BACKGROUND Drug-eluting stents(DES)are used to treat lower extremity arterial *** DES treatment,aneurysmal degeneration occasionally occurs,especially with fluoropolymer-based ***,the incidence of pseudoaneurysms after DES placement is rarely reported in the lower extremity region,although there have been several reports on pseudoaneurysm formation after DES placement in the coronary artery *** SUMMARY We report the case of a 64-year-old man who presented with fever and pain in his left hand after *** was diagnosed by blood culture,and after admission,he developed pain on the medial side of the right thigh.A pseudoaneurysm was observed in the right superficial femoral artery(SFA)at the proximal end of a previously placed *** bacteremia was thought to have been caused by a pseudoaneurysm of the left superficial brachial artery,and the pseudoaneurysm of the left superficial brachial artery was removed after antibiotic *** pseudoaneurysm of the right SFA rapidly expanded after admission,but the expansion rate was reduced after infection *** months after the first admission,the pseudoaneurysm of the left SFA was re-moved and in situ revascularization performed using a rifampicin-soaked Dacron *** Although pseudoaneurysm after DES placement in the lower extremity region is rare,it must be considered in patients with bacteremia.

The quantitation of serum tocilizumab using liquid chromatography tandem-mass spectrometry(LC-MS/MS)method has not been widely applied in clinical settings because of its time-consuming and costly sample *** present study aimed to develop a validated LC-MS/MS method for detecting serum tocilizumab by utilizing immobilized trypsin without an immunoglobulin G purification step and evaluate its applicability in the treatment of rheumatoid arthritis(RA)patients administered intravenously or subcutaneously with *** tocilizumab-derived signature peptide was deciphered using a nano-LC system coupled to a hybrid quadrupole-orbitrap mass *** serum tocilizumab was rapidly digested by immobilized trypsin for 30 *** chromatographic peak of the signature peptide and that of the internal standard were separated from the serum digests for a total run time of 15 *** calibration curve of serum tocilizumab concentration was linear with a range of 2-200 μg/*** intra-and inter-day accuracy and relative standard deviation(RSD)were 90.7%-109.4%and<10%,*** serum tocilizumab concentrations in the RA patients receiving intravenous and subcutaneous injections were 5.8-28.9 and 2.4-63.5 μg/mL,*** serum tocilizumab concentrations using the current method positively correlated with those using the enzyme-linked immunosorbent assay,although a systematic error was observed between these *** conclusion,a validated LC-MS/MS method with minimal sample pretreatments for monitoring serum tocilizumab concentrations in RA patients was developed.

A Case of Warthin’s Tumor Together with IgG4-Related Sialadenitis

Warthin’s tumor is the second most frequent neoplasm next to pleomorphic adenoma in the salivary gland. The tumor contains the epithelial oncocyte cells with the presence of rich-mitochondria and their surrounding abundant lymphocytes. A relatively new disease entity of IgG4-related disease frequently occurs in the salivary gland. However, the coexistence of Warthin’s tumor and IgG4-related disease is scarcely observed. We have recently experienced a rare case of Warthin’s tumor with IgG4-related sialadenitis. A 51-year-old man presented to our hospital, complaining of a mass with right submandibular tenderness and spontaneous pain. A computed tomography scan of the cervical region revealed a suspicion of lymph node proliferative disease, including malignant lymphoma. Elevated serum levels of IL-2R: 1843 U/ml (reference value 122 - 496 U/ml), IgG: 3430 mg/dl (reference value 861 - 1747 mg/dl), and IgG4: 3140 mg/dl (reference value 11 - 121 mg/dl) were observed. Other laboratory data showed within normal ranges. The cervical tumor was diagnosed as Warthin’s tumor by the findings of fine-needle aspiration cytology and biopsy examination. Immunohistochemistry revealed numerous IgG4- and IgG-positive cells with fibrosis surrounding the epithelial component of Warthin’s tumor, suggesting IgG4-rerated sialadenitis. Finally, we diagnosed the cervical tumor as Warthin tumor with IgG4-related sialadenitis. This is the second report describing a case of Warthin’s tumor with possible involvement of IgG4-related sialadenitis.

Jesmanowicz＇conjecture on Pythagorean numbers states that for any positive integer N, the Diophantine equation（aN）x＋（b N）y=（cN）zhas no positive solution（x, y, z） other than x = y = z = 2. In this paper, we prove this conjecture for the case that a or b is a power of 2.

In the new century, energy and environmental problems are becoming more critical, and the development of natural energy is desired. Low-grade Thermal Energy Conversion(LTEC) is refocused as one of the renewable energy methods. The usefulness of LTEC is expected using hot springs and waste heat. In the case of the Rankine cycle using ammonia as the working fluid, the thermal properties of the working fluid changes in the evaporator. The traditional evaluation method of heat exchanger performance is the LMTD(Logarithmic Mean Temperature Difference) method. On the other hand, the GMTD(Generalized Mean Temperature Difference) method allows the variation of thermal properties in the heat exchanger. The aim of this study is to compare the two methods for the calculation of temperature differences and the corresponding influence on the total performance of the Rankine cycle that is operated using ammonia as a working fluid. As a result, the thermal efficiency of the Rankine cycle is greater than that of the LMTD method. Moreover, the computable range of the GMTD calculation method is less than that of the LMTD calculation method.

Aslanger’s sign,also known as the arterial pulse tapping artifact or electromechanical association artifact,is an electrocardiographic artifact caused by arterial pulsation at the site where the limb leads of the standard 12-lead electrocardiogram near the radial or posterior tibial arteries are positioned,particularly in hyperdynamic states.[1–8]It occurs in every cardiac cycle with a constant coupling interval between the QRS complex and *** synchronization with the underlying heart rhythm makes it less likely to be recognized as an artifact compared to unsynchronized artifacts,such as those caused by limb movement and inadequate contact between the electrode and skin.[1,2,7,8]Almost all reported cases of Aslanger’s sign exhibit an unusual waveform morphology in all 12 leads except one of the standard 12-lead *** sign is often confused with an electrocardiographic finding commonly observed during acute coronary events.
