Voluntary Thigh Muscle Strength with Resection Stump-Dependent Blood Flow and Vasodilation in an Amputated Lower Leg with Total Surface Bearing Prosthesis during Dynamic Knee Extensor: A Case Trial

Background: The magnitude of the hyperemic response due to repeated thigh stump exercise on incremental contraction intensity might be useful information in localized exercise tolerance for devising cardiovascular physical therapy for amputees. The effect of exercise on amputated leg blood flow (LBF) may potentially be altered due to voluntary muscle contractions after loss of the lower leg compared with the healthy leg. Case Presentation: A 57-year-old male patient with Burger disease attempted 3 min unilateral repeat/dynamic knee extensor exercise at a target muscle contraction frequency (1 s thigh muscle contraction and 1 s relaxation, 90 repetitions) with each leg at six different contraction intensities (rubber resistance belt). Simultaneous measurement of blood velocity/flow (Doppler ultrasound) in the femoral artery, blood pressure, leg vascular conductance (LVC), and peak muscle strength (PMS) were performed during the 3 min exercise period. The maximum voluntary contraction by one-legged isometric knee muscle contraction was 14.7 kg in non-AL and 7.9 kg in the AL with prosthesis. The relative PMS was defined as “PMS/maximum voluntary contraction × 100 (%)”. Pre-exercise LBF was lower in the AL (200 ± 25 ml/min) than the non-AL (275 ± 74 ml/min). Both the non-AL and AL showed good positive linear relationships between absolute-/relative-PMS and LBF or LVC during 30 s at steady-state before the end of the exercise period. Furthermore, there was also similarity seen in the increase rate in LBF and/or LVC for the incremental relative PMS compared with the absolute PMS. Conclusion: In this case, the muscle strength depended on blood flow increase/vasodilation was seen in this “AL” using a TSB prosthesis for repeated dynamic knee extensor exercise. Psychosocial Factors Affecting the Use of Mammography Testing for Breast Cancer Susceptibility: An Eight-Month Follow-Up Study in a Middle-Aged Japanese Woman Sample

We examined the psychosocial factors affecting middle-aged Japanese women’s intentions to undergo mammography, as well as their actual usage of mammography by applying the Parallel Processing Model (PPM) of self regulation longitudinally. A total of 1030 middle-aged women living in all parts of Japan participated in this study through internet research from September 2010 to May 2011. The participants were evaluated on the basis of a battery of questionnaires mainly including demographics, perceived breast cancer risk, worry about breast cancer, mammography testing beliefs, intentions to use mammography, seeking information about mammography, and actual usage of mammography thrice over an eight-month period. The main results were as follows: 1) Perceived risk and cancer worry affected the intention of undergoing mammography, and this effect was mediated by beliefs about mammography testing. 2) Intention to use mammography and past mammography usage predicted future usage of mammography, with past mammography usage being the strongest predictor. 3) Information seeking about mammography was the strongest predictor of using mammography during the eight-month follow-up period of middle-aged women who had not undergone any mammography testing. PPM was a useful model to explain the mechanism behind middle-aged Japanese women’s intentions to use mammography, as well as their actual usage of mammography. In addition, past mammography experience was the strongest predictor of regular mammography usage and information seeking was a critical factor for the first-usage of mammography.

Proposal and Verification of Simultaneous Measurement Method for Three Thermoelectric Properties with Film-Type Thermocouple Probe

A new simultaneous measurement method for the measurement of the three thermoelectric properties with a film-type thermocouple probe was proposed. Seebeck coefficient was measured using the steady-state condi-tion of the differential method. The electrical resistivity was measured us-ing the four-probe method and the thermal diffusivity is measured using the periodic heating method. The effectiveness of the proposed method was verified using constantan as a reference material. After describing the effectiveness of the method, the measurement of three thermoelectric properties of Bi0.3Sb1.7Te3, which is a thermoelectric material, was per-formed.

PCR-Mediated Detection of Endophytic and Phytopathogenic Fungi from Needles of the Japanese Black Pine, <i>Pinus thunbergii</i>

A specific and sensitive polymerase chain reaction (PCR) assay based on the internal transcribed spacer (ITS) region of rDNA sequences was developed to detect endophytic and phytopathogenic fungi from needles of the Japanese black pine, Pinus thunbergii. Sequences of the ITS regions of Lophodermium conigenum, Lecanosticta acicola, Pestalotiopsis neglecta, Rhizosphaera kalkhoffii, and Septorioides pini-thunbergii were compared, and each specific primer pair for these species was designed. First, the designed primer pairs were tested for their specificity to detect each species. A PCR product was amplified only each combination of species and its specific primer pair, confirming the specificity of the designed primer pairs. These primer pairs were also tested on DNA extracted from the needles of P. thunbergii. The PCR products were amplified not only in needles with lesions but also in healthy needles without symptoms. Furthermore, several endophytic and phytopathogenic fungi could be simultaneously detected from the same region in a needle. The PCR-mediated detection method developed in this study will be a valuable tool for the detection of the endophytic and phytopathogenic fungi, not only as a rapid diagnostic tool for early detection but also for monitoring variations in both the quality and quantity of the endophytic and phytopathogenic fungi in needles in Japanese black pines.

Dear Editor,Microdissection testicular sperm extraction(micro-TESE)is an optimal sperm extraction method for men with nonobstructive azoospermia(NOA).1 According to a recent systematic review by Corona et al.,2 the sperm retrieval rate by micro-TESE and live birth rate per intracytoplasmic sperm injection(ICSI)cycle among NOA patients were 46%and 24%,*** cases where sperm can be collected from NOA patients,the live birth rate remains *** the potential outcomes of ICSI can be predicted by sperm motility assessment,such as pregnancy not anticipated because of immotile sperm,unnecessary ovarian stimulation or oocyte retrieval from female partners could be avoided.

Ductal carcinoma in-situ (DCIS) has been visualized by 2D XDFI (X-ray dark-field imaging) and further by a 3D X-ray CT, and the data was acquired by the X-ray optics DEI (diffraction-enhanced imaging). A newly made algorithm was used for CT. Data of 900 projections with interval of 0.2 degrees were used. Ductus lactiferi, microcalci-fication in a 3D form have been clearly visible. The spatial resolution available was approximately 30μm.

利用GAME Tibet加强观测期间所得到的青藏高原不同地点的降水量资料 ,初步分析了 1 998年 7月和 8月夏降风降水随纬度和海拔高度的变化特征。研究表明藏北高原夏季风降水的分布是纬度效应和海拔高度效应综合作用的结果。 7月份降水量的纬度效应要比高度效应明显 ,而 8月份的降水量不但具有一定的纬度效应 ,也具有一定的海拔高度效应。 7月～ 8月份的降水量主要表现为随纬度的分布特征 ,但它也受到海拔高度效应的修正 ,即由于地形对降水的再分配过程 ,使得某些测点降水随海拔高度的升高而增加。由于青藏高原地表状况复杂 ,其上的降水显然应当同时受到各种因素的影响和制约 ,因此要简单地区分各因子的作用是较困难的。

Preoperative detection and localization of small bowel hemangioma: Two case reports

Among the various diagnostic modalities for small bowel hemangioma, video capsule endoscopy (VCE) and double-balloon enteroscopy (DBE) can be recommended as part of the work-up in patients with obscure gastrointestinal bleeding (OGIB). DBE is superior to VCE in the accuracy of diagnosis and therapeutic potential, while in most cases total enteroscopy cannot be achieved through only the antegrade or retrograde DBE procedures. As treatment for small bowel bleeding, especially spout bleeding, localization of the lesion for the decision of DBE insertion facilitates early treatment, such as endoscopic hemostatic clipping, allowing patients to avoid useless transfusion and the worsening of their disease into life-threatening status. Applying endoscopic India ink marking prior to laparoscopic surgical resection is a particularly useful technique for more minimally invasive treatment. We report two cases of small bowel hemangioma found in examinations for OGIB that were treated with combination of laparoscopic and endoscopic modalities.

We herein report a case of bronchial bleeding afterradical esophagectomy that was treated with lobectomy.A 65-year-old male who underwent subtotal esophagectomy with three-field lymph node dissection for esophageal carcinoma was referred to our hospital because of sudden *** the esophagectomy,bilateral vocal cord paralysis was observed,and the patient suffered from repeated episodes of aspiration *** revealed hemosputum in the right middle lobe bronchus,and contrast-enhanced computed tomography showed tortuous arteries arising from the right inferior phrenic artery and left subclavian artery toward the right middle lobe *** bronchial arterial embolization was performed twice to control the recurrent hemoptysis,the procedures were *** middle lobectomy was therefore performed via video-assisted thoracic *** bronchial arterys with medial hypertrophy and overgrowth of the small branches were noted near the bronchus in the resected *** patient recovered uneventfully and was discharged on postoperative day 14.

Fractalkine and TGF-β1 levels reflect the severity of chronic pancreatitis in humans

MM： To clarify whether serum chemokine and cytokine levels can become useful biological and functional markers to assess the severity of chronic pancreatitis （CP）. This study aimed at clarifying whether serum chemokine and cytokine levels can become useful biological and functional markers to assess the severity of CR METHODS： Serum monocyte chemoattractant protein-1 （MCP-1）, transforming growth factor beta-1 （TGF-βI）, and soluble type fractalkine （s-fractalkine） concentrations were examined in patients with CP （n = 109） and healthy controls （n = 116）. Severity of disease was classified in patients with CP by a staging system. Relationships between stage-specific various clinical factors and serum MCP-1, TGF-β1, and s-fractalkine levels were investigated. Furthermore, 57 patients with non-alcoholic CP were similarly evaluated in order to exclude influence of alcohol intake. RESULTS： Patients with CP showed significant higher levels of serum TGF-β1 and s-fractalkine, but not MCP-1, compared to the controls. Serum TGF-β1 in the severe stage and s-fractalkine in the mild and thesevere stage of CP significantly increased compared to those of controls. However, it was observed that both TGF-β1 and s-fractalkine levels were affected by alcohol intake. In patients with non-alcoholic CP, serum TGF-β1 showed significant increase in the moderate stage of CP, and serum s-fractalkine revealed significant increase in the early stage of CP. CONCLUSION： It is suggested that the measurement of serum F-fractalkine is useful to diagnose early- stage CP. Moreover, the combined determination of both, s-fractalkine and TGF-β1, in human sera may be helpful in evaluating the severity status of CP.
