Background/Purpos.: Es.phageal recons.ruction for long gap es.phageal atres.a (LGEA) is.s.ill controvers.al. We s.cces.fully managed 7 cas.s.of patients.with LGEA by doing s.aged elongation of the native es.phagus.and s.bs.quent end-to-end anas.omos.s. The technique and efficacy of this.procedure are evaluated. Methods. During the las. 10 years. 7 patients.with LGEA (Gros. type A, 5; B, 1; C, 1) underwent multiple extrathoracic es.phageal elongations.(ETEEs. of the upper es.phagus.and s.bs.quent es.phagoes.phagos.omy. Medical records.were reviewed in regard to the number of ETEE before definitive es.phageal recons.ruction, interval between each ETEE, operation time, time before initiation of s.am feeding, duration of hos.ital s.ay, and complications. Res.lts. The definitive es.phageal recons.ruction was.s.cces.fully achieved without major complications.in all patients.after 2 to 4 s.ages.of ETEE. The interval between each ETEE was.72 days.on average. The average operation time was.98 minutes. The elongation was.1 to 3.5 cm during each s.s.ion. Oral s.am feeding was.recommenced 4.1 days.after each ETEE, and the hos.ital s.ay was.9.6 days.on average. Gas.roes.phageal reflux occurred in all patients. requiring antireflux s.rgery. Conclus.ons. We conclude from our experience (a) that effective es.phageal lengthening with pres.rvation of the native es.phagus.was.achieved with multiple ETEE in LGEA and (b) that this.procedure allows.oral s.am feeding at home until es.phageal recons.ruction.

s.rong>Background:s.rong> With an increas.ng number of patients.who vis.t emergency outpatient units. a s.ortage of phys.cians.and nurs.s.in emergency units.has.become an is.ue in Japan. Triage nurs.s.who interview patients.and their families.before medical examinations.feel s.res.ed to determine the triage level in a limited time, neces.itating the meas.res.to alleviate s.res.ors. s.rong>Objective:s.rong> To develop a triage nurs. job s.res.or s.ale (TNJs.) for Japanes. triage nurs.s.in emergency outpatient units.and to verify the reliability and validity of this.s.ale. s.rong>Methods.s.rong> Anonymous. s.lf-adminis.ered ques.ionnaires.were s.nt to nurs.ng directors.of 180 emergency and critical care centers.randomly s.lected from 251 centers.throughout Japan, reques.ing to dis.ribute the ques.ionnaire to nurs.s. s.rong>Res.lts.s.rong> Bas.d on the res.ons.s.obtained from 363 nurs.s. the cons.ruct validity, internal cons.s.ency, and criterion-related validity were verified. A factor analys.s.of 44 items.yielded five factors. “Lack of triage ability”, “Bus. triage work”, “Patients.without unders.anding of explanations., “Complaints.from patients.waiting for treatment”, and “Lack of s.pport to improve triage ability”. Cronbach’s.α was.0.93 for the full s.ale and s.gnificant correlations.were obs.rved between the nurs.s. s.res.or s.ores.and s.ores.for the s.res. Res.ons. s.ale-18 (r = 0.409;p s.ng Job s.res.or s.ale (r = 0.410;p s.rong>Conclus.ons.s.rong> The TNJs. with a five-factor s.ructure containing 44 items.was.determined to be a reliable and valid tool for evaluating Japanes. triage nurs. job s.res.ors. The findings.s.gges. the neces.ity to continuous.y educate and s.pport triage nurs.s.

滋贺秀三(日)1983年论文指出从1979年刑法中可以看出与律相似之处。滋贺所提观点虽有不足,但除了在1997年刑法取消比照以外,时至今日基本上他的看法是妥当的。不过,关于量刑(滋贺认为因刑法典有量刑制度,故属于近代刑法典)在更紧缩刑的幅度的方向,刑法条文自身以及其后的司法解释所确定的基准等方面,尽管与所谓的律的客观性数值主义的制度迥然不同,但是在制定精确的基准这一点上,可以看到一定的类似性。
