Pegylated interferon (PEG-IFN) has become standard therapy for hepatitis C virus (hCV) infection. We evaluated whether PEG-IFN pharmacodynamics and pharmacokinetics account for differences in treatment outcome and whether these parameters might be predictors of therapeutic outcome. Twenty-four IFN-nave, hCV/human immunodeficiency virus-coinfected patients received PEG-IFN α-2b (1.5 μg/kg) once weekly plus daily ribavirin (1,000 or 1,200 mg) for up to 48 weeks. hCV RNA and PEG-IFN αconcentrations were obtained from samples collected frequently after the first 3 PEG-IFN doses. We modeled hCV kinetics incorporating pharmacokinetic and pharmacodynamic parameters. Although PEG-IFN concentrations and pharmacokinetic parameters were similar in sustained virological responders (SVRs) and nonresponders (NRs), the PEG-IFN α-2b concentration that decreases hCV production by 50%(EC50) was lower in SVRs compared with NRs (0.04 vs. 0.45 μg/L [P = .014]). Additionally, the median therapeutic quotient (i.e., the ratio between average PEG-IFN concentration and EC50 [ ˉC/EC50]), and the PEG-IFN concentration at day 7 divided by EC50 (C(7)/EC50) were significantly increased in SVRs compared with NRs after the first (10.1 vs. 1.0 [P = .012], 2.8 vs. 0.3 [P = .007], respectively) and second (14.0 vs. 1.1 [P = .016], 5.4 vs. 0.4 [P = .02], respectively) PEG-IFN doses. All 3 parameters may be used to identify NRs. In conclusion, PEG-IFN concentrations and pharmacokinetic parameters do not differ between SVRs and NRs. In contrast, pharmacodynamic measurements-namely EC50, the therapeutic quotient, and C(7)/EC50-are different in coinfected SVRs and NRs. These parameters might be useful predictors of treatment outcome during the first month of therapy.

Infection with Middle East respiratory syndrome coronavirus (MERS-CoV) emerged in 2012 as an important respiratory disease with high fatality rates of 40%—60%. Despite the increased number of cases over subsequent years, the number of pediatric cases remained low. A review of studies conducted from June 2012 to April 19, 2016 reported 31 pediatric MERS-CoV cases. In this paper, we present the clinical and laboratory features of seven patients with pediatric MERS. Five patients had no underlying medical illnesses, and three patients were asymptomatic. Of the seven cases, four (57%) patients sought medical advice within 1-7 days from the onset of symptoms. The three other patients (43%) were asymptomatic and were in contact with patients with confirmed diagnosis of MERS-CoV. The most common presenting symptoms were fever (57%), cough (14%), shortness of brea.h.(14%), vomiting (28%), and diarrhea (28%). Two (28.6%) patients had platelet counts of had an underlying end-stage renal disease. The remaining patients presented with normal blood count, liver function, and urea and creatinine levels. The documented MERS-CoV Ct values were 32 38 for four of the seven cases. Two patients (28.6%) had abnormal chest radiogra.h.c findings of bilateral infiltration. One patient (14.3%) required ventilator support, and two patients (28.6%) required oxygen supplementation. All the seven patients were discharged without complications.

With the increase of energy consumption worldwide in several domains such as industry,education,and transportation,several technologies played an inﬂuential role in energy conservation such as the Internet of Things(IoT).In this article,we describe the design and implementation of an IoT-based energy conser-vation smart classroom system that contributes to energy conservation in the edu-cation *** proposed system not only allows the user to access and control IoT devices(e.g.,lights,projectors,and air conditions)in real-time,it also has the capability to aggregate the estimated energy consumption of an IoT device,the smart classroom,and the building based on the energy consumption and cost model that we ***,the proposed model aggregates the estimated energy cost according to the Saudi Electricity Company(SEC)***,the model aggregates in real-time the estimated energy conservation percentage and estimated money-saving percentage compared to data collected when the system wasn't *** feasibility and beneﬁts of our system have been validated on a real-world scenario which is a classroom in the college of computer science and engineering,Taibah University,Yanbu *** results of the experimental studies are promising in energy conservation and cost-saving when using our proposed system.

MPDP: A Probabilistic Architecture for Microservice Performance Diagnosis and Prediction

In recent years,container-based cloud virtualization solutions have emerged to mitigate the performance gap between non-virtualized and virtualized physical ***,there is a noticeable absence of techniques for predicting microservice performance in current research,which impacts cloud service users’ability to determine when to provision or de-provision *** microservice performance poses challenges due to overheads associated with actions such as variations in processing time caused by resource contention,which potentially leads to user *** this paper,we propose,develop,and validate a probabilistic architecture named Microservice Performance Diagnosis and Prediction(MPDP).MPDP considers various factors such as response time,throughput,CPU usage,and othermetrics to dynamicallymodel interactions betweenmicroservice performance indicators for diagnosis and *** experimental data fromourmonitoring tool,stakeholders can build various networks for probabilistic analysis ofmicroservice performance diagnosis and prediction and estimate the best microservice resource combination for a given Quality of Service(QoS)*** generated a dataset of microservices with 2726 records across four benchmarks including CPU,memory,response time,and throughput to demonstrate the efficacy of the proposed MPDP *** validate MPDP and demonstrate its capability to predict microservice *** compared various Bayesian networks such as the Noisy-OR Network(NOR),Naive Bayes Network(NBN),and Complex Bayesian Network(CBN),a.h.eving an overall accuracy rate of 89.98%when using CBN.

AIM To understand the role of knowledge as a promoter of hepatitis C virus(hCV) screening among primary care physicians(PCP).METhODS A 45-item online questionnaire assessing knowledge of hCV natural history, risk factors, and treatment was distributed to 163 PCP. Logistic regression, adjusted for survey responses, assessed associations between PCP knowledge of hCV natural history and treatment and birth cohort(i.e., birth between 1945 and 1965) screening. Response stratification and weighting were used to account for nonresponse and to permit extension of responses to the entire survey population. Associations between various predictors including demogra.h.c characteristics, level of training, and hCV treatment experience and hCV knowledge were *** Ninety-one individuals(55.8%) responded. Abnormal liver enzymes(49.4%), assessment of hCV-related risk factors(30.6%), and birth cohort membership(20%) were the leading hCV screening indications. Most PCP(64.7%) felt that the combination of risk-factor and birth cohort screening utilizing a self-administered survey while awaiting the physician(55.3%) were the most efficient screening practices. Implementation of birth cohort screening was associated with awareness of the recommendations(P-value = 0.01), knowledge of hCV natural history(P-value hCV patients(P-value h knowledge of hCV treatment was also knowledgeable about hCV natural history(P-value h hCV treatment knowledge(P-value = 0.03).CONCLUSION Comprehensive knowledge of hCV is critical to motivate hCV screening. PCP-targeted educational interventions are required to expand the hCV workforce and linkageto-care opportunities as we seek global hCV eradication.

Adapting Integrated high Concentrated PV Modules and Evacuated Tube Collectors to Minimize Building Energy Consumption in hot Climate

Energy consumption in buildings is considered a significant portion of gross power dissipation, so a great effort is required to design efficient construction. In severe hot wea.h.r conditions as Kuwait, energy required for building cooling and heating results in a.h.ge energy loads and consumption and accordingly high emission rates of carbon dioxide. So, the main purpose of the current work is to convert the existing institutional building to near net-zero energy building (nNZEB) or into a net-zero energy building (NZEB). A combination of integrated high concentrated photovoltaic (hCPV) solar modules and evacuated tube collectors (ETC) are proposed to provide domestic water heating, electricity load as well as cooling consumption of an institutional facility. An equivalent circuit model for single diode is implemented to evaluate triple junction hCPV modules efficiency considering concentration level and temperature effects. A code compatible with TRNSYS subroutines is introduced to optimize evacuated tube collector efficiency. The developed models are validated through comparison with experimental data available from literature. The efficiency of integrated hCPV-ETC unit is optimized by varying the different system parameters. Transient simulation program (TRNSYS) is adapted to determine the performance of various parts of hCPV-ETC system. Furthermore, a theoretical code is introduced to evaluate the environmental effects of the proposed building when integrated with renewable energy systems. The integrated hCPV-ETC fully satisfies the energy required for building lighting and equipment. Utilizing hCPV modules of orientation 25? accomplishes a minimum energy payback time of about 8 years. Integrated solar absorption chiller provides about 64% of the annual air conditioning consumption needed for the studied building. Parameter Prediction of Stretch-Blow Molding Process of PET Using Neural Networks

This study presents a parameter selection strategy developed for the Stretch-Blow Molding (SBM) process to minimize the weight of preforms used. The method is based on a predictive model developed using Neural Networks. The temperature distribution model of the preform was predicted using a 3-layer NN model with supervised backpropagation learning. In addition, the model was used to predict the uniform air pressure applied inside the preform, taking into account the relationship between the internal air pressure and the volume of the preform. Parameters were validated using in situ tests and measurements performed on several weights and lengths of a 0.330 Liter Polyethylene Terephthalate (PET) bottles. Tests showed that the model adequately predicts both the blowing kinematics, mainly zone temperatures and blowing and stretching pressures along the walls of the bottle while maintaining the bottle strength and top load requirements. In the second step, the model was combined to automatically compute the lowest preform weight that can be used for a particular 330 ml bottle design providing a uniform wall thickness distribution.
