Purpose: To clarify which patients need careful neurourological management after abdominal radical hysterectomy (RH) by investigating the effects of adjuvant radiotherapy and reversibility of neurogenic bladder (NB) on the storage function as well as the effects of urethral resistance on the emptying function. Methods: Data from sixty-two patients referred to our NB clinic after RH were retrospectively reviewed. Findings of urodynamic studies performed at 3 (UDS-1) and 12 (UDS-2) months after treatment were compared, and logistic analysis was used to calculate the odds ratio (OR) of the effects of radiotherapy and irreversible NB on decreased bladder capacity and decreased compliance. Irreversible NB was defined as the need for clean intermittent catheterization at the last follow-up. Results: At the median follow-up period of 41 months, 60% of the patients continued to require clean intermittent catheterization. Of patients with irreversible NB and radiotherapy, 80% had decreased bladder capacity and decreased compliance at UDS-2. For decreased bladder capacity and decreased compliance, ORs of adjuvant radiotherapy at UDS-2 were 38.42 (p Conclusions: Careful neurourological follow-up after RH is mandatory for patients who undergo adjuvant radiotherapy and have irreversible NB with impaired urethral relaxation.

Perinatal Morbidity, Mortality, and Neurodevelopmental Outcomes of Neonates with Fetal Growth Restriction

Objective: This study aimed to assess perinatal morbidity, mortality rates, and neurodevelopmental outcomes in the management of fetal growth restriction (FGR) at a single tertiary institute. Methods: Among 2465 deliveries between 2013 and 2019, 109 cases of FGR were reviewed retrospectively for causes, indications for pregnancy termination, perinatal death, overall neonatal outcomes, and long-term prognosis. Results: Excluding FGR due to congenital anomalies (n = 17), the mortality rate was 3.3% (3/92). One neonate delivered at 23 weeks developed cerebral palsy (1.1%). Retinopathy of prematurity occurred in four neonates (4.3%). Neurodevelopmental disorders were present in six neonates (6.5%), all of whom were delivered at 32 - 38 weeks. Significantly lower gestational age at delivery, lower birth weight, and higher umbilical artery resistance indices were observed in neonates with neurodevelopmental disorders. Conclusions: Intact survival before 27 weeks of gestation at delivery with FGR is uncommon. Neurodevelopmental disorders may still develop after delivery at 32 - 38 weeks;consideration should be given to the timing of delivery usingfetal ductus venosus Doppler waveforms measurements to reduce neurodevelopmental disorders.

This study investigates how financial literacy and behavioral traits affect the adoption of electronic payment(ePayment)services in Japan.We construct a financial literacy index using a representative sample of 25,000 individuals from the Bank of Japan’s 2019 Financial Literacy Survey.We then analyze the relationship between this index and the extensive and intensive usage of two types of payment services:electronic money(e-money)and mobile payment apps.Using an instrumental variable approach,we find that higher financial literacy is positively associated with a higher likelihood of adopting ePayment services.The empirical results suggest that individuals with higher financial literacy use payment services more frequently.We also find that risk-averse people are less likely to adopt and use ePayment services,whereas people with herd behavior tend to adopt and use ePayment services more.Our empirical results also suggest that the effects of financial literacy on the adoption and use of ePayment differ among people with different behavioral traits.

Partial dehydration of brucite and its implications for water distribution in the subducting oceanic slab

Hydrous minerals within the subducting oceanic slab are important hosts for water.Clarification of the stability field of hydrous minerals helps to understand transport and distribution of water from the surface to the Earth’s interior.We investigated the stability of brucite,a prototype of hydrous minerals,by means of electrical conductivity measurements in both open and closed systems at 3 GPa and temperatures up to 1300 K.Dramatic increase of conductivity in association with characteristic impedance spectra suggests that partial dehydration of single-crystal brucite in the open system with a low water fugacity occurs at 950 K,which is about 300 K lower than those previously defined by phase equilibrium experiments in the closed system.By contrast,brucite completely dehydrates at 1300 K in the closed system,consistent with previous studies.Partial dehydration may generate a highly defective structure but does not lead to the breakdown of brucite to periclase and water immediately.Water activity plays a key role in the stability of hydrous minerals.Low water activity(a H_(2)O)caused by the high wetting behavior of the subducted oceanic slab at the transition zone depth may cause the partial dehydration of the dense hydrous magnesium silicates(DHMSs),which significantly reduces the temperature stability of DHMS(this mechanism has been confirmed by previous study on super hydrous phase B).As a result,the transition zone may serve as a‘dead zone’for DHMSs,and most water will be stored in wadsleyite and ringwoodite in the transition zone.

BACKGROUND Synchronous multiple lung cancers are rare and refer to the simultaneous presence of two or more primary lung tumors,which present significant challenges in terms of diagnosis and treatment.CASE SUMMARY We report a case of multiple synchronous lung cancers with hilar lymph node metastasis of small cell carcinoma of unknown origin in a 73-year-old man.Transbronchial lung biopsy revealed squamous cell carcinoma.Although enlargement of lymph node 12u was detected,no distant metastases were observed.The patient was preoperatively diagnosed with T1cN0M0 and underwent thoracoscopic right upper lobectomy with nodal dissection(ND2a).Based on histopathological findings,the primary lesion was squamous cell carcinoma.A microinvasive adenocarcinoma was also observed on the cranial side of the primary lesion.Tumors were detected in two resected lymph nodes(#12u and#11s).Both tumors were pathologically diagnosed as small cell carcinomas.The primary lesion of the small cell carcinoma could not be identified even by whole-body imaging;however,chemotherapy was initiated for hilar lymph node metastasis of the small cell carcinoma of unknown origin.CONCLUSION Multiple synchronous lung cancers can be accompanied by hilar lymph node metastasis of small cell carcinomas of unknown origin.

Aslanger’s sign,also known as the arterial pulse tapping artifact or electromechanical association artifact,is an electrocardiographic artifact caused by arterial pulsation at the site where the limb leads of the standard 12-lead electrocardiogram near the radial or posterior tibial arteries are positioned,particularly in hyperdynamic states.[1–8]It occurs in every cardiac cycle with a constant coupling interval between the QRS complex and artifact.This synchronization with the underlying heart rhythm makes it less likely to be recognized as an artifact compared to unsynchronized artifacts,such as those caused by limb movement and inadequate contact between the electrode and skin.[1,2,7,8]Almost all reported cases of Aslanger’s sign exhibit an unusual waveform morphology in all 12 leads except one of the standard 12-lead electrocardiogram.This sign is often confused with an electrocardiographic finding commonly observed during acute coronary events.

Electrical conductivity of melts: implications for conductivity anomalies in the Earth's mantle

Magmatic liquids,including silicate and carbonate melts,are principal agents of mass and heat transfer in the Earth and terrestrial planets,and they play a crucial role in various geodynamic processes and in Earth’s evolution.Electrical conductivity data of these melts elucidate the cause of electrical anomalies in Earth’s interior and shed light on the melt structure.With the improvement in high-pressure experimental techniques and theoretical simulations,major progress has been made on this front in the past several decades.This review aims to summarize recent advances in experimental and theoretical studies on the electrical conductivity of silicate and carbonate melts of different compositions and volatile contents under high temperature and pressure.The electrical conductivity of silicate melts depends strongly on temperature,pressure,water content and the ratio of non-bridging oxygens to tetrahedral cations(NBO/T).By contrast,the electrical conductivity of carbonate melts exhibits a weak dependence on temperature and pressure due to their fully depolymerized structure.The electrical conductivity of carbonate melts is higher than that of silicate melts by at least two orders of magnitude.Water can increase electrical conductivity significantly and reduce the activation energy of silicate melts.Conversely,this effect is weak for carbonate melts.In addition,the replacement of alkali-earth elements(Ca2+or Mg2+) with alkali elements causes a significant decrease in the electrical conductivity of carbonate melts.A distinct compensation trend is revealed for the electrical conductivity of silicate and carbonate melts under anhydrous and hydrous conditions.Several important applications of laboratory-based melt conductivity are introduced in order to understand the origin of high-conductivity anomalies in the Earth’s mantle.Perspectives for future studies are also provided.

Perioperative fluid management in major hepatic resection: an integrative review

BACKGROUND: Fluid intervention and vasoactive pharmacological support during hepatic resection depend on the preference of the attending clinician, institutional resources, and practice culture. Evidence-based recommendations to guide perioperative fluid management are currently limited. Therefore, we provide a contemporary clinical integrative overview of the fundamental principles underpinning fluid intervention and hemodynamic optimization for adult patients undergoing major hepatic resection. DATA SOURCES: A literature review was performed of MEDLINE, EMBASE and the Cochrane Central Registry of Controlled Trials using the terms 'surgery', 'anesthesia', 'starch', 'hydroxyethyl starch derivatives', 'albumin', 'gelatin', 'liver resection', 'hepatic resection', 'fluids', 'fluid therapy', 'crystalloid', 'colloid', 'saline', 'plasma-Lyte', 'plasmalyte', 'hartmann's', 'acetate', and 'lactate'. Search results for MEDLINE and EMBASE were additionally limited to studies on human populations that included adult age groups and publications in English. RESULTS: A total of 113 articles were included after appropriate inclusion criteria screening. Perioperative fluid management as it relates to various anesthetic and surgical techniques is discussed.CONCLUSIONS: Clinicians should have a fundamental understanding of the surgical phases of the resection, hemodynamic goals, and anesthesia challenges in attempts to individualize therapy to the patient's underlying pathophysiological condition. Therefore, an ideal approach for perioperative fluid therapy is always individualized. Planning and designing large-scale clinical trials are imperative to define the optimal type and amount of fluid for patients undergoing major hepatic resection. Further clinical trials evaluating different intraoperative goal-directed strategies are also eagerly awaited.
