Combined laparoscopic and thoracoscopic repair of adult right-sided Bochdalek hernia with massive liver prolapse: A case report

BACKGROUND A Bochdalek hernia(BH)is a congenital diaphragmatic hernia that often develops in the neonatal period.BH typically occurs on the left side of the diaphragm.A right-sided BH in an adult is rare.CASE SUMMARY A 45-year-old man was referred to our hospital because of an abnormal shadow seen on chest radiography during a medical check-up.A chest radiograph showed elevation of the right hemidiaphragm.Computed tomography showed prolapse of multiple intraabdominal organs into the right thoracic cavity,corresponding to a right-sided BH.The herniated contents included the stomach,transverse colon,and left lobe of the liver.The left lobe of the liver was enlarged,particularly the medial segment.Laparoscopic surgery was performed.However,the left lobe of the liver was completely trapped in the thoracic cavity.Therefore,thoracoscopic manipulation had to be performed to return the liver to the abdominal cavity.The hernia was repaired with interrupted nonabsorbable sutures and reinforced with mesh.CONCLUSION Combined laparoscopic and thoracoscopic surgery was successfully performed for right-sided BH with massive liver prolapse and abnormal liver morphology.

The aging of society has arrived,and is accompanied by an increase in the absolute numbers of patients with neurological disorders,such as Alzheimer's and Parkinson's diseases(Feigin et al.,2020).Such diseases,particularly Alzheimer's disease and other forms of dementia,affect not only the patients themselves,but also the people around them,including family members and care givers.As a result,such neurological disorders are thought to carry a larger social burden compared to other diseases.The most critical point in the current situation is that there is no effective treatment despite the fact that the number of patients increase with the aging of the population.

Spinal cord injury(SCI)is one of the leading causes of disability and is a devastating condition that requires long-term care,reduces social productivity,and gives an immense emotional burden on patients and their fa milies.SCI frequently occurs due to traffic accidents,falls,slips,violence,sports,and medical accidents in today's society.The initial mechanical damage triggers a secondary injury cascade that induces more intractable damage(Silva et al.,2014).Secondary injury mechanisms have been postulated,including neuronal apoptosis,inflammation,oxidative stress,and excitotoxicity.

Comparative evaluation of acute phase proteins by C-reactive protein(CRP)and serum amyloid A(SAA)in nonhuman primates and feline carnivores

The feasibility of a commercially available assay for C-reactive protein(CRP,CRP for humans:hCRP,and CRP for dogs:vCRP)and a trial reagent of serum amyloid A(SAA,vSAA for animals)were applied to the measurement of acute phase proteins in zoo animals,particularly in nonhuman primates and feline carnivores was evaluate.Results showed that hCRP and vSAA methods were applicable to measure CRP and SAA in Haplorhini.There was a highly signifcant correlation between both parameters with remarkably high correlation coefcient.A higher proportion of Bonnet macaques in Haplorhini,and the linear regression with good correlation between hCRP and vSAA levels were observed.Reference values in healthy Bonnet macaques were hCRP(46.86±30.97 nmol/L)and vSAA(9.06±1.95μg/mL).Although Ring-tailed lemur,which belonging to Strepsirrhini,showed low vSAA concentrations(reference values:1.08±0.47μg/mL),vSAA in patients was apparently elevated.The vCRP and vSAA methods were applicable to measurements of CRP and SAA in feline carnivores for highly signifcant correlation between both parameters.Theses two methods were also been deteded in lions,tigers and cheetahs.vSAA assays can be applied to measure SAA levels in other carnivores and herbivores.In conclusion,vSAA systems have potential utility as diagnostic tools for health screening and prediction in zoo animals.

As a candidate bound for the Binary Black Hole(BBH) merger events detected by LIGO/Virgo, Primordial Black Holes(PBHs)provide a useful tool to investigate the primordial curvature perturbations on small scales. Using the GWTC-1 to GWTC-3 catalogs, under the scenario that PBHs originate from large primordial curvature perturbations on small scales during infiationary epoch, we for the first time reconstruct the power spectrum of primordial curvature perturbations on small scales. It is found that the value of the amplitude of the primordial power spectrum is enhanced to O(10-2)on scales O(1) pc. This may imply the validity of PBH as a possible BBH merger candidate.

Surgical treatment for a combined anterior cruciate ligament and posterior cruciate ligament avulsion fracture: A case report

BACKGROUND Independent avulsion fractures with anterior cruciate ligament(ACL)or posterior cruciate ligament(PCL)attachment are relatively common among tibial intercondylar eminence fractures,and their postoperative outcomes are generally favorable.Conversely,huge avulsion fractures of the intercondylar eminence containing the attachment site of both the ACL and the PCL are extremely rare,and the reported clinical outcomes are poor.CASE SUMMARY We describe a 30-year-old Japanese male's huge avulsion fracture of the intercondylar eminence of a tibia containing the attachment site of both the ACL and PCL,together with a complete tear of the medial collateral ligament and a partial tear of both the medial and lateral menisci caused by a fall from a high place.All of these injuries were treated surgically,with anatomical reduction and stable fixation.The limb function at 1 year post-surgery was excellent(Lysholm score:100 points).CONCLUSION Although this patient's complete surgical repair was complex,it should be performed in similar cases for an excellent final clinical outcome.

Microwave dielectric ceramics(MWDCs)with low dielectric constant and low dielectric loss are desired in contemporary society,where the communication frequency is developing to high frequency(sub-6G).Herein,Nd_(2)(Zr_(1−x)Ti_(x))_(3)(MoO_(4))_(9)(NZ_(1−x)T_(x)M,x=0.02-0.10)ceramics were prepared through a solid-phase process.According to X-ray diffraction(XRD)patterns,the ceramics could form a pure crystal structure with the R3c(167)space group.The internal parameters affecting the properties of the ceramics were calculated and analyzed by employing Clausius-Mossotti relationship,Shannon’s rule,and Phillips-van Vechten-Levine(P-V-L)theory.Furthermore,theoretical dielectric loss of the ceramics was measured and analyzed by a Fourier transform infrared(IR)radiation spectrometer.Notably,when x=0.08 and sintered at 700℃,optimal microwave dielectric properties of the ceramics were obtained,including a dielectric constant(ε_(r))=10.94,Q·f=82,525 GHz(at 9.62 GHz),and near-zero resonant frequency temperature coefficient(τ_(f))=−12.99 ppm/℃.This study not only obtained an MWDC with excellent properties but also deeply analyzed the effects of Ti^(4+)on the microwave dielectric properties and chemical bond characteristics of Nd_(2)Zr_(3)(MoO_(4))_(9)(NZM),which laid a solid foundation for the development of rare-earth molybdate MWDC system.
