BACKGROUND Amebic colitis is an infection caused by Entamoeba histolytica and most commonly observed in regions with poor *** is also seen as a sexually transmitted disease in developed *** amebic colitis usually has a chronic course with repeated exacerbations and remissions,it may also manifest as a fulminant form that rapidly progresses and leads to severe,life-threatening complications,such as intestinal perforation,peritonitis,and sepsis,that have a high mortality *** SUMMARY A 68-year-old man was admitted to our hospital with chest pain and acute *** was diagnosed with acute coronary syndrome,acute heart failure,and bacterial *** respiratory condition worsened despite receiving intensive care and intravenous *** the fifth day of hospitalization,he was diagnosed with acute respiratory distress syndrome and was started on steroid *** subsequently developed bloody stools and was diagnosed with cytomegalovirus(CMV)enterocolitis based on biopsy results and a peripheral blood CMV pp65 antigenemia test *** we started antiviral therapy with ganciclovir,which was successful in reducing his antigen titers,he continued to have bloody *** weeks after initiation of ganciclovir therapy and six weeks after his admission,the patient died from intestinal *** only posthumously diagnosed him with amebic colitis and CMV enterocolitis based on autopsy findings of transmural necrosis of the entire colon with massive ameba *** We urge clinicians to consider Entamoeba histolytica infection if severe colitis progresses after steroid *** treatment is recommended then.

Reliability of sawai's classification for dental cervical abrasions:A pilot study

AIM To test the reliability of the sawai’s classification for dental cervical *** photographs of 70 teeth from 23 patients with tooth abrasions were taken by the first examiner *** teeth were marked and the photos were maintained in a soft copy *** other examiners FA and SC were trained in the use of the classification and any clarifications needed were provided at the beginning of the *** examiner was then given the soft copy of the complied photographs and was asked to classify the dental cervical abrasion according to their understanding of the sawai’s *** were given sheets to write their responses for every marked *** the examiners were blinded to each other’s observations which were then tested for inter-rater agreement among the three *** 70 teeth with tooth abrasions from 23 patients were examined by 3 investigators(MS,FA and SC)to test the reliability of the sawai’s classification system for tooth *** examiner marked their responses in separate sheets which were blinded to each *** kappa statistics were performed for inter-rater agreement among the three *** level of agreement was evaluated according to the six-level nomenclature given by Landis and *** and 95%CI between two examiners,i.e.,the inter-rater agreement among 1^(st) examiner(MS) and 2^(nd) examiner(FA) was *** inter-rater agreement among 1^(st) examiner(MS) and 3^(rd) examiner(SC) was *** the inter-rater agreement among 2^(nd) examiner(FA) and 3^(rd) examiner(SC) was *** the three comparisons show an almost perfect agreement between *** is an almost perfect agreement between multiple observers for classifying dental cervical abrasions using sawai’s ***,this classification is reliable.

Selection of treatment modality for hepatocellular carcinoma according to the modified Japan Integrated Staging score

AIM： To compare the prognosis of patients who underwent hepatectomy and ablation using the modified Japan Integrated Staging score （mJIS）. METHODS： We examined the clinicopathologic records and patient outcomes in 278 HCC patients including 226 undergoing hepatectomy and 52 undergoing ablation therapy. RESULTS： Cirrhosis was more frequent in the ablation group. Tumor size, number and presence of vascular invasion were significantly higher in the operation group compared to the ablation group. The local recurrence rate adjacent to treated lesions was significantly higher in the ablation group compared to the operation group （P 〈 0.05）. The 3- and 5-year survival rates in the ablation and the operation group were 66% and 78%, and 50% and 63%, respectively, but not significantly different. Over 50% survival rates were observed in patients with a *** score of 0-2 in both groups. However, survival rates with a score of 3-5 in both groups were significantly lower. CONCLUSION： According to the mJIS system, both local treatments could be selected for patients with a score of 0-2. However, for patients with a score more than 3, liver transplantation might be a better option in patients with HCC.

Occurrence and Bioaccumulation of Micropollutants in Black Cicada

Seven organophosphoric acid triesters （OPEs） and eight polycyclic aromatic hydrocarbons （PAHs） in black cicadas were measured to determine the concentration levels and to investigate how the cicadas are affected by soil contamination. Adult cicadas, nymphal exoskeletons, and soils were sampled in Higashi Osaka, Japan. Four OPEs and six PAHs were detected in the adult cicadas. The total concentrations of OPEs and PAHs ranged from 27.2 ng/g to 824 ng/g and from 4.30 ng/g to 270 ng/g, respectively. Four OPEs and five PAHs were detected in the nymphal exoskeletons. The total concentrations of OPEs and PAHs ranged from 184 ng/g to 1830 ng/g and from 40.3 ng/g to 970 ng/g, respectively. In the soils, three OPEs and six PAHs were detected at the same concentration levels as those detected in the adult cicadas. Significant correlations were observed in the micropollutant concentrations between soils and adult cicadas, and between soils and nymphal exoskeletons. This trend indicates that black cicadas accumulate OPEs and PAHs from contaminated soils.

Relationship between microvessel count and post-hepatectomy survival in patients with hepatocellular carcinoma

AIM: To elucidate the relationship between the microvessel count (MVC) by CD34 analyzed by immunohistochemical method and prognosis in hepatocellular carcinoma (HCC) patients who underwent hepatectomy based on our preliminary study. METHODS: We examined relationships between MVC and clinicopathological factors in 128 HCC patients. The modifi ed Japan Integrated Staging score (mJIS) was applied to examine subsets of HCC patients. RESULTS: Median MVC was 178/mm^2, which was used as a cut-off value. MVC was not signif icantly associated with any clinicopathologic factors or postoperative recurrent rate. Lower MVC was associated with poor disease-free and overall survivals by univariate analysis (P = 0.039 and P = 0.087, respectively) and lower MVC represented an independent poor prognostic factor in disease-free survival by Cox’s multivariateanalysis (risk ratio, 1.64; P = 0.024), in addition to tumor size, vascular invasion, macroscopic fi nding and hepatic dysfunction. Signifi cant differences in disease-free and overall survivals by MVC were observed in HCC patients with mJIS 2 (P = 0.046 and P = 0.0014, respectively), but not in those with other scores. CONCLUSION: Tumor MVC appears to offer a useful prognostic marker of HCC patient survival, particularly in HCC patients with mJIS 2.

Thermal Radiation Effects on MHD Boundary Layer Flow over an Exponentially Stretching Surface

The steady two-dimensional laminar boundary layer flow and heat transfer of a viscous incompressible electrically conducting fluid over an exponentially stretching surface in the presence of a uniform magnetic field with thermal radiation are investigated. The governing boundary layer equations are transformed to ordinary differential equations by taking suitable similarity transformation and solved numerically by shooting method. The effects of various parameters such as magnetic parameter, radiation parameter, Prandtl number and Eckert number on local skin-friction coefficient, local Nusselt number, velocity and temperature distributions are computed and represented graphically.

Fractionation of gamma-glutamyltransferase in patients with nonalcoholic fatty liver disease and alcoholic liver disease

AIM To assess how serum gamma-glutamyltransferase(GGT) fractions vary in patients with alcoholic liver disease(ALD) and non-alcoholic fatty liver disease(NAFLD). METHODS Serum samples were obtained from 14 patients with biopsy-proven alcoholic liver diseases and 9 patients with biopsy proven non-alcoholic fatty liver disease. In addition to these biopsy-proven cases, 16 obese(body mass index > 25) patients without any history of alcohol consumption but with a fatty liver on ultrasound examination and with elevated GGT were included for an additional analysis. Serum GGT fractionation was conducted by high-performance gel filtration liquid chromatography and was separated into the four fractions, big-GGT, medium-GGT, small-GGT(s-GGT), and free-GGT(f-GGT).RESULTS The results were expressed as a ratio of each fraction including the total GGT(t-GGT). The s-GGT/t-GGT ratioswere lowest for the control group and highest for the ALD group. The differences between the control and NAFLD groups and also between the NAFLD and ALD groups were statistically significant. In contrast, the f-GGT/t-GGT ratios were highest in the control group and lowest in the ALD group, with the differences being statistically significant. As a result, the s-GGT/f-GGT ratios were markedly increased in the NAFLD group as compared with the control group. The increase of the s-GGT/t-GGT ratios, the decrease of the f-GGT/t-GGT ratios, and the increase of s-GGT/F-GGT ratios as compared with the control group subjects were also found in obese patients with clinically diagnosed fatty change of the *** Serum GGT fractionation by high-performance gel filtration liquid chromatography is potentially useful for the differential diagnosis of ALD and NAFLD.

Effects of Far-Infrared Irradiative Heating Pasteurization on Fungi

The effect of far-infrared (FIR) irradiation pasteurization on fungi was quantitatively evaluated and compared with the effect of thermal conductive heating. After the bulk temperature of the sterile saline irradiated by FIR reached a steady given temperature, yeast cells (Candida albicans NBRC 1950 and Saccharomyces cerevisiae NBRC 1067) or fungal spores (Aspergillus niger NBRC 4781) were inoculated and FIR heating was conducted. A mullite cylinder FIR heater, with a main wavelength of 4 - 7 μm, was used for FIR heating. Death of fungi by FIR heating and by thermal conductive heating both followed first-order reaction kinetics, and the apparent death rate constants under different temperature conditions were obtained. For the same bulk temperatures, pasteurization by FIR heating was more effective than thermal conductive heating. The activation energy for the death of fungi by FIR irradiation was slightly lower than thermal conductive heating, indicating differences in the mechanism of action.

Recovery of Pentachlorophenol from Aqueous Solution via Silicone Rubber Membrane

Although pentachlorophenol (PCP) has been widely employed as a biocide for over 60 years, its production and use are currently severely curtailed in many countries due to its extreme toxicity. In recent years, the contamination of both soil and surface waters by PCP has become a concern. In this study the permeation characteristics of PCP penetrating silicone rubber membranes (SRM) were studied, in order to determine the feasibility of separation of PCP from water via the permeation and chemical desorption (PCD) method. It was found that efficient separation and recovery of PCP could be obtained using an acidic feed solution and an alkaline recovery solution. The permeation rate of PCP into the SRM was optimized when the feed solution was maintained at a pH of 4 or lower. The SRM thickness did not significantly affect the permeation rate, indicating that the rate determining step for the process is the initial movement of the PCP into the SRM. The activation energy for the penetration process was determined to be quite high, and thus thermal controls will play an important role in the recovery of PCP by this method. The membrane distribution coefficient (mc) for PCP moving into SRM was large and showed a strong correlation to permeation rates reported previously, confirming that PCD is a suitable technique for the separation and recovery of PCP from aqueous solution.

Increased Expression of 11<i>β</i>-Hydroxysteroid Dehydrogenase Type 1 in Experimental Periodontitis Induced by Lipopolysaccharide from <i>Porphyromonas gingivalis</i>

It has been proposed that 11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1), which activates glucocorticoids, plays a role in chronic inflammatory diseases including metabolic diseases, rheumatoid arthritis, and ulcerative colitis. We have recently reported that the expression of 11β-HSD1 is increased in the gingiva of patients with chronic periodontitis and in that of rats with ligature-induced periodontitis. In this study, to further demonstrate the involvement of 11β-HSD1 in chronic periodontitis, the expression of 11β-HSD1 was investigated in another rat model of experimental periodontitis induced by intragingival injection of lipopolysaccharide from Porphyromonas gingivalis (LPS-PG). Alveolar bone loss was observed two weeks after intragingival injection of LPS-PG. The level of 11β-HSD1 mRNA assessed by real-time reverse transcriptase-polymerase chain reaction was significantly elevated in LPS-PG-induced periodontitis compared with controls. The expression of 11β-hydroxysteroid dehydrogenase type 2 (11β-HSD2), which inactivates glucocorticoids, was not significantly different between control and LPS-PG-induced periodontitis. The expression of 11β-HSD1 was significantly correlated with that of TNF in LPS-PG-induced periodontitis. The increased expression of 11β-HSD1 protein in LPS-PG-induced periodontitis was confirmed by immunohistochemistry using anti-11β-HSD1 antibody. These results further suggest a role for 11β-HSD1 in the pathogenesis of chronic periodontitis.
