巴黎正慎重地允许高层建筑回归,Harnonic+Masson与Comte Vollenweider两家法国建筑事务所最近刚刚完成自上世纪70年代以来在巴黎建立的第一座达到50米的住宅项目。这象征着,人们愿意重新考虑在巴黎建造高层建筑的潜在可能性。2011年11月16日,星期二,在巴黎议会会议上,委员会的成员们修订了巴黎的城市法规,修正案将允许在巴黎东南部十三区马沙布鲁诺(Massena-Bruneseau)工业区建造50米高的住宅塔楼和高达180米的写字楼。既可以成为一栋独立建筑,也可以提供社会住房,项目齐整地连接起法兰西大道与更远处的铁路景观,成为通往伊夫里郊区的入口。

A Case of Ewing’s Sarcoma Arising in the Cervical Spine with an Elevation of Serum ProGRP

Multiple instrumental observations including GPS total electron content(TEC),fF and hFfrom ionosondes,vertical ion drift measurements from Communication/Navigation Outage Forecasting System,magnetometer data,and far ultraviolet airglow measured by Thermosphere,Ionosphere,Mesosphere Energetics and Dynamics/Global Ultraviolet Imager(TIMED/GUVI) are used to investigate the profound ionospheric disturbances at midlatitude and low latitude during the 14-17 July 2012 geomagnetic storm event,which was featured by prolonged southward interplanetary geomagnetic field component for about 30 h below-10 *** the East Asian/Australian sector,latitudinal profile of TEC variations in the main phase were characterized by three bands of increments and separated by weak depressions in the equatorial ionospheric anomaly(EIA) crest regions,which were caused by the combined effects of disturbance dynamo electric fields(DDEF) and equatorward neutral *** the recovery phase,strong inhibition of EIA occurred and the summer crest of EIA disappeared on 16 July due to the combined effects of intrusion of neutral composition disturbance zone as shown by the TIMED/GUVI 0/N measurements and long-lasting daytime westward DDEF inferred from the equatorial electrojet *** transit time of DDEF over the dip equator from westward to eastward is around 2200 *** the American longitude,the salient ionospheric disturbances in the summer hemisphere were characterized by daytime periodical intrusion of negative phase for three consecutive days in the recovery phase,preceded by storm-enhanced density plume in the initial *** addition,multiple short-lived prompt penetration electric fields appeared during stable southward interplanetary magnetic field(IMF) B in the recovery phase and were responsible for enhanced the EIA and equatorial ionospheric uplift around sunset.

We experienced a case of small, round-cell malignant neoplasm diagnosed by touch smear cytology and histopathology when an open biopsy was performed in a 50-year-old Japanese woman. She was suspected of having a cervical spine tumor after surgery for cervical spine foraminal stenosis. After consent, the cervical spine tumor histologically diagnosed by an open biopsy was confirmed to be Ewing sarcoma (EWS) by genetic testing. EWS belongs to a group of small, round-cell tumors that are morphologically similar and often difficult to differentiate. After the open biopsy, the present patient received radiotherapy, and her plasma level of Pro-Gastrin-Releasing-Peptide was decreased (217.2 pg/ml before surgery to 30.3 pg/ml;reference value: 0 - 80 pg/ml). We herein report the process for making the final diagnosis by focusing on the intraoperative cytology, histopathology, and immunohistochemical findings. Our diagnosis was validated by karyotyping and a fluorescence in-situ hybridization analysis.

Primary Ovarian Carcinosarcoma: Cytological, Pathological, Immunocytochemical, and Immunohistochemical Features

Ovarian carcinosarcoma composed of high-grade carcinoma and sarcoma is an extremely rare neoplasm and typically occurs in postmenopausal women aged over 60 years. A 73-year-old female, gravida three para three, presented to our hospital with right lower abdominal pain. Right pelvic solid tumor with ascites was detected on pelvic ultrasound examination. She underwent hysterectomy, bilateral salpingo-oophorectomy and partial omentectomy, but the tumor had invaded to the right ureter, and some fragile tumor could not be taken (sub-optimal surgery). On the imprint and ascitic cytology specimens during operation, atypical cells suggestive of adenocarcinoma and spindle atypical cells with immunocytochemically vimentin positive were found. The resected tumor was histopathologically carcinosarcoma consisted of serous adenocarcinoma, chondrosarcoma and fibrosarcoma. Immunohistochemical analysis revealed that adenocarcinoma cells were positive for AE1/AE3 and fibrosarcoma cells stained with vimentin. The final diagnosis was the right ovarian carcinosarcoma (stage pT3CNxMx). Microsatellite instability was stable and BRCA1/2 mutations could not be found in the carcinosarcoma cells. The patient was given four cycles of chemotherapy with paclitaxel, carboplatin and bevacizumab regimen, and thereafter she was treated with the ifosfamide and cisplatin because of slight elevation of serum CA125.

Among the ablation procedures used against advanced liver cancer, thermal ablation therapy is widely applied, and the safety and efficacy of this technique have been well characterized. In contrast, the principle of cryosurgery is not only to control local recurrence, but also to stimulate the immune system into initiating an anti-tumor response. In recent years, we have developed a treatment method for patients with advanced liver tumors. From these experiences, repeated treatment was demonstrated to induce anti-cancer immune reaction-related factors. In the present review, mechanisms for anti-tumor immune reaction will be argued with introducing a novel strategy of a cryoablation.

Echinococcosis is a global and zoonotic helminthic disease caused by Echinococcus sp. A 24-year-old woman, coming from Bolivia two years before, was diagnosed as cystic echinococcosis (CE) by unstained wet mount and cytological findings through aspiration cytology from the liver, suggested as liver abscess by diagnostic images. The patient was classified as CE1, smaller than 5 cm, and was diagnosed as P1N0M0, stage I. The phylogenetic trees of Echinococcus spp., based on cox1 genes showed that the isolates on the patient belonged to E. ortleppi. The patient was conservatively treated with two courses of albendazole therapy. E. ortleppi has been reported to be in regions including Latin America, except with Japan. Although the patient came from Bolivia before two years, this is the first case report of E. ortleppi from liver echinococcosis in Japan.

Pulmonary embolism after arterial chemoembolization for hepatocellular carcinoma: An autopsy case report

We report an extremely rare case of pulmonary lipiodol embolism with acute respiratory distress syndrome(ARDS) after transcatheter arterial chemoembolization(TACE) for hepatocellular carcinoma(HCC). A 77-yearold man who was diagnosed with a huge HCC wasadmitted for TACE. Immediately after the procedure, this patient experienced severe dyspnea. We suspected that his symptoms were associated with a pulmonary lipiodol embolism after TACE, and we began intensive treatment. However, his condition did not improve, and he died on the following day. A subsequent autopsy revealed that the cause of death was ARDS due to pulmonary lipiodol embolism. No cases have been previously reported for which an autopsy was performed to explain the most probable mechanism of pulmonary lipiodol embolism; thus, ours is the first report for such a rare case.

A Case of Serous Tubal Intraepithelial Carcinoma Diagnosed by Cytology during the Operation

We experienced a case of serous tubal intraepithelial carcinoma (STIC), which is the earliest morphologically recognizable precursor of pelvic high grade serous carcinoma, diagnosed by cytology during the operation. A 48-year-old Japanese woman visited to our department because of abnormal cytological result and the left adnexal mass on female cancer screening. Pre-operatively, she was diagnosed as cervical intraepithelial neoplasia (CIN) 3, adenomyosis, and the left adnexal cystic lesion with papillary growth. At laparotomy, the left adnexal tumor turned out to be cystic fallopian tube with small papillary growth on the inner surface, but abnormal findings were not present on bilateral ovaries. The cytology of the imprint smears from the papillary projection and ascitic fluid showed positive, suggesting serous adenocarcinoma. Histopathological examination of the lesion revealed the left fallopian tube lesion was STIC with immunohistochemically positive reactivity against p53. No metastases including disseminated lesions were noted. The patient received four courses of systemic chemotherapy, and had no recurrent signs 10 months after the operation.

Effects of Prolonged Night Shifts on Salivary <i>α</i>-Amylase, Secretory Immunoglobulin, Cortisol, and Chromogranin A Levels in Nurses

Prolonged night shift is associated with high burnout rate, but the physiological effects of a 16 h shift remain undetermined. Here we evaluated fatigue and stress via salivary assays to determine the correlation between stress and fatigue and prolonged night shifts. Twenty-five nurses (9 men, 16 women;16 h night shift (n = 13), 8 h day shift (n = 12)) from Juntendo University Koshigaya Hospital were evaluated for four consecutive workdays separated by off days. Salivary samples were collected upon waking and before sleep on non-working days, before and after the day and night shifts, and before and after the break during the nocturnal schedule, and analyzed for levels of cortisol, chromogranin A, α-amylase activity and secretory immunoglobulin A (sIgA). On non-working days, cortisol levels showed similar kinetic pattern in both nurses. On working days, day-time nurses’ cortisol levels showed normal circadian pattern throughout the shift. Night nurses’ cortisol levels at the beginning of the shift were comparable to that of the normal morning elevation. α-Amylase activity in the night shift nurses was higher than day shift nurses through each period. No significant differences in chromogranin A and sIgA levels were detected between day and night shift workers. A 16 h night shift may cause marked circadian misalignment in cortisol levels.
