A Modified Enhanced Recovery after Surgery (ERAS): Use and Surgical Outcome in Breast Cancer Patients

Background: ERAS protocols are perioperative interventions aimed at reducing postoperative complications, length of hospital stay (LOS) and early return to normal activities. This has improved outcome in many surgical specialties, including breast surgery. We present the surgical outcome of breast cancer (BC) patients treated over a 12-month period following the principle of ERAS protocols and highlight the underpinning evidence. Methods: A retrospective analysis of all BC patients diagnosed and had breast cancer surgery over 12 months. Data collected included patient’s demographics, type of surgery, LOS, other perioperative care and significant postoperative complications. Excluded were patients with bilateral cancer surgeries, diagnostic excision, margin clearance or breast reconstruction. Results: There were 621 BC diagnosed including 5 male and 12 bilateral female BC. The ages ranged from 25 to 93 years. Excluding bilateral BC, 351 patients (70.2 %) had breast conserving surgery (BCS) while 149 (29.8%) patients had mastectomy as index cancer surgery. Sixteen (4.5%) of the women who initially underwent BCS subsequently had a completion mastectomy. The overall rate of successful BCS was 335/500 (67%). 441 (85.5%) of patients were discharged same or next day. 12 (7.2%) cases of postoperative haematoma, 6 cases of wound infection and a case of seroma requiring surgical/radiological drainage recorded. Conclusion: ERAS protocol in BC surgery is associated with decreased LOS and low complication rate. Delayed discharges are mostly due to adverse social factors and medical comorbidity rather than post-operative surgical complications.

Stochastic computing(SC)has a substantial amount of study on application-specific integrated circuit(ASIC)design for artificial intelligence(AI)edge computing,especially the convolutional neural network(CNN)***,SC has little to no optimization on field-programmable gate array(FPGA).Scaling up the ASIC logic without FPGA-oriented designs is inefficient,while aggregating thousands of bitstreams is still challenging in the conventional *** research has reinvented several FPGA-efficient 8-bit SC CNN computing architectures,i.e.,SC multiplexer multiply-accumulate,multiply-accumulate function generator,and binary rectified linear unit,and successfully scaled and implemented a fully parallel CNN model on Kintex7 *** proposed SC hardware only compromises 0.14%accuracy compared to binary computing on the handwriting Modified National Institute of Standards and Technology classification task and achieved at least 99.72%energy saving per image feedforward and 31?more data throughput than modern *** to SC,early decision termination pushed the performance baseline exponentially with minimum accuracy loss,making SC CNN extremely lucrative for AI edge computing but limited to classification *** SC's inherent noise heavily penalizes CNN regression performance,rendering SC unsuitable for regression tasks.

From seawater to hydrogen via direct photocatalytic vapor splitting: A review on device design and system integration

Solar-driven hydrogen production from seawater attracts great interest for its emerging role in decarbonizing global energy consumption. Given the complexity of natural seawater content, photocatalytic vapor splitting offers a low-cost and safe solution, but with a very low solar-to-hydrogen conversion efficiency. With a focus on cutting-edge photothermal–photocatalytic device design and system integration, the recent research advances on vapor splitting from seawater, as well as industrial implementations in the past decades were reviewed. In addition, the design strategies of the key processes were reviewed, including vapor temperature and pressure control during solar thermal vapor generation from seawater, capillary-fed vaporization with salt repellent, and direct photocatalytic vapor splitting for hydrogen production. Moreover, the existing laboratory-scale and industrial-scale systems, and the integration principles and remaining challenges in the future seawater-to-hydrogen technology were discussed.

以某非对称双塔连体超高层结构为研究对象,选取与建筑所在场地相匹配的地震波作为激励,通过对结构进行非线性动力时程分析,研究混合消能减震连体结构在各烈度地震作用下的消能减震效果,探讨了屈曲约束支撑(buckling-restrained brace, BRB)与黏滞阻尼悬臂桁架的位置变化对结构抗震性能的影响,同时对结构进行了全面的抗震性能评估。研究结果表明:在连体结构中、上部的加强层中布设阻尼器时,结构具有更佳的减震效果;在不同水准地震作用下,BRB与黏滞阻尼悬臂桁架耗能阶段不同且具有不同的耗能效率,混合消能减震技术可以综合不同类型阻尼器的优势作用,使连体结构具有更全面的抗震性能。在控制变形方面,其能够降低连体结构的扭转效应和层间位移角;在构件耗能方面,其减轻了主体构件进入塑性的程度,降低了结构主要受力构件的应力水平。将为混合消能减震技术在连体超高层结构中的进一步研究和应用提供借鉴。

Case of Refractory Thrombocytopenia in Pregnancy Associated with May-Heglin Anomaly after Repeated Platelet Transfusions

May-Heglin Anomaly is an autosomal dominant disorder characterized by macrothrombocytopenia with a platelet function that is usually preserved. Platelets play an essential role in hemostasis. During pregnancy, a woman is susceptible to complications, including postpartum hemorrhage. Monitoring patients’ hemostatic functions and observing the patient’s clinical picture to maintain patient safety is paramount, while avoiding unnecessary therapeutic measures. This case report presents a rare instance of May-Heglin Anomaly (MHA) in a 35-year-old pregnant patient, with refractory thrombocytopenia despite receiving multiple platelet transfusions. Initially referred to as gravida 5 para 4 with severe thrombocytopenia at 28 weeks gestation, throughout her pregnancy, she was closely monitored and received over 40 units of platelets, which failed to increase her platelet count significantly. She delivered a healthy baby via vaginal delivery at 38 weeks, with her platelet count still critically low. This report highlights the challenges of managing MHA in pregnancy, the inefficacy of standard thrombocytopenia treatments such as platelet transfusion in MHA patients, and the importance of tailored management strategies to ensure maternal and fetal safety.

Synergistic Interactions of Soil and Vegetation in Agroforestry Systems in Mitigating Climate Change in Upper East Region, Ghana

Climate change has been a global pandemic with its adverse impacts affecting environments and livelihoods. This has been largely attributed to anthropogenic activities which generate large amounts of Green House Gases (GHGs), notably carbon dioxide (CO2), methane (CH4), and nitrous oxide (N2O) among others. In the Upper East of Ghana, climate change manifests in erratic rainfalls, drought, high temperatures, high wind speeds, high intensity rainfall, windstorms, flooding, declining vegetation cover, perennial devastating bushfires etc. Practices such as burning farm residues, use of dung as fuel for cooking, excessive application of nitrogenous fertilizers, and deforestation that are prevalent in the region exacerbate the situation. Although, efforts made by governmental and none-governmental organizations to mitigate climate change through afforestation, agroforestry and promotion of less fuelwood consuming cook stoves, land management practices antagonize these efforts as more CO2 is generated than the carrying capacity of vegetation in the region. Research findings have established the role of trees and soil in carbon sequestration in mitigating climate. However, there is limited knowledge on how the vegetation and soil in agroforestry interplay in mitigation climate change. It is against this background that this review seeks to investigate how vegetation and soil in an agroforestry interact synergistically to sequester carbon and contribute to mitigating climate change in Upper East region of Ghana. In this review, it was discovered soil stored more carbon than vegetation in an agroforestry system and is much effective in mitigating climate change. It was found out that in order to make soil and vegetation in an agroforestry system interact synergistically to effectively mitigate climate change, Climate Smart Agriculture practice which integrates trees, and perennials crops effectively mitigates climate.
