Sitagliptin for Elderly Patients Aged 75 Years or Older with Inadequately Controlled Type 2 Diabetes with Common Antidiabetes Treatments

Background: The purpose of this study was to evaluate the effects of sitagliptin in elderly patients with type 2 diabetes aged 75 years or older versus those aged 65 - 74 years. Methods: Outpatients aged 65 years or older with type 2 diabetes who received sitagliptin at a dose of 50 mg daily for 6 months were divided into two groups: those who were 75 years and older and those who were 65 - 74 years. The efficacy and safety were retrospectively evaluated by comparison of laboratory values before and after the administration of sitagliptin and by review of adverse events after treatment. Results: One hundred and twelve older patients with type 2 diabetes were studied. Six months after the initiation of sitagliptin, participants’ hemoglobin A1c was significantly decreased by 1.09% ± 0.8% in 65 - 74-year-olds (66 patients;mean age, 69.1 ± 3.0 years;mean HbA1c before administration, 8.4% ± 0.8%) and by 1.05% ± 0.8% in patients 75 years or older (46 patients;mean age, 79.8 ± 4.1 years;mean HbA1c before administration, 8.5% ± 0.7%). There was no significant difference in hemoglobin A1c between the two groups. Furthermore, sitagliptin was well tolerated in both age groups. Conclusions: In elderly patients (75 years or older) with type 2 diabetes, the effect of sitagliptin was similar that in older patients (younger than 75 years) with type 2 diabetes.

High-Uptake Areas on 18F-FRP170 PET Image Necessarily Include Proliferating Areas in Glioblastoma: A Superimposed Image Study Combining 18F-FRP170 PET with 11C-methionine PET

Areas of uptake on positron emission tomography with 1-(2-[18F]fluoro-1-[hydroxymethyl]ethoxy)methyl-2-nitroimidazole (FRP170 PET), a hypoxic cell radiotracer, can include regions retaining highly proliferative activity despite tissue hypoxia. The aim of this study was to clarify whether FRP170 image can detect densely populated hypoxic areas without proliferating potential in glioblastoma. We performed FRP170 PET scan and L-methyl-11C-methionine (MET) PET scan in eight patients with non-treated glioblastoma. Standardized uptake values (SUVs) within tumor and apparent normal brain were measured on each FRP170 and MET image for all patients. To visualize actively proliferative areas on MET image we initially extracted pixels showing a ratio of SUV in tumor to SUV in normal brain (T/N) > 1.6. For FRP170 image, we changed the thresholds between minimum SUV and maximum SUV in tumor. Pixels showing SUV above each threshold were extracted and superimposed on previously extracted pixels from MET image. We estimated whether pixels extracted with MET and FRP170 were visually separated on superimposed image for each pa-tient. When no threshold was established, uptake areas on MET image and FRP170 image overlapped widely on superimposed image in all patients. The higher threshold for FRP170 image diminished FRP170-extracted pixels, and shrunk overlapped areas on superimposed image. However, pixels extracted from FRP170 images could not be completely separated from pixels extracted from MET images in all patients, even with threshold raised to almost maximum SUV. The current findings suggest that uptake areas on FRP170 PET scan necessarily include proliferating areas in glioblastoma.

Aerodynamic Sound Radiated from Two-Dimensional Airfoil with Local Porous Material

In the present paper, the attention is focused on the effect of local porous material on aerodynamic sound radiated from two-dimensional airfoil. We measured the aerodynamic sound radiated from the airfoil with porous material, tripping wire and porous plate which are mounted locally on the surface of the airfoils near the leading edge. At the normal airfoil, discrete frequency noise is clearly observed at small attack angle. However, it is clear that its noise generated from the airfoil decreased with the local porous material on the surface of pressure side of the airfoil. The porous material is effective to reduce this noise compared with the others. And the sound absorbing coefficient and the air permeability were measured for test porous material. The sound absorbing coefficient increased at the high frequency band, and the air permeability became small for porous materials. As the attack angle increased, the discrete frequency noise was not generated from the normal airfoil. The broadband noises were almost same for all test airfoils.

AIM To survey the efficacy and safety of dual therapy with daclatasvir and asunaprevir in the elderly hepatitis C virus(HCV) patients *** Interferon-ineligible/intolerant patients and non-responders to previous pegylated-interferon/ribavirin therapy with chronic HCV genotype 1b infection were enrolled. Child B, C cirrhotic patients were *** received oral direct acting antiviral treatment consisting of 60 mg daclatasvir once daily plus 200 mg asunaprevir twice daily for 24 wk. We divided the patients into two groups of 56 elderly patients(≥ 75 years-old) and 141 non-elderly patients(< 75 years old) and compared the efficacy and safety. RESULTS Ninety-one point one percent of elderly patients and 90.1% of non-elderly patients achieved sustained virological response at 24 wk(SVR24). In the former, 1.8% experienced viral breakthrough, as compared with 3.5% in the latter(not significant). Adverse events occurred in 55.4% of the former and 56.0% of the latter. In the former, 7 cases(12.5%) were discontinued due to adverse events, and in the latter 9 cases were discontinued(6.4%, not significant). CONCLUSION Dual therapy with daclatasvir and asunaprevir achieved the same high rates of SVR24 in HCV elderly patients without more adverse events than in the non-elderly patients.

Experimental and Numerical Investigation on the Flow Characteristics around a Cross-Flow Wind Turbine

This study investigated the flow characteristics around a cross-flow wind turbine. A wind tunnel experiment (WTE) was performed to measure the flow characteristics past the wind turbine when operating at the optimal tip-speed ratio of λ = 0.4. In addition, computational fluid dynamics (CFD) simulations were performed for the flow field around the wind turbine that was operating at tip-speed ratios of λ = 0.1, 0.4, and 0.7. The CFD approach was validated against the WTE measurements. CFD results confirmed that with an increase in λ, the velocity deficit was generally increased in the leeward of the return side of the wind turbine, while it was generally decreased in the leeward of the drive side of the wind turbine. It was also confirmed that with an increase in λ, the turbulence kinetic energy was generally increased in the leeward of the return side of the wind turbine, while it generally decreased in the leeward of the drive side of the wind turbine.

Effects of Pre-Schooler Lifestyle on the Circadian Rhythm of Secretory Immunoglobulin A

Assuming that secretory immunoglobulin A (s-IgA) in pre-schoolers is influenced by wake-up time, bedtime, sleeping duration and activity, the current study examined the influence of basic pre-schooler lifestyle on the circadian rhythm of s-IgA. We conducted a questionnaire survey on lifestyle and measured steps, heart rate and salivary s-IgA in 44 pre-schoolers aged 4 or 5 years old. Saliva was collected over 2 days at six time points: bedtime, upon waking up, upon arriving at nursery school, before lunch, after lunch and upon going home. To obtain relatively reliable s-IgA values, the s-IgA concentration/total protein concentration ratio (referred to as s-IgA/T.P.) was calculated. Relationship between the s-IgA/T.P. and heart rate, steps, wake-up time, bedtime and sleeping duration was then examined using Pearson’s correlation coefficient. Accordingly, pre-schooler lifestyle had the following effect on circadian rhythm of s-IgA: 1) s-IgA/T.P. values were higher upon waking up than at other measurement time points;2) pre-schoolers with considerable physical activity had high s-IgA/T.P. at bedtime and upon waking up;and 3) pre-schoolers with long sleeping durations and early bedtime had high s-IgA/T.P. upon waking up. Thus, our results confirmed that pre-schoolers’ lifestyle habits, such as bedtime, sleeping duration, physical activity, had an effect on the circadian rhythm of s-IgA.

Effects of Span Length and Additional Structure on Flow-Induced Transverse Vibration Characteristic of a Cantilevered Rectangular Prism

We consider the effects of the aspect ratio L/H (where L is the length of a prism, and H is the height of a prism normal to the flow direction) and the size of additional structures (which are a plate and a fin on the surface of a prism) on a vibration characteristic of a cantilevered rectangular prism. The present research is intended to support the analysis of energy harvesting research on the flow-induced vibration in water flow using a magnetostrictive phenomenon. The prisms are constructed from stainless steel and mounted elastically to a plate spring attached to the ceiling wall of the water tunnel. The prisms with aspect ratios of L/H ≥ 5 have reasonably identical vibration characteristics. However, the difference in the vibration characteristic appears distinctly on a rectangular prism with an aspect ratio of L/H = 2.5. The rectangular prism with an aspect ratio of L/H = 10 and a side ratio of D/H = 0.2 has a stable and large response amplitude and oscillates with a lower velocity. The length of the added plate and the size of the added fin influence the velocity of vibration onset. If the length of the added plate and fin size on the rectangular prism with D/H = 0.2 becomes large, the curve of the response amplitude shifts to that of the rectangular prism with D/H= 0.5. The response amplitude of the rectangular prism with/without plate or fin is found to be related to the second moment of area of the prism.
