on the high prevalence of complications on a cohort of diabetic subjects. In the current study, we inquire what happens to the risk factors associated with these complications over time. Methods: The study is a prospective observational report over a two-year period where patients presented to the outpatient clinic for their usual care and had laboratory tests, retinal exams, and clinical assessment. They were treated as per American Diabetes Association guidelines in the outpatient department of the American University of Beirut. Results: Out of 220 subjects, 200 completed the two-year evaluation. There was no change in metabolic indices such as HbA1C, BMI, or smoking rates. However, there was mild, but significant improvement in LDL-C levels and blood pressure control. In parallel, there was an increase in the use of statins and ARB/ACEI category. However, their use remained much lower than in the reported literature. The population fared worse than the National Health Assessment and Nutrition Examination Survey (NHANES) population in most diabetes quality of care measures. The predicted 10-year vascular risk was 19%, similar to the calculated risk in a German population study. Conclusion: The lack of significant metabolic improvement points to a lack of effective lifestyle implementation measures, as well as to an under utilization of medical therapy. The approach to complications prevention needs to be more intense and multidisciplinary in nature in order to improve the patients’ outcome.

A critical review of microplastics in aquatic ecosystems: Degradation mechanisms and removing strategies

Plastic waste discarded into aquatic environments gradually degrades into smaller fragments,known as microplastics(MPs),which range in size from 0.05 to 5 *** ubiquity of MPs poses a significant threat to aquatic ecosystems and,by extension,human health,as these particles are ingested by various marine organisms including zooplankton,crustaceans,and fish,eventually entering the human food *** contamination threatens the entire ecological balance,encompassing food safety and the health of aquatic ***,developing effective MP removal technologies has emerged as a critical area of ***,we summarize the mechanisms and recently reported strategies for removing MPs from aquatic *** combining physical and chemical pretreatments with microbial degradation have shown promise in decomposing *** such as bacteria,fungi,algae,and specific enzymes are being leveraged in MP remediation *** advancements have focused on innovative methods such as membrane bioreactors,synthetic biology,organosilane-based techniques,biofilm-mediated remediation,and nanomaterial-enabled strategies,with nano-enabled technologies demonstrating substantial potential to enhance MP removal *** review aims to stimulate further innovation in effective MP removal methods,promoting environmental and social well-being.

Standardization of meibomian gland dysfunction in an Egyptian population sample using a non-contact meibography technique

AIM:To develop normative data for meibomian gland dysfunction(MGD)parameters,using non-contact meibography technique of Sirius Costruzione Strumenti Oftalmici(CSO)machine,in an Egyptian population ***:Observational,cross-sectional,analytic study,in which 104 Egyptian volunteers were *** upper lids were examined,using“Sirius CSO”*** eyelid was given a degree of meibomian gland loss(MGL),which was calculated by the software of the ***:Mean percentage MGL in right upper lid was of 30.9%±12.6%,and that of left upper lid was 32.6%±11.8%.Thirty-four volunteers(32.7%)had first-degree MGL in their right upper lid,and 67.3%had second-degree *** volunteer(1%)had zero-degree MGL in left upper lid,28(26.9%)had first-degree loss,and 75(72.1%)had second-degree *** of MGL in right upper eyelid was not related to age,but degree of MGL in left upper eyelid increased with *** was statistically significant difference between both genders for degree of MGL in right eye(P=0.036)and in left eye(P=0.027).CONCLUSION:Noncontact meibography is a useful non-invasive tool for diagnosing *** is diagnosed in 100%of apparently normal individuals;26.9%-32.7%of which have first-degree MGL,and 67.3%-72.1%have second-degree MGL.

Understanding the solid target dynamics resulting from the interaction with an ultrashort laser pulse is a challenging fundamental multi-physics problem involving atomic and solid-state physics,plasma physics,and laser *** of the initial interplay of the underlying processes is essential to many applications ranging from lowpower laser regimes like laser-induced ablation to high-power laser regimes like laser-driven ion *** the properties of the so-called pre-plasma formed as the laser pulse’s rising edge ionizes the target is complicated from the theoretical and experimental point of view,and many aspects of this laser-induced transition from solid to overdense plasma over picosecond timescales are still open *** the one hand,laser-driven ion acceleration requires precise control of the pre-plasma because the efficiency of the acceleration process crucially depends on the target properties at the arrival of the relativistic intensity peak of the *** the other hand,efficient laser ablation requires,for example,preventing the so-called“plasma shielding”.By capturing the dynamics of the initial stage of the interaction,we report on a detailed visualization of the pre-plasma formation and ***-thin diamond-like carbon foils are shown to transition from solid to plasma during the laser rising edge with intensities<10^(16)W/cm^(2).Single-shot near-infrared probe transmission measurements evidence sub-picosecond dynamics of an expanding plasma with densities above 10^(23)cm^(−3)(about 100 times the critical plasma density).The complementarity of a solid-state interaction model and kinetic plasma description provides deep insight into the interplay of initial ionization,collisions,and expansion.

Introduction:Cupping therapy is an integrative and complementary medicine therapy that practiced worldwide by several civilizations for thousands of *** therapy is reported to be a relatively safe practice especially if performed by qualified *** hemorrhage(SCH)is a common cause of eye redness and can be classified into traumatic and *** presentation:This case report discussed an adverse event related to wet cupping *** patient developed SCH one day after performing cupping *** was completely recovered after 2 *** case report should increase the knowledge of cupping therapists regarding increasing safety of patient when performing wet cupping therapy on specific body parts especially,head,neck and cervical ***,it highlighted the importance of conducting research regarding the mechanical and physiological effects of cupping ***:Adverse events related to cupping therapy should be reported with every *** therapists should be aware of this possible mild adverse *** the best of our knowledge,this is the first published adult case of SCH related to wet cupping therapy *** studies are critically needed for evaluation of cupping therapy effects and adverse events especially when performed on head and neck.

Building Custom Spreadsheet Functions with Python: End-User Software Engineering Approach

End-user computing empowers non-developers to manage data and applications, enhancing collaboration and efficiency. Spreadsheets, a prime example of end-user programming environments widely used in business for data analysis. However, Excel functionalities have limits compared to dedicated programming languages. This paper addresses this gap by proposing a prototype for integrating Python’s capabilities into Excel through on-premises desktop to build custom spreadsheet functions with Python. This approach overcomes potential latency issues associated with cloud-based solutions. This prototype utilizes Excel-DNA and IronPython. Excel-DNA allows creating custom Python functions that seamlessly integrate with Excel’s calculation engine. IronPython enables the execution of these Python (CSFs) directly within Excel. C# and VSTO add-ins form the core components, facilitating communication between Python and Excel. This approach empowers users with a potentially open-ended set of Python (CSFs) for tasks like mathematical calculations, statistical analysis, and even predictive modeling, all within the familiar Excel interface. This prototype demonstrates smooth integration, allowing users to call Python (CSFs) just like standard Excel functions. This research contributes to enhancing spreadsheet capabilities for end-user programmers by leveraging Python’s power within Excel. Future research could explore expanding data analysis capabilities by expanding the (CSFs) functions for complex calculations, statistical analysis, data manipulation, and even external library integration. The possibility of integrating machine learning models through the (CSFs) functions within the familiar Excel environment.

Robust Designs Through Risk Sensitivity:An Overview

This is an overview paper on the relationship between risk-averse designs based on exponential loss functions with or without an additional unknown(adversarial)term and some classes of stochastic *** particular,the paper discusses the equivalences between risk-averse controller and filter designs and saddle-point solutions of some corresponding risk-neutral stochastic differential games with different information structures for the *** of the by-products of these analyses is that risk-averse controllers and filters(or estimators)for control and signal-measurement models are robust,through stochastic dissipation inequalities,to unmodeled perturbations in controlled system dynamics as well as signal and the measurement *** paper also discusses equivalences between risk-sensitive stochastic zero-sum differential games and some corresponding risk-neutral three-player stochastic zero-sum differential games,as well as robustness issues in stochastic nonzero-sum differential games with finite and infinite populations of players,with the latter belonging to the domain of mean-field games.

Increasing spatial coverage in rough terrain and vegetated areas using InSAR optimized pixel selection:application to Tohoku,Japan

One of the major limitations of using Interferometric Synthetic Aperture Radar(InSAR)in time series analysis is the low-phase coherence associated with rough terrain and vegetated areas,which results in limited spatial coverage in such *** scatterers technique was introduced to overcome this limitation using time-series ***,identifying major scatterers within a pixel requires the single-looked pixels oversampling which can be a demanding process especially with large interferometric stacks and vast study ***,using multilooked temporal coherent pixels was proposed to increase processing efficiency and coverage by utilizing distributed targets,but this technique may exclude pixels with reliable phase returns because of their temporal varying neighboring *** this paper,we propose a technique to identify multilooked temporal stable pixels with reliable phase returns independent of their neighboring *** conduct a simulation analysis to relate the spatial coherence of a pixel with its expected temporal correlation in the time series analysis *** found that a liberal temporal correlation threshold of 0.53 in multilooked pixels stack is equivalent to a spatial coherence threshold of 0.2 when using number of looks of 9,which is considered acceptable in temporal coherent pixels,in terms of phase standard *** these findings to study the 2011 Tohoku earthquake in the northeastern part of Japan resulted in increasing the number of usable pixels and spatial coverage index by nearly 50.4%and 36.8%,respectively,compared to the temporal coherent ***,we propose an approach to integrate GPS observations with InSAR time series analysis,which resulted in deformation maps of the megathrust 2011 Tohoku earthquake with mean RMSE of 11.4 mm and a correlation of 98%in comparison to GPS observations.

A survey of RDF data management systems

RDF is increasingly being used to encode data for the semantic web and data exchange. There have been a large number of works that address RDF data manage- ment following different approaches. In this paper we pro- vide an overview of these works. This review considers cen- tralized solutions （what are referred to as warehousing ap- proaches）, distributed solutions, and the techniques that have been developed for querying linked data. In each category, further classifications are provided that would assist readers in understanding the identifying characteristics of different approaches.
