Non-Neutropenic Sepsis in Cancer Patients: The Luminal Obstruction Syndrome

Background: Neutropenic sepsis is a frequent complication of cytotoxic chemotherapy delivered for cancer patients. Its management is well determined by clinical guidelines. Non-neutropenic sepsis is another potential complication in cancer patients. Its management is less established in the medical literature. Materials and Methods: Three cases are presented to illustrate favourable evolution of non-neutropenic biliary-, uro- and bronchogenic-sepsis in cancer patients with poor prognosis. Results: All three patients had a survival over six months after the management of the septic complication. Two of them received subsequent systemic anticancer treatment. Conclusions: Survival benefit offered by the management of cancer-related non-neutropenic sepsis may be comparable to the benefit obtained by systemic anticancer treatments. Cost-effectiveness of sepsis management may be better than that of anticancer treatments.

Severity of gastroesophageal reflux disease influences daytime somnolence: A clinical study of 134 patients underwent upper panendoscopy

AIM: To asses the relationship between severity of gastroesophageal refluxe disease and Epworth sleepiness scale as an indicator of daytime somnolence. METHODS: One hundred and thirty-four patients underwent an upper panendoscopy as indicated by the typical reflux symptoms and were also investigated with regard to somnolence. Sleepiness was evaluated by Epworth Sleepiness Scale, which was compared to the severity of endoscopic findings (Savary-Miller/modified by Siewert). Patients with psychiatric disorders or being on sedato-hypnotics as well as shift workers were excluded from the study. The relationship between the severity of the reflux disease and daytime somnolence was analyzed with the help of multivariate regression analysis. RESULTS: A positive tendency was found between the severity of the reflux disease and the corresponding Epworth Sleepiness Scale. In the case of the more severe type-Savary-Miller Ⅲ- at least a mild hypersomnia was found. For this group daytime somnolence was significantly higher than in the case of the non-erosive type of Gastroesophageal Reflux Disease representing the mildest stage of reflux disease. CONCLUSION: The severity of Gastroesophageal Reflux Disease influences daytime somnolence.

Objective:To investigate the antioxidant activity,total phenolic and total tannin content of the pericarp and the seed of Coffea benghalensis(***) and Coffea liberica compared to Coffea arabica(***).Methods:The antioxidant potential,total tannin and polyphenol contents of the immature and mature seed and pericarp of *** and Coffea liberica were quantified and compared to *** chemiluminescence(ECL).2,2-diphenyl-1-picrylhydruzyl(DPPH).oxygen radical absorbance capacity,Folin-Ciocaltcau method and total tannin content assays were ***:Trolox equivalent(TE/g plant material) values obtained by ECL and DPPH methods showed loose correlation(r^2=0.587)while those measured by oxygen radical absorbance capacity assay were higher without correlation in each plant.A closer correlation was detected between the ECL method and the percentage antioxidant activity of the DPPH technique(r^2=0.610 7) in each species,however the immature pericarp of *** showed much higher DPPH scavenging potential than was seen in the ECL *** immature pericarp of *** expressed the highest tannin and polyphenol content,and a high polyphenol level was also detected in the immature seed of *** immature pericarp of Bengal and Liberian coffees showed the largest amount of phenolic ***:The obtained data highlight the potential role of *** as a new source of natural antioxidants and polyphenols compared to ***.

Hyperuricemia-mediated uric acid crystal formation may cause joint inflammation and provoke the destruction of joints through the activation of inflammasome-mediated innate immune ***,the immunopathological effects and underlying intracellular regulatory mechanisms of uric acid crystal-mediated activation of fibroblast-like synoviocytes(FLS)in rheumatoid arthritis(ra)have not been ***,we investigated the in vitro effects of monosodium urate crystals,alone or in combination with the inflammatory cytokines tumor-necrosis factor(TNF)-a or interleukin(IL)-1b,on the activation of human FLS from ra patients and normal control subjects and the underlying intracellular signaling mechanisms of treatment with these *** urate crystals were able to significantly increase the release of the inflammatory cytokine IL-6,the chemokine CXCL8 and the matrix metalloproteinase(MMP)-1 from both normal and ra-FLS(all P,0.05).Moreover,the additive or synergistic effect on the release of IL-6,CXCL8 and MMP-1 from both normal and ra-FLS was observed following the combined treatment with monosodium urate crystals and TNF-a or *** experiments showed that the release of the measured inflammatory cytokine,chemokine and MMP-1 stimulated by monosodium urate crystals were differentially regulated by the intracellular activation of extracellular signal-regulated kinase and c-Jun N-terminal kinase pathways but not the p38 mitogen-activated protein kinase *** results therefore provide a new insight into the uric acid crystal-activated immunopathological mechanisms mediated by distinct intracellular signal transduction pathways leading to joint inflammation in ra.

Electrorheological(ER) finishing utilizes the flow of electrically stiffened abrasive fluid through a preset converging gap formed by the work-piece surface and a moving tool. An ER finishing tool characterized by cathode integrated with anode together is proposed,whose electric field distribution is finite-element-analyzed(FEA) and is useful to finish both conductive work-piece and non-conductive ones. Experiments were performed to finish a K9 glass by this tool. After 30 minutes polishing,the surface roughness was reduced from 8.46 to 2.53 nm ra which is better than previously reported 2.9 nm. The result verified the validity of the integrated-electrodes tool for non-conductive optical glasses.

Electrorheological（ER） finishing is a promising technique for polishing optical mirrors lenses. Silicone oil-based ER finishing fluid with ceria particles as abrasive was developed, whose dispersed phase was alumina,titanium dioxide, silica, and starch, respectively. Experiments were performed in detail under the voltage of 3000V, after 25 min of polishing a K9 glass. By using the ER fluid developed in this paper, with 47.62% starch, 47.62%silicone oil, and 4.76% ceria, the surface roughness of the K9 glass was reduced from 8.46 nm rato 3.45 nm ra. The result verifies the validity of the developed ER fluid.

Firms need cloud computing adoption for strategic and competitive goals, generating business value, and at last gaining competitive advantage. This study reviews the literature regarding cloud computing and IT governance, and presents a research model along with its hypotheses formulation to examine the factors impacting cloud computing perceived importance in several Arab firms, specifically Jordan, Saudi Arabia and United Arab Emirates by using the integration of Technology Acceptance Model (tam) model and Technology-Organizational-Environmental (TOE) framework as adapted from [1]. 329 returned surveys from top, middle-level IT managers, and IT employees from the operational level of the studied firms were analyzed using the structural equation modeling technique. The study found relative advantage, compatibility, complexity, organizational readiness, top management commitment, and training and education as important variables for impacting cloud computing adoption using perceived ease of use and perceived usefulness as mediating variables. The model explained 61%, 63%, and 74% of cloud computing adoption for perceived usefulness, perceived ease of use and perceived importance respectively.

This paper focuses on the process of electrorheological （ER） finishing optical surfaces. Experiments on K9 mirrors were conducted. In one experiment, the operating distance was varied over 0.5-0.8 mm with the voltage at 2000 V. The maximum peak-to-valley （PV） reduction was obtained at the distance of 0.5 ram, where the PV value was reduced from 58.71 to 25.03nm. In another experiment, the voltage was varied over 1500- 3000V with operating distance at 0.5mm. The final surface roughness （ra） achieved was as low as 2.5 rim. A higher voltage produced a higher relative reduction of the ra. These experimental results validated the process.

用形态计量学方法对口腔颊粘膜的正常粘膜上皮(N)、慢性非特异性炎症(IF)、单纯性过角化(HK)、纤维增生(FH)、扁平苔藓(LI)、白斑(LP)、乳头状瘤(PP)、癌旁上皮(CE)、鳞癌癌巢(CI)的上皮基底层细胞及胞核的大小(包括体积、面积和胞核/胞浆比值)、进行了测量和形态计量相关分析(MCA)。结果显示 CI 组在分布图上最偏离 N 组,其次是 CE 组,LI 与 LP 组位置非常接近并邻近于 CE和 CI 组;此外发现 IF,FH,HK 及 PP 组与 N 组的位置最为接近。对于病变个体的 MCA 分析显示 N 组最为集中,IF,FH,HK 及 PP 的分布相对比较集中,围绕或与 N 组相重叠;而 CI 组与 N 组无重叠且偏离一边,LI,LP 及 CE 组位于 CI 与 N 及其它良性病变组之间并部分与 CI 重叠。本文结果揭示通过 MCA方法可以较好的反映颊部粘膜各病变所处的病变状态、各病变之间的关系以及个体病变的病变程度。应用 MCA 的方法将有助于癌前病变的早期发现以及确定癌变组内的不同级别。
