Surgical management of high-grade pancreatic injuries: Insights from a high-volume pancreaticobiliary specialty unit

BACKGROUND The management of high-grade pancreatic trauma is *** To review our single-institution experience on the surgical management of blunt and penetrating pancreatic *** A retrospective review of records was performed on all patients undergoing surgical intervention for high-grade pancreatic injuries [American Association for the Surgery of Trauma(AAST) Grade Ⅲor greater] at the Royal North Shore Hospital in Sydney between January 2001 and December 2022. Morbidity and mortality outcomes were reviewed, and major diagnostic and operative challenges were *** Over a twenty-year period, 14 patients underwent pancreatic resection for highgrade injuries. Seven patients sustained AAST Grade Ⅲinjuries and 7 were classified as Grades Ⅳ or Ⅴ. Nine underwent distal pancreatectomy and 5 underwent pancreaticoduodenectomy(PD). Overall, there was a predominance of blunt aetiologies(11/14). Concomitant intra-abdominal injuries were observed in 11 patients and traumatic haemorrhage in 6 patients. Three patients developed clinically relevant pancreatic fistulas and there was one in-hospital mortality secondary to multi-organ failure. Among stable presentations, pancreatic ductal injuries were missed in two-thirds of cases(7/12) on initial computed tomography imaging and subsequently diagnosed on repeat imaging or endoscopic retrograde cholangiopancreatography. All patients who sustained complex pancreaticoduodenal trauma underwent PD without mortality. The management of pancreatic trauma is evolving. Our experience provides valuable and locally relevant insights into future management *** We advocate that high-grade pancreatic trauma should be managed in high-volume hepatopancreato-biliary specialty surgical units. Pancreatic resections including PD may be indicated and safely performed with appropriate specialist surgical, gastroenterology, and interventional radiology support in tertiary centres.

Clinical analysis of propofol deep sedation for 1,104 patients undergoing gastrointestinal endoscopic procedures:A three year prospective study

AIM： To analyze the hemodynamic and respiratory effects of propofol on patients undergoing gastroscopy and colonoscopy. METHODS： In this prospective study, conducted over a period of three years, 1,104 patients referred for a same day GI endoscopy procedure were analyzed. All patients were given a propofol bolus （0.5-1.5 mg/kg）. Arterial blood pressure （BP） was monitored at 3 rain intervals and heart rate and oxygen saturation （SpO2） were recorded continuously by pulse oximetry. Analyzed data acquisition was carried out before, during, and after the procedure. RESULTS： A statistically significant reduction in mean arterial pressure was demonstrated （P〈0.001） when compared to pre-intervention values, but severe hypotension, defined as a systolic blood pressure below 60mmHg, was noted in only 5 patients （0.5%）. Oxygen saturation decreased from 96.5% to 94.4 % （P〈0.001）. A critical decrease in oxygen saturation （〈90%） was documented in 27 patients （2.4%）. CONCLUSION： Our results showed that propofol provided good sedation with excellent pain control, a short recovery time and no significant hemodynamic side effelts if carefully titrated. All the patients （and especially ASA Ⅲ group） require monitoring and care of an anesthesiologist.

Biochemical markers for non-invasive assessment of disease stage in patients with primary biliary cirrhosis

AIM: To evaluate different biochemical markers and their ratios in the assessment of primary biliary cirrhosis (PBC) stages. METHODS: This study included 112 patients with PBC who underwent a complete clinical investigation. We analyzed the correlation (Spearman's test) between ten biochemical markers and their ratios with different stages of PBC. The discriminative values were compared using areas under receiver operating characteristic (ROC) curves. RESULTS: The mean age of patients included in the study was 53.88 ± 10.59 years, including 104 females and 8 males. We found a statistically significant correlation between PBC stage and Aspartate aminotransferase (AST), Alanine aminotransferase (ALT) to platelet ratio (APRI), ALT/platelet count, AST/ALT, ALT/AST and ALT/Cholesterol ratios, with the values of Spearman's rho of 0.338, 0.476, 0.404, 0.356, 0.351 and 0.325, respectively. The best sensitivity and specificity was shown for AST/ALT, with an area under ROC of 0.660. CONCLUSION: Biochemical markers and their ratios do correlate with different sensitivity to and specificity of PBC disease stage. The use of biochemical markers and their ratios in clinical evaluation of PBC patients may reduce, but not eliminate, the need for liver biopsy.

Feasibility of breaking up sitting time in mainstream and special schools with a cognitively challenging motor task

Background: Children spend 70% of the school day sitting in class. Classroom-based active breaks can benefit children's physical health, but if the breaks are cognitively demanding(i.e., combine physical exertion and mental engagement), they may also improve focus and cognitive functions. Teachers and students play a crucial role in the successful implementation of active breaks, and their perspectives are critical to the feasibility of these strategies. The aim of this study was to assess the feasibility of implementing a cognitively challenging motor task as an active break in mainstream and special primary ***: A total of 5 teachers in 2 mainstream schools and 7 teachers in 1 special school(attended by children with neurodevelopmental disorders) attended a 20-min training on how to implement a 4-min cognitively challenging active break, before conducting a feasibility trial(twice a day for 1 week). To understand individual perceptions, one-on-one semistructured interviews were conducted before and after the trial with teachers, and focus group interviews were conducted with typically developing children after the trial. Questions were based on a predefined framework for feasibility studies. All interviews were audio recorded, transcribed and analyzed in NVivo 11 using a framework approach. A total of 12 teachers(11 females; 7 between 20 and 34 years old) and 34 children(16 girls; 9.3 § 1.7 years, mean § SD) participated in the ***: In mainstream schools, teachers viewed the cognitively challenging motor task as appropriate and potentially beneficial for children's health and focus. Children reported enjoying the active breaks. Teachers in special schools viewed the task as complex and potentially frustrating for children. Biomass responses to intraspecific competition differ between wild species and CO_(2)

Aims How growth of wild and crop species responds to global environmental perturbations has both ecological and agricultural significance in a changing *** primary aim of this synthesis was to quantitatively assess the interactive effects of intraspecific competition and elevated CO_(2) on biomass production in herbaceous *** Using meta-analytical techniques,we synthesized data from publications before 2006 that reported biomass responses to elevated CO_(2) in 321 herbaceous species grown in isolation or in competition with con-specific *** findings Intraspecific competition differentially modified biomass responses to elevated CO_(2) in wild and crop *** example,competition reduced CO_(2) stimulation of total biomass(WT)from 27 to 23%in wild species,but by a much greater magnitude,i.e.,from 43 to 32%in *** had no effect on responses of either above-(WAG)or below-ground(WBG)biomass to elevated CO_(2) in wild species,but significantly diminished CO_(2) enhancement of WAG,although not of WBG,in *** variations were found among functional groups in the modification of growth responses to elevated CO_(2) by intraspecific competition,which exerted greater depression on CO_(2) enhancement in C3 than in C4 species and in non-legumes than in *** CO_(2) affected leaf and stem growth of individually grown C3 graminoids and forbs similarly,but increased leaf growth only in C4 graminoids and stem growth only in C4 *** from this synthesis demonstrated that intraspecific competition differentially affected growth responses to elevated CO_(2) in wild and crop *** wildcrop species differences will have important implications for understanding primary production by herbaceous species in both natural and agricultural ecosystems in the future when atmospheric CO_(2) is significantly higher than the current level.

Ultra-processed foods have known negative implications for health;however,their effect on skeletal development has never been ***,we show that young rats fed ultra-processed food rich in fat and sugar suffer from growth retardation due to lesions in their tibial growth *** bone mineral density decreases significantly,and the structural parameters of the bone deteriorate,presenting a sieve-like appearance in the cortices and poor trabecular parameters in long bones and *** results in inferior mechanical performance of the entire bone with a high fracture *** sequence analysis of the growth plates demonstrated an imbalance in extracellular matrix formation and degradation and impairment of proliferation,differentiation and mineralization *** findings highlight,for the first time,the severe impact of consuming ultra-processed foods on the growing *** pathology extends far beyond that explained by the known metabolic effects,highlighting bone as a new target for studies of modern diets.

Parkinson’s disease is the most common movement disorder,affecting about 1%of the population over the age of 60 ***’s disease is characterized clinically by resting tremor,bradykinesia,rigidity and postural instability,as a result of the progressive loss of nigrostriatal dopaminergic *** addition to this neuronal cell loss,Parkinson’s disease is characterized by the accumulation of intracellular protein aggregates,Lewy bodies and Lewy neurites,composed primarily of the proteinα-*** it was first described almost 200 years ago,there are no disease-modifying drugs to treat patients with Parkinson’s *** addition to conventional therapies,non-pharmacological treatment strategies are under investigation in patients and animal models of neurodegenerative *** such strategies,environmental enrichment,comprising physical exercise,cognitive stimulus,and social interactions,has been assessed in preclinical models of Parkinson’s *** enrichment can cause structural and functional changes in the brain and promote neurogenesis and dendritic growth by modifying gene expression,enhancing the expression of neurotrophic factors and modulating *** this review article,we focus on the current knowledge about the molecular mechanisms underlying environmental enrichment neuroprotection in Parkinson’s disease,highlighting its influence on the dopaminergic,cholinergic,glutamatergic and GABAergic systems,as well as the involvement of neurotrophic *** describe experimental pre-clinical data showing how environmental enrichment can act as a modulator in a neurochemical and behavioral context in different animal models of Parkinson’s disease,highlighting the potential of environmental enrichment as an additional strategy in the management and prevention of this complex disease.
