BACKGROUND The impacts of chemotherapy on patients with malignant gastrointestinal obstructions remain unclear,and multicenter evidence is *** To evaluate the effectiveness and safety of chemotherapy in patients with unresectable malignant gastrointestinal *** We conducted a multicenter retrospective cohort study that compared the chemotherapy group who received any chemotherapeutics after interventions,including palliative surgery or selfexpandable metal stent placement,for unresectable malignant gastrointestinal obstruction vs the best supportive care(BSC)group between 2014 and 2019 in nine *** primary outcome was overall survival,and the secondary outcomes were patency duration and adverse events,including gastrointestinal perforation and gastrointestinal *** In total,470 patients in the chemotherapy group and 652 patients in the BSC group were *** the follow-up period of 54.1 mo,the median overall survival durations were 19.3 mo in the chemotherapy group and 5.4 mo in the BSC group(log-rank test,P<0.01).The median patency durations were 9.7 mo[95% confidence interval(CI):7.7-11.5 mo]in the chemotherapy group and 2.5 mo(95%CI:2.0-2.9 mo)in the BSC group(log-rank test,P<0.01).The perforation rate was 1.3%(6/470)in the chemotherapy group and 0.9%(6/652)in the BSC group(P=0.567).The gastrointestinal bleeding rate was 1.5%(7/470)in the chemotherapy group and 0.5%(3/652)in the BSC group(P=0.105).CONCLUSION Chemotherapy after interventions for unresectable malignant gastrointestinal obstruction was associated with increased overall survival and patency duration.

Since first establishing thermal measurement techniques for micrometer-scale wires,various methods have been devised and improved ***,the uncertainty of different measurements on the same sample has not yet been *** this work,a round robin test was performed to compare the thermal conductivity and thermal diffusivity measurement methods for a fine metal *** tested material was a pure gold wire,with a diameter of 30μ*** wire was cut into certain lengths and distributed to four institutions using five different measurement methods:the direct current(DC)self-heating method,the DC heating T-type method,the 3ωmethod for thermal conductivity,the scanning laser heating alternating current(AC)method,and the spot periodic heating radiation thermometry method for thermal *** completing the measurements,the reported thermal conductivity and thermal diffusivity at room temperature,i.e.,317 W·m^(-1)·K^(-1)and 128×10^(-6)m^(2)·s^(-1),respectively,were adopted as references for comparison with the measurement *** advantages and disadvantages of each method are described in terms of the effect of electrical and thermal junctions fabricated on a wire,such as an electrode,a thermocouple,and a heat *** knowledge obtained from the tested methods will be useful for selecting and designing a measurement technique for various wire-like materials.

Investigate Targeted Factors to Achieve Prediction Goal in Stroke Convalescence in Terms of Causal Relationships of Prediction Error

Background and Purpose: To investigate target functional independence measure (FIM) items to achieve the prediction goal in terms of the causal relationships between prognostic prediction error and FIM among stroke patients in the convalescent phase using the structural equation modeling (SEM) analysis. Methods: A total of 2992 stroke patients registered in the Japanese Rehabilitation Database were analyzed retrospectively. The prediction error was calculated based on a prognostic prediction formula proposed in a previous study. An exploratory factor analysis (EFA) then the factor was determined using confirmatory factorial analysis (CFA). Finally, multivariate analyses were performed using SEM analysis. Results: The fitted indices of the hypothesized model estimated based on EFA were confirmed by CFA. The factors estimated by EFA were applied, and interpreted as follows: “Transferring (T-factor),” “Dressing (D-factor),” and “Cognitive function (C-factor).” The fit of the structural model based on the three factors and prediction errors was supported by the SEM analysis. The effects of the D- and C-factors yielded similar causal relationships on prediction error. Meanwhile, the effects between the prediction error and the T-factor were low. Observed FIM items were related to their domains in the structural model, except for the dressing of the upper body and memory (p < 0.01). Conclusions: Transfer, which was not heavily considered in the previous prediction formula, was found in causal relationships with prediction error. It is suggested to intervene to transfer together with positive factors to recovery for achieving the prediction goal.

Lateral Stress Concentration in Localized Interlayer Rock Stratum and the Impact on Deep Multi-Seam Coal Mining

To explore the impact of lateral stress concentration in interlayer rock stratum on the exploitation of protected coal seam, a field experiment was carried out in a multi-seam mining structure. Lateral stress redistribution and interlayer rock failure behavior were surveyed. Then an assistant numerical investigation was implemented to evolve the effect of liberated seam mining and its influence on stress reconstruction in surrounding rock mass. The cause of lateral stress concentration and its impact were discussed finally. Key findings turn out that a certain lateral stress increases in interlayer rock stratum and concentrates on its lower region. Lateral stress concentration and interlayer rock failure are interactional. The former is an inducing factor of the latter;the latter promotes the increase of concentration degree. Extent of lateral stress concentration increases to the maximum as seam distance is about 50 m. But the efficacy of liberated seam mining decreases as the seam spacing gets larger. Protected seam mining is then classified based upon the impact of lateral stress concentration, which helps to prevent the rock burst hazard and then to achieve a reliable mining in deep mines.

Interaction between Caregiving Ability and Home Discharge Using Severity Classification upon Discharge

Background: The interaction between self-care and caregiving ability in home discharged stroke patients is unclear. Purpose: This study investigates the interaction between caregiving ability and individual self-care activities for post-stroke patients upon home discharge based on their motor-Functional Independence Measure (m-FIM) score. Methods: A total of 2626 stroke patients registered in the Japanese Rehabilitation Database were retrospectively analyzed. Extracted data were divided into three groups based on the m-FIM score at discharge and classified into two groups based on their discharge destination (home discharge, non-home discharge). After the data were modified as mean-centered values, a hierarchical multiple regression analysis evaluated the change in variance amount (ΔR2) using discharge destination as the dependent variable. Model 1 used two independent variables (Rankin Scale and cognitive FIM score), model 2 used two independent variables (caregiving ability and individual self-care), which were added to model 1, and model 3 used an interaction term value, which was added to model 2. Furthermore, a simple slope analysis was performed for these interaction effects. Results: The ΔR2 exhibited six and five self-care items in the moderate and mild groups, respectively. The interaction was significant on simple slope analysis in the moderate group for six self-care items (except dressing upper body, toileting, and bowel management) and in the mild group for three self-care items, including dressing upper body, bladder management, and climbing stairs). Conclusion: This study suggests the need for intervention, especially bladder management and single-foot standing ability during step climbing, in stroke patients discharged to home. In addition, caregiving ability is one of the factors that should be considered in mild group.

Relation between Fluidability of Flyash Cement and Properties of Flyash Particles

A large amount of waste is produced by preparation plants. The properties of the waste vary depending on the mineralogical contents of the mother rock in which the coal is embedded. The waste quality depends on the method of mining and cleaning. The waste mainly consists of clays, quartz, carbonaceous materials, mica, pyrites, and so on. About 80% of coal-ash is now utilized and the remainder is disposed of at disposal sites. However, the life span of disposal sites is limited, and it is difficult to find new disposal sites. It is therefore requested that the percentage of utilized flyash be increased in all applicable fields in Japan. Thus, it is very desirable that cement supplemented with flyash be used as injection material for natural/artificial openings. With this perspective in mind, we attempt to clarify how much the size and shape of the flyash affects the properties of the injection mixture by way of an injection test.

Numerical Study on an Applicable Underground Mining Method for Soft Extra-Thick Coal Seams in Thailand

The EGAT Mae Moh Mine is the largest open pit lignite mine in Thailand and it produces lignite about 16 million tons annually. In the near future, the pit limit of the mine will be reached and underground mine will then be developed through the open pit in the depth of 400 - 600 m from the surface. However, due to the challenges for underground mining such as poor geological conditions, extra thickness (20 - 30 m) of coal seams, and weak mechanical properties of coal seams and the surrounding rock, the success possibility of underground mining and an applicable underground mining method is being investigated at the present. The paper discusses the applicability of multi-slice bord-and-pillar method for the soft extra thick coal seams in the Mae Moh mine by means of numerical analyses using the 3D finite difference code “FLAC3D”.

Impact of Underground Mining on Shaft Lining and Aquifer in Eastern China

Serious shaft lining ruptures have often occurred in the eastern part of China since 1987 due to the complicated geological conditions. This paper tries to find out the relationship between mechanisms of shaft lining rupture and the underground mining process. The analysis is based on the existence typical engineering and geological conditions in eastern China;the impact of underground mining on the shaft lining and aquifer layer is analyzed by using numerical method. The impact factors such as different depths, thicknesses, mining widths of coal seam and different distances to the shaft are used in the analysis. The mining area under the aquifer which near the shaft lining has a significant impact on the shaft lining due to mining process, and increases the risk of occurrence of shaft lining rupture.
