Optimized quantitative angiographic and intravascular ultrasound parameters predicting the functional significance of single de novo lesions in the left anterior descending artery

Background The correlation betw.en angiographic or intravascular ultrasound （IVUS） variables and fractional flow.reserve （FFR） in patients w.th single left anterior descending artery （LAD） lesion has not been studied. The current study aimed at determining the best cutoff value of angiographic and IVUS parameters for defining FFR 〈0.80 in patients w.th LAD lesion. Methods Quantitative coronary analysis, IVUS and FFR measurements w.re undergone in 169 patients w.th single LAD lesion, The best angiographic and IVUS cutoff value and their predictive value for FFR 〈0.80 w.re compared using area under the receiver-operator characteristic curve （AUC） in overall patients or in subgroups stratified by lesion sites. Results FFR 〈0.80 w.s found in 99 lesions （58.6%）. Minimal lumen area （MLA）, and plaque burden （PB） w.re tw. predictors of FFR 〈0.80. Lesion length had less value in predicting FFR 〈0.80. The cutoff value of PB and MLA for FFR 〈0.80 w.s 75.4% and 3.03 mm2. MLA and PB had similar high diagnostic value for proximal （cutoff value 3.04 mm2 and 76.5%） and distal LAD lesion （2.82 mm2 and 80.6%）. Combination of MLA （2.82 mm2） and PB （80.6%） had increased diagnostic value for distal LAD lesion. Conclusions MLA and plaque burden had equivalent diagnostic value for FFR 〈0.80 w.en lesion localized in LAD. The predictive value of combination of MLA and plaque burden for distal LAD lesion w.s strengthened.

Background Because no data regarding the comparison of crush stenting w.th paclitaxel （PES） or sirolimus eluting stents （SES） for coronary bifurcate lesions have been reported, w. compared the clinical outcomes of these tw. types of stents. Methods Tw. hundred and thirty patients w.th 242 bifurcate lesions w.re enrolled in a prospective, nonrandomized trial Primary endpoints included myocardial infarction, cardiac death and target vessel revascularization at 8 months. Results All patients w.re follow.d up clinically and 82% angiographically at 8 months. Final kissing balloon inflation w.s performed in 72% in the PES and 75% in the SES groups （P〉0.05）. Compared to the SES group, PES group had a higher late loss and incidence of restenosis （P=0.04） in the prebifurcation vessel segment. The postbifurcation vessel segment in the PES group had a greater late loss （（0.7±0.6） mm vs （0.3±0.4） mm, P〈0.001） and higher restenosis in the side branch （25.5% vs 15.6%, P=0.04） w.en compared to the SES group. There w.s significant difference of insegment restenosis in the entire main vessel betw.en PES and SES groups （P=0.004）. Target lesion revascularization w.s more frequently seen in the PES group as compared to the SES group （P=0.01）. There w.s significant difference in the accumulative MACE betw.en these tw. groups （P=-0.01）. The survival rate free from target lesion revascularization w.s significantly higher in the SES group w.en compared to the PES group （P〈0.001）. Conclusion SES is superior to PES in reducing restenosis and target lesion revascularization by 8-month follow.up after crush stenting for bifurcate lesions.

w. proposed a novel solution schema called the Hierarchical Labeling Schema （HLS） to answ.r reachability queries in directed graphs. Different from many existing approaches that focus on static directed acyclic graphs （DAGs）, our schema focuses on directed cyclic graphs （DCGs） w.ere vertices or arcs could be added to a graph incrementally. Unlike many of the traditional approaches, HLS does not require the graph to be acyclic in constructing its index. Therefore, it could, in fact, be applied to both DAGs and DCGs. w.en vertices or arcs are added to a graph, the HLS is capable of updating the index incrementally instead of re-computing the index from the scratch each time, making it more efficient than many other approaches in the practice. The basic idea of HLS is to create a tree for each vertex in a graph and link the trees together so that w.enever tw. vertices are given, w. can immediately know.w.ether there is a path betw.en them by referring to the appropriate trees. w. conducted extensive experiments on both real-w.rld datasets and synthesized datasets. w. compared the performance of HLS, in terms of index construction time, query processing time and space consumption, w.th tw. state-of-the-art methodologies, the path-tree method and the 3-hop method. w. also conducted simulations to model the situation w.en a graph is updated incrementally. The performance comparison of different algorithms against HLS on static graphs has also been studied. Our results show.that HLS is highly competitive in the practice and is particularly useful in the cases w.ere the graphs are updated frequently.

A comparison of the cardiovascular actions of the extract of Radix Stephaniae Tetrandrae (RST), the root of a Chinese herb Stephania tetrandra S Moore, in rats w.th those of tetrandrine (Tet), the best know. active component of RST w.s review.d. The RST extract inhibits Ca2+ influx into the myocyte and reduces protein release during reperfusion w.th a Ca2+ containing solution follow.ng perfusion w.th a Ca2+ free solution (Ca2+ paradox) , and arrhythmia during reperfusion in the isolated perfused heart. It also reduces the infarct size induced by ischemia/reperfusion in vitro and in vivo. In addition, the RST extract suppresses elevation of arterial blood pressure in DOCA-salt hypertensive rats. It does not further reduce the heart rate and coronary flow.significantly during myocardial ischemia. The effects are similar to those of Tet. w.en compared w.th the same doses of Tet alone, the RST extract, of w.ich 9 % is Tet, produces equally potent effects on infarction, arrhythmias, coronary flow.and heart r

Apoptosis or programmed cell death (PCD) is an evolutionarily conserved cellular process that is essential for normal development and homeostasis of multicellular organisms. Defects in the apoptosis signaling result in many diseases including autoimmune diseases and cancer. The apoptosis signaling pathw.y w.s first described genetically in the nematode Caenorhabditis elegans w.ich serves as a framew.rk for the more complex apoptotic pathw.ys that exist in mammals. Background:Our clinical practice of laparoscopic liver resection(LLR)had achieved better short-term and long-term benefits for patients w.th hepatocellular carcinoma(HCC)over open liver resection(OLR),but the underlying mechanisms are not *** study w.s to find out w.ether systemic inflammation plays an important ***:A total of 103 patients w.th early-stage HCC under liver resection w.re enrolled(LLR group,n=53;OLR group,n=50).The expression of 9 inflammatory cytokines in patients at preoperation,postoperative day 1(POD1)and POD7 w.s quantified by Luminex Multiplex *** relationships of the cytokines and the postoperative outcomes w.re compared betw.en LLR and ***:Seven of the circulating cytokines w.re found to be significantly upregulated on POD1 after LLR or OLR compared to their preoperative *** to OLR,the POD1 levels of granulocytemacrophage colony-stimulating factor(GM-CSF),interleukin-6(IL-6),IL-8,and monocyte chemoattractant protein-1(MCP-1)in the LLR group w.re significantly *** POD1 levels of these cytokines w.re significantly correlated w.th longer operative time and higher volume of blood loss during *** levels of these cytokines w.re positively associated w.th postoperative liver injury,and the length of hospital ***,a high level of IL-6 at POD1 w.s a risk factor for HCC recurrence and poor disease-free survival after liver ***:Significantly low.r level of GM-CSF,IL-6,IL-8,and MCP-1 after liver resection represented a milder systemic inflammation w.ich might be an important mechanism to offer better short-term and long-term outcomes in LLR over OLR.

Human embryonic kidney 293 cells w.re transfected w.th plasmid pcDNA-H520C9scFv-rhIL- 2 containing a chimeric cDNA encoding the humanized 520C9 scFv/recombinant human IL-2 fusion protein (H520C9scFv-rhIL-2). The transfected cells in plateau grow.ng phase w.re cultured in serum-free medium for three days. The supernatant w.s collected, concentrated and purified using an affinity column packed w.th CNBr-activated Sepharose 4B coupled w.th anti-rhIL-2 mouse monoclonal antibody. The purified fusion protein w.s analyzed by ELISA, SDS-PAGE and w.stern blot. The fusion protein show.d only one band in both silver stained electrophoresis gel and w.stern blot developed by ECL chemiluminescence system. Its molecular w.ight w.s confirmed to be about 45 kD. This fusion protein possessed binding specificity against p185 positive SKOV3 and B16/neu cells, and it might stimulate IL-2-dependent CTLL-2 cell prolifera- tion as w.ll.
