Effectiveness of sub-maximal intermittent exercise on muscle glycogen depletion, PGC-1<i>α</i>and PDK-4 gene expression

Several metabolic gene expressions are regulated in concert with muscle glycogen status. We hypothesized that intermittent exercise performed at high but sub-maximal intensities with long recovery periods would induce a low glycogen state that would stimul- ate peroxisome proliferator-activated receptor-γ coa- ctivator-1α (PGC1-α) and pyruvate dehydrogenase kinase-4 (PDK-4) gene expression in muscle. Nine young human subjects performed two intermittent exercise sessions. One session consisted of 60 s cycling bouts at VO2max (IE100%), and the other session consisted of 75 s cycling bouts at 80% VO2max (IE80%). Twelve bouts of exercise were completed in both sessions with a 4 min rest between each bout. Muscle specimens were obtained at pre-exercise and immediately, 1.5 h and 3 h post-exercise. Muscle gly- cogen was significantly decreased after both sessions (IE100%, 94.1 ± 5.8 to 38.7 ± 5.5 mmol/kg w.w.;IE80%, 94.6 ± 9.1 to 53.3 ± 4.8 mmol/kg w.w.;both P α and PDK- 4 mRNA expression were significantly increased after exercise in both IE100% and IE80% (PGC-1α: ~3.7 and ~2.9-fold, respectively;PDK-4: ~11.1 and ~3.5-fold, respectively;all P 100% than in IE80% (P a and PDK-4 mRNA expression, suggesting that increasing exercise intensity contributes to muscle glycogen depletion and PDK-4 mRNA expression in human skeletal muscle.

Efficacy of 308-nm Monochromatic Excimer Lamp and Topical Tacrolimus Combination Therapy in Localized Vitiligo:A Study in Vietnam

Objective:Combination therapy of a 308-nm monochromatic excimer lamp and topical tacrolimus is widely used in the treatment of localized vitiligo;however,there are no published studies investigating this combination therapy in Vietnamese patients with *** study was performed to evaluate the efficacy of combined excimer lamp and topical tacrolimus therapy for patients with localized vitiligo in ***:We performed a pre-post clinical trial of 30 patients diagnosed with localized vitiligo in *** patients were treated with an excimer lamp twice weekly and with topical tacrolimus twice *** was evaluated based on the reduction rate of the vitiligo area scoring index (VASI).Data were analyzed using descriptive statistics,Fisher’s exact test,Kruskal-WallisH test,and Spearman correlation coefficient ***:After 12 weeks of treatment,response was excellent or good in 16 patients (53.3%),moderate in 8 patients (26.7%),and poor in 6 patients (20.0%).The excellent or good responses increased over time from 3 patients (10.0%) at 6 weeks to 16 patients (53.3%) at 12 weeks (P = 0.001).Earlier initiation of repigmentation was associated with a better response to treatment (r = -0.71;P = 0.020).Patients with a perifollicular repigmentation pattern responded better to treatment than those with combined or marginal repigmentation patterns (P = 0.003).Side effects occurred in 13 patients (43.3%),including burning (43.3%),pruritus (10.0%),and mild blistering (6.7%),and none of these interrupted the ***:Combination therapy with excimer lamp and topical tacrolimus appears to be effective and safe in treating patients with localized vitiligo in Vietnam.

BACKGROUND Fistula formation is a severe adverse event related to antiangiogenetic agents such as bevacizumab and inferior mesenteric arteriovenous fistula(IMAVF) is a result of acquired factor, especially colon surgery. However, IMAVF occurs very rarely and there are few reports in patients during chemotherapy. We report a case of a patient who developed IMAVF during treatment with bevacizumab in metastatic colorectal cancer(mCRC) after colon *** SUMMARY An 81-year-old man was diagnosed with descending colon cancer and underwent left hemicolectomy without any complications. He was definitely diagnosed with high-risk stage 2 and received tegafur-uracil plus leucovorin as adjuvant chemotherapy. Three years and 6 mo after the operation, the cancer relapsed with peritoneal dissemination. The patient underwent CyberKnife radiosurgery targeting the recurrent tumor and received chemotherapy with S-1 plus bevacizumab. At 1 year after chemotherapy, he complained of severe diarrhea, which is suspected drug-induced colitis. As diarrhea worsened despite the termination of treatment, he underwent colonoscopy and computed tomography (CT) scans that revealed edematous change from sigmoid to rectosigmoid colon. CT scans also revealed an aneurysm adjacent to the inferior mesenteric vein and multidetector CT angiography showed the IMAVF. Elective angiography confirmed the diagnosis of an IMAVF and it was successfully treated by arterial embolization. The patient resumed chemotherapy with only S-1 6 mo after *** Clinicians should keep in mind the probability of severe diarrhea arose from IMAVF in mCRC patients treated with bevacizumab.

Incidental Serous Tubal Intraepithelial Carcinoma Detected by a Surgery for Ectopic Pregnancy

Serous tubal intraepithelial carcinoma is a putative precursor of high-grade serous carcinoma, which is the most common histological type of ovarian or pelvic peritoneal cancer. Serous tubal intraepithelial carcinoma is commonly found in patients with breast cancer susceptibility gene mutations who undergo risk-reducing salpingo-oophorectomy. Incidental serous tubal intraepithelial carcinoma found by a non-prophylactic surgery is rare. A 33-year-old woman referred to our hospital for a diagnosis of ectopic pregnancy. She underwent a laparoscopic right salpingectomy. Pathologically, ectopic pregnancy in the ampulla of the right fallopian tube was confirmed and serous tubal intraepithelial carcinoma was observed in the fallopian tube. Subsequently, she underwent a laparoscopic hysterectomy, bilateral oophorectomy, and left salpingectomy as additional treatment. She has experienced no recurrence thus far for 37 months since the surgery.

Colonoscopy can miss diverticula of the left colon identified by barium enema

AIM: To identify the diagnostic value of colonoscopy for diverticulosis as determined by barium enema. METHODS: A total of 65 patients with hematochezia who underwent colonoscopy and barium enema were analyzed, and the diagnostic value of colonoscopy for diverticula was assessed. The receiver operating characteristic area under the curve was compared in relation to age (< 70 or ≥ 70 years), sex, and colon location. The number of diverticula was counted, and the detection ratio was calculated. RESULTS: Colonic diverticula were observed in 46 patients with barium enema. Colonoscopy had a sensitivity of 91% and specificity of 90%. No significant differences were found in the receiver operating characteristic area under the curve (ROC-AUC) for age group or sex. The ROC-AUC of the left colon was significantly lower than that of the right colon (0.81 vs 0.96, P=0.02). Colonoscopy identified 486 colonic diverticula, while barium enema identified 1186. The detection ratio for the entire colon was therefore 0.41 (486/1186). The detection ratio in the left colon (0.32, 189/588) was significantly lower than that of the right colon (0.50, 297/598) (P < 0.01). CONCLUSION: Compared with barium enema, only half the number of colonic diverticula can be detected by colonoscopy in the entire colon and even less in the left colon.

Analysis on Temperature Distributions in Single Cell of Polymer Electrolyte Fuel Cell When Operated in High Temperature Range

This study is to understand the impact of operating condition, especially initial operation temperature （Tini） which is set in high temperature range, on the temperature profile of the interface between PEM （polymer electrolyte membrane） and catalyst layer at the cathode （i.e., the reaction surface） in a single PEFC （polymer electrolyte fuel cell）. A 1D multi-plate heat transfer model based on the temperature data of separator measured using thermograph in a power generation experiment was developed to evaluate the reaction surface temperature （Treact）. This study investigated the effects of flow rate, relative humidity and type of supply gas as well as Tini on the temperature distribution on reaction surface. The results obtained in 02 supply case show that, the temperature rise at the segments near the outlet of cell decreases with increasing Tini irrespective of relative humidity of supply gas （RH）, while it is not seen in air supply case. Regarding the segments except near the outlet in 02 supply case, Treact - Tini increases with increasing Tini for 40% RH. The temperature distribution on reaction surface in 02 supply case is wider with increasing Tini as well as decreasing RH, though that in air supply case is relatively even.

Characteristic Changes Defined via Comparison of the Big-Five Personalities in Japanese University Freshmen from Years 2000 to 2016

Introduction: With the arrival of the 21st century, drastic worldwide changes to natural, economic and social events have occurred. These include the Great East Japan Earthquake, and the global financial crisis. Furthermore, Japan experienced the introduction of a specific education system. In this study, we traced and compared the psychological characters of university freshmen for 16 years (years 2000-2016) using Big-Five personalities inventory (Japanese version). Methods: A total of 1189 physically and mentally healthy university freshmen participated in this study. They were divided into four groups according to the year they entered university. They completed a self-filled questionnaire, Big-Five personalities inventory, which consists of Extraversion, Agreeableness, Neuroticism, Conscientiousness and Openness. Result and Discussion: Compared with the year 2008, Extraversion nominally increased in 2012, however, it statistically significantly decreased in 2016. Agreeableness notably decreased from 2000 to 2008, then showed an increase in 2012. Compared to 2000, Neuroticism was statistically significantly lower in 2008 and 2016. Such statistically significant differences were not observed in the other two personalities. The personalities of university freshmen have changed in the last 16 years, which may be been caused by drastic changes. However, further research (e.g., larger-scale studies or studies including other university students) is required to support the current findings.

Treatment strategies for advanced hepatocellular carcinoma:Sorafenib vs hepatic arterial infusion chemotherapy

Sorafenib is used worldwide as a first-line standardsystemic agent for advanced hepatocellular carcinoma(HCC) on the basis of the results of two large-scale Phase Ⅲ trials. Conversely,hepatic arterial infusion chemotherapy(HAIC) is one of the most recommended treatments in Japan. Although there have been no randomized controlled trials comparing sorafenib with HAIC,several retrospective analyses have shown no significant differences in survival between the two therapies. Outcomes are favorable for HCC patients exhibiting macroscopic vascular invasion when treated with HAIC rather than sorafenib,whereas in HCC patients exhibiting extrahepatic spread or resistance to transcatheter arterial chemoembolization,good outcomes are achieved by treatment with sorafenib rather than HAIC. Additionally,sorafenib is generally used to treat patients with Child-Pugh A,while HAIC is indicated for those with either Child-Pugh A or B. Based on these findings,we reviewed treatment strategies for advanced HCC. We propose that sorafenib might be used as a first-line treatment for advanced HCC patients without macroscopic vascular invasion or Child-Pugh A,while HAIC is recommended for those with macroscopic vascular invasion or Child-Pugh A or B. Additional research is required to determine the best second-line treatment for HAIC non-responders with Child-Pugh B through future clinical trials.

Current status of Radiologic Diagnosis for Mediastinal Lymph Node Metastases of Non-Small-Cell Lung Cancer: Retrospective Study of pN2 Cases

Objective: Advances in diagnostic imaging techniques, such as 18F-fluorodeoxyglucose positron emission tomography (FDG-PET), have led to greater accuracy in preoperative mediastinal staging for patients with non-small-cell lung cancer (NSCLC), but surgical staging remains the “gold standard” for diagnosis. A proper understanding of the current accuracy of diagnostic imaging is needed for further improvements. Methods: Forty-three patients who underwent resection for NSCLC involving mediastinal lymph node (MLN) metastasis at our hospital between June 2003 and May 2011 were enrolled in this study. We conducted a retrospective study of the radiological and pathological findings for 53 metastatic MLNs in the 43 patients. Results: The preoperative imaging modality was computed tomography (CT) alone for 18 patients (22 MLNs) and CT and FDG-PET for 25 patients (31 MLNs). The sensitivities of CT and FDG-PET were 41.5% and 58.0%, respectively. The sensitivity of CT did not differ according to any clinicopathological factors, but the sensitivity of FDG-PET tended to be higher for primary tumors with high SUVmax values and for non-adenocarcinomas. In the lymph nodes, all micrometastatic foci ≤ 2 mm were PET-negative, but 4 lymph nodes with metastatic foci larger than 10 mm were also PET-negative. Conclusions: For the diagnostic imaging of MLN, FDG-PET has a greater sensitivity than contrast-enhanced CT based on “size criteria”, but it is still not sufficiently sensitive and is influenced by various factors. At present, histological confirmation of MLNs is necessary when making decisions regarding treatment plans and the type of surgical procedure that should be performed.
