The effects of oral smokeless tobacco administration on endurance performance

Background: Smokeless tobacco is widely used by athletes to enhance performance. nicotine is a central nervous system stimulant and acts on cardiocirculatory and metabolic systems, involving tissue blood flow and circulatory vasoreactivity. The aim of this study was to investigate the effects of the oral smokeless tobacco(Swedish snus(SS)) on the perception of fatigue and time to exhaustion(TTE) during moderate-intensity aerobic ***: Fourteen healthy non-tobacco male users were recruited for a double-blind, controlled crossover design(SS vs. snus placebo(SP)).Subjects were tested for 3 sessions: experimental session 1(Exp1) consisted of an incremental test to determine the maximal aerobic power output(Wmax), whereas Exp2 and Exp3 consisted of exercising at 65%Wmaxuntil exhaustion in SS or SP conditions. During Exp2 and Exp3, muscle and cerebral oxygenation was assessed by means of near-infrared spectroscopy, and the rating of perceived exertion(RPE) was ***: Comparing SS with SP tests, significant differences(p nd in the values of cerebral(～3%) and muscular tissues oxygenation(～4%) in the first 30 min of exercise. The RPE values were not significantly different between the 2 conditions(SS vs. SP). no significant difference was found in TTE(SS: 54.25 § 21.84 min; SP: 50.01 § 17.03 min).Conclusion: This study showed that muscular and cerebral oxygenation increased significantly with snus administration during an endurance exercise until exhaustion, but this did not affect fatigue perception and TTE. The results showed that snus could not be considered an ergogenic substance in non-tobacco users.

nine-year experience of doxorubicin-eluting beads chemoembolization for hepatocellular carcinoma

BACKGROUnD: Chemoembolization with doxorubucin-eluting beads (DEB) has been used to treat hepatocellular carcinoma (HCC) since 2007. This study compared the efifcacy and sur-vival between transarterial chemoembolization (TACE) with DEB and conventional approach (cTACE) in HCC treatment. METHODS: This retrospective case-control study compared the overall survival and tumor response of HCC patients to cTACE (n=190) and DEB (n=143) by the reassessment of com-puted tomography and serum alpha-fetoprotein (AFP). Mul-tivariate analysis was used to determine the factors affecting tumor response. RESULTS: The median post-treatment to pre-treatment AFP level was 0.8 for a DEB session (n=258) and 1.0 for a cTACE session (n=452), showing a signiifcantly greater decrease in AFP after DEB (Pnts in the DEB group achieved objective response (complete and partial) compared with those in the cTACE group (Pnse after DEB vs cTACE was 34.8% vs 15.4% in 0-3 months (P=0.001), 37.1% vs 20.0% in 3-6 months (Pnd 50.0%vs 30.0% in 6-12 months (P=0.093). DEB predicted a 3.604 times odds of achieving at least one objective tumor response in a patient when compared to cTACE (Pn survival from ifrst transcatheter therapy of patients having undergone at least once DEB was 12.53 months, while those having received cTACE only was 10.53 months (P=0.086). A tendency of improved survival appeared to maintain until >80 months after the ifrst TACE session in the DEB group. COnCLUSIOn: DEB is a safe alternative to cTACE in HCC pa-tients with better therapeutic efifcacy.

利用紫外光谱、荧光光谱、红边激发荧光位移(REES)法、圆二色谱(CD)结合分子模拟技术共同研究了模拟生理条件下杨梅素与牛血清白蛋白(BSA)的相互作用,阐述了相互作用机制.分子模拟结果表明,杨梅素与蛋白在亚结构域II A的疏水腔内结合,主要作用力为疏水作用力和氢键.依据荧光猝灭法判断猝灭机制为静态猝灭,并得到不同温度下药物与蛋白相互作用的结合常数(Ka)及结合位点数(n),根据热力学参数判断出作用力类型,并且计算出杨梅素与蛋白的结合距离,与分子模拟得到的判定结果基本一致.通过紫外光谱、同步荧光光谱以及REES法获得的信息讨论了相互作用时BSA中色氨酸(Trp)微环境的变化;并利用CD谱的测定结果定量计算了BSA二级结构中α-螺旋含量的变化.

Hybrid Procedure Utilizing Stent Grafts to Stabilize Distal Flaps after Common and Superficial Femoral Endarterectomy

Background: Endarterectomy has long been the standard for common femoral artery (CFA) occlusive disease. Hybrid procedures utilizing endovascular and open techniques have recently been used for revascularization. The purpose of this study was to evaluate the effectiveness of the use of a stent graft to stabilize the distal flap and prevent further dissection after extensive endarterectomy. Methods: All patients from Monmouth Medical Center in Long Branch, nJ from September 2008 to March 2013 who underwent an extensive common and superficial femoral (SFA) endarterectomy combined with the use of a Viabahn (Gore Medical) stent graft to stabilize the distal flap were included in the study. These stents were deployed in the proximal SFA after extensive endarterectomy, under direct visualization, without the aid of fluoroscopy. Due to the location in the SFA, these flaps were not amenable to suture tacking. Results: Fifteen patients met these criteria and were included in our study. Twelve patients underwent femoral endarterectomy for severe claudication and three patients for limb salvage. Technical success was achieved in all 15 patients. Five patients also had stents placed proximally to increase inflow and one patient had an additional stent placed distally to improve outflow. There were no intraoperative or postoperative complications. Conclusion: Stent graft placement allows a more extensive endarterectomy to be performed by stabilizing the distal flap allowing a safe transition into the true lumen that is not possible with suture tacking.

Stress Analysis of Bolted Joints Part II. Contact and Slip Analysis of a Four Bolt Joint

For up-to-date bolted joints, first of all in vehicles, high strength bolts of 10.9 or even 12.9 are used, which are preloaded up to 90% or even 100% of the yield strength. The primary aim of this high degree utilization is the weight reduction. For the analytic dimensioning of bolted joints, the VDI 2230 Richtlinien German standard [1] provides support. However, the analytic model can mostly consider the true structural characteristics only in a limited way. The analytic modeling is especially uncertain in case of multiple bolted joints when the load distribution among the bolts depends reasonably upon the elastic deformation of the participating elements in the joints over the geometry of the bolted joint. The first part of this paper deals with the problems of numerical modeling and stress analysis, respectively specifying the analytic dimensioning procedure by applying elastic or rather elastic-plastic material law. The error magnitude in bolted joint calculation was examined in case of omitting the existing threaded connection—between the bolt and the nut—in order to simplify the model. The second part of the paper deals with the dimensioning of stands and cantilevers’ multi-bolt fixing problems, first of all, with the load distribution among the bolts keeping in view the analysis of the local slipping relations. For demonstrating the above technique, an elaborated numeric procedure is presented for a four-bolted cantilever, having bolted joints pre-tightened to the yield strength.
