Use of color Doppler ultrasonography in the diagnosis of anomalous connection in pancreatobiliary disease

AIM: To estimate the detectability of anomalous connection in pancreatobiliary disease (ACPBD) cases, measuring gallbladder wall blood flow (GWBF).METHODS: In the retrospective study, we enrolled 42subjects with gallbladder wall thickening. GWBF velocity was determined as an average value of the peak velocity of color signals on the gallbladder wall, three times in each case. Based on the findings on endoscopic ultrasonography (EUS) or endoscopic retrograde cholangiopancreatography (ERCP), the 42 subjects were divided into 11 cases with ACPBD and 31 cases without ACPBD. In the prospective study, the subjects were 92 cases with gallbladder wall thickening. Using the cut-off level of the flow velocity obtained in the retrospective study, the usefulness of measuring GWBF velocity in diagnosing ACPBD was ***: In the retrospective study, imaging of GWBF was obtained in 40 of the 42 subjects. The mean GWBF velocity of the ACPBD cases was 29.4±3.9 cm/s(mean±SD), which was significantly different (P＜0.0001;95% CI 5.48-13.2) from that of the without ACPBD cases(20.1±5.9 cm/s). Based on this result, we prepared a receiver operating characteristic curve, and the cut-off level appropriate for diagnosing ACPBD was estimated to be 25 cm/s. In the prospective study, GWBF was detected in 86 of the 92 subjects. Based on the EUS or ERCP findings, the 92 subjects were divided into 15 cases with ACPBD and 77 cases without ACPBD. When a cut-off level of 25 cm/s was employed, ACPBD could be diagnosed with a sensitivity of 87.0% (13/15) and a specificity of87.3% (62/71).CONCLUSION: Measurement of GWBF velocity, which is less invasive and provides objective values, is very useful for diagnosing ACPBD prior to the development of malignant tumors in cases with gallbladder wall thickening.

The effect of soybean trypsin inhibitor(STI)on the degradation of crucian carp myofibrillar proteins caused by an endogenous myofibril-bound serine proteinase (MBSP)was *** trypsin inhibitor was purified to high homogeneity and mixed with myofibrils and its inhibitory effect on myofibrillar protein degradation was *** the absence of STI,proteolysis of myofibrillar proteins including myosin heavy chain,α-actinin,actin and tropomyosin could be identified after incubation at 55℃ for 5 to 20 min depending on the kind of the *** contrast,in the presence of STI,with final concentration of 0.75μg/ml,proteolysis of these proteins was greatly suppressed even after incubation for 1 h,suggesting STI is an effective inhibitor in preventing myofibrillar protein degradation caused by a serine proteinase that is quite possibly *** STI reveals disadvantage for food digestion,as a native food grade inhibitor,it has the safety as food additive,especially at low *** in surimi production,the decrease of elasticity is always accompanied with the degradation of myofibrillar proteins,thus,the present result suggested the possibility that STI be applicable in surimi production in order to enhance the elasticity that is the quality of the final products.

NiFe alloy particles doping effect of Gd-Ba-Cu-O bulks processed by a new cold-seeding technology

The process of cold seeding melt growth of GdBa2Cu3Oy （Gd123） bulk superconductors using NdBa2Cu3Oy （Nd123） thin films was reported. In addition, a novel cold seeding concept of combining MgO crystal and buffer pellet was also introduced. The misorientation caused by the lattice mismatch between MgO and Gd123 melt was overcome by choosing suitable heat treatment program and Gd2BaCuO5 （Gd211） content of the buffer pellet. The doping effect of soft ferromagnetic NiFe alloy particles was also reported. The bulk sample with 0.4% （mole fraction） doping amount shows the best performance on the flux trapping. The critical current density is largely enhanced under the external field of 1-2 T, which is promising for large-scale applications. This effect is originated from the substitution of Fe and Ni ions for the Cu sites contributing to magnetic flux pinning.

Increased Cerebral Cortex Perfusion in Stroke Patients with Cognitive Disorder Following Cilostazol Administration, Two Cases Report

Objective: In our search for a new augmentation therapy for stroke patients, we administered cilostazol, an antiplatelet agent. Subjects: The patients suffered from mild hemiparesis or cognitive disorder showing reduced cerebral perfusion in the prefrontal cortex. Methods: We evaluated the functional cerebral blood flow (CBF) before and after the administration of cilostazol using near-infrared resonance spectroscopy (NIRS) during a verbal fluency task (VFT). Results: For the patient with cognitive disorder, statistically significant improvements were observed in the number of generated words in the VFT before and after administration of 50 mg cilostazol (p 0.05, Mann-Whitney U test). Another patient without cognitive disorder, however, showed no significant VFT improvement after administration of cilostazol. Effect size data revealed large or very large effects of cilostazol on brain activation (oxy-Hb levels) at the affected side prefrontal cortex for both patients. The patient with cognitive disorder showed significant improvement in VFT performance as well as an increase in bilateral prefrontal CBF after cilostazol administration. Discussion: These findings suggest that, for patients with cerebrovascular lesions suffering from cognitive disorder, cilostazol may be promising as a drug to improve cognitive function in addition to preventing recurrent cerebral infarction.

Formation of Gel-Like Shear-Induced Structure with Dosing of Dilute Surfactant Solution and Its Effect on Turbulent Channel Flow

A shear-induced structure (SIS) is formed under appropriate concentration and shear conditions in a surfactant micellar solution. In this study, we performed experiments of surfactant solution dosing in a fully developed two-dimensional turbulent channel flow from a sintered metallic wire mesh plate attached to a side wall. We investigated the behavior of the solution under the elongation during its passing through the wire mesh and under the strong shear due to the channel flow. It was confirmed that the dosed solution containing a laser dye was visualized by a laser sheet, and the accumulated gel from the wire mesh formed a layer and developed with time. Consequently, on dosing the dilute surfactant solution from the wire mesh, a gel-like SIS layer was formed, which majorly covered the wire mesh plate. The gel-like SIS layer on the wire mesh plate acted as a sticky solid and restricted the flow in the channel. This layer continued to grow while dosing, owing to which the pressure drop of the channel flow significantly increased. The gel-like SIS layer grew rapidly even in the turbulent flow and reached the equilibrium thickness. After the termination of the dosing, the gel layer collapsed gradually. In addition, the thickness of the gel-like SIS layer (indicating the strength indirectly) strongly depended on the surfactant concentration and the elongation rate in the wire mesh.

Association between Systolic Blood Pressure Difference ≥10 mm Hg and Ankle-Brachial Index

Background: In new outpatients, blood pressure should be measured in both arms. A previous study reported that an inter-arm systolic blood pressure difference (ΔSBP) of ≥10 mm Hg is associated with an increased risk of mortality. Aim: The aim was to identify the associations with absolute values of ΔSBP (|ΔSBP|) ≥10 mm Hg. Subjects and Methods: This study included 2481 patients. Patients with a body mass index ≥25 kg/m2 were defined as obese. The group of A was defined as following: ankle-brachial index (ABI) was <0.9 or ≥1.3. ΔSBP was expressed as right arm BP minus left arm BP. |ΔSBP| ≥10 mm Hg were analyzed using multivariate logistic analysis. Results: |ΔSBP| ≥10 mm Hg was found in 6.0% of patients and |ΔSBP| < 5 mm Hg in 80.4%. In multivariate analysis, the odds ratios (ORs) of the associations with |ΔSBP| ≥10 mm Hg were significantly associated with abnormal ABI and obesity regardless of sex and age. Moreover, the OR of the combined effects of abnormal ABI and obesity was higher than that of abnormal ABI and obesity alone. Conclusion: |ΔSBP| ≥10 mm Hg was associated with abnormal ABI and obesity. In a primary care setting, blood pressure should be actively measured in both arms. This study suggests that the associations with |ΔSBP| ≥10 mm Hg may be a useful part of screening for abnormal ABI.

A carbon-supported Ru catalyst, Ru/BP2000, is able to simultaneously convert cellobiose into sorbitol and gluconic acid. This reaction occurs as the result of hydrolytic disproportionation in water at 393 K under an Ar atmosphere, without bases or sacrificial reagents. In-situ XANES measurements suggest that the active Ru species involved is composed of partially oxidized Ru metal.

Macrophage colony-stimulating factor expressed in non-cancer tissues provides predictive powers for recurrence in hepatocellular carcinoma

AIM To investigate the role of macrophage colony-stimulating factor(M-CSF) in patients with hepatocellular carcinoma(HCC) after surgery. METHODS Expression of M-CSF, distribution of M2 macrophages(Mφs), and angiogenesis were assessed in the liver, including tumors and peritumoral liver tissues. The prognostic power of these factors was assessed. Mouse isolated hepatic Mφs or monocytes were cultured with media containing M-CSF. The concentration of vascular endothelial growth factor(VEGF) in media was assessed. Furthermore, the role of the M-CSF-matured hepatic Mφs on proliferation of the vascular endothelial cell(VEC) was investigated. RESULTS A strong correlation between the expressions of M-CSF and CD163 was observed in the peritumoral area. Also, groups with high density of M-CSF, CD163 or CD31 showed a significantly shorter time to recurrence(TTR) than low density groups. Multivariate analysis revealedthe expression of M-CSF or hepatic M2Mφs in the peritumoral area as the most crucial factor responsible for shorter TTR. Moreover, the expression of M-CSF and hepatic M2Mφs in the peritumoral area had better predictable power of overall survival. Values of VEGF in culture media were significantly greater in the hepatic Mφs compared with the monocytes. Proliferation of the VEC was greatest in the cells co-cultured with hepatic Mφs when M-CSF was present in *** M-CSF increases hepatocarcinogenesis, most likely by enhancing an angiogenic factor derived from hepatic Mφ and could be a useful target for therapy against HCC.

Dear editor,The current treatment strategy for advanced non-small cell lung cancer(NSCLC)patients harboring epider-mal growth factor receptor(EGFR)mutations involves EGFR-tyrosine kinase inhibitors(EGFR-TKIs).Several reports on EGFR-TKI resistance have shown that the T790M mutation within the EGFR gene is present in approximately half of the patients who develop resist-ance to a first-or second-generation EGFR-TKI[1,2].In Japan,osimertinib monotherapy is the standard therapy for patients harboring the EGFR T790M mutation arising after the development of resistance to a first-or second-generation EGFR-TKI[3,4].Herein,we report the case of a patient who underwent four tumor biopsies over the treatment *** no detection of this mutation after developing resistance to the first-line EGFR-TKI,in the fourth biopsy specimen,which was collected after progression following EGFR-TKI re-challenge,EGFR T790M mutation was detected using the cobas®EGFR mutation test(Roche Molecular Systems,Pleasanton,CA,USA).Additionally,we analyzed the T790M allele frequency(AF)in longitudinal biopsy samples obtained at four different time points using droplet digital PCR(ddPCR)(PrimePCR™,Bio-Rad Laboratories,Inc.,Her-cules,CA,USA).We discuss the clinical usefulness of longitudinal assessment of the EGFR T790M AF,espe-cially for deciding the appropriate treatment(e.g.,osi-mertinib)in patients with EGFR mutations.
