Background Although adhesive strapping(AS)for pediatric umbilical hernia(UH),which was once obsolete,has been reconsidered as a common practice in Japan,its efficacy is still *** study aimed to evaluate its efficacy by reviewing related *** A comprehensive literature search of PubMed,Cochrane,Google Scholar,and Igaku Chuo Zasshi via Ichushi-Web was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses *** studies reporting on the UH closure rate after AS compared with observation-only management were *** A total of 10 cohort studies were included,and the overall UH closure rate was not statistically significant(p=0.31,risk ratio(RR)=0.76,95%confidence interval(CI)0.45 to 1.28).However,there were significant differences in the UH closure rate at the age of 6 months(p<0.01,RR=0.55,95%CI 0.41 to 0.75)and the efficacy of preventing protruding umbilici with redundant skin(p=0.049,RR=0.16,95%CI 0.03 to 0.99).Conclusions Although the efficacy of AS on UH compared with observation-only management did not differ in terms of the UH closure rate,the application of AS may be effective for faster UH closure and the prevention of protruding ***,due to the high heterogeneity of the study,further large-scale studies,particularly randomized controlled trials,are warranted to reach a conclusion.

A 77-year-old man who received transarterial chemoembolization for large hepatocellular carcinomas complained of acute pain in the upper abdomen and suddenly developed severe jaundice, anemia, and massive hematuria. Abdominal computed tomography demonstrated gas gangrene at the tumor site. Clostridium perfringens was identified from blood samples and drainage cultures from the liver abscess. Despite intensive treatment, the patient died 55 hours after chemoembolization due to multiple organ failure. When treating patients with large tumors, such as in our case, this rare but fatal complication that causes sepsis and hemolysis with lightning-like rapidity should be considered.

We report a case of an asymptomatic 36-year-old man with a bronchial artery aneurysm in the right hilum. Selective angiography revealed a 25mmsaccular aneurysm and an efferent artery of the aneurysm forming a high flow bronchial artery-pulmonary artery fistula. Because of dilatation and tortuosity of the bronchial artery, the microcatheter could reach the efferent artery but not the fistula. Therefore, we embolized the fistula by sending microcoils through the bloodstream from the efferent artery to the fistula (the “flow-dependent” coil embolization technique), and further embolized the aneurysm by coil isolation and packing technique.

A 65-year-old man was admitted to our hospital following 6 months of dysphagia. At first, conventional endoscopy showed a reddish and depressed lesion in the stomach and an elevated lesion in the posterior wall of the hypopharynx. An endoscopic biopsy showed adenocarcinoma in the stomach, and squamous cell carcinoma in the hypopharynx. On the further examination, trans-nasal endoscopy with narrow band imaging (NBI) was performed. During the trumpet maneuver, a huge protruded lesion was observed and it reached to the orifice of the esophagus. Other superficial lesion located at left pyriform sinus was detected by NBI system as brownish area with brown dots. Furthermore, superficial esophageal cancer in the cervical esophagus was detected. Finally, 4 carcinomas in upper gastrointestinal tract were detected. Among them, the hypopharyngeal cancer was the most advanced (T3N0). The patient hoped to preserve his voice and swallowing function, endoscopic laryngo-pharyngeal surgery (ELPS) was performed for the hypopharyngeal cancer. Endoscopic mucosal resection (EMR) was performed for the esophageal cancer, and Endoscopic submucosal dissection (ESD) was performed for the gastric cancer. Under collaboration between a head and neck surgeon and an endoscopist, the tumor was resected en-bloc. The histopathological findings of hypopharyngeal cancer were squamous cell carcinoma, subeipthelial invasion, 29 × 28 × 4.2 mm. The others were diagnosed as mucosal cancers. The patient is currently alive with no recurrence at 28 months after the surgery;there is no stricture at the cervical esophagus. Endoscopic laryngopharyngeal surgery for the tumor of pharyngo-esophageal junction can provide a less invasive treatment.
