Efficacy of percutaneous radiofrequency ablation for hepatocellular carcinoma treatment in patients aged≥80 years

Background:Although radiofrequency ablation(RFA)is a minimally invasive treatment for early-stage hepatocellular carcinoma(HCC),it remains unclear whether RFA achieves favorable outcomes in pa-tients aged≥80 *** study aimed to determine the efficacy and safety of RFA for HCC in patients aged≥80 ***:A total of 512 naïve patients with HCC who had undergone RFA from January 2001 to December 2016 were *** were categorized into the≥80-year-old group and the control group(aged<80 years).The primary endpoint was overall survival(OS),and the secondary endpoints were recurrence-free survival,complications associated with RFA,and cause of *** score matching was performed to adjust for patients’sex,liver function,tumor number,tumor diameter,and hepatitis C virus ***,the data of 68 patients in the≥80-year-old group and 68 in the control group were analyzed;their baseline characteristics,primary endpoint,and secondary endpoints were ***:There were significant differences in the alanine aminotransferase level and prothrombin time between the *** cumulative OS rate was not significantly different between the groups(P=0.83):98.5%,87.9%,and 50.5%in the≥80-year-old group and 94.1%,72.8%,and 49.3%in the control group at 1,3,and 5 years,***≥80 years was not significantly associated with OS in multivariate ***-related death occurred in 17 patients in the≥80 year-old group and in 16 patients in the control group(P=1.00).Conclusions:RFA is safe and effective for the treatment of patients with HCC aged≥80 years.

Efficacy of tolvaptan in patients with refractory ascites in a clinical setting

AIM: To elucidate the efficacies of tolvaptan(TLV) as a treatment for refractory ascites compared with conventional treatment. METHODS: We retrospectively enrolled 120 refractory ascites patients between January 1, 2009 and September 31, 2014. Sixty patients were treated with oral TLV at a starting dose of 3.75 mg/d in addition to sodium restriction(> 7 g/d), albumin infusion(10-20 g/wk), and standard diuretic therapy(20-60 mg/d furosemide and 25-50 mg/d spironolactone) and 60 patients with large volume paracentesis in addition to sodium restriction(less than 7 g/d), albumin infusion(10-20 g/wk), and standard diuretic therapy(20-120 mg/d furosemide and 25-150 mg/d spironolactone). Patient demographics and laboratory data, including liver function, were not matched due to the small number of patients. Continuous variables were analyzed by unpaired t-test or paired t-test. Fisher's exact test was applied in cases comparing two nominal variables. We analyzed factors affecting clinical outcomes using receiver operating characteristic curves and multivariate regression analysis. We also used multivariate Cox's proportional hazard regression analysis to elucidate the risk factors that contributed to the increased incidence of ***: TLV was effective in 38(63.3%) patients. The best cut-off values for urine output and reduced urine osmolality as measures of refractory ascites improvement were > 1800 mL within the first 24 h and > 30%, respectively. Multivariate regression analysis indicated that > 25% reduced urine osmolality [odds ratio(OR) = 20.7; P < 0.01] and positive hepatitis C viral antibodies(OR = 5.93; P = 0.05) were positively correlated with an improvement of refractory ascites, while the total bilirubin level per 1.0 mg/dL(OR = 0.57;P = 0.02) was negatively correlated with improvement. Thermal ageing of soft and hard epoxy resins

Surface flashover is a devastating electronic avalanche along the gas–solid interface when a high electric field is applied,which is a potential issue that threatens the safe operations of advanced power electronic,electrical,and spacecraft ***,the underlying physical mechanisms for surface flashover development are still under investigation owing to the complex charge transport processes through the gas phase,solid phase,and gas–solid *** this study,the history of surface flashover theory in the last 50 years is introduced,and several key questions are reviewed from the perspective of the competing mechanisms of charge transport:the role of each phase in a surface flashover,the origin of surface charging,and effects of traps in solid on surface ***,some suggestions involve charge transport processes in each phase,and their correlations are put forward,and a predictable‘charge transport competitive flashover model’is proposed by clarifying the competing mechanisms of charge transport processes through multiple *** study summarises the history and hot topics of physical mechanisms of surface flashover proposed based on classic and recent progress and offers promising routes for developing a more precise surface flashover theory and improving surface flashover performances.

Finite-difference time-domain simulation of partial discharges in a gas insulated switchgear

It is important to estimate the current magnitude and charge amount of a partial discharge (PD) in a gasinsulatedswitchgear (GIS). In this study, electromagnetic fields generated by a PD in an 84 kV-class three-phase GIS fortesting have been measured with a voltage probe installed on the outer surface of an insulation spacer of the GIS tank,and the corresponding simulations have been carried out using the three-dimensional finite-difference time-domain(FDTD) method. FDTD-computed time-domain voltages across spacers agree reasonably well with the correspondingmeasured ones, and the current magnitude of PD and its charge amount have been estimated.

Influence of morphology of hollow silica-alumina composite spheres on their activity for hydrolytic dehydrogenation of ammonia borane

Hollow silica-alumina composite spheres were prepared by a polystyrene(PS)template method using various amounts of PS *** hollow spheres prepared using 40 g were found to be with a diameter of about 300 nm in scanning electron microscopy,and transmission electron microscopy demonstrated their hollow sphere *** the nitrogen adsorption isotherm results,the homogeneous hollow spheres prepared using 40 g of the PS suspension were found to be an ordered pore *** activities of the hollow spheres prepared using various amounts of the PS suspension for hydrolytic dehydrogenation of ammonia borane were *** results showed that 10,7,and 6 mL of hydrogen were evolved from the aqueous ammonia borane solution in about 40 min in the presence of the hollow spheres prepared using 40,80,and 120 g of PS suspension,*** homogeneous hollow spheres with an ordered pore structure showed the highest activity among all the hollow *** amount of acid sites and the coordination number of aluminum active species were characterized using neutralization titration and solid-state ^(27)Al magic angle spinning nuclear magnetic resonance *** homogeneous hollow spheres with an ordered pore structure had high amount of acid sites and 4-coordinated aluminum *** relative proportion of 4-coordinated aluminum species was related to the dispersion of aluminum *** results indicate that the homogeneous hollow spheres with an ordered pore structure showed the high activity because of high amount of acid sites induced by the highly dispersed aluminum species.
