Direct-acting antivirals for hepatitis C virus-infected patients with hepatocellular carcinoma

BACKGROUND Hepatocellular carcinoma(HCC)in hepatitis C virus(HCV)-infected patients has a high risk of recurrence.Although eradication of HCV is expected to reduce this risk,the risk in patients with a history of HCC may be high after treatment with direct-acting antivirals(DAAs).AIM To determine the risk factors for HCC recurrence in patients with HCV and a history of HCC.METHODS The risk of HCC recurrence in patients with a history of HCC and/or of HCC occurrence in patients without a history of HCC after DAA therapy was retrospectively analyzed in 311 HCV patients treated at our institution and several neighboring hospitals.The frequency and predictors of HCC recurrence/occurrence after DAA treatment were included in these analyses.The clinical course of HCC before and after DAA treatment was also evaluated.RESULTS HCV patients with a history of HCC were older and had greater progression of liver fibrosis and diabetes than patients without a history of HCC.Median recurrence-free survival(RFS)was 1092 d in patients with a history of HCC,and post-DAA HCC recurrence/occurrence was observed in 29 patients(53.7%)with and 5(1.9%)without a history of HCC over 6 years(P<0.001).RFS in patients with a history of HCC did not differ significantly before and after DAA treatment.The frequency of HCC recurrence/occurrence in patients with a history of HCC was lower after than before DAA treatment.Multivariate analysis showed that the incidence rate of HCC recurrence/occurrence before DAA treatment was the only independent predictor of HCC recurrence/occurrence after DAA treatment.Liver function was well preserved and clinical course was good in patients with HCC recurrence/occurrence after DAA therapy.CONCLUSION DAA therapy in patients infected with HCV is also effective in patients with a history of HCC.Curative treatment for HCC is desirable before DAA therapy.The frequency of HCC recurrence/occurrence before DAA therapy was associated with a significantly increased risk of HCC recurrence after DAA therapy.Careful observat

SYSTEMATIC DESIGN OF SINGLE CARRIER OVERLAP FREQUENCY DOMAIN EQUALIZATION

This paper proposes a systematic design method of overlap frequency domain equalization(FDE) for single carrier (SC) transmission without a guard interval (GI).Based on the analysis of signal-to-interference-plus-noise ratio (SINR) of the equalizer output for each symbol,the authors adaptivelydetermine the block of the overlap FDE,where the block is defined as a set of symbols at the equalizeroutput with sufficiently low error rate,for a certain fixed sliding window size,which corresponds toa fast Fourier transform (FFT) window size.The proposed method takes advantage of the fact thatthe utility part of the equalized signal is localized around the center of the FFT window.In addition,the authors also propose to adjust the block size in order to control the computational complexity ofthe equalization per processed sample associating with the average bit error rate (BER) of the system.Simulation results show that the proposed scheme can achieve comparable BER performance to theconventional SC-FDE scheme with sufficient GI insertion for both the coded and uncoded cases withvarious modulation levels,while requiring lower computational complexity compared to the SC overlapFDE transmission with the fixed block.

A case of intrahepatic clear cell cholangiocarcinoma

Intrahepatic clear cell cholangiocarcinoma is very rareonly 8 cases have been reported.A 56-year-old Japanese man with chronic hepatitis B infection was diagnosed with a 2.2 cm hepatocellular carcinoma on imaging,and hepatic segmentectomy was performed.Histopathologically,the tumor cells had copious clear cytoplasm and formed glandular structures or solid nests.These pathological findings suggested the tumor was a clear cell variant of intrahepatic cholangiocarcinoma.Particular stains and radiological images suggested that the cause of the clear cell change had been glycogen,not mucin nor lipid.On immunohistochemical staining,cytokeratin(CK)7 and CK19 were positive,whereas CK20 was negative.Vimentin was detected on the cell membranes,and CD56 was focally positive.The patient was given adjuvant chemotherapy and is currently free from the tumor 7 mo postoperatively.Careful follow-up with adequate postoperative supplementary chemotherapy is necessary because the characteristics of this type of tumor are unknown.

BACKGROUND Acute portal vein thrombosis(PVT)with bowel necrosis is a fatal condition with a 50%-75%mortality rate.This report describes the successful endovascular treatment(EVT)of two patients with severe PVT.CASE SUMMARY The first patient was a 22-year-old man who presented with abdominal pain lasting 3 d.The second patient was a 48-year-old man who presented with acute abdominal pain.Following contrast-enhanced computed tomography,both patients were diagnosed with massive PVT extending to the splenic and superior mesenteric veins.Hybrid treatment(simultaneous necrotic bowel resection and EVT)was performed in a hybrid operating room(OR).EVTs,including aspiration thrombectomy,catheter-directed thrombolysis(CDT),and continuous CDT,were performed via the ileocolic vein under laparotomy.The portal veins were patent 4 and 6 mo posttreatment in the 22-year-old and 48-year-old patients,respectively.CONCLUSION Hybrid necrotic bowel resection and transileocolic EVT performed in a hybrid OR is effective and safe.

A fully inkjet-printed disposable gas sensor matrix with molecularly imprinted gas-selective materials

A method was used to fabricate a fully inkjet-printed gas sensor matrix on photographic paper.An electrode matrix comprising 36 interdigital electrodes in a high-density layout that is easy to integrate has been fabricated using a combination of insulating ink and commercial silver ink.Molecular-imprinted polymer(MIP)inks were then made using a simple solution mixing method,and these inks were printed together with carbon black ink on the electrode matrix to complete production of the sensor.Finally,experimental dynamic sensing of volatile organic compounds verifies that for detection of gases corresponding to the MIP template molecules,the MIP layer offers improvements in both sensitivity and selectivity when compared with non-imprinted polymer layers.The matrix can produce a response of more than 20%to 3 ppm propenoic acid gas through adjustment of the printing times for the carbon black layer and the MIP layer.

Precision surgery with the genetic assessment for operable pancreatic cancer beyond the radiological assessment alone

We congratulate Dr.Dreyer and colleagues for their recently published study entitled“Precision oncology in surgery:Patient selection for operable pancreatic cancer”in Annals of Surgery(1).In this unique study,the authors tried to define preoperative clinical and molecular characteristics in order to improve patient selection for operable pancreatic cancer.Pancreatic ductal adenocarcinoma(PDAC)remains the most lethal type of human cancer due to its high chemoresistance and invasiveness(2).In resected PDAC,the median overall survival has increased from 22.1 to 35 months during the past 10 years,largely owing to improvements in adjuvant therapies(3,4).On the other hand,the high recurrence rate even in patients who underwent curative resection,and chemoresistance in the recurrent sites to current systemic chemotherapies(FOLFIRINOX and nab-paclitaxel plus gemcitabine),are major issues in the treatment for unselected PDAC patient populations.Surgical resection offers the only chance of cure,but surgery can be associated with significant morbidity and decreased activity of daily life(ADL),especially in the Whipple procedure(pancreaticoduodenectomy).Recently,the importance of preoperative chemotherapy even in operable PDAC has been increasingly recognized(5).Although molecular markers are being used more frequently to select patients for systemic targeted agents,only imaging modalities are used to stage patients and to assess their suitability for operative resection.Decisions on primary surgery or a neoadjuvant approach are made without evaluation of biological measures of tumor aggressiveness,or of the risk of occult metastatic disease.

Numerical Modeling of Neotectonic Movements and State of Stresses in the Central Seismic Gap Region,Garhwal Himalaya

This paper presents finite element modeling(FEM) to simulate the present-day stress field and crustal deformation using NE-SW structural section in the central Seismic Gap region of the Garhwal Himalaya.Our study deals with the effect of geometrical characteristics and rock layer parameters on the upper crust.Modeling results show that two types of tectonic regimes developed in the central Seismic Gap region:the geotectonics of the northern part has been controlled by regional compression,whereas southern part is characterized by regional extension.Correspondingly,thrust faults are induced in the northern part and normal faults are extensively developed in the southern front.Those evidences noticeably indicate that the compressive tectonic environment of the Himalaya becomes change into the extensional tectonic regime in its front.The computed shear stress accumulation along the northern flat of Main Himalayan Thrust(MHT) implies that considerable amount of interseismic stress is building up along the MHT system in the Himalaya,which ultimately release through the possible future great Himalayan earthquake(M>8).The comparison between our modeled stress field,faulting pattern and horizontal shortening rate with the distribution of the microseismic events,focal mechanism solutions,active faulting and GPS data in the central Seismic Gap region shows good agreement.

Modeling the Contemporary Stress Field and Deformation Pattern of Eastern Mediterranean

The contemporary stress field in the earth＇s crust is important and provides insights into mechanisms that drive plate motions. In this study, elastic plane stress finite element modeling incorporating realistic rock parameters was used to calculate the stress field, displacement field, and defor- mation of the plate interactions in the eastern Mediterranean. Modeled stress data for the African- Arabian-Anatolian plate interactions with fixed European platform correlate well with observed contemporary stress indicator from the world stress map （WSM） and focal mechanism of earthquakes; while displacement field agrees qualitatively well with GPS vectors and sense of motion indicated by focal mechanisms for large crustal earthquakes （Ms〉6） and plate motion models. Modeling result shows the direction of maximum horizontal compressive stress （σHmax） toward the direction of absolute motion of these plates. Large perturbations in σHmax orientations are shown to occur in and around tectonic boundaries between those plates. It is observed that, although the African plate acts mostly as indenter, which transmits the collisional motion from the Arabian plate to the Anatolian plate, in the current situation, the far-field stress, probably from the subduction in Aegean Arc, is needed to satisfy the contemporary stress field in Anatolia.

Fault Type Analysis along the San Andreas Fault Zone: A Numerical Approach

Finite Element (FE) modeling under plane stress condition is used to analyze the fault type variation with depth along and around the San Andreas Fault (SAF) zone. In this simulation elastic rheology was used and was thought justifiable as the variation in depth from 0.5 km to 20 km was considered. Series of calculations were performed with the variation in domain properties. Three types of models were created based on simple geological map of California, namely, 1) single domain model considering whole California as one homogeneous domain, 2) three domains model including the North American plate, Pacific plate, and SAF zone as separate domains, and 3) Four domains model including the three above plus the Garlock Fault zone. Mohr-Coulomb failure criterion and Byerlee's law were used for the calculation of failure state. All the models were driven by displacement boundary condition imposing the fixed North American plate and Pacific plate motion along N34°W vector up to the northern terminus of SAF and N50°E vector motion for the subducting the Gorda and Juan de Fuca plates. Our simulated results revealed that as the depth increased, the fault types were generally normal, and at shallow depth greater strike slip and some thrust faults were formed. It is concluded that SAF may be terminated as normal fault at depth although the surface expression is clearly strike slip.

Inhibitory effects of carbon dioxide insufflation on pneumoperitoneum and bowel distension after percutaneous endoscopic gastrostomy

AIM:To evaluate the inhibitory effects of carbon dioxide (CO2) insufflation on pneumoperitoneum and bowel distension after percutaneous endoscopic gastrostomy (PEG).METHODS:A total of 73 consecutive patients who were undergoing PEG were enrolled in our study.After eliminating 13 patients who fitted our exclusion criteria,60 patients were randomly assigned to either CO2 (30 patients) or air insufflation (30 patients) groups.PEG was performed by pull-through technique after threepoint fixation of the gastric wall to the abdominal wall using a gastropexy device.Arterial blood gas analysis was performed immediately before and after the procedure.Abdominal X-ray was performed at 10 min and at 24 h after PEG to assess the extent of bowel distension.Abdominal computed tomography was performed at 24 h after the procedure to detect the presence of pneumoperitoneum.The outcomes of PEG for 7 d postprocedure were also investigated.RESULTS:Among 30 patients each for the air and the CO2 groups,PEG could not be conducted in 2 patients of the CO2 group,thus they were excluded.Analyses of the remaining 58 patients showed that the patients' backgrounds were not significantly different between the two groups.The elevation values of arterial partial pressure of CO2 in the air group and the CO2 group were 2.67 mmHg and 3.32 mmHg,respectively (P = 0.408).The evaluation of bowel distension on abdominal X ray revealed a significant decrease of small bowel distension in the CO2 group compared to the air group (P < 0.001) at 10 min and 24 h after PEG,whereas there was no significant difference in large bowel distension between the two groups.Pneumoperitoneum was observed only in the air group but not in the CO2 group (P = 0.003).There were no obvious differences in the laboratory data and clinical outcomes after PEG between the two groups.CONCLUSION:There was no adverse event associated with CO2 insufflation.CO2 insufflation is considered to be safer and more comfortable for PEG patients because of the lower incidence of pneumoperitoneum
