Perioperative intensive insulin therapy using artificial endocrine pancreas in patients undergoing pancreatectomy

Perioperative glycemic control is important for reducing postoperative infectious complications. However, clinical trials have shown that efforts to maintain normoglycemia in intensive care unit patients result in deviation of glucose levels from the optimal range, and frequent attacks of hypoglycemia. Tight glycemic control is even more challenging in those undergoing pancreatic resection. Removal of lesions and surrounding normal pancreatic tissue often cause hormone deficiencies that lead to the destruction of glucose homeostasis, which is termed pancreatogenic diabetes. Pancreatogenic diabetes is characterized by the occurrence of hyperglycemia and iatrogenic severe hypoglycemia, which adversely effects patient recovery. Postoperatively, a variety of factors including surgical stress, inflammatory cytokines, sympathomimetic drug therapy, and aggressive nutritional support can also affect glycemic control. This review discusses the endocrine aspects of pancreatic resection and highlights postoperative glycemic control using a closed-loop system or artificial pancreas. In previous experiments, we have demonstrated the reliability of the artificial pancreas in dogs with total pancreatectomy, and its postoperative clinical use has been shown to be effectiveand safe, without the occurrence of hypoglycemic episodes, even in patients after total pancreatectomy. Considering the increasing requirement for tight perioperative glycemic control and the recognized risk of hypoglycemia, we propose the use of an artificial endocrine pancreas that is able to monitor continuously blood glucose concentrations with proven accuracy, and administer automatically substances to return blood glucose concentration to the optimal narrow range.

A nucleotide substitution at the 5′ splice site of intron 1 of rice HEADING DATE 1(HD1) gene homolog in foxtail millet, broadly found in landraces from Europe and Asia

We investigated genetic variation of a rice HEADING DATE 1(HD1) homolog in foxtail ***, we searched for a rice HD1 homolog in a foxtail millet genome sequence and designed primers to amplify the entire coding sequence of the gene. We compared full HD1 gene sequences of 11 accessions(including Yugu 1, a Chinese cultivar used for genome sequencing) from various regions in Europe and Asia, found a nucleotide substitution at a putative splice site of intron 1, and designated the accessions with the nucleotide substitution as carrying a splicing variant. We verified by RT-PCR that this single nucleotide substitution causes aberrant splicing of intron 1. We investigated the geographical distribution of the splicing variant in 480 accessions of foxtail millet from various regions of Europe and Asia and part of Africa by d CAPS and found that the splicing variant is broadly distributed in Europe and Asia. Differences of heading times between accessions with wild type allele of the HD1 gene and those with the splicing variant allele were unclear. We also investigated variation in 13 accessions of ssp. viridis, the wild ancestor, and the results suggested that the wild type is predominant in the wild ancestor.

High-velocity frictional behavior of Longmenshan fault gouge from Hongkou outcrop and its implications for dynamic weakening of fault during the 2008 Wenchuan earthquake

High-velocity friction experiments were conducted on clayey fault gouge collected from Hongkou outcrop of Beichuan fault, located at the southwestern part of Longmenshan fault system that caused the disastrous 2008 Wenchuan earthquake. The ultimate purpose of this study is to reproduce this earthquake by modeling based on measured frictional properties. Dry gouge of about 1 mm in thickness was deformed dry at slip rates of 0.01 to 1.3 m/s and at normal stresses of 0.61 to 3.04 MPa, using a rotary-shear high-velocity frictional testing machine. The gouge displays slip weakening behavior as initial peak friction decays towards steady-state values after a given displacement. Both peak friction and steady-state friction remain high at slow slip rates are exam- ined and gouge only exhibits dramatic weakening at high slip rates, with steady-state friction coefficient values of about 0.1 to 0.2. Specific fracture energy ranges from 1 to 4 MN/m in our results and this is of the same order as seismically determined values. Low friction coefficients measured on experimental faults are in broad agree- ment with lack of thermal anomaly observed from temperature measurements in WFSD-1 drill hole （Wenchuan Earthquake Fault Scientific Drilling Project）, which can be explained by even smaller friction coefficient for the Wenchuan earthquake fault. High-velocity friction experiments with pore water needs to be done to see if even smaller friction is attained or not. Shiny slickenside surfaces form at high slip rates, but not at slow slip rates. Slip zone with slickenside surface changes its color to dark brown and forms duplex-like microstructures, which are similar to those microstructures found in the fault gouges from the Hongkou outcrop. Detailed comparisons between experimentally deformed gouge samples and WFSD drill cores in the future will reveal how much we could reproduce the dynamic weakening processes in operation in fault zones during Wenchuan earthquake at present.

Postoperative hyperglycemia is common in critically ill patients, even in those without a prior history of diabetes mellitus. It is well known that hyperglycemia induced by surgical stress often results in dysregulation of liver metabolism and immune function, impairing postoperative recovery. Current evidence suggests that maintaining normoglycemia postoperatively improves surgical outcome and reduces the mortality and morbidity of critically ill patients. On the basis of these observations, several large randomized controlled studies were designed to evaluate the benefit of postoperative tight glycemic control with intensive insulin therapy. However, intensive insulin therapy carries the risk of hypoglycemia, which is linked to serious neurological events. Recently, we demonstrated that perioperative tight glycemic control in surgical patients could be achieved safely using a closed-loop glycemic control system and that this decreased both the incidence of infection at the site of the surgical incision, without the appearance of hypoglycemia, and actual hospital costs. Here, we review the benefits and requirements of perioperative intensive insulin therapy using a dosed-loop artificial endocrine pancreas system in hepatectomized patients. This novel intensive insulin therapy is safe and effectively improves surgical outcome after hepatic resection.

Inflammatory cytokines promote inducible nitric oxide synthase-mediated DNA damage in hamster gallbladder epithelial cells

AIM: To investigate the link between chronic biliary inflammation and carcinogenesis using hamster gallbladder epithelial cells. METHODS: Gallbladder epithelial cells were isolated from hamsters and cultured with a mixture of inflammatory cytokines including interleukin-1β,interferon-γ,and tumor necrosis factor-α. Inducible nitric oxide synthase (iNOS) expression,nitric oxide (NO) generation,and DNA damage were evaluated. RESULTS: NO generation was increased significantly following cytokine stimulation,and suppressed by an iNOS inhibitor. iNOS mRNA expression was demonstrated in the gallbladder epithelial cells during exposure to inflammatory cytokines. Furthermore,NO-dependent DNA damage,estimated by the comet assay,was significantly increased by cytokines,and decreased to control levels by an iNOS inhibitor. CONCLUSION: Cytokine stimulation induced iNOS expression and NO generation in normal hamster gallbladder epithelial cells,which was sufficient to cause DNA damage. These results indicate that NO-mediated genotoxicity induced by inflammatory cytokines through activation of iNOS may be involved in the process of biliary carcinogenesis in response to chronic inflammation of the biliary tree.

Protein translocation across membranes is a fundamental cellular process. The majority of the proteins of organelles such as mitochondria and chloroplasts is synthesized in the cytosol and subsequently imported in a posttranslational manner. The precursor proteins have to be unfolded at least for translocation, but it has also been assumed that they are unfolded during transport to the organelle in the cytosol. Unfolding is governed by chaperones and the translocon itself. At the same time, chaperones provide the energy for the import process. The energetic properties of the chloroplast translocon were studied by import of the Ig-like module of the muscle protein titin fused to the transit peptide of the chloroplast targeted oxygen evolving complex subunit of 33 kDa （OE33）. Our results suggest that p（OE33）titin is folded prior to import and that translocation is initiated by unfolding after having bound to the translocon at the chloroplast surface. Using a set of stabilizing and destabilizing mutants of titin previously analyzed by atomic force microscopy and as passenger for mitochondrial translocation, we studied the unfolding force provided by the chloroplast translocon. Based on these results, a model for translocation is discussed.

Diffuse malignant pleural mesothelioma （DMPM） is a rare thoracic malignancy, but its incidence isdramatically increasing worldwide as a result ot widespread use of asbestos. The World Health Organization classifies DMPM into three types： epithelioid, sarcomatoid, and biphasic types. DMPM remains suffering poor prognosis and the diagnosis should always be based on adequate, representative tissue samples. There still remains a considerable number of patients with DMPM who are misdiagnosed after a complete investigation including thoracoscopic biopsies.

Fabrication of Silver Nanowire Transparent Electrodes at Room Temperature

Silver nanowires （AgNWs） surrounded by insulating poly（vinylpyrrolidone） have been synthesized by a polyol process and employed as transparent electrodes. The AgNW transparent electrodes can be fabricated by heattreatment at about 200 ℃ which forms connecting junctions between AgNWs. Such a heating process is, however, one of the drawbacks of the fabrication of AgNW electrodes on heat-sensitive substrates. Here it has been demonstrated that the electrical conductivity of AgNW electrodes can be improved by mechanical pressing at 25 MPa for 5 s at room temperature. This simple process results in a low sheet resistance of 8.6 Ω/square and a transparency of 80.0%, equivalent to the properties of the AgNW electrodes heated at 200 ℃. This technique makes it possible to fabricate AgNW transparent electrodes on heat-sensitive substrates. The AgNW electrodes on poly（ethylene terephthalate） films exhibited high stability of their electrical conductivities against the repeated bending test. In addition, the surface roughness of the pressed AgNW electrodes is one-third of that of the heat-treated electrode because the AgNW junctions are mechanically compressed. As a result, an organic solar cell fabricated on the pressed AgNW electrodes exhibited a power conversion as much as those fabricated on indium tin oxide electrodes. These findings enable continuous roll-to-roll processing at room temperature, resulting in relatively simple, inexpensive, and scalable processing that is suitable for forthcoming technologies such as organic solar cells, flexible displays, and touch screens.

Pharmacokinetic study of paclitaxel in malignant ascites from advanced gastric cancer patients

AIM： To examine the paclitaxel concentrations in plasma and ascites after its intravenous administration in patients with ascites due to peritonitis carcinomatosa resulting from advanced gastric cancer. METHODS： Two patients with ascites due to peritonitis carcinomatosa resulting from gastric cancer were included in this study. The paclitaxel concentrations in plasma and ascites were investigated for 72 h in case 1 and 168 h in case 2 after intravenous administration. RESULTS： The paclitaxel concentration in plasma peaked immediately after administration, followed by rapid decrease below the threshold value of 0.1 μmol （85 ng/mL） within 24 h. In contrast, the paclitaxel concentration in ascites increased gradually for 24 h after administration to a level consistent with the level found in plasma. After 24 h the level of paclitaxel in ascites and plasma became similar, with the optimal level being maintained up to 72 h following administration. CONCLUSION： The concentration of paclitaxel in ascites is maintained within the optimal level for the treatment of cancer cells for up to 72 h after intravenous administration. Paclitaxel is a promising drug for the treatment of malignant ascites of gastric cancer.
