Factors associated with residual gastroesophageal reflux disease symptoms in patients receiving proton pump inhibitor maintenance therapy

AIM To elucidate the factors associated with residual gastroesophageal reflux disease(GERD) symptoms in patients receiving proton pump inhibitor(PPI) maintenance therapy in clinical practice.METHODS The study included 39 GERD patients receiving maintenance PPI therapy. Residual symptoms were assessed using the Frequency Scale for Symptoms of GERD(FSSG) questionnaire and the Gastrointestinal Symptom Rating Scale(GSRS). The relationships between the FSSG score and patient background factors, including the CYP2C19 genotype, were analyzed.RESULTS The FSSG scores ranged from 1 to 28 points(median score: 7.5 points), and 19 patients(48.7%) had a score of 8 points or more. The patients' GSRS scores were significantly correlated with their FSSG scores(correlation coefficient = 0.47, P < 0.005). In erosive esophagitis patients, the FSSG scores of the CYP2C19 rapid metabolizers(RMs) were significantly higher than the scores of the poor metabolizers and intermediate metabolizers(total scores: 16.7 ± 8.6 vs 7.8 ± 5.4, P < 0.05; acid reflux-related symptom scores: 12 ± 1.9 vs 2.5 ± 0.8, P < 0.005). In contrast, the FSSG scores of the CYP2C19 RMs in the non-erosive reflux disease patients were significantly lower than those of the other patients(total scores: 5.5 ± 1.0 vs 11.8 ± 6.3, P < 0.05; dysmotility symptom-related scores: 1.0 ± 0.4 vs 6.0 ± 0.8, P < 0.01). CONCLUSION Approximately half of the GERD patients receiving maintenance PPI therapy had residual symptoms associated with a lower quality of life, and the CYP2C19 genotype appeared to be associated with these residual symptoms.

The diagnosis of the Middle Jurassic spinicaudatan genus Shizhuestheria is revised following the re-examination under a scanning electron microscope of a type specimen from Yanxi of Shizhu, and the newly collected specimens from Xintiangou of Chengjiang Town, Beibei, Chongqing. Small-sized reticulations occur not only in the middle-sized reticulations on growth bands on the dorsal side of the carapace, but also on growth bands on the ventral side of the carapace, which have been overlooked in the original description. The shift from wide dorsal growth bands to narrow ventral ones might indicate an onset of sexual maturity, and the transition in biological priorities from rapid juvenile growth to reproductive activities.

Lodging has been a major roadblock to attaining increased crop productivity. In an attempt to understand the mechanism for culm strength in rice, we isolated an effective quantitative trait locus （QTL）, STRONG CULM3 （SCM3）, the causal gene of which is identical to rice TEOSINTE BRANCHED1 （OsTB1）, a gene previously reported to positively control strigolactone （SL） signaling. A near-isogenic line （NIL） carrying SCM3 showed enhanced culm strength and increased spikelet number despite the expected decrease in tiller number, indicating that SL also has a positive role in enhancing culm strength and spikelet number. We produced a pyramiding line carrying SCM3 and SCM2, another QTL encoding AP01 involved in panicle development. The NIL-SCM2＋SCM3 showed a much stronger culm than NIL-SCM2 and NIL-SCM3 and an increased spikelet number caused by the additive effect of these QTLs. We discuss the importance of utilizing suitable alleles of these STRONG CULM QTLs without inducing detrimental traits for breeding.

Clinical analysis of high serum IgE in autoimmune pancreatitis

AIM: To clarify the clinical significance of high serum IgE in autoimmune pancreatitis (AIP). METHODS: Forty-two AIP patients, whose IgE was measured before steroid treatment, were analyzed. To evaluate the relationship between IgE levels and the disease activity of AIP, we examined (1) Frequency of high IgE (> 170 IU/mL) and concomitant allergic dis-eases requiring treatment; (2) Correlations between IgG, IgG4, and IgE; (3) Relationship between the presence of extrapancreatic lesions and IgE; (4) Re-lationship between clinical relapse and IgE in patients treated with steroids, and (5) Transition of IgE before and after steroid treatment. RESULTS: IgE was elevated in 36/42 (86%) patients.Concomitant allergic disease was observed in seven patients (allergic rhinitis in three, bronchial asthma in three, and urticaria in one). There were no signifi-cant correlations between IgG, IgG4, and IgE (r = -0.168 for IgG, and r = -0.188 for IgG4). There was no significant difference in IgE in the patients with and without extrapancreatic lesions (526 ± 531 IU/mL vs 819 ± 768 IU/mL, P = 0.163), with and without clini-cal relapse (457 ± 346 IU/mL vs 784 ± 786 IU/mL, P = 0.374). There was no significant difference in IgE between before and after steroid treatment (723 ± 744 IU/mL vs 673 ± 660 IU/mL, P = 0.633). CONCLUSION: Although IgE does not necessarily reflect the disease activity, IgE might be useful for the diagnosis of AIP in an inactive stage.

东亚地区森林类型多样,开展区域生态系统碳循环模拟时应考虑森林类型的差异。论文利用基于叶氮浓度-最大净光合作用速率关系的PnET-CN模型,对东亚地区8个森林生态系统通量观测站点的总生态系统碳交换(GEE)和生态系统呼吸(RE)进行模拟,以探讨模型的适用性并对不确定性来源进行分析。研究结果表明:1PnET-CN模型能较为准确地模拟东亚地区大部分森林生态系统站点的GEE和RE;2模型的适用性排序依次为温带、寒温带、亚热带、热带,模型未能很好地模拟热带湿地森林GEE和RE的季节与年际变异;3在同一气候区中,PnET-CN模型更加适用于针叶林碳交换的模拟;4PnET-CN模型比较准确地反映了东亚森林生态系统GEE、RE对气候因子(例如,温度或辐射)的响应,但在低温、较弱辐射条件下模型低估了GEE,在高温或较强辐射条件下高估了GEE;在低温条件下模型低估了RE,在高温条件下模型高估了RE。针对东亚多个森林通量站点的模拟情况,论文提出模型应在以下方面进行改进:1PnET-CN模型计算物候时除了考虑温度之外还应加入土壤湿度的影响,并对不同气候区森林生态系统赋予不同叶片凋落时长;2PnET-CN模型中温度对GEE的限制以及光合最适温度应该根据不同站点设置;3PnET-CN模型应该考虑森林生态系统对环境胁迫的适应性,加强对干旱等干扰的模拟;4同时对于拥有复杂水文条件的森林生态系统应该改进土壤含水量的算法,以准确反映该类型森林生态系统GEE和RE的季节变化。

Basket pattern blood flow signals discovered in a case of splenic hamartoma by power Doppler ultrasonography

We present the gray-scale ultrasonography (GSUS), power Doppler ultrasonography (PDUS), abdominal computed tomography (CT), and magnetic resonance imaging (MRI) findings for a case of splenic hamartoma in a 27-year-old man, showing a ψ 50 mm homogeneous, iso- and hypo-echoic splenic mass with evidence of a small plural cystic lesion.This splenic hamartoma showed increased vascularity on power Doppler sonograms. PDUS showed multiple circular blood flow signals inside the mass (i.e. a basket pattern),which was consistent with the small plural cystic lesion shown by GSUS. Spectral analysis also confirmed arterial and venous flow. CT scans showed that the mass had lowdensity relative to the normal spleen and MRI showed that the mass was isodense, relative to the normal spleen.Therefore, CT and MRI are not useful for the diagnosis of splenic hamartoma. Ultrasonography can be used to diagnose splenic hamartoma without administration of a contrast material and therefore is an indispensable method for the diagnosis of splenic hamartoma.

A Development of Rom-Exercise Machine Using Linkages(Design of Leg Mechanism by Walking Simulation)

In this study, the design of an automatic ROM-Exercise machine that is constructed witha planar multililnk mechamsm consisting of only revolute pairs is investigated. Namely, equations thatdetermine the minimum moving spaces and relative positions of link required to construct the legmechanism are formulated with consideration of transform functions. For the leg mechanism that isconstructed with a planar eleven-link mechanism, arrangements of each link and optimum linkprofiles avoided mutual interferences among moving links are determined wb consideration of therelative locations of each link in the same plane during a cycle of motion of the mechanism. Based onthe above analytical results, an automatic ROM-Exercise machine that performs within a minimum moving spaces is proposed as a prachcal example. ms study is carried out as part of the students'computcr education to the graduation thesis, in order to improve their creativity and machine designtechnology skills in coniunction with educational advantages. Significam educational results areobtained by using the design techniques mentioned above.

Visceral Fat Accumulation Is Associated with Increased Mortality Rate after Transcatheter Arterial Chemoembolization in Patients with Hepatocellular Carcinoma

Aim: Transcatheter arterial chemoembolization (TACE) is thought to be a safe and effective treatment for hepatocellular carcinoma (HCC). However, in some HCC patients, it potentially shortens survival due to liver damage. We aimed to identify independent factors to predict overall survival of HCC after TACE. Methods: We included a total of 96 consecutive HCC patients who underwent TACE at Kobe University Hospital. Areas of skeletal muscle and fat tissue were measured by computed tomography (CT) scan before TACE. We divided the patients into two groups in terms of the presence or absence of 1-year mortality after TACE. Factors associated with 1-year mortality after TACE were assessed by multivariate analyses, and the optimal cut-off values were evaluated using a propensity score. Results: Multivariate analyses showed that visceral fat accumulation on CT was an independent factor associated with 1-year mortality after TACE (p = 0.033). There were no differences in skeletal muscle area and subcutaneous and intermuscular fat area between the two groups. Cut-off values for visceral fat area associated with 1-year mortality after TACE were defined as 33.3 cm2/m2 for males and 24.4 cm2/m2 for females. Conclusions: High visceral fat area was a prognostic factor associated with increased mortality rate in HCC patients undergoing TACE. Using this value, 1-year mortality risk after TACE would be better estimated before the day TACE was performed.
