Objective The changes in serum adipokines and cytokines related to oxidative stress were examined during 3 months ‘Off to On’ and ‘On to Off’ periods using negatively charged particle-dominant indoor air conditions（NCPDIAC）.Methods Seven volunteers participated in the study,which included ‘OFF to 3 months ON’ periods（ON trials） for a total of 16 times,and ‘ON to 3 months OFF’（OFF trials） periods for a total of 13 *** With the exception of one case,serum amyloid A（SAA） levels decreased significantly during the ON *** Considering that SAA is an acute phase reactive protein such as C reactive protein（CRP）,this observed decrease might indicate the prevention of cardiovascular and atherosclerotic changes,since an increase in high-sensitive CRP is associated with the subsequent detection of these events.

Silicosis patients （SILs） and patients who have been exposed to asbestos develop not only respiratory diseases but also certain immunological disorders. In particular, SIL sometimes complicates autoimmune diseases such as systemic scleroderma, rheumatoid arthritis （known as Caplan syndrome）, and systemic lupus erythematoses. In addition, malignant complications such as lung cancer and malignant mesothelioma often occurr in patients exposed to asbestos, and may be involved in the reduction of tumor immunity. Although silica-induced disorders of autoimmunity have been explained as adjuvant-type effects of silica, more precise analyses are needed and should reflect the recent progress in immunomolecular findings. A brief summary of our investigations related to the immunological effects of silica/asbestos is presented. Recent advances in immunomolecular studies led to detailed analyses of the immunological effects of asbestos and silica. Both affect immuno-competent cells and these effects may be associated with the pathophysiological development of complications in silicosis and asbestos-exposed patients such as the occurrence of autoimmune disorders and malignant tumors, respectively. In addition, immunological analyses may lead to the development of new clinical tools for the modification of the pathophysiological aspects of diseases such as the regulation of autoimmunity or tumor immunity using cellmediated therapies, various cytokines, and molecule-targeting therapies. In particular, as the incidence of asbestosrelated malignancies is increasing and such malignancies have been a medical and social problem since the summer of 2005 in Japan, efforts should be focused on developing a cure for these diseases to eliminate nationwide anxiety.

BACKGROUND Many advanced hepatocellular carcinoma(HCC) patients are receiving sorafenib treatment. Sorafenib reportedly improves overall survival(OS) significantly in patients with HCC. Prediction of sorafenib response and prognosis in patients with HCC receiving sorafenib treatment are important due to the potentially serious side effects of sorafenib. A disintegrin-like and metalloproteinase with thrombospondin type-1 motifs 13(ADAMTS13) and von Willebrand factor(VWF) are associated with the pathophysiology of liver cirrhosis and HCC through their roles in hypercoagulability; they are also associated with angiogenesis via vascular endothelial growth factor(VEGF). The imbalance between ADAMTS13 and VWF was associated with prognosis of various cancers in patients undergoing *** To investigate ADAMTS13 and VWF as potential biomarkers for sorafenib response and prognosis in patients with HCC receiving sorafenib *** Forty-one patients with HCC receiving sorafenib treatment were included in this study. The initial daily sorafenib dose was 400 mg in all patients. ADAMTS13 activity(ADAMTS13:AC), VWF antigen(VWF:Ag), VEGF levels were determined by enzyme-linked immunosorbent assay. Univariate andmultivariate analyses were used to determine predictive factors for sorafenib response and prognosis in patients with HCC receiving sorafenib *** ADAMTS13:AC was significantly higher in patients with stable disease(SD),partial response(PR), and complete response(CR) than in those with progressive disease(PD)(P < 0.05). In contrast, VWF:Ag and the VWF:Ag/ADAMTS13:AC ratio were significantly lower in patients with SD, PR, and CR than in those with PD(P < 0.05 for both). Multivariate analysis showed that the VWF:Ag/ADAMTS13:AC ratio was the only predictive factor for sorafenib response and ADAMTS13:AC was the only prognostic factor in patients with HCC receiving sorafenib treatment. Rifaximin ameliorates hepatic encephalopathy and endotoxemia without affecting the gut microbiome diversity

AIM To determine the efficacy of rifaximin for hepatic encephalopathy(HE) with the linkage of gut microbiome in decompensated cirrhotic *** Twenty patients(12 men and 8 women; median age, 66.8 years; range, 46-81 years) with decompensated cirrhosis(Child-pugh score > 7) underwent cognitive neuropsychological testing, endotoxin analysis, and fecal microbiome assessment at baseline and after 4 wk of treatment with rifaximin 400 mg thrice a day. HE was determined by serum ammonia level and number connection test(NCT)-A. Changes in whole blood endotoxin activity(EA) was analyzed by endotoxinactivity assay. Fecal microbiome was assessed by 16 S ribosome RNA(rR NA) gene *** Treatment with rifaximin for 4 wk improved hyperammonemia(from 90.6 ± 23.9 μg/d L to 73.1 ± 33.1 μg/dL; P < 0.05) and time required for NCT(from 68.2 ± 17.4 s to 54.9 ± 20.3 s; P < 0.05) in patients who had higher levels at baseline. Endotoxin activity was reduced(from 0.43 ± 0.03 to 0.32 ± 0.09; P < 0.05) in direct correlation with decrease in serum ammonia levels(r = 0.5886, P < 0.05). No statistically significant differences were observed in the diversity estimator(Shannon diversity index) and major components of the gut microbiome between the baseline and after treatment groups(3.948 ± 0.548 at baseline vs 3.980 ± 0.968 after treatment; P = 0.544), but the relative abundances of genus Veillonella and Streptococcus were *** Rifaximin significantly improved cognition and reduced endotoxin activity without significantly affecting the composition of the gut microbiome in patients with decompensated cirrhosis.

The aim of this study was to measure the motion of the body surface induced by heartbeat by using a high-precision laser sensor in order to provide a reference for other research on medical remote sensing techniques that are unconstrained and non-contact. Body surface motions at some points on the chest and on the dorsal region were measured for eight men with a laser displacement sensor. For comparison, an electrocardiograph was also used to monitor the heartbeat. As a result, we confirmed cyclic motion of about 0.1 mm in amplitude everywhere on the body surface, not only from the front of the body but also from the back and the neck. In contrast, which part on the body was most suitable for measurement had to be taken into account because a difference in motion amplitudes was observed in different parts on the body. The amplitude of the motions also depended on the conditions of the body surface as well as underneath the skin. These results show the possibility of acquiring information about heartbeats from anywhere on the body by using an unconscious sensing technique, and the potential for the technique to monitor the condition of the body and personal physical characteristics.

Objective The custom-homebuilding company, Cosmic Garden Co. Ltd., located in Okayama City, Japan was established in 1997 and uses specific natural ore powder （SNOP） in wall materials and surveys customers in order to improve allergic symptoms. Methods To investigate the biological effects of SNOP, patients with a pollen allergy were recruited to stay in a room surrounded by cloth containing SNOP （CCSNOP）, and their symptoms and various biological parameters were compared with those of individuals staying in a room surrounded by control non-woven cloth （NWC）. Each stay lasted 60 min. Before and immediately after the stay, a questionnaire regarding allergic symptoms, as well as POMS （Profile of Mood Status） and blood sampling, was performed. Post-stay minus pre-stay values were calculated and compared between CCSNOP and NWC groups. Results Results indicated that some symptoms, such as nasal obstruction and lacrimation, improved, and POMS evaluation showed that patients were calmer following a stay in CCSNOP. Relative eosinophils, non-specific Ig E, epidermal growth factor, monocyte chemotactic protein-I, and tumor necrosis factor-a increased following a stay in CCSNOP. Conclusion This ore powder improved allergic symptoms, and long-term monitoring involving 1 to 2 months may be necessary to fully explore the biological and physical effects of SNOP on allergic patients.

Objective: Influenza is a highly infectious viral disease, which occurs epidemically almost every winter in Japan. Rapid screening of patients with suspected influenza in places of mass gathering is important to delay or prevent transmission of the infection. The aim of this study was to assess the effectiveness of our newly developed infection screening system that employed vital signs and percutaneous oxygen saturation (SpO2) as parameters in a clinical setting. Methods: Since SpO2 accurately reflects respiratory status during influenza virus infection, we upgraded our previous system by adding SpO2 as a new parameter to improve the screening accuracy. This system instantly measures SpO2 and vital signs (i.e., heart rate, respiration rate, and facial temperature), which automatically detects infected individuals via a neural network-based nonlinear discriminant function using these derived parameters. We tested the system on 45 patients with seasonal influenza (35.8℃ < body temperature < 40.0℃, 18-35 years) and 64 normal control subjects (35.0℃ < body temperature < 37.5℃, 18-30 years) at Japan Self-Defense Central Hospital in 2012. Results: The system identified 40/45 patients with influenza and 60/64 normal control subjects, and provided sensitivity, specificity, and positive and negative predictive value (PPV, NPV) of 88.8%, 93.8%, 90.9%, and 92.3%, respectively. By including SpO2 as a screening parameter, we achieved superior sensitivity and NPV compared to that reported in our previous paper (sensitivity = 88%;NPV = 82%). Conclusions: Our results suggest that SpO2 is a good screening parameter that improves the accuracy of infection screening. The proposed system has the potential to efficiently identify infected individuals, thereby delaying or preventing the spread of infection during epidemic seasons.
