Delirium Severity and Correlates among Medical Inpatients Admitted through Accident and Emergency Unit of j.s University Teaching Hospital

Background: Delirium is one of the most common medical emergencies and is associated with poor outcomes including: mortality, prolonged length of stay and poor functional outcome. The more severe delirium is the worst the clinical outcomes of medical illness. Despite the obvious, not much has been documented on delirium severity and its associated factors among medical inpatients in low income countries including Nigeria. Obj.ctives: This study was conducted to examine the severity of delirium among medical inpatients admitted through the Accident and Emergency unit of j.s University Teaching Hospital as well as to assess its associated Sociodemographic and Clinical characteristics. Method: This was a cross-sectional study that employed a consecutive sampling technique to select 290 eligible subj.cts from medical inpatients that presented to Accident and Emergency unit of j.s University Teaching Hospital. On admission, patients were assessed for delirium using Confusion Assessment Method and severity of delirium was evaluated using Delirium Rating Scale-Revised-98. Results: Of the 105 delirious medical inpatients, 48 (45.7%) had severe delirium, 41 (39.1%) developed less severe delirium while 16 (15.2) presented with no severe delirium. Sociodemographic characteristics found to be significantly associated with less to more severe delirium include male gender (P = 0.001), nonprofessionals (P = 0.003), income range of N20,000 - N49,000 (P Conclusion: This study demonstrates that delirium is often associated with higher severity in Accident and Emergency unit admission and there are strongly associated clinical characteristics to watch out for in high risk medical inpatients.

The ever-growing human population,coupled with the exigent need to meet the increasing demand for poultry meat and egg,has put the onus on poultry nutritionists and farmers to identify alternative feed ingredients that could assure the least-cost feed *** addition,the public desire for nonantibiotic-treated poultry products has also necessitated the ultimate search for potent antibiotic alternatives for use in poultry *** some identified alternatives are promising,their cost implications and technical know-how requirements may discourage their ease of adoption in *** use of plants and/or their by-products,like fruit pomaces,present a pocket-friendly advantage and as a result,are gaining much *** is traceable to their rich phytochemical profile,nutritional composition,ready availability,and relatively cheap *** fruit j.ice and wine pressing industries generate a plethora of fruit wastes ***,fruit pomaces contain appreciable dietary fibre,protein,and phenolic compounds,and thus,their adoption could serve the poultry industry in dual capacities including as substitutes to antibiotics and some conventional ***,there is a possibility to reduce fruit wastes produced and feed-cost in poultry farming from environmental and economical standpoints,*** review seeks to provide reinforcing evidence on the applicability and impact of fruit pomaces in poultry nutrition.

Effect of Antenatal Depression on Fetal Growth Outcomes at the j.s University Teaching Hospital j.s, Plateau State, Nigeria

Background: Depression is the most prevalent psychiatric disorder in pregnancy and it is associated with psychosocial and obstetric factors. Studies have shown that pregnancy does not prevent women from becoming depressed;rather, it may be a time when depression occurs for the first time in some women. Antenatal depression has been identified as a risk factor for post natal depression, adverse obstetric outcomes, poorer neonatal outcomes and higher growth retardation in infants. Purpose: This study aimed to determine the fetal growth outcomes among depressed pregnant women in their third trimester attending antenatal clinic at the j.s University Teaching Hospital (j.TH), j.s, Plateau State, Nigeria. Method: A prospective cohort study design was used to assess 514 women who consented to the study (256 cases and 258 controls). A socio-demographic questionnaire was given to the women to fill out the study entry. Edinburgh Post Natal Depression Scale (EPDS) was used to screen for depression and MINI neuropsychiatric interviews were used to diagnose depression in those women found to be at risk of depression using the EPDS. Ultrasonography was used to determine the fetal weight in the third trimester of pregnancy. The birth weight of the babies born to the women was obtained from the birth register in the labor ward and the fetal growth rate was calculated from the estimated fetal weight on ultrasound scan in late pregnancy and the birth weight of babies. Result: The mean fetal weight in the third trimester for non-depressed women was slightly higher than in depressed women though the difference was not statistically significant (P = 0.431). The difference in the mean calculated fetal growth rate for fetuses of non-depressed women in the third trimester was statistically significantly higher than in depressed women (p = 0.000). Depressed women also had babies with lower birth weight than non-depressed women and the difference was statistically significant (p = 0.00).

Organic wastes are one of the greatest challenges that cities face worldwide. In numerous underdeveloped nations, like Cameroon, waste is often disposed of through landfills, composting, or open-air combustion. Unfortunately, the concept of waste sorting and organic waste processing is new to many individuals. This has led to an increase in the amount of organic waste and the costs connected with its management. Consequently, the maj.rity of developing nations have sought out waste management solutions that are more cost-effective. Therefore, it has been determined that the bioconversion of organic wastes by black soldier fly larvae (BSFLs) (Hermetia illucens) into multifunctional prepupae is a viable alternative. Appreciation is given to the employment of the organic waste management approach in developing nations since it is not only environmentally friendly and economically viable, but also provides a means for waste valorisation through the production of diverse resources and potential economic benefits. Studies have proved the usefulness of the insect in controlling organic wastes, but countries such as Cameroon are still unfamiliar with the nuances of this method. Consequently, this timely review examined the performance of the BSFL, specifically in organic waste treatment, as well as the best practices for multiplying them to determine its viability for use in a waste treatment plant, the production of high-quality larvae as a source of protein for livestock, and the production of diesel fuel.

Antipredatory call behavior of lapwing species in an Afrotropical environment

Depression in patients with irritable bowel syndrome in j.s,Nigeria

AIM: To study the brain-gut interaction and the effect of behavioral or psychiatric conditions on irritable bowel syndrome (IBS) in an African population. METHODS: IBS was diagnosed using the Rome Ⅱ diagnostic criteria. The entry of each patient was confirmed following detailed explanations of the questions. Four hundred and eighteen patients were studied. Subj.cts satisfying the Rome Ⅱ criteria for IBS were physically examined and stool microscopy was done to identify the presence of “alarm factors”. Depression was diagnosed using the symptom-check list adapted from the Research Diagnostic Criteria (DSM-Ⅳ) of the American Psychiatric Association. RESULTS: Seventy-five (56.8%) of the 132 IBS patients were depressed whereas only 54 (20.1%) of the 268 non-IBS patients were depressed. There was a significant relationship between IBS and depression (χ2 = 54.29, Odds ratio = 5.21, 56.8 ± 8.4 vs 20.1 ± 5.2, P = 0.001). Even though constipation predominant IBS patients were more likely to be depressed, no significant relationship was found between the subtype of IBS and depression (χ2 = 0.02, OR = 0.95, P = 0.68). CONCLUSION: IBS is significantly associated with maj.r depression but not gender and bowel subtypes of the patients. Patients with IBS need to be evaluated for depression due to the highly significant relationship between the two conditions.

The August 2011 Flood in Ibadan, Nigeria: Anthropogenic Causes and Consequences

Determining when and where flood strikes can be a daunting task. Apart from heavy and prolonged rainfalls and river overflows, there are anthropogenic causal factors of flooding. These anthropogenic factors are significantly variable and exacerbate floods, but may be difficult to measure. This study aims to unravel some of the anthropogenic factors, particularly with respect to their contributions to the flood in Ibadan City on 26 August 2011. Data were collected through structured questionnaire and key informant interviews. The August 2011 Ibadan flood was perhaps the worst in the history of this ancient city. Twelve anthropogenic factors are broadly identified as having contributed to the flood and the flood had five maj.r consequences. Some policy implications that can help prevent future flood occurrence are outlined.

Diagnostic Utility of Salivary Matrix Metalloproteinase-8 (MMP-8) in Chronic Periodontitis: A Novel Approach

Currently, diagnosis of chronic periodontitis is based on clinical methods which are cumbersome and have been shown to have inherent errors and drawbacks including inability to detect active disease. There is therefore an urgent need for a timely, cost-effective and less cumbersome method. Salivary matrix metalloproteinase-8 has been shown in some studies to be a promising bio-marker of chronic periodontitis. This was a case-control study conducted at the University College Hospital, Ibadan. It had 40 cases and 40 controls designed to assess the diagnostic ability of salivary MMP-8 in periodontal disease. Unstimulated whole saliva was assessed using the Quantikine human total MMP-8 ELISA kit from R&D? systems Europe.

Solitary Plasmacytoma of the Zygoma in a 70-Year-Old Nigerian: A Case Report and Review of Literature

Purpose: Solitary bone plasmacytoma [SBP] is a localised variant of plasma cell tumor which commonly affects the axial skeleton but rarely found in the maxillofacial region. We hereby report an unusual case of solitary plasmacytoma of the right zygomatic bone in a 70-year-old Nigerian. Case Report: The patient presented with a painless right zygomatic swelling of 2 years duration. Examination showed that the swelling was of mixed consistency [mostly bony but firm in some areas] and it measured about 8 × 8 cm. Radiographic examination revealed an osteolytic lesion over the right zygoma with involvement of the apex and lateral wall of the antrum. An initial working diagnosis of ossifying fibroma was made and the patient was scheduled for surgery. Histological staining with H & E and immunohistochemistry of the surgical specimen however confirmed a diagnosis of SBP. Serum monoclonal protein and Bence j.nes proteinuria was negative throughout the follow up period. Conclusion: SBP of the maxillofacial region is amenable to surgery combined with postoperative radiotherapy. However, prognosis can be worsened with tumor recurrence or dissemination into MM. Thus patients with SBP must be closely followed up after treatment so that immediate therapeutic steps can be taken if recurrence or systemic dissemination is encountered.
