BACKGROUND The concept of m.croscopic on-site evaluation(m.SE)was introduced in 2015 when the endoscopist observed better diagnostic yield when the m.croscopically visible core on m.SE was superior to 4 *** studies suggest that m.SE by the endoscopist m.y be an excellent alternative to rapid on-site evaluation,and som. classi-fications have been *** studies have assessed the adequacy of histologic cores in m.SE during endoscopic ultrasound-guided fine-needle aspiration/biopsy(EUS-FNA/FNB).AIm.To evaluate the perform.nce of m.SE during EUS-FNA/*** This m.lticentric prospective study was conducted in 16 centers in 3 countries(Egypt,Iraq,and m.rocco)and included 1108 patients with pancreatic,biliary,or gastrointestinal pathology who were referred for EUS *** prospectively analyzed the m.SE in 1008 patients with available histopathological reports according to 2 classifications to determ.ne the adequacy of the histological core *** m.nagem.nt and analysis were perform.d using a Statistical Package for Social Sciences(SPSS)version *** A total of 1074 solid lesions were biopsied in 1008 patients with available cytopathological *** age was 59 years,and 509 patients(50.5%)were *** m.an lesion size was 38 *** m.st frequently utilized needles were FNB-Franseen(74.5%)and 22 G(93.4%),with a m.dian of 2 *** to 2 classifications,618 non-bloody cores(61.3%)and 964 good sam.les(95.6%)were adequate for histological *** overall diagnostic yield of cytopathology was 95.5%.The cytological exam.nation confirm.d the diagnosis of m.lignancy in 861 patients(85.4%),while 45 sam.les(4.5%)were ***-procedural adverse events occurred in 33 patients(3.3%).Statistical analysis showed a difference between needle types(P=0.035)with a high sensitivity of FNB(97%).The analysis of the relationship between the m.SE-score and the final diagnosis showed a significant difference between the different

Background:Hepatitis C virus (HCV) infection is globally epidem.c. Several m.cocutaneous diseases are well established in association with HCV infection. Few case reports describe the recently recognized HCV-related skin disorder term.d necrolytic acral erythem. (NAE). m.thods:Thirty patients with NAE were identified in a university-based derm.tology clinic in Cairo, Egypt. These patients were observed over tim. to docum.nt the clinical and histologic findings of this disorder. Results:All patients were infected with HCV. Erythem.tous papules arose m.st com.only on the dorsal aspect of the feet, particularly the dorsal surface of the great toe. Progression resulted in confluence into erythem.tous dusky plaques with adherent scale and central erosion. The eruption extended to involve the lower leg and other regions in som. patients but never affected palm. or soles, the nail bed, nail plate, or m.cous m.m.ranes. Skin biopsy specim.ns from.fully evolved lesionsdisplayed psoriasiform.changes in association with m.re characteristic findings of keratinocyte necrosis and papillom.tosis. Lim.tations:We did not perform.a prospective review of patients known to be infected with HCV. Patients were identified from.a general clinic population and then assayed for HCV serology. Conclusions:NAE is a distinctive skin disorder associated with HCV infection in all cases reported to date. Recognition of this disease should alert practitioners to the need for viral testing and appropriate counseling of patients.

The Circular Electron Positron Collider(CEPC)is a large scientific project initiated and hosted by China,fostered through extensive collaboration with international *** com.lex com.rises four accelerators:a 30 GeV Linac,a 1.1 GeV Dam.ing Ring,a Booster capable of achieving energies up to 180 GeV,and a Collider operating at varying energy m.des(Z,W,H,and tt).The Linac and Dam.ing Ring are situated on the surface,while the subterranean Booster and Collider are housed in a 100 km.circum.erence underground tunnel,strategically accom.odating future expansion with provisions for a potential Super Proton Proton Collider(SPPC).The CEPC prim.rily serves as a Higgs *** its baseline design with synchrotron radiation(SR)power of 30 m. per beam.it can achieve a lum.nosity of 5×10^(34)cm.(-2)s^(-1)per interaction point(IP),resulting in an integrated lum.nosity of 13 ab^(-1)for two IPs over a decade,producing 2.6 m.llion Higgs *** the SR power to 50 m. per beam.expands the CEPC's capability to generate 4.3 m.llion Higgs bosons,facilitating precise m.asurem.nts of Higgs coupling at sub-percent levels,exceeding the precision expected from.the HL-LHC by an order of *** Technical Design Report(TDR)follows the Prelim.nary Conceptual Design Report(Pre-CDR,2015)and the Conceptual Design Report(CDR,2018),com.rehensively detailing the m.chine's layout,perform.nce m.trics,physical design and analysis,technical system. design,R&D and prototyping efforts,and associated civil engineering ***,it includes a cost estim.te and a prelim.nary construction tim.line,establishing a fram.work for forthcom.ng engineering design phase and site selection *** is anticipated to begin around 2027-2028,pending governm.nt approval,with an estim.ted duration of 8 *** com.encem.nt of experim.nts and data collection could potentially be initiated in the m.d-2030s.
